| 1 | Xanthogranulomatous cholecystitis: What every radiologist should know显示文摘Xanthogranulomatous cholecystitis(XGC) is an uncommon variant of chronic cholecystitis characterized by xanthogranulomatous inflammation of the gallbladder. Intramural accumulation of lipid-laden macrophages and acute and chronic inflammatory cells is the hallmark of the disease. The xanthogranulomatous inflammation of the gallbladder can be very severe and can spill over to the neighbouring structures like liver, bowel and stomach resulting in dense adhesions, perforation, abscess formation, fistulous communication with adjacent bowel. Striking gallbladder wall thickening and dense local adhesions can be easily mistaken for carcinoma of the gallbladder, both intraoperatively as well as on preoperative imaging. Besides, cases of concomitant gallbladder carcinoma complicating XGC have also been reported in literature. So, we have done a review of the imaging features of XGC in order to better understand the entity as well as to increase the diagnostic yield of the disease summarizing the characteristic imaging findings and associations of XGC. Among other findings, presence of intramural hypodense nodules is considered diagnostic of this entity. However, in some cases, an imaging diagnosis of XGC is virtually impossible. Fine needle aspiration cytology might be handy in such patients. | 4 | Perivascular adipose tissue dysfunction aggravates adventitial remodeling in obese mini pigs via NLRP3 inflammasome/IL-1 signaling pathway显示文摘Perivascular adipose tissue (PVAT), a special type of adipose tissue, closely surrounds vascular adventitia and produces numerous bioactive substances to maintain vascular homeostasis. PVAT dysfunction has a crucial role in regulating vascular remodeling, but the exact mechanisms remain unclear. In this study, we investigated whether and how obesity-induced PVAT dysfunction affected adventitia remodeling in early vascular injury stages. Mini pigs were fed a high sugar and fat diet for 6 months to induce metabolic syndrome and obesity. In the mini pigs, left carotid vascular injury was then generated using balloon dilation. Compared with normal mini pigs, obese mini pigs displayed significantly enhanced vascular injury-induced adventitial responses, evidenced by adventitia fibroblast (AF) proliferation and differentiation, and adventitia fibrosis, as well as exacerbated PVAT dysfunction characterized by increased accumulation of resident macrophages, particularly the M1 pro-infiammatory phenotype, increased expression of leptin and decreased expression of adiponectin, and production of pro-infiammatory cytokines interleukin (IL)-1β and IL-18. Primary AFs cultured in PVAT-conditioned medium from obese mini pigs also showed significantly increased proliferation and differentiation. We further revealed that activated nod-like receptor protein 3 (NLRP3) infiammasome and its downstream products, i.e., IL-1 family members such as IL-1β and IL-18 were upregulated in the PVAT of obese mini pigs;PVAT dysfunction was also demonstrated in preadipocytes treated with palmitic acid. Finally, we showed that pretreatment with IL-1 receptor (IL-1R) antagonist or IL-1R knockdown blocked AF proliferation and differentiation in AFs cultured in PVAT-conditioned medium. | 5 | Imaging and radiological interventions in extra-hepatic portal vein obstruction显示文摘Extrahepatic portal vein obstruction(EHPVO) is a primary vascular condition characterized by chronic long standing blockage and cavernous transformation of portal vein with or without additional involvement of intrahepatic branches, splenic or superior mesenteric vein. Patients generally present in childhood with multiple episodes of variceal bleed and EHPVO is the predominant cause of paediatric portal hypertension(PHT) in developing countries. It is a pre-hepatic type of PHT in which liver functions and morphology are preserved till late. Characteristic imaging findings include multiple parabiliary venous collaterals which form to bypass the obstructed portal vein with resultant changes in biliary tree termed portal biliopathy or portal cavernoma cholangiopathy. Ultrasound with Doppler, computed tomography, magnetic resonance cholangiography and magnetic resonance portovenography are non-invasive techniques which can provide a comprehensive analysis of degree and extent of EHPVO, collaterals and bile duct abnormalities. These can also be used to assess in surgical planning as well screening for shunt patency in post-operative patients. The multitude of changes and complications seen in EHPVO can be addressed by various radiological interventional procedures. | 6 | Implications of Climate Change on Streamflow of a Snow-Fed River System of the Northwest Himalaya显示文摘Air temperature and snow cover variability are sensitive indicators of climate change. This study was undertaken to forecast and quantify the potential streamflow response to climate change in the Jhelum River basin. The implications of air temperature trends (+0.11°C/decade) reported for the entire north-west Himalaya for past century and the regional warming (+0.7°C/decade) trends of three observatories analyzed between last two decades were used for future projection of snow cover depletion and stream flow. The streamflow was simulated and validated for the year 2007-2008 using snowmelt runoff model (SRM) based on in-situ temperature and precipitation with remotely sensed snow cover area. The simulation was repeated using higher values of temperature and modified snow cover depletion curves according to the assumed future climate. Early snow cover depletion was observed in the basin in response to warmer climate. The results show that with the increase in air temperature, streamflow pattern of Jhelum will be severely affected. Significant redistribution of streamflow was observed in both the scenarios. Higher discharge was observed during spring-summer months due to early snowmelt contribution with water deficit during monsoon months. Discharge increased by 5% 40% during the months of March to May in 2030 and 2050. The magnitude of impact of air temperature is higher in the scenario-2 based on regional warming. | 7 | Tomato lycopene attenuates myocardial infarction induced by isoproterenol:Electrocardiographic,biochemical and anti-apoptotic study显示文摘Objective:To assess the protective effects of lycopene on electrocardiographic,hemodynamic, biochemical and apoptotic changes in isoproterenol induced myocardial infarction.Methods: Myocardial infarction was induced in rats by subcutaneous injection of isoproterenol(200 mg/kg) for two consecutive days at an interval of 24 h.Rats were treated with lycopene(10 mg/kg/day, p.o.) for a period of 30 days and isoproterenol(ISO) was injected on the 29th and 30th day.At the end of experiment i.e.on the 31st day electrocardiographic,hemodynamic,biochemical and apoptotic changes were monitored from control and experimental groups.Results:ISO injected ruts showed a significant alteration in electrocardiograph pattern and hemodynamic changes(i.e. systolic,diastolic and mean arterial pressure).It also showed significant increase in C-reactive protein,myeloperoxidase,nitrite levels and Caspase-3 protease activity.In addition,it also exhibited alteration in the levels of electrolytes(Na^+,K^+ and Ca^(2+),vitamin E,uric acid and serum protein.Cel electrophoresis of ISO injected rats showed increase in DNA fragmentation.Triphenyl tetrazolium chloride staining of the heart section shows increase area of infarction in ISO injected rats.Pre-co-treatment with lycopene significantly prevented the ISO induced ullerution in ECC, haemodynamic,biochemical and apoptotic changes.Conclusions:The present result shows that treatment of lycopene in ISO injected rats significantly attenuates induced myocardial infarction. | Aman Upaganlawar Vaibhav Patel Balaraman R | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,5: | 6 |

| 9 | Snow Cover Variation and Streamflow Simulation in a Snow-fed River Basin of the Northwest Himalaya显示文摘Snowmelt is an important component of any snow-fed river system.The Jhelum River is one such transnational mountain river flowing through India and Pakistan.The basin is minimally glacierized and its discharge is largely governed by seasonal snow cover and snowmelt.Therefore,accurate estimation of seasonal snow cover dynamics and snowmeltinduced runoff is important for sustainable water resource management in the region.The present study looks into spatio-temporal variations of snow cover for past decade and stream flow simulation in the Jhelum River basin.Snow cover extent(SCE) was estimated using MODIS(Moderate Resolution Imaging Spectrometer) sensor imageries.Normalized Difference Snow Index(NDSI) algorithm was used to generate multi-temporal time series snow cover maps.The results indicate large variation in snow cover distribution pattern and decreasing trend in different sub-basins of the Jhelum River.The relationship between SCE-temperature,SCE-discharge and discharge-precipitation was analyzed for different seasons and shows strong correlation.For streamflow simulation of the entire Jhelum basin Snow melt Runoff Model(SRM) used.A good correlation was observed between simulated stream flow and in-situ discharge.The monthly discharge contribution from different sub-basins to the total discharge of the Jhelum River was estimated using a modified version of runoff model based on temperature-index approach developed for small watersheds.Stream power - an indicator of the erosive capability of streams was also calculated for different sub-basins. | Vaibhav SHARMA V.D.MISHRA P.K.JOSHI | 2012 | Journal of Mountain Science2012,9,6: | 5 |
| 10 | Clinical practice of early extubation after liver transplantation显示文摘BACKGROUND: Anesthetic practices such as early tracheal extubation facilitate postoperative recovery. Early extubation after liver transplantation has been adopted by some centers in the recent two decades. No universal clinical guidelines are used and questions remain. This review aimed to address the current status of early extubation after liver transplantation. DATA SOURCES: A literature search of MEDLINE and ISI Web of Knowledge databases was performed using terms such as liver transplantation, early extubation, immediate tracheal extubation fast tracking or fast track anesthesia and postoperative tracheal extubation. Additional papers were identified by a manual search of the references in the key articles. RESULTS: Review of the available literature provided an insight into the definition, evolution, advantages and risks of early extubation, and anesthetic techniques that prompt early extubation in liver transplant patients. Early extubation has proved to be feasible and safe in these patients, but the outcomes are still uncertain. CONCLUSIONS: Early extubation after liver transplantation is feasible, safe and cost-effective in the majority of patients and has been increasingly accepted as an option for conventional postoperative ventilation. | 12 | Healthcare utilization and costs associated with gastroparesis显示文摘AIM To use a national database of United States hospitals to evaluate the incidence and costs of hospital admissions associated with gastroparesis.METHODS We analyzed the National Inpatient Sample Database(NIS)for all patients in whom gastroparesis(ICD-9 code:536.3)was the principal discharge diagnosis during the period,1997-2013.The NIS is the largest publicly available all-payer inpatient care database in the United States.It contains data from approximately eight million hospital stays each year.The statistical significance of the difference in the number of hospital discharges,length of stay and hospital costs over the study period was determined by regression analysis.RESULTS In 1997,there were 3978 admissions with a principal discharge diagnosis of gastroparesis as compared to16460 in 2013(P<0.01).The mean length of stay for gastroparesis decreased by 20%between 1997 and 2013from 6.4 d to 5.1 d(P<0.001).However,during this period the mean hospital charges increased significantly by 159%from$13350(after inflation adjustment)per patient in 1997 to$34585 per patient in 2013(P<0.001).The aggregate charges(i.e.,'national bill') for gastroparesis increased exponentially by 1026%from$50456642±4662620 in 1997 to$568417666±22374060 in 2013(P<0.001).The percentage of national bill for gastroparesis discharges(national bill for gastroparesis/total national bill)has also increased over the last 16 years(0.0013%in 1997 vs 0.004%in2013).During the study period,women had a higher frequency of gastroparesis discharges when compared to men(1.39/10000 vs 0.9/10000 in 1997 and 5.8/10000vs 3/10000 in 2013).There was a 6-fold increase in the discharge diagnosis of gastroparesis amongst type 1 DM and 3.7-fold increase amongst type 2 DM patients over the study period(P<0.001).CONCLUSION The number of inpatient admissions for gastroparesis and associated costs have increased significantly over the last 16 years.Inpatient costs associated with gastroparesis contribute significantly to the national healthcare bill.Further research on cost-effective evaluation and management of gastroparesis is required. | Vaibhav Wadhwa Dhruv Mehta Yash Jobanputra Rocio Lopez Prashanthi N Thota Madhusudhan R Sanaka | 2017 | World Journal of Gastroenterology2017,23,24: | 3 |

| 14 | Efficacy of dietary phytase supplementation on laying performance and expression of osteopontin and calbindin genes in eggshell gland显示文摘This study was conducted to evaluate the effects of different levels of dietary phytase supplementation in the layer feed on egg production performance,egg shell quality and expression of osteopontin(OPN) and calbindin(CALB1) genes.Seventy-five White Leghorn layers at 23 weeks of age were randomly divided into 5 groups consisting of a control diet with 0.33% non-phytate phosphorus(NPP) and 4 low phosphorus(P) diets: 2 diets(T1 and T2) with 0.24% NPP t 250 FTU/kg laboratory produced phytase or commercial phytase and another 2 diets(T3 and T4) with 0.16% NPP t 500 FTU/kg laboratory produced phytase or commercial phytase with complete replacement of inorganic P.The results indicated that there were no significant differences(P > 0.05) in egg production performance and quality of egg during the first 2 months of trial.However,in next 2 months,a significant drop in egg production and feed intake was observed in birds fed diets with low P and 500 FTU/kg supplementation of laboratory produced phytase.Osteopontin gene was up-regulated whereas the CALB1 gene was down regulated in all phytase treatment groups irrespective of the source of phytase.The current data demonstrated that250 FTU/kg supplementation of laboratory produced phytase with 50% less NPP supplementation and500 FTU/kg supplementation of commercial phytase even without NPP in diet can maintain the egg production.The up-regulation of OPN and down regulation of CALB1 in egg shell gland in the entire phytase treated group birds irrespective of the source of enzymes is indicative of the changes in P bioavailability at this site. | Divya Shet Jyotirmoy Ghosh Sreeja Ajith Vaibhav B.Awachat Arumbackam V.Elangovan | 2018 | Animal Nutrition2018,,1: | 2 |
| 15 | Re-look to conventional techniques for trapping efficiency estimation of a reservoir显示文摘All reservoirs are subjected to sediment inflow and deposition up to a certain extent leading to reduction in their capacity. Thus, the important practical problem related to the life of reservoir is the estimation of sedimentation quantity in the reservoirs. Large number of methods and models are available for estimation of reservoir sedimentation process. However, each model differs greatly in terms of their complexity, inputs and other requirements. In the simplest way, the fraction of sediment deposit in the reservoir can be determined through the knowledge of its trap efficiency. Trap efficiency (Te) is the proportion of the incoming sediment that is deposited or trapped in a reservoir. Most of the Te estimation methods define a relationship of the Te of the reservoir to their capacity and annual inflow, generally through curves. In this study, the empirical relationships given by Brune and Brown were used and compared for estimating the trap efficiency of Gobindsagar Reservoir (Bhakra Dam) on Satluj River in Bilaspur district of Himachal Pradesh, in the Himalayan region of India. A new set of regression equations has been developed for Brune's method and compared with Brown and other available Brune's equations. It has been found that Brune's equations developed in the present study estimated better than the other Brune's equations reported in literature. Later, in the present study it was found that Brown's approach was over estimating the Te. Hence it was again modified for Gobindsagar reservoir. It was also identified that sediments coming to this particular reservoir were mainly of coarse nature. | V. | 16 | A mass-customizable dermal patch with discrete colorimetric indicators for personalized sweat rate quantification显示文摘In this paper,we present a disposable,colorimetric,user-friendly and mass-customizable dermal patch for chronological collection and discrete real-time in situ measurement of sweat secretion over a small area of skin.The patch consists of a laminated filter paper patterned into radially arranged channels/fingers with water-activated dyes at their tips.As channels are filled during perspiration,their tips change color once fully saturated,providing easily identifiable levels of water loss which in turn can be mapped to personal dehydration levels.The patch can be manufactured at low cost in a variety of sizes to allow hydration monitoring for individuals participating in activities under different conditions(intensity,temperature,humidity,etc.).Furthermore,we describe an analytical model that enables mass customization of such a flexible wearable system accommodating a broad range of sweat rates and volumes to generate patch designs that are personalized to an individual’s sweat rate,desired time of usage,and the temporal resolution of the required feedback.As a proof-of-concept demonstration,we characterized laser-fabricated patches that cover(7 cm×5 cm)area of skin having various wicking materials,thicknesses(180–540μm),and pore sizes(3–11μm).Tests were conducted at various flow rates simulating different sweating intensities in the range of 1.5–15 mg/cm^(2)/min.Experimental results for the case of a half-marathon runner targeting 90 min of usage and sweating at a rate of 1.5 mg/cm2/min indicated measurement accuracy of 98.3%when the patch is completely filled. | Vaibhav Jain Manuel Ochoa Hongjie Jiang Rahim Rahimi Babak Ziaie | 2019 | Microsystems & Nanoengineering2019,5,1: | 2 |
| 17 | Shock wave mitigation using zig-zag structures and cylindrical obstructions显示文摘The present study focuses on the mitigation of shock wave using novel geometric passages in the flow field.The strategy is to produce multiple shock reflections and diffractions in the passage with minimum flow obstruction,which in turn is expected to reduce the shock wave strength at the target location.In the present study the interaction of a plane shock front(generated from a shock tube)with various geometric designs such as,1)zig-zag geometric passage,2)staggered cylindrical obstructions and 3)zigzag passage with cylindrical obstructions have been investigated using computational technique.It is seen from the numerical simulation that,among the various designs,the maximum shock attenuation is produced by the zig-zag passage with cylindrical obstructions which is then followed by zig-zag passage and staggered cylindrical obstructions.A comprehensive investigation on the shock wave reflection and diffraction phenomena happening in the proposed complex passages have also been carried out.In the new zig-zag design,the initial shock wave undergoes shock wave reflection and diffraction process which swaps alternatively as the shock front moves from one turn to the other turn.This cyclic shock reflection and diffraction process helps in diffusing the shock wave energy with practically no obstruction to the flow field.It is found that by combining the shock attenuation ability of zig-zag passage(using shock reflection and diffraction)with the shock attenuation ability of cylindrical blocks(by flow obstruction),a drastic attenuation in shock strength can be achieved with moderate level of flow blocking. | Arun Kumar R Vaibhav Pathak | 2021 | Defence Technology(防务技术)2021,17,6: | 2 |

Simulation is performed at 70 nm technology node with supply voltage 1V for domino logic and DOIND logic based AND, OR, XOR and Half Adder circuits using the tanner EDA tool. Simulation results show that the proposed DOIND approach decreases the average leakage current by 68.83%, 66.6%, 77.86% and 74.34% for 2 input AND, OR, XOR and Half Adder respectively.
