2Minilaparotomy to rectal cancer has higher overall survival rate and earlier short-term recovery显示文摘AIM:To report our experience using mini-laparotomy for the resection of rectal cancer using the total mesorectal excision (TME) technique METHODS:Consecutive patients with rectal cancer who underwent anal-colorectal surgery at the authors' hospital between March 2001 and June 2009 were included In total, 1415 patients were included in the study The cases were divided into two surgical procedure groups (traditional open laparotomy or mini-lap-arotomy) The mini-laparotomy group was defined as having an incision length ≤ 12 cm Every patient underwent the TME technique with a standard operation performed by the same clinical team The multimodal preoperative evaluation system and postoperative fast track were used To assess the short-term outcomes, data on the postoperative complications and recovery functions of these cases were collected and analysed The study included a plan for patient follow-up, to ob- tain the long-term outcomes related to 5-year survival and local recurrenceRESULTS: The mini-laparotomy group had 410 patients, and 1015 cases underwent traditional laparotomy There were no differences in baseline characteristics between the two surgical procedure groups The overall 5-year survival rate was not different between the mini-laparotomy and traditional laparotomy groups (80 6% vs 79 4%, P = 0 333), nor was the 5-year local recurrence (1 4% vs 1 5%, P = 0 544) However, 1-year mortality was decreased in the mini-laparotomy group compared with the traditional laparotomy group (0% vs 4 2%, P < 0 0001) Overall 1-year survival rates were 100% for Stage Ⅰ, 98 4% for Stage Ⅱ, 97 1% for Stage Ⅲ, and 86 6% for Stage Ⅳ Local recurrence did not differ between the surgical groups at 1 or 5 years Local recurrence at 1 year was 0 5% (2 cases) for mini-laparotomy and 0 5% (5 cases) for traditional laparotomy (P = 0 670) Local recurrence at 5 years was 1 5% (6 cases) for mini-laparotomy and 1 4% (14 cases) for traditional laparotomy (P = 0 544) Days to first ambulation (3.2 ± 0.8 dvs 3.9 ± 2.3 d, P = 0 000) and passing of gas (3.5 ± 1.1 d vs 4.3 ± 1.8 d, P = 0.000), length of hospital stay (6.4 ± 1.5 dvs 9.7 ± 2.2 d, P = 0 000), anastomotic leakage (0 5% vs 4 8%, P = 0 000), and intestinal obstruction (2 2% vs 7 3%, P = 0 000) were decreased in the mini-laparotomy group compared with the traditional laparotomy group The results for other postoperative recovery function indicators, such as days to oral feeding and defecation, were similar, as were the results for immediate postoperative complications, including the physiologic and operative severity score for the enumeration of mortality and morbidity score CONCLUSION:Mini-laparotomy, as conducted in a single-centre series with experienced TME surgeons, is a safe and effective new approach for minimally invasive rectal cancer surgery Further evaluation is required to evaluate the use of this approach in a larger patient sample and by other surgical teams.Xiao-Dong Wang Ming-Jun Huang Chuan-Hua Yang Ka Li Li Li 2012World Journal of Gastroenterology2012,18,37:6
3Command and Control Cities in Global Space-economy Before and after 2008 Geo-economic Transition显示文摘The global economic downturn caused primarily by the US sub-prime mortgage crisis in 2007 engendered revenue loss of the multinational corporations.Existing studies have yet to depict the detrimental impacts on city's command and control functions induced by the sub-prime mortgage crisis together with its residual wave of global economic recession on the global spatial economy.Recent and previous studies have produced an 'instant history' of the global spatial economy before the global economic downturn undermines the global economy in late 2008.However,the waxes and wanes of major cities' command and control functions on the global economic arena before and after the outbreak of financial crisis and its associated geo-economic transitions are still poorly understood.This paper attempts to contribute a new set of customized data to update and fill in the gap in the literature with the investigation of the command and control functions of cities around the world from 2005 to 2009.Particular attentions are paid to the time-space relationship of the geo-economic transition that can capture the recent historical images of the command and control situation of different cities in the world.Ernest Ka Shing LEE Simon Xiaobin ZHAO Yang XIE 2012Chinese Geographical Science2012,22,3:5
4Cell cycle-related kinase reprograms the liver immune microenvironment to promote cancer metastasis显示文摘The liver is an immunologically tolerant organ and a common metastatic site of multiple cancer types.Although a role for cancer cell invasion programs has been well characterized,whether and how liver-intrinsic factors drive metastatic spread is incompletely understood.Here,we show that aberrantly activated hepatocyte-intrinsic cell cycle-related kinase(CCRK)signaling in chronic liver diseases is critical for cancer metastasis by reprogramming an immunosuppressive microenvironment.Using an inducible liverspecific transgenic model,we found that CCRK overexpression dramatically increased both B16F10 melanoma and MC38 colorectal cancer(CRC)metastasis to the liver,which was highly infiltrated by polymorphonuclear-myeloid-derived suppressor cells(PMNMDSCs)and lacking natural killer T(NKT)cells.Depletion of PMN-MDSCs in CCRK transgenic mice restored NKT cell levels and their interferon gamma production and reduced liver metastasis to 2.7% and 0.7%(metastatic tumor weights)in the melanoma and CRC models,respectively.Mechanistically,CCRK activated nuclear factor-kappa B(NF-κB)signaling to increase the PMN-MDSC trafficking chemokine C-X-C motif ligand 1(CXCL1),which was positively correlated with liver-infiltrating PMN-MDSC levels in CCRK transgenic mice.Accordingly,CRC liver metastasis patients exhibited hyperaaivation of hepatic CCRK/NF-κB/CXCL1 signaling,which was associated with accumulation of PMN-MDSCs and paucity of NKT cells compared to healthy liver transplantation donors.In summary,this study demonstrates that immunosuppressive reprogramming by hepatic CCRK signaling undermines antimetastatic immunosurveillance.Our findings offer new mechanistic insights and therapeutic targets for liver metastasis intervention.Xuezhen Zeng Jingying Zhou Zhewen Xiong Hanyong Sun Weiqin Yang Myth T.S.Mok Jing Wang Jingqing Li Man Liu Wenshu Tang Yu Feng Hector Kwong-Sang W ang Shun-Wa Tsang King-Lau Chow Philip Chun Yeung John Wong Paul Bo-San Lai Anthony Wing-Hung Chan Ka Fai To Stephen Lam Chan Qiang Xia Jing Xue Xiao Chen Jun Yu Sui Peng Joseph Jao-Yiu Sung Ming Kuang Alfred Sze-Lok Cheng 2021Cellular & Molecular Immunology2021,18,4:5
5Flexible microbubble-based Fabry-Pérot cavity for sensitive ultrasound detection and wide-view photoacoustic imaging显示文摘Interaction of acoustic waves and microbubbles occurs in numerous biomedical applications including ultrasound imaging,drug delivery,lithotripsy treatment,and cell manipulation,wherein the acoustically driven microbub-blcs routincly act as active microscale oscillators or actuators.In contrast,microbubbles were utilized here as passive reccivers to detect broadband ultrasound waves in aqueous environments.The microbubble was photo-thermally generated on a microstructured optical fiber(MOF)tip,forming a flexible Fabry-Perot cavity whose gas-water interface was sensitive to ultrasound waves.The MOF severed as both a low-loss waveguide and a compact light condenser,allowing high-efficiency generation and stabilization of ultrasmall microbubbles.Integrated with all-fiber interferometry,a 10 um diameter microbubble exhibited a low noise-cquivalcnt pressur elevei of^3.4 mPa/Hz^1/2 and a broad bandwidth of^0.8 MHz,capable of detecting weak ultrasounds emittedfrom red blood cells irradiated by pulsed laser light.With advantages of high sensitivity,compact size,and low cost,the microbubble-based ultrasound sensor has great potential in biomedical imaging and sensing applications.JUN MA YANG HE XUE BAI LI-PENG SUN KA CHEN KYUNGHWAN OH BAI-OU GUAN 2020Photonics Research2020,8,10:4
6Development of nanoscale drug delivery systems of dihydroartemisinin for cancer therapy: A review显示文摘Dihydroartemisinin(DHA),a first-line antimalarial drug,has demonstrated great anticancer effects in many types of tumors,including liver cancer,glioblastoma,and pancreatic cancer.Due to its abilities to induce programmed cell death(PCD;apoptosis,autophagy and ferroptosis),inhibit tumor metastasis and angiogenesis,and modulate the tumor microenvironment,DHA could become an antineoplastic agent in the foreseeable future.However,the therapeutic efficacy of DHA is compromised owing to its inherent disadvantages,including poor stability,low aqueous solubility,and short plasma halflife.To overcome these drawbacks,nanoscale drug delivery systems(NDDSs),such as polymeric nanoparticles(NPs),liposomes,and metal-organic frameworks(MOFs),have been introduced to maximize the therapeutic efficacy of DHA in either single-drug or multidrug therapy.Based on the beneficial properties of NDDSs,including enhanced stability and solubility of the drug,prolonged circulation time and selective accumulation in tumors,the outcomes of DHA-loaded NDDSs for cancer therapy are significantly improved compared to those of free DHA.This reviewfirst summarizes the current understanding of the anticancer mechanisms of DHA and then provides an overview of DHA-including nanomedicines,aiming to provide inspiration for further application of DHA as an anticancer drug.Ka Hong Wong Donglin Yang Shanshan Chen Chengwei He Meiwan Chen 2022Asian Journal of Pharmaceutical Sciences2022,17,4:3

8Weather Information Acquisition and Health Significance during Extreme Cold Weather in a Subtropical City:A Crosssectional Survey in Hong Kong显示文摘Health and disaster risk reduction are important and necessary components in building a smart city,especially when climate change may increase the frequency of extreme temperatures and the health risks of urban dwellers.However,limited knowledge is available about the best way to disseminate weather warnings and health protection information.This study explores the weather information acquisition patterns of the Hong Kong public and examines the sociodemographic predictors of these patterns to establish the potential public health implications of smart city development.A population-based,stratified crosssectional,random digit dialing telephone survey was conducted among the Cantonese-speaking population aged over 15 years in Hong Kong in early 2016.Analyses were conducted based on 1017 valid samples,with a response rate of 63.6%.Cold Weather Warnings were well disseminated in Hong Kong,with 95.7% of the respondents reporting awareness of the public warnings.Television and smartphone apps were the two most important channels for weather information acquisition.Age and education level are the main social-demographic variables associated with the current utilization and future preference of smartphone technology.Among those who were not using a preferredchannel to acquire weather information,61.3% considered switching to a smartphone app.Moreover,the patterns of individual health protection measures and self-reported health impacts were significantly different between smartphone app users and non-users.Weather information dissemination should be tailored to the sociodemographic characteristics of the users.Emily Ying Yang Chan Zhe Huang Carman Ka Man Mark Chunlan Guo 2017International Journal of Disaster Risk Science2017,8,2:2
