| 3 | Mechanosignaling activation of TGFβmaintains intervertebral disc homeostasis显示文摘Intervertebral disc(IVD) degeneration is the leading cause of disability with no disease-modifying treatment.IVD degeneration is associated with instable mechanical loading in the spine, but little is known about how mechanical stress regulates nucleus notochordal(NC) cells to maintain IVD homeostasis. Here we report that mechanical stress can result in excessive integrin α_vβ_6-mediated activation of transforming growth factor beta(TGFβ), decreased NC cell vacuoles, and increased matrix proteoglycan production, and results in degenerative disc disease(DDD). Knockout of TGFβ type II receptor(TβRII) or integrin α_v in the NC cells inhibited functional activity of postnatal NC cells and also resulted in DDD under mechanical loading.Administration of RGD peptide, TGFβ, and α_vβ_6-neutralizing antibodies attenuated IVD degeneration. Thus,integrin-mediated activation of TGFβ plays a critical role in mechanical signaling transduction to regulate IVD cell function and homeostasis. | 5 | Effect of hydroalcoholic extract of Hibiscus rosa sinensis Linn.leaves in experimental colitis in rats显示文摘Objective:To elucidate the ameliorative effect of liydrualcuholic extract of leaves of Hibiscus rosa sinensis(HRS) in acetic acid induced experimental colitis in male wistar rats.Methods: The animals were administered with 2 mL acetic acid(4%) via intra rectal.The animals were divided into various treatment groups(n=6).Prednisolone was used as standard drug and HRS was administered at a dose of 50,100 and 200 mg/kg p.o.The control group of animals received 1 mL of vehicle(distilled water).Ulcer area,ulcer index,spleen weight,colon weight to length ratio, macroscopic score,haemalological parameters,colonic superoxide dismutase(SOD),glutathione(GSH),myeloperoxidase(MPO),malondialdchyde(MDA),tumor necrosis factor-α(TNK- α), nitric oxide(NO) and histological changes were recorded after the treatment regimen of 11 days. | 7 | Novel technique for anastomotic salvage using transanal minimally invasive surgery:A case report显示文摘Anastomotic leak(AL)after low anterior resection(LAR)can be a highly morbid complication.The incidence of AL ranges from 5%to 20%depending on patient characteristics and the distance of the anastomosis from the anal verge.Low anastomoses and leaks pose technical challenges for endoscopic treatment.The aim of this report was to describe the use of a commercially available laparoscopic energy device through a transanal minimally invasive surgery(TAMIS)port for the management of a symptomatic leak not requiring relaparotomy(grade B)after a LAR with diverting loop ileostomy.CASE SUMMARY A TAMIS GelPOINT Path port was inserted into the anus to access the distal rectum.Pneumorectum was achieved with AirSeal insufflation and a 30 degree laparoscope was introduced through a trocar.A LigaSureTM Retractable L-Hook device was then used to perform a septotomy of the chronic sinus tract identified posterior to the coloproctostomy.The procedure was then repeated twice in three weeks intervals with ultimate resolution of the chronic leak cavity.Several months after serial TAMIS septotomies,barium enema demonstrated a patent anastomosis with no evidence of persistent leak or stricture.The patient subsequently underwent ileostomy reversal and has had no significant postoperative issues.CONCLUSION TAMIS septotomy with the LigaSureTM Retractable L-Hook is a feasible and effective,minimally invasive salvage technique for the treatment of grade B ALs.Larger studies are needed to assess the generalizability and long-term results of this technique. | Oscar A Olavarria Robert L Kress Shinil K Shah Amit K Agarwal | 2019 | World Journal of Gastrointestinal Surgery2019，11，5： | 2 |
