11Effects of chronic and subtoxic chlorobenzenes on adrenocorticotrophic hormone release显示文摘Many environmental chemicals and pesticides have been found to alter neuroendocrine communication in exposed biological objects. The environmental loads have primary and secondary effects that can alter the homeostatic regulation potential. Since it is difficult to avoid human exposition, a potentially important area of research to develop in vivo and in vitro experimental models. In this context, the primary aim of this study was to demonstrate the effects of chlorobenzenes on adrenocorticotrophic hormone(ACTH) release. In our experimental study, male Wistar rats were exposed to 0.1, 1.0 and 10 μg/b.w.(body weight) kg of 1,2,4-trichlorobenzene and hexachlorobenzene(Cl B) mix via gastric tube for 30, 60 or 90 days. At the endpoints of the experiment blood samples were taken and animals were decapitated. Primary,monolayer adenohypophysis cell cultures were prepared by enzymatic and mechanical digestion. The ACTH hormone content in serum and supernatant media was measured by immuno-chemiluminescence assay. The Mg2+-dependent ATPase activity was determined by modified method of Martin and Dotty. Significant differences were detected in the hormone release between the control and treated groups. The hormone release was enhanced characteristically in exposed groups depending upon the dose and duration of exposure. The Mg2+-ATPase activity enhanced after chronic and subtoxic Cl B exposition. Light microscopy revealed that the adenohypophysis seemed to be more abundant. 12Long-term outcomes after endoscopic removal of malignant colorectal polyps:Results from a 10-year cohort显示文摘BACKGROUND Choosing an optimal post-polypectomy management strategy of malignant colorectal polyps is challenging,and evidence regarding a surveillance-only strategy is limited.AIM To evaluate long-term outcomes after endoscopic removal of malignant colorectal polyps.METHODS A single-center retrospective cohort study was conducted to evaluate outcomes after endoscopic removal of malignant colorectal polyps between 2010 and 2020.Residual disease rate and nodal metastases after secondary surgery and local and distant recurrence rate for those with at least 1 year of follow-up were invest-igated.Event rates for categorical variables and means for continuous variables with 95%confidence intervals were calculated,and Fisher’s exact test and Mann-Whitney test were performed.Potential risk factors of adverse outcomes were RESULTS In total,135 lesions(mean size:22.1 mm;location:42%rectal)from 129 patients(mean age:67.7 years;56%male)were enrolled.The proportion of pedunculated and non-pedunculated lesions was similar,with en bloc resection in 82%and 47%of lesions,respectively.Tumor differentiation,distance from resection margins,depth of submucosal invasion,lymphovascular invasion,and budding were reported at 89.6%,45.2%,58.5%,31.9%,and 25.2%,respectively.Residual tumor was found in 10 patients,and nodal metastasis was found in 4 of 41 patients who underwent secondary surgical resection.Univariate analysis identified piecemeal resection as a risk factor for residual malignancy(odds ratio:1.74;P=0.042).At least 1 year of follow-up was available for 117 lesions from 111 patients(mean follow-up period:5.59 years).Overall,54%,30%,30%,11%,and 16%of patients presented at the 1-year,3-year,5-year,7-year,and 9-10-year surveillance examinations.Adverse outcomes occurred in 9.0%(local recurrence and dissemination in 4 patients and 9 patients,respectively),with no difference between patients undergoing secondary surgery and surveillance only.CONCLUSION Reporting of histological features and adherence to surveillance colonoscopy needs improvement.Long-term adverse outcome rates might be higher than previously reported,irrespective of whether secondary surgery was performed.Anna Fábián Renáta Bor Béla Vasas Mónika Szűcs Tibor Tóth Zsófia Bősze Kata Judit Szántó Péter Bacsur Anita Bálint Bernadett Farkas Klaudia Farkas Ágnes Milassin Mariann Rutka Tamás Resál Tamás Molnár Zoltán Szepes 2024World Journal of Gastrointestinal Endoscopy2024,16,4:0
13Immunosuppressive treatment and radiotherapy in kidney transplant patients:A systematic review显示文摘BACKGROUND Immunosuppression(IS)therapy may contribute to cancer development.Some authors have proposed to reduce immunosuppression drugs dose in case of viral infections,in immunosuppression-related diseases,and in patients undergoing radiotherapy.The present analysis reports the results of a systematic review on kidney transplant recipients undergoing immunosuppression and radiotherapy.AIM To define if it is necessary reduce immunosuppression drugs during radiotherapy.METHODS The literature search was based on three electronic databases(Pubmed,Scopus,and Web of Science)using selected keywords linked through the'AND'and'OR'Boolean operators to build specific strings for each electronic search engine.Two researchers independently screened the citations,and disagreement was resolved by discussion or through the intervention of a third author.The review was conducted and reported according to the PRISMA statement.Extracted data were narratively synthesized,and,where possible,frequencies,percentages,and ranges were calculated.RESULTS The literature search resulted in 147 citations.After abstracts screening,21 records were selected for full-text evaluation.Fifteen of these were excluded,leaving six papers considered suitable for analysis.There is still no clear evidence that withdrawing antimetabolites and/or calcineurin inhibitors and/or mammalian target of rapamycin-inhibitors,as opposed to continuing maintenance IS,improves patient survival in kidney transplant recipients with cancer undergoing radiotherapy.Only few retrospective studies on small cancer patient cohorts are available in this setting,but without comparison of different immunosuppression treatments.Even where immunosuppression therapy was described,patient survival seemed to be correlated only with cancer stage and type.CONCLUSION The results of this systematic review do not support the reduction of immunosuppression dose in patients undergoing radiotherapy.Valentina Lancellotta Andrea D'Aviero Bruno Fionda Calogero Casà Ilaria Esposito Francesco Preziosi Anna Acampora Fabio Marazzi György Kovács Barbara Alicja Jereczek-Fossa Alessio Giuseppe Morganti Vincenzo Valentini Maria Antonietta Gambacorta Jacopo Romagnoli Luca Tagliaferri 2022World Journal of Radiology2022,14,3:0
14Predictive biomarkers of immunotherapy response with pharmacological applications in solid tumors显示文摘Immune-checkpoint inhibitors show promising effects in the treatment of multiple tumor types.Biomarkers are biological indicators used to select patients for a systemic anticancer treatment,but there are only a few clinically useful biomarkers such as PD-L1 expression and tumor mutational burden,which can be used to predict immunotherapy response.In this study,we established a database consisting of both gene expression and clinical data to identify biomarkers of response to anti-PD-1,anti-PD-L1,and anti-CTLA-4 immunotherapies.A GEO screening was executed to identify datasets with simultaneously available clinical response and transcriptomic data regardless of cancer type.The screening was restricted to the studies involving administration of anti-PD-1(nivolumab,pembrolizumab),anti-PD-L1(atezolizumab,durvalumab)or anti-CTLA-4(ipilimumab)agents.Receiver operating characteristic(ROC)analysis and Mann-Whitney test were executed across all genes to identify features related to therapy response.The database consisted of 1434 tumor tissue samples from 19 datasets with esophageal,gastric,head and neck,lung,and urothelial cancers,plus melanoma.The strongest druggable gene candidates linked to anti-PD-1 resistance were SPIN1(AUC=0.682,P=9.1E-12),SRC(AUC=0.667,P=5.9E-10),SETD7(AUC=0.663,P=1.0E-09),FGFR3(AUC=0.657,P=3.7E-09),YAP1(AUC=0.655,P=6.0E-09),TEAD3(AUC=0.649,P=4.1E-08)and BCL2(AUC=0.634,P=9.7E-08).In the anti-CTLA-4 treatment cohort,BLCAP(AUC=0.735,P=2.1E-06)was the most promising gene candidate.No therapeutically relevant target was found to be predictive in the anti-PD-L1 cohort.In the anti-PD-1 group,we were able to confirm the significant correlation with survival for the mismatch-repair genes MLH1 and MSH6.A web platform for further analysis and validation of new biomarker candidates was set up and available at http://gffzz1931668773ea4c46snkxbf6vq9p906u5q.ffgz.tsg.suse.edu.cn/immune.In summary,a database and a web platform were established to investigate biomarkers of immunotherapy response in a large cohort of solid tumor samples.Our results could help to identify new patient cohorts eligible for immunotherapy.Szonja Anna Kovács János Tibor Fekete Balázs Győrffy 2023Acta Pharmacologica Sinica2023,44,9:0
