| 1 | Current status of preoperative drainage for distal biliary obstruction显示文摘Preoperative biliary drainage(PBD) was developed to improve obstructive jaundice, which affects a number of organs and physiological mechanisms in patients waiting for surgery. However, its role in patients who will undergo pancreaticoduodenectomy for biliary obstruction remains controversial. This article aims to review the current status of the use of preoperative drainage for distal biliary obstruction. Relevant articles published from 1980 to 2015 were identified by searching MEDLINE and Pub Med using the keywords 'PBD', 'pancreaticoduodenectomy', and 'obstructive jaundice'. Additional papers were identified by a manual search of the references from key articles. Current studies have demonstrated that PBD should not be routinely performed because of the postoperative complications. PBD should only be considered in carefully selected patients, particularly in cases where surgery had to be delayed. PBD may be needed in patients with severe jaundice, concomitant cholangitis, or severe malnutrition. The optimal method of biliary drainage has yet to be confirmed. PBD should be performed by endoscopic routes rather than by percutaneous routes to avoid metastatic tumor seeding. Endoscopic stenting or nasobiliary drainage can be selected. | 2 | Short-and long-term results of endoscopic ultrasoundguided transmural drainage for pancreatic pseudocysts and walled-off necrosis显示文摘AIM To evaluate the short-and long-term results of endoscopic ultrasound-guided transmural drainage(EUS-GTD) for pancreatic fluid collection(PFC) and identify the predictive factors of treatment outcome for walled-off necrosis(WON) managed by EUS-GTD alone.METHODS We investigated 103 consecutive patients with PFC who underwent EUS-GTD between September 1999 and August 2015. Patients were divided into four groups as follows: WON(n = 40), pancreatic pseudocyst(PPC; n = 11), chronic pseudocyst(n = 33), and others(n = 19). We evaluated the short-and long-term outcomes of the treatment. In cases of WON, multiple logistic regression analyses were performed to identify the predictor variables associated with the treatment success. In addition, PFC recurrence was examined in patients followed up for more than 6 mo and internal stent removal after successful EUS-GTD was confirmed.RESULTS In this study, the total technical success rate was 96.1%. The treatment success rate of WON, PPC, chronic pseudocyst, and others was 57.5%, 90.9%, 91.0%, and 89.5%, respectively. Contrast-enhanced computed tomography using the multivariate logistic regression analysis revealed that the treatment success rate of WON was significantly lower in patients with more than 50% pancreatic parenchymal necrosis(OR = 17.0; 95%CI: 1.9-150.7; P = 0.011) and in patients with more than 150 mm of PFC(OR = 27.9; 95%CI: 3.4-227.7; P = 0.002).The recurrence of PFC in the long term was 13.3%(median observation time, 38.8 mo). Mean amylase level in the cavity was significantly higher in the recurrence group than in the no recurrence group(P = 0.02).CONCLUSION The reduction of WON by EUS-GTD alone was associated with the proportion of necrotic tissue and extent of the cavity. | 4 | Repeat endoscopic submucosal dissection for recurrent gastric cancers after endoscopic submucosal dissection显示文摘AIM:To clarify the safety and efficacy of repeat endoscopic submucosal dissection(re-ESD)for locally recurrent gastric cancers after ESD.METHODS:A retrospective evaluation was performed of the therapeutic efficacy,complications and followup results from ESD treatment for early gastric cancers in 521 consecutive patients with 616 lesions at St.Luke`s International Hospital between April 2004 and November 2012.In addition,tumor size,the size of resected specimens and the operation time were compared between re-ESD and initial ESD procedures.A flex knife was used as the primary surgical device and a hook knife was used in cases with severe fibrosis in the submucosal layer.Continuous variables were analyzed using the non-parametric Mann-Whitney U test and are expressed as medians(range).Categorical variables were analyzed using a Fisher’s exact test and are reported as proportions.Statistical significance was defined as a P-value less than 0.05.RESULTS:The number of cases in the re-ESD group and the initial ESD group were 5 and 611,respectively.The median time interval from the initial ESD to re-ESD was 14(range,4-44 mo).En bloc resection with free lateral and vertical margins was successfully performed in all re-ESD cases without any complications.No local or distant recurrence was observed during the median follow-up period of 48(range,11-56 mo).Tumor size was not significantly different between the re-ESD group and the initial ESD group(median 22 mm vs 11mm,P=0.09),although the size of resected specimens was significantly larger in the re-ESD group(median 47 mm vs 34 mm,P<0.05).There was a nonsignificant increase observed in re-ESD operation time compared to initial ESD(median 202 min vs 67 min,respectively,P=0.06).CONCLUSION:Despite the low patient number and short follow-up,the results suggest that re-ESD is a safe and effective endoscopic treatment for recurrent gastric cancer after ESD. | Yuto Shimamura Naoki Ishii Kaoru Nakano Takashi Ikeya Kenji Nakamura Koichi Takagi Katsuyuki Fukuda Koyu Suzuki Yoshiyuki Fujita | 2013 | World Journal of Gastrointestinal Endoscopy2013,5,12: | 5 |
| 5 | Depolymerization of polyamide 6 in hydrophilic ionic liquids显示文摘Polyamide 6 underwent an efficient depolymerization in hydrophilic ionic liquids under microwave irradiation at 300C. The depolymerization completed within 60 min. Caprolactam was readily separated by simple extraction procedure and the ionic liquids were recovered and reused for several times. Addition of catalytic amounts of DMAP(N,N-dimethylaminopyridine) promoted the depolymerization effectively.The present improved procedure provides a method to avoid direct distillation procedure, which consumes energy for the separation of caprolactam from ionic liquids. Although some contamination of ionic liquids was observed, the present procedure provides a new possibility for the use of ionic liquids for plastic chemical recycling from the viewpoint of development of an energy-saving methodology. | 6 | New model for cardiomyocyte sheet transplantation using a virus-cell fusion technique显示文摘AIM: To facilitate close contacts between transplanted cardiomyocytes and host skeletal muscle using cell fusion mediated by hemagglutinating virus of Japan envelope(HVJ-E) and tissue maceration. METHODS: Cardiomyocytes(1.5 × 106) from fetal rats were first cultured. After proliferation, some cells were used for fusion with adult muscle fibers using HVJ-E. Other cells were used to create cardiomyocyte sheets(area: about 3.5 cm2 including 2.1 × 106 cells), which were then treated with Nile blue, separated, and transplanted between the latissimus dorsi and intercostal muscles of adult rats with four combinations of HVJ-E and/or Na OH maceration: G1: HVJ-E(+), Na OH(+), Cardiomyocytes(+); G2: HVJ-E(-), NaO H(+), Cardiomyocytes(+); G3: HVJ-E(+),Na OH(-), Cardiomyocytes(+); G4: HVJ-E(-), Na OH(-), Cardiomyocytes(-). At 1 and 2 wk after transplantation, the four groups were compared by detection of beating domains, motion images using moving target analysis software, action potentials, gene expression of MLC-2v and Mesp1 by reverse transcription-polymerase chain reaction, hematoxylin-eosin staining, and immunostaining for cardiac troponin and skeletal myosin.RESULTS: In vitro cardiomyocytes were fused with skeletal muscle fibers using HVJ-E. Cardiomyocyte sheets remained in the primary transplanted sites for 2 wk. Although beating domains were detected in G1, G2, and G3 rats, G1 rats prevailed in the number, size, motion image amplitudes, and action potential compared with G2 and G3 rats. Close contacts were only found in G1 rats. At 1 wk after transplantation, the cardiomyocyte sheets showed adhesion at various points to the myoblast layer in the latissimus dorsi muscle. At 2 wk after transplantation, close contacts were seen over a broad area. | 7 | Reinforcement Learning Based Dynamic Spectrum Access in Cognitive Internet of Vehicles显示文摘Cognitive Internet of Vehicles(CIoV)can improve spectrum utilization by accessing the spectrum licensed to primary user(PU)under the premise of not disturbing the PU’s transmissions.However,the traditional static spectrum access makes the CIoV unable to adapt to the various spectrum environments.In this paper,a reinforcement learning based dynamic spectrum access scheme is proposed to improve the transmission performance of the CIoV in the licensed spectrum,and avoid causing harmful interference to the PU.The frame structure of the CIoV is separated into sensing period and access period,whereby the CIoV can optimize the transmission parameters in the access period according to the spectrum decisions in the sensing period.Considering both detection probability and false alarm probability,a Q-learning based spectrum access algorithm is proposed for the CIoV to intelligently select the optimal channel,bandwidth and transmit power under the dynamic spectrum states and various spectrum sensing performance.The simulations have shown that compared with the traditional non-learning spectrum access algorithm,the proposed Q-learning algorithm can effectively improve the spectral efficiency and throughput of the CIoV as well as decrease the interference power to the PU. | Xin Liu Can Sun Mu Zhou Bin Lin Yuto Lim | 2021 | China Communications2021,18,7: | 3 |

| 17 | New tapered metallic stent for unresectable malignant hilar bile duct obstruction显示文摘AIM: To examine the usefulness of a new tapered metallic stent(MS) in patients with unresectable malignant hilar bile duct obstruction.METHODS: This new tapered MS was placed in 11 patients with Bismuth Ⅱ or severer unresectable malignant hilar bile duct obstruction, as a prospective study. The subjects were six patients with bile duct carcinoma, three with gallbladder cancer, and two with metastatic bile duct obstruction. Stenosis morphology was Bismuth Ⅱ: 7, Ⅲa: 3, and Ⅳ: 1. UMIN Clinical Trial Registry(UMIN000004758).RESULTS: MS placement was 100%(11/11) successful. There were no procedural accidents. The mean patency period was 208.401 d, the median survival period was 142.000 d, and the mean survival period was 193.273 d. Occlusion rate was 36.4%(4/11); the causes of occlusion were ingrowth and overgrowth in 2 patients each, 18.2%, respectively. Patients with occlusion underwent endoscopic treatment one more time and all were treatable.
