1Intussusception of the bowel in adults:A review显示文摘Intussusception of the bowel is defined as the telescop-ing of a proximal segment of the gastrointestinal tract within the lumen of the adjacent segment.This condi-tion is frequent in children and presents with the classic triad of cramping abdominal pain,bloody diarrhea and a palpable tender mass.However,bowel intussusception in adults is considered a rare condition,accounting for 5% of all cases of intussusceptions and almost 1%-5%of bowel obstruction.Eight to twenty percent of cases are idiopathic,without a lead point lesion.Secondary intus-susception is caused by organic lesions,such as inflam-matory bowel disease,postoperative adhesions,Meckel's diverticulum,benign and malignant lesions,metastatic neoplasms or even iatrogenically,due to the presence of intestinal tubes,jejunostomy feeding tubes or after gas-tric surgery.Computed tomography is the most sensitive diagnostic modality and can distinguish between intus-susceptions with and without a lead point.Surgery is the definitive treatment of adult intussusceptions.Formal bowel resection with oncological principles is followed for every case where a malignancy is suspected.Reduction of the intussuscepted bowel is considered safe for benign lesions in order to limit the extent of resection or to avoid the short bowel syndrome in certain circumstances.Athanasios Marinis Anneza Yiallourou Lazaros Samanides Nikolaos Dafnios Georgios Anastasopoulos Ioannis Vassiliou Theodosios Theodosopoulos 2009World Journal of Gastroenterology2009,15,4:31
2Cardiovascular disease: Risk factors and applicability of a risk model in a Greek cohort of renal transplant recipients显示文摘AIM To investigate the incidence and the determinants of cardiovascular morbidity in Greek renal transplant recipients(RTRs) expressed as major advance cardiac event(MACE) rate. METHODS Two hundred and forty-two adult patients with a functioning graft for at least three months and availabledata that were followed up on the August 31, 2015 at two transplant centers of Western Greece were included in this study. Baseline recipients' data elements included demographics, clinical characteristics, history of comorbid conditions and laboratory parameters. Follow-up data regarding MACE occurrence were collected retrospectively from the patients' records and MACE risk score was calculated for each patient. RESULTS The mean age was 53 years(63.6% males) and 47 patients(19.4%) had a pre-existing cardiovascular disease(CVD) before transplantation. The mean estimated glomerular filtration rate was 52 ± 17 mL /min per 1.73 m2. During follow-up 36 patients(14.9%) suffered a MACE with a median time to MACE 5 years(interquartile range: 2.2-10 years). Recipients with a MACE compared to recipients without a MACE had a significantly higher mean age(59 years vs 52 years, P < 0.001) and a higher prevalence of pre-existing CVD(44.4% vs 15%, P < 0.001). The 7-year predicted mean risk for MACE was 14.6% ± 12.5% overall. In RTRs who experienced a MACE, the predicted risk was 22.3% ± 17.1% and was significantly higher than in RTRs without an event 13.3% ± 11.1%(P = 0.003). 3Managing injuries of hepatic duct confluence variants after major hepatobiliary surgery:An algorithmic approach显示文摘AIM:To investigate injuries of anatomy variants of hepatic duct confluence during hepatobiliary surgery and their impact on morbidity and mortality of these procedures. An algorithmic approach for the management of these injuries is proposed. METHODS:During a 6-year period 234 patients who had undergone major hepatobiliary surgery were retrospectively reviewed in order to study postoperative bile leakage. Diagnostic workup included endoscopic and magnetic retrograde cholangiopancreatography (E/MRCP), scintigraphy and fistulography. RESULTS:Thirty (12.8%) patients who developed postoperative bile leaks were identified. Endoscopic stenting and percutaneous drainage were successful in 23 patients with bile leaks from the liver cut surface. In the rest seven patients with injuries of hepatic duct confluence, biliary variations were recognized and a stepwise therapeutic approach was considered. Conservative management was successful only in 2 patients. Volume of the liver remnant and functional liver reserve as well as local sepsis were used as criteria for either resection of the corresponding liver segment or construction of a biliary-enteric anastomosis. Two deaths occurred in this group of patients with hepatic duct confluence variants (mortality rate 28.5%). CONCLUSION:Management of major biliary fistulaethat are disconnected from the mainstream of the biliary tree and related to injury of variants of the hepatic duct confluence is extremely challenging. 4Simplified approach for design of raft foundations against fault rupture.Part II:soil-structure interaction显示文摘This is the second paper of two, which describe the results of an integrated research effort to develop a four-step simplified approach for design of raft foundations against dip-slip (normal and thrust) fault rupture. The first two steps dealing with fault rupture propagation in the free-field were presented in the companion paper. This paper develops an approximate analytical method to analyze soil-foundation-structure interaction (SFSI), involving two additional phenomena: (i) fault rupture diversion (Step 3); and (ii) modification of the vertical displacement profile (Step 4). For the first phenomenon (Step 3), an approximate energy-based approach is developed to estimate the diversion of a fault rupture due to presence of a raft foundation. The normalized critical load for complete diversion is shown to be a function of soil strength, coefficient of earth pressure at rest, bedrock depth, and the horizontal position of the foundation relative to the outcropping fault rupture. For the second phenomenon (Step 4), a heuristic approach is proposed, which 'scans' through possible equilibrium positions to detect the one that best satisfies force and moment equilibrium. Thus, we account for the strong geometric nonlinearities that govern this interaction, such as uplifting and second order (P-Δ) effects. Comparisons with centrifuge-validated finite element analyses demonstrate the efficacy of the method. Its simplicity makes possible its utilization for preliminary design.I. Anastasopoulos N. 6Simplified approach for design of raft foundations against fault rupture.Part I:free-field显示文摘Over the past few decades, earthquake engineering research mainly focused on the effects of strong seismic shaking. After the 1999 earthquakes in Turkey and Taiwan, and thanks to numerous cases where fault rupture caused substantial damage to structures, the importance of faulting-induced deformation has re-emerged. This paper, along with its companion (Part II), exploits parametric results of finite element analyses and centrifuge model testing in developing a four-step semi-analytical approach for analysis of dip-slip (normal and thrust) fault rupture propagation through sand, its emergence on the ground surface, and its interaction with raft foundations. The present paper (Part I) focuses on the effects of faulting in the absence of a structure (i.e., in the free-field). The semi-analytical approach comprises two-steps: the first deals with the rupture path and the estimation of the location of fault outcropping, and the second with the tectonically-induced displacement profile at the ground surface. In both cases, simple mechanical analogues are used to derive simplified semi-analytical expressions. Centrifuge model test data, in combination with parametric results from nonlinear finite element analyses, are utilized for model calibration. The derived semi-analytical expressions are shown to compare reasonably well with more rigorous experimental and theoretical data, thus providing a useful tool for a first estimation of near-fault seismic hazard.I.
