| 3 | Efficacy of dexamethasone and N-acetylcysteine combination in preventing post-embolization syndrome after transarterial chemoembolization in hepatocellular carcinoma显示文摘BACKGROUND Conventional transarterial chemoembolization(cTACE)is the current standard treatment for intermediate-stage hepatocellular carcinoma(HCC).Postembolization syndrome(PES)is complex clinical syndrome that presents as fever,abdominal pain,nausea,and vomiting.Either dexamethasone(DEXA)or Nacetylcysteine(NAC)is used to prevent PES;however,the synergistic effect of their combined therapy for preventing PES and liver decompensation has not been determined.AIM To evaluate the efficacy of DEXA and NAC combination in preventing PES and liver decompensation after cTACE.METHODS Patients with Barcelona Clinic Liver Cancer stage A or B HCC who were scheduled for TACE were prospectively enrolled.All patients were randomly stratified to receive NAC and DEXA or placebo.The dual therapy(NAC+DEXA)group received intravenous administration of 10 mg DEXA every 12 h,NAC 24 h prior to cTACE(150 mg/kg/h for 1 h followed by 12.5 mg/kg/h for 4 h),and a continuous infusion of 6.25 mg/h NAC plus 4 mg DEXA every 12 h for 48 h after cTACE.The placebo group received an infusion of 5%glucose solution until 48 h after procedure.PES was defined by South West Oncology Group toxicity code grading of more than 2 that was calculated using incidence of fever,nausea,vomiting,and pain.RESULTS One-hundred patients were enrolled with 50 patients in each group.Incidence of PES was significantly lower in the NAC+DEXA group compared with in the placebo group(6%vs 80%;P<0.001).Multivariate analysis showed that the dual treatment is a protective strategic therapy against PES development[odds ratio(OR)=0.04;95%confidence interval(CI):0.01-0.20;P<0.001).Seven(14%)patients in the placebo group,but none in the NAC+DEXA group,developed post-TACE liver decompensation.A dynamic change in Albumin-Bilirubin score of more than 0.5 point was found to be a risk factor for post-TACE liver decompensation(OR=42.77;95%CI:1.01-1810;P=0.049).CONCLUSION Intravenous NAC+DEXA administration ameliorated the occurrence of PES event after cTACE in patients with intermediate-stage HCC. | Nitipon Simasingha Wasu Tanasoontrarat Torpong Claimon Supatsri Sethasine | 2023 | World Journal of Gastroenterology2023,29,5: | 1 |

| 9 | Abiotic and Biotic Factors Controlling Grain Aroma along Value Chain of Fragrant Rice:A Review显示文摘The aroma of fragrant rice is one of the grain quality attributes that significantly influenceconsumer preferences and prices in world markets. The volatile compound 2-acetyl-1-pyrroline (2AP) isrecognized as a key component of the aroma in fragrant rice. The variation in grain 2AP content amongvarious fragrant rice varieties is associated with the expression of the badh2 gene, with 19 alleles havingbeen identified so far. The grain 2AP content is strongly influenced by environmental and managementfactors during cultivation as well as post-harvest conditions. This review pinpointed the major abiotic andbiotic factors that control grain 2AP content. Abiotic factors refer to water, temperature, light quality,fertilizer application (both macro- and micro-nutrients), and soil properties, including salinity, while bioticfactors include microorganisms that produce aromatic compounds, thus influencing the grain aroma infragrant rice. Post-harvest management, including storage and drying conditions, can significantly impactthe grain 2AP content, and proper post-harvest conditions can intensify the grain aroma. This reviewsuggests that there are rice varieties that can serve as potential sources of genetic material for breedingrice varieties with high grain aroma content. It offers an overview of recent research on the major factorsaffecting the aroma content in fragrant rice. | 10 | Clinical outcome of Yttrium-90 selective internal radiation therapy (Y-90 SIRT) in unresectable hepatocellular carcinoma: Experience from a tertiary care center显示文摘Background and aim:Whereas Yttrium-90 selective internal radiation therapy(Y-90 SIRT)was shown to improve local tumor control in non-Asian population,the efficacy of this therapy for Asian population in real-world setting remains poorly detailed.We aimed to determine outcomes and identify predictors of response in hepatocellular carcinoma(HCC)patients treated by Y-90 SIRT.Methods:We retrospectively enrolled 52 HCC patients receiving Y-90 SIRT at our tertiary center between 2014 and 2019.Overall survival(OS),progression free survival(PFS),and predictive factors were determined by KaplaneMeier method and Cox-proportional hazard analysis.Results:Of the 52 patients(81% male,mean age 64.9 years),71%and 29% were classified as Barcelona Clinic Liver Cancer stage C and B HCC,respectively;63% had portal vein thrombosis,and 35% had objective tumor response defined by the modified Response Evaluation Criteria in Solid Tumors(mRE-CIST)criteria.OS and PFS were 11.0 and 2.4 months,respectively.Two patients were successfully down-staged and further underwent surgical resection.Multifocal lesion,alpha-fetoprotein(AFP)≥200 ng/mL,and Eastern Cooperative Oncology Group(ECOG)score≥1 were significantly associated with poor sur-vival,with adjusted hazard ratio(95% confidence interval)of 7.7(2.0e29.8),5.4(2.0e14.7),and 3.1(1.0 e9.6),respectively(all in P<0.05).Conclusions:Y-90 SIRT is an effective treatment for the local tumor control of HCC without serious adverse events.Single lesion,AFP level and ECOG status were predictors of response. | Jukkaphop Chaikajornwat Wasu Tanasoontrarat Chonlada Phathong Nutcha Pinjaroen Roongruedee Chaiteerakij | 2022 | Liver Research2022,6,1: | 0 |
