| 1 | Update on hepatocellular carcinoma: Pathologists' review显示文摘Histopathologic diversity and several distinct histologic subtypes of hepatocellular carcinoma(HCC) are well-recognized. Recent advances in molecular pathology and growing knowledge about the biology associated with distinct histologic features and immuno-profile in HCC allowed pathologists to update classifications. Improving sub-classification will allow for more clinically relevant diagnoses and may allow for stratification into biologically meaningful subgroups. Therefore, immuno-histochemical and molecular testing are not only diagnostically useful, but also are being incorporated as crucial components in predicting prognosis of the patients with HCC. Possibilities of targeted therapy are being explored in HCC, and it will be important for pathologists to provide any data that may be valuable from a theranostic perspective. Herein, we review and provide updates regarding the pathologic sub-classification of HCC.Pathologic diagnostic approach and the role of biomarkers as prognosticators are reviewed. Further, the histopathology of four particular subtypes of HCC:Steatohepatitic, clear cell, fibrolamellar and scirrhous-and their clinical relevance, and the recent consensus on combined HCC-cholangiocarcinoma is summarized. | 3 | Apple dwarfing rootstocks exhibit an imbalance in carbohydrate allocation and reduced cell growth and metabolism显示文摘Apple dwarfing rootstocks cause earlier shoot termination and reduced root and shoot mass.To identify physiological factors responsible for rootstock-induced growth restriction,we compared vascular-enriched gene expression between two dwarfing rootstocks(‘M27’and‘M9’)and the vigorous rootstock‘M793’using RNA sequencing and quantitative reverse transcriptase PCR.Differentially expressed genes common to both dwarfing rootstocks belonged to five main biological processes:(1)primary metabolism,(2)cell wall synthesis and modification,(3)secondary metabolism,(4)hormone signalling and response and(5)redox homeostasis.Genes promoting the biosynthesis of amino acids,lipids and cell walls were downregulated in dwarfing rootstocks,whereas genes promoting the breakdown of these compounds were upregulated.The only exception to this trend was the upregulation of starch synthesis genes in dwarfing rootstocks.Non-structural carbohydrate analysis demonstrated that starch concentrations in‘M9’roots,stems and grafted‘Royal Gala’(‘RG’)scions were double that of equivalent tissues from‘RG’homo-grafted trees(‘RG’/‘RG’).Fructose and glucose concentrations were much lower in all three tissues of the‘RG’/‘M9’trees.Together,these data indicate that dwarfing rootstocks are in a state of sugar depletion and reduced cellular activity despite having large starch reserves.Another significant finding was the over-accumulation of flavonoids and the downregulation of auxin influx transporters MdAUX1 and MdLAX2 in dwarfing rootstocks.We propose that both factors reduce polar auxin transport.The results of this study contribute novel information about the physiological state of dwarfing rootstocks. | Toshi M Foster Peter A McAtee Chethi N Waite Helen L Boldingh Tony K McGhie | 2017 | Horticulture Research2017，4，1： | 4 |

| 5 | Aspartate transaminase to platelet ratio index in hepatitis C virus and Schistosomiasis coinfection显示文摘AIM: To assess the diagnostic accuracy,of aminotransferase-to-platelet ratio index(APRI) alone and with antischistosomal antibody(Ab) in patients with hepatitis C virus(HCV) and schistosomiasis coinfection. METHODS: This retrospective study included medical records of three hundred and eighty three Egyptianmen patients who had undergone percutaneous liver biopsy between January 2006 to April 2014 in tertiary care hospital in Qatar for diagnosis or monitoring purpose were selected. Data of patients > 18 years of age were included in the study. The values of HCV RNA titer and antischistosomal antibody titer were also taken into consideration. Patients were excluded from the study if they had any other concomitant chronic liver disease,including; history of previous antiviral or interferon therapy,immunosuppressive,therapy,chronic hepatitis B infection,human immunodeficiency virus co-infection,autoimmune hepatitis,decompensated liver disease,hepatocellular carcinoma,prior liver transplantation,and if no data about the liver biopsy present. RESULTS: Median age of patients was 46 years. About 7.1% had no fibrosis,whereas 30.4%,37.5%,20.4%,and 4.6% had fibrosis of stage Ⅰ,Ⅱ,Ⅲ,and Ⅳ respectively. In bivariate analysis,APRI score,levels of AST,platelet count and age of patient showed statistically significant association with liver fibrosis(P < 0.0001); whereas antischistosomal antibody titer(P = 0.52) and HCV RNA titer(P = 0.79) failed to show a significant association. The respective AUC values for no fibrosis,significant fibrosis,severe fibrosis and cirrhosis of APRI score were 63%,73.2%,81.1% and 88.9% respectively. This showed good sensitivity and specificity of APRI alone for grading of liver fibrosis. But the inclusion of anti-Schistosoma antibody did not improve the prediction of fibrosis stage. CONCLUSION: The study results suggest that noninvasive biochemical markers like APRI are sensitive and specific in diagnosing the degree of fibrosis and cirrhosis in patients with coinfection of HCV and schistosomiasis as compared to biopsy.
