1无创筛查酒精相关性肝纤维化的成本效果分析显示文摘【据《Hepatology》2019年10月报道】题:无创筛查酒精相关性肝纤维化的成本效果分析(作者Asphaug L等)酒精相关性肝病常常难以被发现,直到终末期出现肝功能失代偿表现时才被引起注意。该研究旨在分析针对进展期酒精相关性肝纤维化的经济、高效的无创筛查方法的成本效果。来自挪威的Asphaug等进行了一项现实世界数据的研究,作者从丹麦多中心招募了具有肝活检病理结果的大量饮酒者,评价一级诊疗中心采用的4种筛查策略:(1)传统的肝功能化验,如果结果阳性则进一步完善腹部超声;(2)肝纤维化血清学标志物:强化的肝纤维化(ELF)试验,如果结果阳性则进一步完善肝硬度(LSM)检查;(3)三级策略:在进行第2种策略前进行Forns索引(根据年龄、血小板计数、GGT、胆固醇水平得到的无创预测公式);(4)直接进行LSM检查。应用决策树和Markov联合模型进一步评价短期结果(每项准确诊断的成本)和长期结果[质量调整生命年(QALYs)]。周胜云 段志辉 ASPHAUG L THIELE M KRAG A 2020临床肝胆病杂志2020,36,1:2
