| 5 | Tai Ji Quan as antihypertensive lifestyle therapy:A systematic review and meta-analysis显示文摘Background:Professional health organizations are not currently recommending Tai Ji Quan alongside aerobic exercise to treat hypertension.We aimed to examine the efficacy of Tai Ji Quan as antihypertensive lifestyle therapy.Methods:Tai Ji Quan interventions published in English and Chinese were included when they involved healthy adults,reported pre-and postintervention blood pressure(BP),and had a non-exercise/non-diet control group.We systematically searched 11 electronic databases for studies published through July 31,2018,yielding 31 qualifying controlled trials.We(1)evaluated the risk of bias and methodological study quality,(2)performed meta-regression analyses following random-effects assumptions,and(3)generated additive models representing the largest possible clinically relevant BP reductions.Results:Participants(n=3223)were middle-aged(56.6±15.1 years of age,mean±SD)adults with prehypertension(systolic BP(SBP)=136.9±15.2 mmHg,diastolic BP(DBP)=83.4±8.7 mmHg).Tai Ji Quan was practiced 4.0±1.4 sessions/week for 54.0§10.6 min/session for 22.3±20.2 weeks.Overall,Tai Ji Quan elicited significant reductions in SBP(-11.3 mmHg,95%CI:-14.6 to-8.0;d+=-0.75)and DBP(-4.8 mmHg,95%CI:-6.4 to-3.1;d+=-0.53)vs.control(p<0.001).Controlling for publication bias among samples with hypertension,Tai Ji Quan trials published in English elicited SBP reductions of 10.4 mmHg and DBP reductions of 4.0 mmHg,which was half the magnitude of trials published in Chinese(SBP reductions of 18.6 mmHg and DBP reductions of 8.8 mmHg).Conclusion:Our results indicate that Tai Ji Quan is a viable antihypertensive lifestyle therapy that produces clinically meaningful BP reductions(i.e.,10.4 mmHg and 4.0 mmHg of SBP and DBP reductions,respectively)among individuals with hypertension.Such magnitude of BP reductions can lower the incidence of cardiovascular disease by up to 40%. | Yin Wu Blair T.Johnson Shiqi Chen Yiyang Chen Jill Livingston Linda S.Pescatello | 2021 | Journal of Sport and Health Science2021,10,2: | 11 |

Selection criteria have been optimized to increase the yield of capsule endoscopy in patients suspected to have Crohn's disease. Capsule endoscopy allows for earlier diagnosis of Crohn's disease of the small bowel and improved diagnosis of colitis in patients where it is unclear if they suffer from Crohn's or ulcerative colitis. A test capsule is available to assess for small bowel strictures and thus avoid capsule retention. A common language has been developed and a new scoring index will be added to capsule software. | 10 | Variable retention harvesting in Victoria’s Mountain Ash (Eucalyptus regnans) forests (southeastern Australia)显示文摘Variable retention harvesting is a silvicultural system that focuses on retaining key elements of stand structure at the time of logging and is increasingly being used worldwide.We describe the design and establishment of a variable retention harvesting experiment established in the Mountain Ash(Eucalyptus regnans)forests of the Central Highlands of Victoria,south-eastern Australia.The experiment was instigated in 2003,and the work to date has shown that it has environmental benefits for certain groups of small mammals,birds,and vascular plants.The experiment has been integrated with an ongoing long-term monitoring program as well as other experiments such as those in post-fire salvage-logged areas.Collectively,the results of various studies suggest that the potential value of variable retention harvesting extends beyond green-tree logging to post-fire salvage logging environments.We outline some of the challenges in,and new perspectives derived from,implementing and maintaining our experiment.This included difficulties protecting islands from high-intensity post-harvest regeneration burns and threat of declining funding undermining ongoing project viability.A critically important perspective concerns the ecological and economic context in which variable retention harvesting is implemented.In the particular case of Mountain Ash forests,assessments using formal IUCN criteria classify the ecosystem as being Critically Endangered under the Red Listed Ecosystem approach.As a result,Mountain Ash forests are at a high risk of ecosystem collapse.Further logging will increase that risk,making the basis for continued harvesting questionable.In addition,economic analyses suggest that the value of natural assets,like water production,far outweigh the value of the wood products harvested from the Mountain Ash ecosystem,again leading to questions about the viability of ongoing harvesting.We therefore conclude that whilst variable retention harvesting has the potential to contribute to biodiversity conservation in Mountain Ash forests,broader ecological and economic contextual issues(such as the values of competing resources like water yields and the heavily degraded state of the forest)may erode the case for its broader application. | David Lindenmayer David Blair Lachlan McBurney | 2019 | Ecological Processes2019,8,1: | 5 |

| 12 | An integrative Tai Chi program for patients with breast cancer undergoing cancer therapy:study protocol for a randomized controlled feasibility study显示文摘Background:Breast cancer patients experience various side effects during cancer therapy, often resulting in reduced quality of life and poor adherence to treatment. A limited range of proven interventions has been developed to target such side effects. While Tai Chi offers benefits for the health and well-being of breast cancer survivors, the effectiveness of Tai Chi across the treatment continuum has not been evaluated. Improved patient education and support has been suggested as a priority for breast cancer care. This pilot study assesses the feasibility of a randomized controlled trial(RCT) to evaluate the effectiveness of‘‘an integrative Tai Chi'(ANITA) program for breast cancer patients undergoing cancer therapy.Methods/design: This is a single-centre, two-arm feasibility RCT. Twenty-four patients with breast cancer who have undergone surgical treatment will be recruited from the Dunedin Hospital(New Zealand) over a 12-month period(from August 2017 to July 2018). Subject to informed consent, patients will be randomized to receive standard cancer treatment alone or standard cancer treatment plus the ANITA program, consisting of peer support, health education, and Tai Chi Ruler exercise. The program runs alongside the patient's adjuvant cancer therapy, which may include chemotherapy, radiation therapy,antibody treatment, and/or antihormonal therapy. Analysis in this study will focus on process evaluation of participant recruitment, retention, treatment fidelity, acceptability of the program, and occurrence of adverse events. We present new geologic and structural maps, geochemical plots, geo- and thermo-chronology, and geophysical data from seven silicic, mafic to ultramafic complexes separated by major shear systems across the southern Makhonjwa Mountains. All reveal signs of modern oceanic back-arc crust and subductionrelated processes. We compare the rates of processes determined from this data and balance these against plate tectonic and plume related models. Robust rates of both horizontal and vertical tectonic processes derived from the Makhonjwa Mountain complexes are similar, well within an order of magnitude, to those encountered across modern oceanic and orogenic terrains flanking Western Pacific-like subduction zones. We conclude that plate tectonics and linked plate-boundary processes were well established by 3.2-3.6 Ga. | 18 | Laparoscopic resection of pancreatic adenocarcinoma: Dream or reality?显示文摘Laparoscopic pancreatic surgery is in its infancy despite initial procedures reported two decades ago. Both laparoscopic distal pancreatectomy(LDP) and laparoscopic pancreaticoduodenectomy(LPD) can be performed competently; however when minimally invasive surgical(MIS) approaches are implemented the indication is often benign or low-grade malignant pathologies. Nonetheless, LDP and LPD afford improved perioperative outcomes, similar to those observed when MIS is utilized for other purposes. This includes decreased blood loss, shorter length of hospital stay, reduced post-operative pain, and expedited time to functional recovery. What then is its role for resection of pancreatic adenocarcinoma? The biology of this aggressive cancer and the inherent challenge of pancreatic surgery have slowed MIS progress in this field. In general, the overall quality of evidence is low with a lack of randomized control trials, a preponderance of uncontrolled series, short follow-up intervals, and small sample sizes in the studies available. Available evidence compiles heterogeneous pathologic diagnoses and is limited by case-by-case follow-up, which makes extrapolation of results difficult. Nonetheless, shortterm surrogate markers of oncologic success, such as margin status and lymph node harvest, are comparable to open procedures. Unfortunately disease recurrence and long-term survival data are lacking. | 20 | The next detectors for gravitational wave astronomy显示文摘This paper focuses on the next detectors for gravitational wave astronomy which will be required after the current ground based detectors have completed their initial observations, and probably achieved the first direct detection of gravitational waves. The next detectors will need to have greater sensitivity, while also enabling the world array of detectors to have improved angular resolution to allow localisation of signal sources. Sect. 1 of this paper begins by reviewing proposals for the next ground based detectors,and presents an analysis of the sensitivity of an 8 km armlength detector, which is proposed as a safe and cost-effective means to attain a 4-fold improvement in sensitivity. The scientific benefits of creating a pair of such detectors in China and Australia is emphasised. Sect. 2 of this paper discusses the high performance suspension systems for test masses that will be an essential component for future detectors, while sect. 3 discusses solutions to the problem of Newtonian noise which arise from fluctuations in gravity gradient forces acting on test masses. Such gravitational perturbations cannot be shielded, and set limits to low frequency sensitivity unless measured and suppressed. Sects. 4 and 5 address critical operational technologies that will be ongoing issues in future detectors. Sect. 4 addresses the design of thermal compensation systems needed in all high optical power interferometers operating at room temperature. Parametric instability control is addressed in sect. 5. Only recently proven to occur in Advanced LIGO, parametric instability phenomenon brings both risks and opportunities for future detectors. The path to future enhancements of detectors will come from quantum measurement technologies.
