| 1 | APACHE system is better than Ranson system in the prediction of severity of acute pancreatitis显示文摘BACKGROUND: It has been suggested that addition of obesity score to the APACHE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APACHE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APACHE-Ⅱ and APACHE-O systems in assessing severity of acute pancreatitis in a local Chinese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APACHE-Ⅱ and APACHE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APACHE-Ⅱ, and APACHE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APACHE-Ⅱ and APACHE-O systems was 0.808, 0.955 and 0.951, respectively. CONCLUSIONS: The APACHE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. | 2 | Advances in Computer Vision-Based Civil Infrastructure Inspection and Monitoring显示文摘Computer vision techniques, in conjunction with acquisition through remote cameras and unmanned aerial vehicles (UAVs), offer promising non-contact solutions to civil infrastructure condition assessment. The ultimate goal of such a system is to automatically and robustly convert the image or video data into actionable information. This paper provides an overview of recent advances in computer vision techniques as they apply to the problem of civil infrastructure condition assessment. In particular, relevant research in the fields of computer vision, machine learning, and structural engineering is presented. The work reviewed is classified into two types: inspection applications and monitoring applications. The inspection applications reviewed include identifying context such as structural components, characterizing local and global visible damage, and detecting changes from a reference image. The monitoring applications discussed include static measurement of strain and displacement, as well as dynamic measurement of displacement for modal analysis. Subsequently, some of the key challenges that persist toward the goal of automated vision-based civil infrastructure and monitoring are presented. | 3 | Environmental application of three-dimensional graphene materials as adsorbents for dyes and heavy metals: Review on ice-templating method and adsorption mechanisms显示文摘The remediation of wastewater requires treatment technologies which are robust, efficient,simple to operate and affordable such as adsorption. Lately, three-dimensional(3D)graphene based materials have attracted significant attention as effective adsorbents for wastewater treatment. The intrinsic properties of 3D graphene structure such as large surface area and interconnected porous structure can facilitate the transport of pollutants into the 3D network and provide abundant active sites for trapping the pollutants. For the synthesis of 3D graphene structure, ice-templating is commonly practiced due to its facile steps, cost effectiveness and high scalability potential. This review covers the icetemplating fabrication technique for 3D graphene based materials and their application as adsorbents in eliminating dyes and heavy metals from aqueous media. The assembly mechanisms of the ice-templating fsynthesis are comprehensively discussed. Further discussion on the fundamental principles, critical process parameters and characteristics of ice-templated 3D graphene structures is also included. A thorough review on the mechanisms for batch adsorption of dyes and heavy metals is presented based on the structures and properties of the 3D graphene materials. | 9 | Gastrointestinal endoscopy biopsy derived proteomic patterns predict indeterminate colitis into ulcerative colitis and Crohn's colitis显示文摘Patients with indeterminate colitis(IC) are significantly younger at diagnosis with onset of symptoms before the age of 18 years with significant morbidity in the interim. The successful care of IC is based on microscopic visual predict precision of eventual ulcerative colitis(UC) or Crohn's colitis(CC) which is not offered in 15%-30% of inflammatory bowel disease(IBD) patients even after a combined state-of-the-art classification system of clinical, visual endoscopic, radiologic and histologic examination. These figures have not changed over the past 3 decades despite the introduction of newer diagnostic modalities. The patient outcomes after restorative proctocolectomy and ileal pouch-anal anastomosis may be painstaking if IC turns into CC. Our approach is aiming at developing a single sensitive and absolute accurate diagnostic test tool during the first clinic visit through endoscopic biopsy derived proteomic patterns. Matrix-assisted-laser desorption/ionization mass spectrometry(MS) and/or imaging MS technologies permit a histology-directed cellular test of endoscopy biopsy which identifies phenotype specific proteins, as biomarker that would assist clinicians more accurately delineate IC as beingeither a UC or CC or a non-IBD condition. | 12 | Damper placement for seismic control of super-long-span suspension bridges based on the first-order optimization method显示文摘To ensure the anti-earthquake performances of super-long-span suspension bridges, effective devices should be employed to control the seismic response of key sections. In this paper, four kinds of assessment functions for seismic response control effect are formulated based on the mechanism of seismic response control with dampers and the seismic response characteristics of long-span suspension bridges. A new optimal placement method of dampers using penalty function and first-order optimization theory is then proposed. Runyang suspension bridge (RSB) with a main span of 1490 m is then taken as an example. After seismic response time-history analyses on RSB using different placements of dampers, the analysis results are optimized by employing the optimal placement method and the optimal placements of dampers with the four assessment functions are then achieved respectively. Comparison of the four optimal control effects show that different assessment functions can lead to different optimal placements when the number of dampers is certain, but all placements of dampers can reduce the seismic response of RSB significantly. The selection of assessment functions and damper optimal placement should be determined by the structural characteristics and by what is considered in the structures. | 13 | Impaired granulocyte-macrophage colony-stimulating factor bioactivity accelerates surgical recurrence in ileal crohn's disease显示文摘AIM To examine the relationship between elevated granulocyte-macrophage colony-stimulating factor(GMCSF) auto-antibodies(Ab) level and time to surgical recurrence after initial surgery for Crohn's disease(CD). METHODS We reviewed 412 charts from a clinical database at tertiary academic hospital. Patients included in the study had ileal or ileocolonic CD and surgical resection of small bowel or ileocecal region for management of disease. Serum samples were analyzed for serological assays including GM-CSF cytokine, GM-CSF Ab, ASCA Ig G and Ig A, and genetic markers including SNPs rs2066843, rs2066844, rs2066845, rs2076756 and rs2066847 in NOD2, rs2241880 in ATG16 L1, and rs13361189 in IRGM. Cox proportional-hazards models were used to assess the predictors of surgical recurrence.RESULTS Ninety six percent of patients underwent initial ileocecal resection(ICR) or ileal resection(IR) and subsequently 40% of patients required a second ICR/IR for CD. GMCSF Ab level was elevated at a median of 3.81 mcg/mL. Factors predicting faster time to a second surgery included elevated GM-CSF Ab [hazard ratio(HR) 3.52, 95%CI: 1.45-8.53, P = 0.005] and elevated GM-CSF cytokine(HR = 2.48, 95%CI: 1.31-4.70, P = 0.005). Factors predicting longer duration between first and second surgery included use of Immunomodulators(HR = 0.49, 95%CI: 0.31-0.77, P = 0.002), the interaction effect of low GM-CSF Ab levels and smoking(HR = 0.60, 95%CI: 0.45-0.81, P = 0.001) and the interaction effect of low GM-CSF cytokine levels and ATG16 L1(HR = 0.65, 95%CI: 0.49-0.88, P = 0.006).CONCLUSION GM-CSF bioavailability plays a critical role in maintaining intestinal homeostasis. | 14 | maturity of associating liver partition and portal vein ligation for staged hepatectomy-derived liver regeneration in a rat model显示文摘AIM To establish a rat model for evaluating the maturity of liver regeneration derived from associating liver partition and portal vein ligation for staged hepatectomy(ALPPS).METHODS In the present study, ALPPS, partial hepatecotmy(PHx), and sham rat models were established initially, which were validated by significant increase of proliferative markers including Ki-67, proliferating cell nuclear antigen, and cyclin D1. In the setting of accelerated proliferation in volume at the second and fifth day after ALPPS, the characteristics of newborn hepatocytes, as well as specific markers of progenitor hepatic cell, were identified. Afterwards, the detection of liver function followed by cluster analysis of functional gene expression were performed to evaluate the maturity.RESULTS Compared with PHx and sham groups, the proliferation of f LR was significantly higher in ALPPS group(P = 0.023 and 0.001 at second day, P = 0.034 and P < 0.001 at fifth day after stage I). Meanwhile, the increased expression of proliferative markers including Ki-67, proliferating cell nuclear antigen, and cyclin D1 verified the accelerated liver regeneration derived from ALPPS procedure. However, ALPPS-induced Sox9 positive hepatocytes significantly increased beyond the portal triad, which indicated the progenitor hepatic cell was potentially involved. And the characteristics of ALPPSinduced hepatocytes indicated the lower expression of hepatocyte nuclear factor 4 and anti-tryptase in early proliferative stage. Both suggested the immaturity of ALPPS-derived liver regeneration. Additionally, the detection of liver function and functional genes expression confirmed the immaturity of renascent hepatocytes derived in early stage of ALPPS-derived liver regeneration.CONCLUSION Our study revealed the immaturity of ALPPS-derived proliferation in early regenerative response, which indicated that the volumetric assessment overestimated the functional proliferation. | 15 | Flash flood sediment transport in a steep sand-bed ephemeral stream显示文摘This paper reports about flow and sediment transport measurements undertaken during the rainy season(July-August) 2008 on Gereb Oda,a steep,sand-bed ephemeral stream,draining the western margin of the Kobo structural basin in northern Ethiopia.Gereb Oda streambed is dry for most of the year since it is subjected only to sporadic flash floods in response to individual,high intensity and spatially confined rainstorms.Information about hydraulic and sediment transport processes in steep sand-bed,ephemeral stream is very poor and,though only two flash floods occurred during the field campaign,the data gathered may contribute to improve our knowledge on these kind of rivers.Froude numbers were calculated in order to verify the occurrence of supercritical flow conditions as postulated by a few authors from the analysis of sedimentary structures characteristics.Flow data are also compared to simple models to predict flow velocity and discharge since flow recording systems are seldom installed on ephemeral streams in remote areas and developing countries.Dryland rivers are known for their very high suspended sediment transport,but very little bedload data exist for sand-bed,ephemeral streams.The variation of suspended sediment concentration with discharge is analysed and simple rating curves for both suspended and bed load transport are derived.A few equations to predict bedload are tested against Gereb Oda data and the relationship between en masse bedload transport processes as thin sand sheets and the development of horizontal lamination through the migration of leaf-shaped,sheet-like bedforms is investigated.Individual,large boulders were observed to move at flow depths of the same order of magnitude of their size.A few functions to predict the threshold conditions for large particles entrainment are used to verify if and to what extent they match Gereb Oda field conditions.The increased density of the water-sediment mixture,for the very high suspended sediment concentrations ranging from 100,000 to 200,000 ppm,is considered as well. | Paolo BILLI | 2011 | International Journal of Sediment Research2011,26,2: | 5 |
| 16 | Model-based framework for multi-axial real-time hybrid simulation testing显示文摘Real-time hybrid simulation is an efficient and cost-effective dynamic testing technique for performance evaluation of structural systems subjected to earthquake loading with rate-dependent behavior. A loading assembly with multiple actuators is required to impose realistic boundary conditions on physical specimens. However, such a testing system is expected to exhibit significant dynamic coupling of the actuators and suffer from time lags that are associated with the dynamics of the servo-hydraulic system, as well as control-structure interaction(CSI). One approach to reducing experimental errors considers a multi-input, multi-output(MIMO) controller design, yielding accurate reference tracking and noise rejection. In this paper, a framework for multi-axial real-time hybrid simulation(ma RTHS) testing is presented. The methodology employs a real-time feedback-feedforward controller for multiple actuators commanded in Cartesian coordinates. Kinematic transformations between actuator space and Cartesian space are derived for all six-degrees-offreedom of the moving platform. Then, a frequency domain identification technique is used to develop an accurate MIMO transfer function of the system. Further, a Cartesian-domain model-based feedforward-feedback controller is implemented for time lag compensation and to increase the robustness of the reference tracking for given model uncertainty. The framework is implemented using the 1/5 th-scale Load and Boundary Condition Box(LBCB) located at the University of Illinois at UrbanaChampaign. To demonstrate the efficacy of the proposed methodology, a single-story frame subjected to earthquake loading is tested. One of the columns in the frame is represented physically in the laboratory as a cantilevered steel column. For realtime execution, the numerical substructure, kinematic transformations, and controllers are implemented on a digital signal processor. | 17 | The effects of 12 weeks of functional strength training on muscle strength,volume and activity upon exposure to elevated Gz forces in high-performance aircraft personnel显示文摘Background:Technological advancements in modern military and acrobatic jet planes have resulted in extraordinary psychophysiological loads being exerted upon flying personnel,including inducing neck and back pain.The purpose of this study was to examine the effects of 12 weeks of functional strength training on 1)the volume and strength of the neck and shoulder muscles and 2)muscular activity upon exposure to helmets of different masses and elevated Gz forces in a long-arm centrifuge in high-performance aircraft personnel.Methods:Eighteen participants underwent 12 weeks of functional strength training(n=12)or the control protocol(n=6)without additional strength training.Pre-and post-intervention tests included evaluations of isometric strength of the head extensor muscles,flexion,and lateral flexion and rotation,as well as magnetic resonance imaging(MRI)to measure the volume of the m.sternocleidomastoideus,m.trapezius,and deep neck muscles.Furthermore,during a long-arm centrifuge(+1.4 and+3Gz)protocol,the muscular activity levels of the m.sternocleidomastoideus,m.trapezius and m.erector spinae muscles were assessed without a flight helmet,with a helmet,and with a helmet and night vision goggles.Each participant’s perception of muscular strain was noted immediately after the long-arm centrifuge protocol.Results:The maximal isometric strength in all exercises and muscle volumes increased in the training group but not the control group(P<0.05).Relative muscle activity(%MVC)with a helmet decreased after the intervention in the training but not the control group(P=0.01).Relative muscle activity while wearing a helmet and night vision goggles was higher after intervention in the control group than in the training group(P<0.01).The perceived muscular strain of the neck muscles induced by the long-arm centrifuge did not differ between the groups.Conclusions:Twelve weeks of functional strength training improves the maximal isometric strength and volume of neck and shoulder muscles and leads to lower relative muscle activation upon exposure to elevated Gz forces in a long-arm centrifuge. | Monika Rausch Frank Weber Sven Kühn Carla Ledderhos Christoph Zinner Billy Sperlich | 2021 | Military Medical Research2021,8,4: | 4 |
