| 2 | Risk factors for fever and sepsis after percutaneous nephrolithotomy显示文摘Objective:Percutaneous nephrolithotomy(PCNL)is commonly used in the management of large renal stones.Postoperative infections are one of the most common complications of this procedure.The present study is to determine and assess the factors that may increase the risk to develop fever and urinary sepsis after PCNL.Methods:A total of 60 patients(38 males and 22 females)with a mean age of 40.25 years enrolled in this study in Sulaimania Teaching Hospital.Patients had renal stone disease need operation with different socioeconomic status,body mass index and different type and size of stones were included in this study.Patients with preoperative positive urine culture and sensitivity were excluded.Preoperative investigations done for all patients.All Patients received prophylactic antibiotic gentamicin intravenously at the induction of anaesthesia.Renal pelvis urine sample were taken from all patients after puncturing the pelvicalyceal system and send for culture and sensitivity.Patients were monitored closely in the postoperative period for the development of fever and sepsis.Results:Mean duration of the operations was 77.08 min ranged 40e120 min.All patients had postoperative nephrostomy tube.Seventeen(28.33%)patients developed post PCNL fever and the statistically significant factors for post PCNL fever were diabetes mellitus(DM)(p Z 0.001),stone burden(p Z 0.001),number of the stones(p<0.001),degree of hydronephrosis(p Z 0.001),duration of the operation(p<0.001),residual stones(p Z 0.001)and number of tracts(p Z 0.038).Three(5.00%)patients developed post PCNL sepsis,and the statistically significant risk factors for post PCNL sepsis were duration of the operation(p Z 0.013)and intraoperative blood loss,postoperative drop in haemoglobin(HB)level(p Z 0.046).Conclusion:DM,staghorn stones,degree of hydronephrosis,duration of the operation and number of tracts are risk factors for post PCNL fever,while number of stones,intraoperative blood loss,duration of the operation and residual stones are risk factors for post PCNL sepsis. | Aso Omer Rashid Saman Salih Fakhulddin | 2016 | Asian Journal of Urology2016，3，2： | 21 |
| 3 | Molecular Cloning and Functional Characterization of Porcine MyD88 Essential for TLR Signaling显示文摘We isolated cDNA encoding porcine MyD88 (poMyD88) from Peyer's patches (Pps) of GALT. The complete open reading frame (ORF) of poMyD88 contains 879 bp encoding a deduced 293 aa residues. The amino acid sequence of poMyD88 was characterized by N-terminal death,intermediate and C-terminal Toll/IL-1 receptor (TIR) domains. The putative poMyD88 protein shares a higher level of homology with its human (87.2% amino acid identity) than with its mouse (77.4% amino acid identity) counterpart. Overexpression of poMyD88 participated in the further enhanced activation of NF-κB in human embryonic kidney (HEK) 293 cells expressing porcine TLR2 and porcine TLR4/MD-2,but not porcine RP105/MD-1 after stimulation with the corresponding ligands. The expression levels of MyD88 were highest in the spleen and mesenteric lymph nodes (MLNs),and lower in digestive tissues of newborn swine. In adult swine,the expression levels in the digestive tissues were lower than those in MLNs and the spleen. | 4 | Hepatic schwannoma:Imaging findings on CT,MRI and contrast-enhanced ultrasonography显示文摘A primary benign schwannoma of the liver is extremely rare and is difficult to preoperatively discriminate from a malignant tumor.We compared the imaging and pathological findings,and examined the possibility of preoperatively diagnosing a benign liver schwannoma.A 72-year-old woman was admitted to our hospital because of a 4.6-cm mass in the liver.A malignant tumor was suspected,and a right hepatectomy was performed.After this,the diagnosis of a primary benign schwannoma of the liver was made through pathological examination.Contrast-enhanced ultrasonography(CEUS) with Sonazoid showed minute blood flows into the septum and solid areas of the tumor in the vascular phase;most likely due to increased arterial flow associated with infiltration of chronic inflammatory cells.In the postvascular phase,CEUS showed contrast defect of cystic areas and delayed enhancement of solid areas;most likely due to aggregation of siderophores.Because discriminating between a benign and malignant schwannoma of the liver is difficult,surgery is generally recommended.However,the two key findings from CEUS may be useful in discriminating ancient schwannoma by recognizing the hemorrhage involved in the secondary degeneration and aggregation of siderophores. | Yu Ota Kazunobu Aso Kenji Watanabe Takahiro Einama Koji Imai Hidenori Karasaki Ryuji Sudo Yosui Tamaki Mituyoshi Okada Yosihiko Tokusashi Toru Kono Naoyuki Miyokawa Masakazu Haneda Masahiko Taniguchi Hiroyuki Furukawa | 2012 | World Journal of Gastroenterology2012，18，35： | 11 |
| 5 | Inhibitory effect of angiotensinⅡreceptor antagonist on hepatic stellate cell activation in non-alcoholic steatohepatitis显示文摘AIM: To investigate the efficacy of angiotensinⅡreceptor antagonist on hepatic stellate cells (HSCs) activation in the patients with non-alcoholic steatohepatitis (NASH). METHODS: Seven patients with NASH were prescribed losartan, a selective angiotensinⅡtype 1 receptor antagonist (50 mg/d) for 48 wk. Liver biopsies were performed both at the entry and end of the study in all patients. Quiescent and activated HSCs were identified by double immunostaining using anti-p75 andα-smooth muscle actin antibodies, and the number of each phenotype was counted. Similarly, the liver specimens obtained from the eight patients with non-alcoholic fatty liver (NAFL) were also examined as controls. RESULTS: In NASH hepatic tissues, activated HSCs were dominantly distributed as compared with those in NAFL. The 48-wk losartan treatment induced a remarkable decrease in activated HSCs and a mild increase in quiescent phenotypes. | 6 | Lactogenic hormones alter cellular and extracellular microRNA expression in bovine mammary epithelial cell culture显示文摘Background: Bovine milk contains not only a variety of nutritional ingredients but also micro RNAs(mi RNAs) that are thought to be secreted by the bovine mammary epithelial cells(BMECs). The objective of this study was to elucidate the production of milk-related mi RNAs in BMECs under the influence of lactogenic hormones.Results: According to a microarray result of milk exosomal mi RNAs prior to cellular analyses, a total of 257 mi RNAs were detected in a Holstein cow milk. Of these, 18 major mi RNAs of interest in the milk were selected for an expression analysis in BMEC culture that was treated with or without dexamethasone, insulin, and prolactin(DIP) to induce a lactogenic differentiation. Quantitative polymerase chain reaction(q PCR) results showed that the expressions of mi R-21–5p(P = 0.005), mi R-26a(P = 0.016), and mi R-320a(P = 0.011) were lower in the DIP-treated cells than in the untreated cells. In contrast, the expression of mi R-339a(P = 0.017) in the cell culture medium were lower in the DIP-treated culture than in the untreated culture. Intriguingly, the mi R-148 a expression in cell culture medium was elevated by DIP treatment of BMEC culture(P = 0.018). The medium-to-cell expression ratios of mi R-103(P = 0.025), mi R-148a(P < 0.001), and mi R-223(P = 0.013) were elevated in the DIP-treated BMECs, suggesting that the lactogenic differentiation-induced secretion of these three mi RNAs in BMECs. A bioinformatic analysis showed that the mi RNAs down-regulated in the BMECs were associated with the suppression of genes related to transcriptional regulation, protein phosphorylation, and tube development.Conclusion: The results suggest that the mi RNAs changed by lactogenic hormones are associated with milk protein synthesis, and mammary gland development and maturation. | 9 | Endoscopic mucosal resection vs endoscopic submucosal dissection for superficial non-ampullary duodenal tumors显示文摘BACKGROUND The selection of endoscopic treatments for superficial non-ampullary duodenal epithelial tumors(SNADETs)is controversial.AIM To compare the efficacy and safety of endoscopic mucosal resection(EMR)and endoscopic submucosal dissection(ESD)for SNADETs.METHODS We retrospectively analyzed the data of patients with SNADETs from a database of endoscopic treatment for SNADETs,which included eight hospitals in Fukuoka,Japan,between April 2001 and October 2017.A total of 142 patients with SNADETs treated with EMR or ESD were analyzed.Propensity score matching was performed to adjust for the differences in the patient characteristics between the two groups.We analyzed the treatment outcomes,including the rates of en bloc/complete resection,procedure time,adverse event rate,hospital stay,and local or metastatic recurrence.RESULTS Twenty-eight pairs of patients were created.The characteristics of patients between the two groups were similar after matching.The EMR group had a significantly shorter procedure time and hospital stay than those of the ESD group[median procedure time(interquartile range):6(3-10.75)min vs 87.5(68.5-136.5)min,P<0.001,hospital stay:8(6-10.75)d vs 11(8.25-14.75)d,P=0.006].Other outcomes were not significantly different between the two groups(en bloc resection rate:82.1%vs 92.9%,P=0.42;complete resection rate:71.4%vs 89.3%,P=0.18;and adverse event rate:3.6%vs 17.9%,P=0.19,local recurrence rate:3.6%vs 0%,P=1;metastatic recurrence rate:0%in both).Only one patient in the ESD group underwent emergency surgery owing to intraoperative perforation.CONCLUSION EMR has significantly shorter procedure time and hospital stay than ESD,and provides acceptable curability and safety compared to ESD.Accordingly,EMR for SNADETs is associated with lower medical costs. | Mitsuru Esaki Kazuhiro Haraguchi Kazuya Akahoshi Naru Tomoeda Akira Aso Soichi Itaba Haruei Ogino Yusuke Kitagawa Hiroyuki Fujii Kazuhiko Nakamura Masaru Kubokawa Naohiko Harada Yosuke Minoda Sho Suzuki Eikichi Ihara Yoshihiro Ogawa | 2020 | World Journal of Gastrointestinal Oncology2020，12，8： | 2 |
