| 2 | Three-dimensional endoanal ultrasonographic assessment of an anal fistula with and without H_2O_2 enhancement显示文摘AIM:To evaluate the effectiveness of three-dimensional endoanal ultrasound (3D-EAUS) in the assessment of anal fistulae with and without H2O2 enhancement.METHODS: Sixty-one patients (37 males, aged 17-74 years) with anal fi stulae, which were not simple low types, were evaluated by physical examination and 3D-EAUS with and without enhancement. Fistula classification was determined with each modality and compared to operative findings as the reference standard.RESULTS: The accuracy of 3D-EAUS was significantly higher than that of physical examination in detecting the primary tract (84.4% vs 68.7%, P=0.037) and secondary extension (81.8% vs 62.1%, P=0.01) and localizing the internal opening (84.2% vs 59.7%, P=0.004). A contrast study with H2O2 detected several more fistula components including two primary suprasphincteric fistula tracks and one supralevator secondary extension, which were not detected on non-contrast study. However, there was no significant difference in accuracy between 3D-EAUS and H2O2-enhanced 3D-EAUS with respect to classification of the primary tract (84.4% vs 89.1%, P=0.435) or secondary extension (81.8% vs 86.4%, P=0.435) or localization of the internal opening (84.2% vs 89.5%, P=0.406).CONCLUSION: 3D-EAUS was highly reliable in the diagnosis of an anal fistula. | 12 | Interactions between wind and water erosion change sediment yield and particle distribution under simulated conditions显示文摘Wind and water erosion are among the most important causes of soil loss, and understanding their interactions is important for estimating soil quality and environmental impacts in regions where both types of erosion occur. We used a wind tunnel and simulated rainfall to study sediment yield, particle-size distribution and the fractal dimension of the sediment particles under wind and water erosion. The experiment was conducted with wind erosion firstly and water erosion thereafter, under three wind speeds(0, 11 and 14 m/s) and three rainfall intensities(60, 80 and 100 mm/h). The results showed that the sediment yield was positively correlated with wind speed and rainfall intensity(P<0.01). Wind erosion exacerbated water erosion and increased sediment yield by 7.25%–38.97% relative to the absence of wind erosion. Wind erosion changed the sediment particle distribution by influencing the micro-topography of the sloping land surface. The clay, silt and sand contents of eroded sediment were also positively correlated with wind speed and rainfall intensity(P<0.01). Wind erosion increased clay and silt contents by 0.35%–19.60% and 5.80%–21.10%, respectively, and decreased sand content by 2.40%–8.33%, relative to the absence of wind erosion. The effect of wind erosion on sediment particles became weaker with increasing rainfall intensities, which was consistent with the variation in sediment yield. However, particle-size distribution was not closely correlated with sediment yield(P>0.05). The fractal dimension of the sediment particles was significantly different under different intensities of water erosion(P<0.05), but no significant difference was found under wind and water erosion. We analyzed the relationship among the tumor makers, curative resection and recurrence, retrospectively.RESULTS: Among the 679 patients with gastric cancer, curative resection was 93.6%(n = 636) and noncurative resection was 6.4%(n = 43). The independent risk factors for the non-curative resection were tumor location and the positivity of preoperative serum CA 19-9 and CA 125 levels. After curative resection, the independent prognostic risk factors for recurrence in curative resection were gender, stage, and preoperative increased serum CA 125 level(HR = 2.431, P =0.020), in a multivariate analysis.
