| 5 | Role of diffusion-weighted magnetic resonance imaging in the differential diagnosis of focal hepatic lesions显示文摘AIM:To evaluate the utility of diffusion-weighted im-aging(DWI)in screening and differential diagnosis of benign and malignant focal hepatic lesions.METHODS:Magnetic resonance imaging(MRI)ex-aminations were performed using the Signa Excite Xl Twin Speed 1.5T system(GE Healthcare,Milwaukee,WI,USA).Seventy patients who had undergone MRI of the liver [29 hepatocellular carcinomas(HCC),four cholangiocarcinomas,34 metastatic liver cancers,10 hemangiomas,and eight cysts] between April 2004 and August 2008 were retrospectively evaluated.Visu-alization of lesions,relative contrast ratio(RCR),and apparent diffusion coefficient(ADC)were compared between benign and malignant lesions on DWI.Su-perparamagnetic iron oxide(SPIO)was administered to 59 patients,and RCR was compared pre-and post-administration.RESULTS:DWI showed higher contrast between ma-lignant lesions(especially in multiple small metastatic cancers)and surrounding liver parenchyma than did contrast-enhanced computed tomography.ADCs(mean ± SD × 10-3 mm2/s)were signif icantly lower(P < 0.05)in malignant lesions(HCC:1.31 ± 0.28 and liver me-tastasis:1.11 ± 0.22)and were signifi cantly higher in benign lesions(hemangioma:1.84 ± 0.37 and cyst:2.61 ± 0.45)than in the surrounding hepatic tissues.RCR between malignant lesions and surrounding he-patic tissues signifi cantly improved after SPIO adminis-tration,but RCRs in benign lesions were not improved.CONCLUSION:DWI is a simple and sensitive method for screening focal hepatic lesions and is useful for dif-ferential diagnosis. | Naoto Koike Akihiro Cho Katsuhiro Nasu Kazuhiko Seto Shigeyuki Nagaya Yuji Ohshima Nobuhiro Ohkohchi | 2009 | World Journal of Gastroenterology2009,15,46: | 38 |
| 6 | Pollution characteristics and health risk assessment of benzene homologues in ambient air in the northeastern urban area of Beijing, China显示文摘Ambient benzene homologues were measured at a site in the northeastern urban area of Beijing, China, from August 24 to September 4, 2012 by SUMMA canister sampling followed by laboratory determination using cryogenic cold trap pre-concentration-GC-MS/FID, and their health risks were also assessed. Daily total benzene homologues ranged from 0.99 to 49.71 μg/m3with an average of 11.98 μg/m3. Benzene homologues showed higher concentrations in the morning and evening than that at noontime. Comparison with previous studies revealed a trend of decrease for ambient benzene homologues probably due to the efective emission control in Beijing in recent years. Vehicular exhaust was the main source while volatilization of paints and solvents also made substantial contributions. Health risk assessment showed that BTEX(benzene, toluene, ethylbenzene, o-xylene, m-xylene and p-xylene) and styrene had no appreciable adverse non-cancer health risks for the exposed population, while benzene has potential cancer risk of 1.34E-05. | 7 | Changes in Mean and Extreme Temperature and Precipitation over the Arid Region of Northwestern China: Observation and Projection显示文摘This paper reports a comprehensive study on the observed and projected spatiotemporal changes in mean and extreme climate over the arid region of northwestern China, based on gridded observation data and CMIP5 simulations under the RCP4.5 and RCP8.5 scenarios. The observational results reveal an increase in annual mean temperature since 1961, largely attributable to the increase in minimum temperature. The annual mean precipitation also exhibits a significant increasing tendency. The precipitation amount in the most recent decade was greater than in any preceding decade since 1961. Seasonally,the greatest increase in temperature and precipitation appears in winter and in summer, respectively. Widespread significant changes in temperature-related extremes are consistent with warming, with decreases in cold extremes and increases in warm extremes. The warming of the coldest night is greater than that of the warmest day, and changes in cold and warm nights are more evident than for cold and warm days. Extreme precipitation and wet days exhibit an increasing trend, and the maximum number of consecutive dry days shows a tendency toward shorter duration. Multi-model ensemble mean projections indicate an overall continual increase in temperature and precipitation during the 21 st century. | 8 | A novel m6A reader Prrc2a controls oligodendroglial specification and myelination显示文摘While N6-methyladenosine (m6A), the most abundant internal modification in eukaryotic mRNA, is linked to cell differentiation and tissue development, the biological significance of m6A modification in mammalian glial development remains unknown. Here, we identify a novel m6A reader, Prrc2a (Proline rich coiled-coil 2A), which controls oligodendrocyte specification and myelination. Nestin-Cre-mediated knockout of Prrc2a induces significant hypomyelination, decreased lifespan, as well as locomotive and cognitive defects in a mouse model. Further analyses reveal that Prrc2a is involved in oligodendrocyte progenitor cells (OPCs) proliferation and oligodendrocyte fate determination. Accordingly, oligodendroglial-lineage specific deletion of Prrc2a causes a similar phenotype of Nestin-Cre-mediated deletion. Combining transcriptome-wide RNA-seq, m6A-RIP-seq and Prrc2a RIP-seq analysis, we find that Olig2 is a critical downstream target gene of Prrc2a in oligodendrocyte development. Furthermore, Prrc2a stabilizes Olig2 mRNA through binding to a consensus GGACU motif in the Olig2 CDS (coding sequence) in an m6A-dependent manner. Interestingly, we also find that the m6A demethylase, Fto, erases the m6A modification of Olig2 mRNA and promotes its degradation. Together, our results indicate that Prrc2a plays an important role in oligodendrocyte specification through functioning as a novel m6A reader. | 9 | Safe procedure in endoscopic submucosal dissection for colorectal tumors focused on preventing complications显示文摘Endoscopic submucosal dissection (ESD) is efficient for en bloc resection of large colorectal tumors. However, it has several technical difficulties, because the wall of the colon is thin and due to the winding nature of the colon. The main complications of ESD comprise postoperative perforation and hemorrhage, similar to endoscopic mucosal resection (EMR). In particular, the rate of perforation in ESD is higher than that in EMR. Perforation of the colon can cause fatal peritonitis. Endoscopic clipping is reported to be an efficient therapy for perforation. Most cases with perforation are treated conservatively without urgent surgical intervention. However, the rate of postoperative hemorrhage in ESD is similar to that in EMR. Endoscopic therapy including endoscopic clipping is performed and most of the cases are treated conservatively without blood transfusion. In blood examination, some degree of inflammation is detected after ESD. For the standardization of ESD, it is most important to decrease the rate of perforation. Adopting a safe strategy for ESD and a suitable choice of knife are both important waysof preventing perforation. Moreover, appropriate training and increasing experience can improve the endoscopic technique and can decrease the rate of perforation. Recent advances in our understanding of the biological functions of natural-origin biomacromolecules and the progress in the study of biological drug carriers indicate that such carriers may have advantages over synthetic material-based carriers in terms of half-life, stability, safety and ease of manufacture. In this review, we give a brief introduction to the biochemical properties of the widely used biomacromoleculebased carriers such as albumin, lipoproteins and polysaccharides. Then examples from the clinic and in recent laboratory development are summarized. | 11 | Changes in rice cropping systems in the Poyang Lake Region, China during 2004-2010显示文摘Rice cropping systems not only characterize comprehensive utilization intensity of agricultural resources but also serve as the basis to enhance the provision services of agro-ecosystems.Yet,it is always affected by external factors,like agricultural policies.Since 2004,seven consecutive No.1 Central Documents issued by the Central Government have focused on agricultural development in China.So far,few studies have investigated the ef-fects of these policies on the rice cropping systems.In this study,based upon the long-term field survey information on paddy rice fields,we proposed a method to discriminate the rice cropping systems with Landsat data and quantified the spatial variations of rice cropping systems in the Poyang Lake Region(PLR),China.The results revealed that:(1) from 2004 to 2010,the decrement of paddy rice field was 46.76 km2 due to the land use change.(2) The temporal dynamics of NDVI derived from Landsat historical images could well characterize the temporal development of paddy rice fields.NDVI curves of single cropping rice fields showed one peak,while NDVI curves of double cropping rice fields displayed two peaks an-nually.NDVI of fallow field fluctuated between 0.15 and 0.40.NDVI of the flooded field during the transplanting period was relatively low,about 0.20±0.05,while NDVI during the period of panicle initiation to heading reached the highest level(above 0.80).Then,several temporal windows were determined based upon the NDVI variations of different rice cropping systems.(3) With the spatial pattern of paddy rice field and the NDVI threshold within optimum tem-poral windows,the spatial variation of rice cropping systems was very obvious,with an in-creased multiple cropping index of rice about 20.2% from 2004 to 2010.The result indicates that agricultural policies have greatly enhanced the food provision services in the PLR,China. | LI Peng FENG Zhiming JIANG Luguang LIU Yujie XIAO Xiangming | 2012 | Journal of Geographical Sciences2012,22,4: | 23 |
| 12 | Clinicopathologic characteristics and prognosis of gastroenteropancreatic neuroendocrine neoplasms: a multicenter study in South China显示文摘Background: Gastroenteropancreatic neuroendocrine neoplasms(GEP-NENs) are a heterogeneous group of rare tumors. Many issues in terms of epidemiologic features, pathogenesis, and treatment of GEP-NENs are still under discussion. Our study aimed to analyze the clinicopathologic characteristics and prognosis of Chinese patients with GEP-NENs.Methods: Complete clinicopathologic data and survival information of 1183 patients with GEP-NENs treated between 2005 and 2015 were collected from five medical centers in Guangdong Province, China. Patient survival was estimated using the Kaplan–Meier method and analyzed using the log-rank test; prognostic factors were analyzed using the Cox proportional hazards model.Results: The most common tumor location was the rectum(37.4%), followed by the pancreas(28.1%), stomach(20.7%), small intestine(7.2%), appendix(3.4%), and colon(3.3%). After initial definitive diagnosis, 1016(85.9%) patients underwent surgery. The 1-, 3-, and 5-year overall survival(OS) rates for the entire cohort were 87.9%, 78.5%, and 72.8%, respectively. The 3-year OS rates of patients with G1, G2, and G3 tumors were 93.1%, 82.7%, and 43.1%, respectively(P < 0.001). The 3-year OS rates of patients with stage I, II, III, and IV tumors were 96.0%, 87.3%, 64.0%, and 46.8%, respectively(P < 0.001). Patients with distant metastasis who underwent palliative surgery had a longer survival than those who did not(P = 0.003). Similar survival benefits of palliative surgery were observed in patients with neuroendocrine tumor(P y, M category, and sur= 0.031) or neuroendocrine carcinoma(P gery were found to be independent prog= 0.046). In multivariate analysis, age, grade, N categornostic factors.Conclusions: Patients with GEP-NENs who are women, younger than 50 years old, have smaller tumor size, have lower tumor grade, have lower T/N/M category, and who undergo surgery can have potentially longer survival time. Our data showed that surgery can improve the prognosis of GEP-NEN patients with distant metastasis. | 13 | GSDMB promotes non-canonical pyroptosis by enhancing caspase-4 activity显示文摘Gasdermin B (GSDMB) has been reported to be associated with immune diseases in humans, but the detailed molecular mechanisms remain unsolved. The N-terminus of GSDMB by itself, unlike other gasdermin family proteins, does not induce cell death. Here, we show that GSDMB is highly expressed in the leukocytes of septic shock patients, which is associated with increased release of the gasdermin D (GSDMD) N-terminus. GSDMB expression and the accumulation of the N-terminal fragment of GSDMD are induced by the activation of the non-canonical pyroptosis pathway in a human monocyte cell line. The downregulation of GSDMB alleviates the cleavage of GSDMD and cell death. Consistently, the overexpression of GSDMB promotes GSDMD cleavage, accompanied by increased LDH release. We further found that GSDMB promotes caspase-4 activity, which is required for the cleavage of GSDMD in non-canonical pyroptosis, by directly binding to the CARD domain of caspase-4. | 14 | Chronic ankle instability is associated with proprioception deficits:A systematic review and meta-analysis显示文摘Background:Acute ankle injury causes damage to joint mechanoreceptors and deafferentation and contributes to proprioception deficits in patients with chronic ankle instability(CAI).We aimed to explore whether deficits of proprioception,including kinesthesia and joint position sense(JPS),exist in patients with CAI when compared with the uninjured contralateral side and healthy people.We hypothesized that proprioception deficits did exist in patients with CAI and that the deficits varied by test methodologies.Methods:The study was a systematic review and meta-analysis.We identified studies that compared kinesthesia or JPS in patients with CAI with the uninjured contralateral side or with healthy controls.Meta-analyses were conducted for the studies with similar test procedures,and narrative syntheses were undertaken for the rest.Results:A total of 7731 studies were identified,of which 30 were included for review.A total of 21 studies were eligible for meta-analysis.Compared with the contralateral side,patients with CAI had ankle kinesthesia deficits in inversion and plantarflexion,with a standardized mean difference(SMD)of 0.41 and 0.92,respectively,and active and passive JPS deficits in inversion(SMD=0.92 and 0.72,respectively).Compared with healthy people,patients with CAI had ankle kinesthesia deficits in inversion and eversion(SMD=0.64 and 0.76,respectively),and active JPS deficits in inversion and eversion(SMD=1.00 and 4.82,respectively).Proprioception deficits in the knee and shoulder of patients with CAI were not statistically significant.Conclusion:Proprioception,including both kinesthesia and JPS,of the injured ankle of patients with CAI was impaired,compared with the uninjured contralateral limbs and healthy people.Proprioception varied depending on different movement directions and test methodologies.The use of more detailed measurements of proprioception and interventions for restoring the deficits are recommended in the clinical management of CAI. | Xiao’ao Xue Tengjia Ma Qianru Li Yujie Song Yinghui Hua | 2021 | Journal of Sport and Health Science2021,10,2: | 21 |
| 15 | Efficacy of mosapride citrate with polyethylene glycol solution for colonoscopy preparation显示文摘AIM:To evaluate the efficacy and safety of adjunctive mosapride citrate for bowel preparation before colonoscopy. METHODS:We conducted a randomized,double-blind, placebo-controlled study with mosapride in addition to polyethylene glycol(PEG)-electrolyte solution.Of 250 patients undergoing colonoscopy,124 were randomized to receive 2 L PEG plus 15 mg of mosapride citrate (mosapride group),and 126 received 2 L PEG plus placebo(placebo group).Patients completed a questionnaire reporting the acceptability and tolerability of the bowel preparation process.The efficacy of bowel preparation was assessed by colonoscopists using a 5-point scale based on Aronchick's criteria.The primary end point was optimal bowel preparation rates(scores of excellent/good/fair vs poor/inadequate). RESULTS:A total of 249 patients were included in the analysis.In the mosapride group,optimal bowel preparation rates were significantly higher in the left colon compared with the placebo group(78.2%vs 65.6%,P<0.05),but not in the right colon(76.5%vs 66.4%,P=0.08).After excluding patients with severe constipation,there was a significant difference in bowel preparation in both the left and right colon(82.4%vs 66.7%,80.8%vs 67.5%,P<0.05,P<0.01).The incidence of adverse events was similar in both groups. Among the subgroup who had previous colonoscopy experience,a significantly higher number of patients in the mosapride group felt that the current preparation was easier compared with patients in the placebo group(34/72 patients vs 24/74 patients,P<0.05). METHODS:A prospective,randomized,single-center study without crossover was conducted.After endoscopic diagnosis of esophageal variceal rupture,patients were randomized into groups for SCL or BL treatment.Sclerotherapy was performed by ethanolamine oleate intravascular injection both above and below the rupture point,with a maximum volume of 20 mL.For BL patients,banding at the rupture point was attempted,followed by ligation of all variceal tissue of the distal esophagus.Primary outcomes for both groups were initial failure of bleeding control(5 d) ,early re-bleeding(5 d to 6 wk),and complications,including mortality.From May 2005 to May 2007,100 patients with variceal bleeding were enrolled in thestudy:50 SCL and 50 BL patients.No differences between groups were observed across gender,age,ChildPugh status,presence of shock at admission,mean hemoglobin levels,and variceal size. RESULTS:No differences were found between groups for bleeding control,early re-bleeding rates,complications,or mortality.After 6 wk,36(80%) SCL and 33(77 %) EBL patients were alive and free of bleeding.A statistically significant association between Child-Pugh status and mortality was found,with 16%mortality in Child A and B patients and 84%mortality in Child C patients(P<0.001) . | 18 | Therapeutic efficacy of the Qing Dai in patients with intractable ulcerative colitis显示文摘Ulcerative colitis (UC) is a chronic inflammatory bowel disease that may become intractable when treated with conventional medications such as aminosalicylates, corticosteroids, and azathioprine. The herbal medicine Qing Dai has traditionally been used in Chinese medicine to treat UC patients, but there is a lack of published data on the efficacy of Qing Dai in UC treatment. We report several cases of patients with intractable UC who take Qing Dai in a retrospective observational study. Furthermore, we explore the mechanisms of action of Qing Dai. Nine patients with active UC who received conventional medications but wished to receive Qing Dai as an alternative medication were included in our analysis. The UC severity level was determined based on the clinical activity index (CAI). Additionally, 5 of the 9 patients were endoscopically evaluated according to the Matts grading system. Each patient received 2 g/d of Qing Dai orally and continued taking other medications for UC as prescribed. Electron spin resonance was applied to explore the mechanisms of action of Qing Dai. After 4 mo of treatment with Qing Dai, the CAI score decreased from 8.3 ± 2.4 to 2.4 ± 3.4 (mean ± SD; P < 0.001). Similarly, the endoscopic Matts grade decreased from 3.4 ± 0.5 to 2.2 ± 0.8 (P = 0.02). Six of 7 patients who were on prednisolone upon enrollment in the study were able to discontinue this corticosteroid. Electron spin resonance revealed that Qing Dai possesses strong hydroxyl radical scavenging activity. Qing Dai showed significant clinical and endoscopic efficacy in patients who failed to respond to conventional medications. | 20 | Substance P-modified human serum albumin nanoparticles loaded with paclitaxel for targeted therapy of glioma显示文摘The blood–brain barrier(BBB) and the poor ability of many drugs to cross that barrier greatly limits the efficacy of chemotherapies for glioblastoma multiforme(GBM). The present study exploits albumin as drug delivery vehicle to promote the chemotherapeutic efficacy of paclitaxel(PTX) by improving the stability and targeting efficiency of PTX/albumin nanoparticles(NPs). Here we characterize PTX-loaded human serum albumin(HSA) NPs stabilized with intramolecular disulfide bonds and modified with substance P(SP) peptide as the targeting ligand. The fabricated SP-HSA-PTX NPs exhibited satisfactory drug-loading content(7.89%) and entrapment efficiency(85.7%) with a spherical structure(about 150 nm) and zeta potential of -12.0 mV. The in vitro drug release from SP-HSA-PTX NPs occurred in a redox-responsive manner.
