| 1 | 改良早期预警评分在评估院前急性中毒患者病情及预后的应用研究显示文摘目的 研究改良早期预警评分(modified early warning scores,MEWS)在院前急性中毒患者病情评估和死亡预测的价值及可行性.方法 以2010年1月1日至2010年12月31日院前急性中毒患者作为研究对象,进行前瞻性观察性研究,现场采集相关数据,进行MEWS评分,以收入院后90 d为观察终点,结局作为观察指标.计数资料以百分比表示,用x2检验,计量资料以均数±标准差((x)±s)表示,用成组t检验,以P <0.05为差异具有统计学意义,对患者进行MEWS评分工作特征曲线(receiver operating characteristic curve,ROC)下面积计算,计算患者相应的预测指标.结果 0~6分的患者287例次,占94.4%;7~13分共17例次,占5.59%;死亡组MEWS评分较存活组高,差异有统计学意义(P<0.05);MEWS评分ROC曲线下面积为0.99,判断院前中毒患者“潜在危重症”的最佳截断点为≥7分,对危重症患者死亡预测的敏感度91.7%,特异度为97.9%,准确度为97.7%,约登指数为0.896.表明MEWS评分在急性中毒患者病情评估及死亡的预测具有较强的应用价值.结论 MEWS评分对院前急性中毒患者病情评估及死亡的预测具有较高的分辨能力,此评分方法简单、实用、可操作性强,有较强的应用价值. | 黄翔 阮海林 黄伟 方量 | 2014 | 中华急诊医学杂志2014,23,4: | 36 |
| 2 | 建立急诊潜在危重病评分预测短期死亡风险的前瞻性研究显示文摘目的:在改良早期预警评分( MEWS)的基础上进行改进、综合,以建立一种新的评分方法,并探讨这种新的评分方法早期识别急诊潜在危重病患者的可行性。方法记录1235例急诊观察室患者的临床资料,分别进行MEWS和新评分表评分,追踪所有患者的去向和预后。分别比较MEWS和新评分方法不同分数段患者14 d内死亡、>14 d死亡、住院时间>14 d、住院时间≤14 d、门诊治疗的构成比;比较MEWS和新评分方法对患者病情评价和预测预后分辨度的ROC曲线差异;比较两种评分方法及与预后的相关性。结果 MEWS 5分以上,危重患者的构成比明显增加,与5分以下比较差异有统计学意义(P<0.05)。新评分方法5分以上、住院时间>14 d的患者构成比明显增加(P<0.05);10分以上、危重患者构成比明显增加(P<0.05);15分以上、<14 d短期死亡的患者构成比明显增加( P<0.05)。就患者短期死亡危险性的预测和评估的鉴别能力而言,新评分方法ROC曲线下面积明显高于MEWS ( P<0.001)。新评分方法与预后的相关性高于MEWS(P<0.05),而且转归愈严重的组别新评分分值越大。结论 MEWS和新评分方法均可用于判断急诊患者的病情严重程度,有一定识别“急诊潜在危重病”的能力。而新评分方法可以更好地判断患者的住院周期及短期死亡风险,且只需增加血常规检查,其快速、费用低廉和操作简捷性并不亚于MEWS,更适用于国内的急诊科。 | 陈芙蓉 赵文穗 徐仲卿 | 2014 | 中国急救医学2014,34,4: | 15 |
| 3 | Comprehensive early warning of rock burst utilizing microseismic multi-parameter indices显示文摘Rock bursts have become one of the most severe risks in underground coal mining and its early warning is an important component in the safety management. Microseismic(MS) monitoring is considered potentially as a powerful tool for the early warning of rock burst. In this study, an MS multi-parameter index system was established and the critical values of each index were estimated based on the normalized multi-information warning model of coal-rock dynamic failure. This index system includes bursting strain energy(BSE) index, time-space-magnitude independent information(TSMII) indices and timespace-magnitude compound information(TSMCI) indices. On the basis of this multi-parameter index system, a comprehensive analysis was conducted via introducing the R-value scoring method to calculate the weights of each index. To calibrate the multi-parameter index system and the associated comprehensive analysis, the weights of each index were first confirmed using historical MS data occurred in LW402102 of Hujiahe Coal Mine(China) over a period of four months. This calibrated comprehensive analysis of MS multi-parameter index system was then applied to pre-warn the occurrence of a subsequent rock burst incident in LW 402103. | 6 | Early Warning Systems: Lost in Translation or Late by Definition? A FORIN Approach显示文摘Early warning systems(EWSs) are widely considered to be one of the most important mechanisms to prevent disasters around the globe. But as disasters continue to affect countries where EWSs have already been implemented, the striking disaster consequences have led us to reflect on the focus, architecture, and function of the warning systems. Since the 2004 Indian Ocean tsunami there has been a rapid rise in the promotion and use of EWSs to minimize disaster losses and damage. However,few researchers have addressed the question of their acceptability as an adaptive measure to the existing exposure conditions. EWSs are far more linked to emergency response and humanitarian crises and accepted technological interventions as solutions than they are to explicitly advance integrated analysis, disaster risk reduction, and policy making. | 7 | Real-time seismology for the 05/12/2008 Wenchuan earthquake of China: A retrospective view显示文摘The devastating 05/12/2008 Wenchuan earthquake (Mw7.9) in Sichuan Province of China showed very few precursory phenomena and occurred on a fault system once assigned to be of moderate long term seismic risk. Given the existing coverage of seismograph stations in Sichuan Province, real-time seis-mology could have been effective in avoiding some earthquake damage and helping post-earthquake emergency response. In a retrospective view, we demonstrated that the epicenter can be located with 20 km accuracy using just two broadband stations with three-component, which takes only about 10 s after the onset of the earthquake. Initial magnitude is estimated to be M7 with the Tc measurement over first 4 seconds of P waves. Better magnitude estimate can be obtained within 2 min by modeling Pnl waves for stations about 500 km away where the S waveforms are clipped. The rupture area is well revealed by teleseismically-recorded >M5 early aftershocks within two hours after the mainshock. | 8 | A study of the early warning signals of abrupt change in the Pacific decadal oscillation显示文摘In recent years, the phenomenon of a critical slowing down has demonstrated its major potential in discovering whether a complex dynamic system tends to abruptly change at critical points. This research on the Pacific decadal oscillation(PDO) index has been made on the basis of the critical slowing down principle in order to analyze its early warning signal of abrupt change. The chaotic characteristics of the PDO index sequence at different times are determined by using the largest Lyapunov exponent(LLE). The relationship between the regional sea surface temperature(SST) background field and the early warning signal of the PDO abrupt change is further studied through calculating the variance of the SST in the PDO region and the spatial distribution of the autocorrelation coefficient, thereby providing the experimental foundation for the extensive application of the method of the critical slowing down phenomenon. Our results show that the phenomenon of critical slowing down, such as the increase of the variance and autocorrelation coefficient, will continue for six years before the abrupt change of the PDO index. This phenomenon of the critical slowing down can be regarded as one of the early warning signals of an abrupt change. Through calculating the LLE of the PDO index during different times, it is also found that the strongest chaotic characteristics of the system occurred between 1971 and 1975 in the early stages of an abrupt change(1976), and the system was at the stage of a critical slowing down, which proves the reliability of the early warning signal of abrupt change discovered in 1970 from the mechanism. | 10 | Participatory Early Warning Systems: Youth, Citizen Science,and Intergenerational Dialogues on Disaster Risk Reduction in Brazil显示文摘Building national people-centered early warning systems(EWS) is strongly recommended by the United Nations International Strategy for Disaster Reduction(UNISDR). Most of the scientific literature is critical of the conventional view of EWS as a linear model with a topdown approach, in which technological features are given more attention than human factors. It is argued that EWS should be people-centered, and used for risk prevention,with an emphasis on resilience, rather than only being triggered when a hazard occurs. However, both the UNISDR and the literature fail to say how a people-centered EWS should be built, and what steps are needed to put EWS into effect. This article examines the obstacles and measures required to promote people-centered EWS,with a focus on the situation in Brazil. | 12 | Probabilistic rainfall thresholds in Chibo, India: estimation and validation using monitoring system显示文摘The Himalayan region has been severely affected by landslides especially during the monsoons. In particular, Kalimpong region in Darjeeling Himalayas has recorded several landslides and has caused significant loss of life, property and agricultural land. The study region, Chibo has experienced several landslides in the past which were mainly debris and earth slide. Globally, several types of rainfall thresholds have been used to determine rainfall-induced landslide incidents. In this paper, probabilistic thresholds have been defined as it would provide a better understanding compared to deterministic thresholds which provide binary results, i.e., either landslide or no landslide for a particular rainfall event. Not much research has been carried out towards validation of rainfall thresholds using an effective and robust monitoring system. The thresholds are then validated using a reliable system utilizing Microelectromechanical Systems(MEMS) tilt sensor and volumetric water content sensor installed in the region. The system measures the tilt of the instrument which is installed at shallow depths and is ideal for an early warning system for shallow landslides. The change in observed tilt angles due to rainfall would give an understanding of the applicability of the probabilistic model. The probabilities determined using Bayes' theorem have been calculated using the rainfall parameters and landslide data in 2010-2016. The rainfall values were collected from an automatic rain gauge setup near the Chibo region. The probabilities were validated using the MEMS based monitoring system setup in Chibo for the monsoon season of 2017. This is the first attempt to determine probabilities and validate it with a robust and effective monitoring system in Darjeeling Himalayas. | 16 | Clinical assessment indicators of postpartum hemorrhage: A systematic review显示文摘Objective: The assessment indicators of postpartum hemorrhage(PPH) were systematically reviewed to explore their relationship with blood loss as an early warning sign to identify potential PPH for prompt intervention.Methods: A systematic literature search of 9 databases was conducted according to inclusion and exclusion criteria using a comprehensive strategy. Strengthening the Reporting of Observational Studies in Epidemiology(STROBE) strategy was chosen as the tool for quality assessment of included studies, and data concerning the basic study characteristics and assessment indicators were extracted.Results: Eleven studies were included in this review. The assessment indicators in different reports covered heart rate, blood pressure, placental and birth canal check, hemoglobin(Hb), hematocrit(HCT),coagulation function tests, hemodynamic parameters and arterial blood gas, but no data on the assessment of postpartum uterine contraction were found. The relationship between indicators and blood loss varies.The overall quality of 9 manuscripts was comparatively high or moderate, and 2 were rated as low.Conclusions: The association between blood loss and clinical symptoms is not conclusive based on the present studies, and specific cut-off points could not be formulated to determine the approximate amount of blood loss. However, some cut-off points can be considered warning signs for the need of advanced interventions, such as heart rate, prothrombin time, and placental implantation.
