| 1 | AFP level and histologic differentiation predict the survival of patients with liver transplantation for hepatocellular carcinoma显示文摘BACKGROUND: In liver transplantation or resection for hepatocellular carcinoma (HCC), patient selection depends on morphological features. In patients with HCC, we performed a clinicopathological analysis of risk factors that affected survival after liver transplantation. METHODS: In 389 liver transplantations performed from 2004 to 2010, 102 were for HCC patients. Data were collected retrospectively from the Organ Transplantation Center Database. Variables were as follows: age, gender, preoperative alpha-fetoprotein (AFP) levels, Child-Pugh and MELD scores, prognostic staging criteria (Milan and UCSF), etiology, number of tumors, the largest tumor size, total tumor size, multifocality, intrahepatic portal vein tumor thrombosis, bilobarity, and histological differentiation. RESULTS: One hundred and two patients were evaluated. The 5-year overall survival rate was 56.5%. According to the UCSF criteria, 63% of the patients were within and 37% were beyond UCSF (P=0.03). Ten patients were excluded (one with fibrolamellary HCC and 9 because of early postoperative death without HCC recurrence), and 92 patients were assessed. The mean age of the patients was 56.5±6.9 years. Sixty-two patients underwent living donor liver transplantations. The mean follow-up time was 29.4±22.6 months. Fifteen patients (16.3%) died in the follow-up period due to HCC recurrence. Univariate analysis showed that AFP level, intrahepatic portal vein tumor thrombosis, histologic differentiation and UCSF criteria were significant factors related to survival and tumor recurrence. The 5-year estimated overall survival rate was 62.2% in allpatients. According to the UCSF criteria, and the 5-year overall survival rate was 66.7% within and 52.7% beyond the criteria (P=0.04). Multivariate analysis showed that AFP level and poor differentiation were independent factors. CONCLUSIONS: For proper patient selection in liver trans- plantation for HCC, prognostic criteria related to tumor biology (especially AFP level and histological differentiation) should be considered. | 2 | Cryptosporidiosis as threatening health problem:A review显示文摘The protozoa under the genus Cryplosporidium is a zoonotic apicomplexan obligate intracellular parasite.Cryptosporidiosis,the term used to designate infection caused by Cryptosporidium sp.,is considered as one of the most common food and waterborne diseases with worldwide spread,acting as a common cause of diarrhoea in animals and man.In immunocompetent individuals,Cryptosporidium typically induces self-limiting diarrhoea,which may resolve on its own after 2-3d.However,cryptosporidiosis may turn life-threatening and subsequently lead to death in small children,the elderly and immunocompromised person,especially in AIDS patient.The diagnosis for Cryptosporidium infection is usually carried out through examination of stool for the presence of oocysts which measured 4-6μm with spherical appearance.Morphometric identification is often difficult because of the diminutive size and obscure internal structure of the protozoa.Often,the identification of Cryptosporidium is realised through the combination of methods incorporating data from morphometries,molecular techniques,and host specificity.However,limitations to some of these techniques still exist whether because of eost,duration,expertise,or reliability.Drugs combination is implemented in treatment of cryptosporidiosis.The efficiency of paromomycin,an aminocyclitol antibiotic isolated from Streptomyces,can be effective when combined use with protease inhibitors or recombinant IL-12.Since there is no drug that achieves the complete removal of Cryptosporidium from the host,supportive therapy was preferred in both human and domestic animals. | Nurul Fariza Rossle Baha Latif | 2013 | Asian Pacific Journal of Tropical Biomedicine2013，3，11： | 3 |
| 3 | Clinical outcomes of a new diffractive multifocal intraocular lens显示文摘AIM:To evaluate clinical outcomes after implantation of a new diffractive aspheric multifocal intraocular lens(IOL) with +3.00 addition power.METHODS:This is a retrospective,consecutive case series of cataract patients who underwent bilateral implantation of the Optiflex MO/HF D012(Moss Vision Inc.Ltd,London,UK) multifocal IOL.Patients followed for 6 mo were included in the study.Data on distance,intermediate and near visual acuity,refractive error [manifest spherical equivalent(MSE)],contrast sensitivity,adverse events,subjective symptoms,spectacle independence and patient satisfaction [visual function questionnaire(VFQ)-25 questionnaire] were retrieved from electronic medical records and analyzed.RESULTS:Forty eyes of 20 patients with a mean age of 66.7±8.5 y(range:53-82) were included in the study.Mean uncorrected distance,near and intermediate visual acuity remained stable through postoperative visits and was 0.19±0.19 log MAR,Jaeger 4 and Jaeger 3 respectively at the 6 mo visit.At the end of postoperative 6 mo,MSE was-0.14±0.42 diopters(D) and 98% of the eyes were within 1.00 D of target refraction.Postoperative low contrast(10%) visual acuity remained stable(P=0.54) through follow up visits with a mean of 0.35±0.17 log MAR at the 6 mo visit.There were no reported adverse events.None of the patients reported subjective symptoms of halo or glare.Spectacle independence rate was 90%.Mean VFQ-25 questionnaire score was 93.5±6.12.CONCLUSION:The Optiflex MO/HF-DO12 IOL was safely implanted and successfully restored distance,intermediate and near visual acuity without impairing contrast sensitivity.High levels of spectacle independence were achieved at all distances including intermediate distance. | Baha Toygar Ozge Yabas Kiziloglu Okan Toygar Ali Murat Hacimustafaoglu | 2017 | International Journal of Ophthalmology(English edition)2017，10，12： | 2 |

| 18 | Cryptosporidiosis among children with diarrhoea in three Asian countries:A review显示文摘This review focuses on studies concerning cryptosporidiosis in three Asian countries.Cryptosporidium spp. infection was investigated in children < 12 years old afflicted with diarrhoea and admitted to the paediatric hospitals in Iraq, Jordan and Malaysia. Most of the patients complained of abdominal pain, watery diarrhoea and mild-to-severe dehydration. Stool samples were collected from children and five methods were used to detect oocysts of Cryptosporidium spp. including: direct wet mount, Sheather's sugar flotation,formalin-ether sedimentation, modified Ziehl–Neelsen and direct fluorescent antibody(DFA). The infection rate was 8.56, 37.3 and 4.6 in Iraq, Jordan and Malaysia, respectively. A combination offormalin ether sedimentation and acid fast stain was used to detect Cryptosporidium oocysts in Iraq. The DFA test showed the highest sensitivity for samples of children in Jordan. In Malaysia, direct wet mount, formalin-ether sedimentation, modified Ziehl–Neelsen and DFA gave the same results(4.62%) while Sheather's sugar flotation was 3.85%. Source of drinking water appeared to be an important risk factor in transmission of infection.
