1Comparative evaluation of microscopy,OptiMAL® and 18S rRNA gene based multiplex PCR for detection of Plasmodium falciparum & Plasmodium vivax from field isolates of Bikaner,India显示文摘Objective:To evaluate microscopy,OptiMAL? and multiplex PCR for the identification of Plasmodium falciparum(P.falciparum) and Plasmodium vivax(P.vivax) from the field isolates of Bikaner,Rajasthan(Northwest India).Methods:In this study,a multiplex PCR(P.falciparum and P.vivax) was further developed with the incorporation of Plasmodium malariae(P.malariae) specific primer and also a positive control.The performance of microscopy,plasmodium lactate dehydrogenase(pLDH) based malaria rapid diagnostic test OptiMAL? and 18S rRNA gene based multiplex PCR for the diagnosis of P.falciparum and P.vivax was compared.Results:The three species multiplex PCR if.falciparum,P.vivax and P.malariae) with an inbuilt positive control was developed and evaluated.In comparison with multiplex PCR,which showed the sensitivity and specificity of 99.36%(95%CI,98.11%-100.00%) and 100.00%(95%CI,100.00%-100.00%),the sensitivity and specificity of microscopy was 90.44%(95%CI,88.849-95.04%) and 99.22%(95% CI,97.71%-100.00%),and OptiMAL? was 93.58%(95%CI,89.75%-97.42%) and 97.69%(95%CI, 95.10%-100.00%).The efficiencies were 99.65%,95.10%and 95.45%for multiplex PCK.microscopy and OptiMAL?.respectively.Conclusions:Our results raise concerns over the overall sensitivities of microscopy and OptiMAL?,when compared to the multiplex PCR and thus stress the need for new molecular interventions in the accurate detection of the malarial parasites.This further highlights the fact that further developments are needed to improve the performance of rapid diagnostic tests at field level.Deepak Pakalapati Shilpi Garg Sheetal Middha Abhishek Kochar Amit Kumar Subudhi Boopathi Pon Arunachalam Sanjay Kumar Kochar Vishal Saxena Pareek RP Jyoti Acharya Dhanpat Kumar Kochar Ashis Das 2013Asian Pacific Journal of Tropical Medicine2013,6,5:3
2A novel ZnO/reduced graphene oxide and Prussian blue modified carbon paste electrode for the sensitive determination of Rutin显示文摘A carbon paste modified sensor based on a novel composite of zinc oxide nanoparticles deposited on reduced graphene oxide(ZnO-rGrO) and Prussian blue(PB) was drop-cast(ZnO-rGrO-PB/MCPE) for the sensitive estimation of Rutin(Rtn) at pH 7.0.The high surface area of ZnO-rGrO and electrocatalytic property of PB promotes the oxidation of Rtn. Field emission scanning electron microscope(FE-SEM) and energy-dispersive X-ray spectroscopy(EDX) techniques were employed to confirm the deposition of ZnO-rGrO and PB on carbon paste electrode(CPE). The ability of ZnO-rGrO-PB/MCPE in charge transfer at the interface was investigated using electrochemical impedance spectroscopy(EIS). The heterogeneous rate constant(ks) and the charge transfer coefficient(α) have been calculated as 6.08 s^(-1) and 0.74 respectively. This sensor showed a wide linear response for Rtn from 7.0×10^(-8)to 7.0×10^(-6) M and 7.0×10^(-6) to 1.0×10^(-4) M with a limit of detection(2.05±0.04)×10^(-8) M(S/N=3). The application of ZnO-rGrO-PB/MCPE was found in the analysis of Rtn in fruit juice samples using standard addition method. 4Fabrication of the electrochemically reduced graphene oxide-bismuth nanoparticles composite and its analytical application for an anticancer drug gemcitabine显示文摘The present study explores an electroreduced graphene oxide-bismuth nanoparticles composite(ErGOBi) as an electrochemical sensor for the determination of an anticancer drug, gemcitabine hydrochloride(GMB). The Er-GOBi interface was prepared by drop casting of bismuth nitrate-graphene oxide suspension on a glassy carbon electrode(GCE) followed by electro-reduction in the potential range of 0.6 V to 1.7 V. SEM, FTIR, EDAX and AFM techniques were employed for the characterization of prepared materials. Cyclic voltammetric and electrochemical impedance spectroscopic methods were used to understand the charge transfer properties of stepwise modification of Er-GOBi/GCE. GMB exhibited an irreversible oxidation peak at 1.144 V on Er-GOBi/GCE in phosphate buffer of p H 3. A 100-fold enhanced oxidation peak current was observed at Er-GOBi/GCE when compared to that at bare GCE.Sensing performance of Er GO-Bi/GCE was optimized by varying peak current dependent parameters.Linear relationship between the peak current and concentration of GMB was observed in the range of 0.1–51.1 mmol/L in differential pulse voltammetric method and 2.1–61.1 mmol/L in linear sweep voltammetric method. 5Intranasal ketamine for the treatment of patients with acute pain in the emergency department显示文摘BACKGROUND:Pain in the emergency department(ED)is common but undertreated.The objective of this study was to examine the efficacy and safety of intranasal(IN)ketamine used as an analgesic for patients with acute injury with moderate to severe pain.METHODS:This study was a cross sectional,observational study of patients more than 8 years old experiencing moderate to severe pain[visual analog score(VAS)>50 mm].The initial dose of IN ketamine was 0.7 mg/kg with an additional dose of 0.3 mg/kg if VAS was more than 50 mm after 15minutes.Pain scores and vital signs were recorded at 0,15,30 and 60 minutes.Side-effects,sedation level and patient's satisfaction were also recorded.The primary outcome was the number of patients achieving≥20 mm reductions in VAS at 15 minutes.Other secondary outcome measures were median reduction in VAS at 15,30 and 60 minutes,changes of vital signs,adverse events,satisfaction of patients,and need for additional ketamine.RESULTS:Thirty-four patients with a median age of 29.5 years(IQR 17.5–38)were enrolled,and they had an initial median VAS of 80 mm(IQR 67–90).The VAS decreased more than 20 mm at15 minutes in 27(80%)patients.The reduction of VAS from baseline to 40 mm(IQR 20–40),20 mm(IQR 14–20)and 20 mm(IQR 10–20)respectively at 15,30 and 60 minutes(P<0.001).No critical changes of vital signs were noted and adverse effects were mild and transient.CONCLUSION:This study showed that IN ketamine is an analgesic choice for patients with acute injury in moderate to severe pain in an overcrowded and resource limited ED.Roshana Shrestha Samita Pant Ashis Shrestha Kabita Hada Batajoo Rashmi Thapa Sumana Vaidya 2016World Journal of Emergency Medicine2016,7,1:2
