1Does a staged treatment of high energy tibial plateau fractures affect functional results and bony union?A case series显示文摘Purpose:Tibial plateau fracture(TPF)is a devastating injury as it shatters lower articular surface of the largest joint.Apart from bony injury,TPF can lead to great soft tissue envelope compromise which affects the treatment plan and outcome.In the present study,clinical results were assessed in cases of high energy TPFs treated in staged manner.Methods:Twenty-three(20 males and 3 females)patients of high energy communited TPFs(Schatzker typeⅤandⅥ)were consecutively treated.1 All the patient had compromise of overlying skin conditions.They were all successively scheduled for staged treatment plan which comprised of application of bridging knee external fixator on the first day of admission and definitive internal fixation after skin and soft tissue overlying the fracture were healed.Schatzker typeⅠ,Ⅱ,ⅢandⅣwere excluded from the study.Primary survey was done and patient who had head injury,chest and abdominal injury,pelvic injury and contralateral limb injury and open fractures were excluded from the study.The patients were also evaluated in terms of wound complications,axial and rotary alignment of limb,fixation failure,articular congruity and range of motion of the knees and post injury employment.Statistical analysis was done using SPSS software.Results:Maximum follow-up period was 13 months.All the fractures were united at final follow-up.Clinical evaluation was done with the Tegner Lysholm knee scoring scale.2 Excellent results were found in 78%cases and good and fair results in 22%cases.There was significant correlation between range of motion and the Tegner Lysholm knee score(p<0.001,Pearson correlation coefficient=0.741).The correlation between the score and the radiographical union duration was significant(p=0.006,Pearson correlation coefficient=-0.554).Conclusion:A staged treatment plan allows healing of soft tissue envelope,with avoidance of dreadful complications such as compartment syndrome and chronic infection.In addition,a staged treatment strategy does not hamper the fracture reduction,bony union and the functional results.Nilesh Barwar Abhay Elhence Sumit Banerjee Nitesh gahlot 2020Chinese Journal of Traumatology2020,23,4:7
2Differential methylation landscape of pancreatic ductal adenocarcinoma and its precancerous lesions显示文摘Background:Pancreatic cancer is one of the most lethal diseases with an incidence almost equal to the mortality.In addition to having genetic causes,cancer can also be considered an epigenetic disease.DNA methylation is the premier epigenetic modification and patterns of aberrant DNA methylation are recognized to be a common hallmark of human tumor.In the multistage carcinogenesis of pancreas starting from precancerous lesions to pancreatic ductal adenocarcinoma(PDAC),the epigenetic changes play a significant role.Data sources:Relevant studies for this review were derived via an extensive literature search in Pub Med via using various keywords such as pancreatic ductal adenocarcinoma,precancerous lesions,methylation profile,epigenetic biomarkers that are relevant directly or closely associated with the concerned area of our interest.The literature search was intensively done considering a time frame of 20 years(1998–2018).Result:In this review we have highlighted the hypermethylation and hypomethylation of the precancerous PDAC lesions(pancreatic intra-epithelial neoplasia,intraductal papillary mucinous neoplasm,mucinous cystic neoplasm and chronic pancreatitis)and PDAC along with the potential biomarkers.We have also achieved the early epigenetic driver that leads to progression from precancerous lesions to PDAC.A bunch of epigenetic driver genes leads to progression of precancerous lesions to PDAC(pp ENK,APC,p14/5/16/17,h MLH1 and MGMT)are also documented.We summarized the importance of these observations in therapeutics and diagnosis of PDAC hence identifying the potential use of epigenetic biomarkers in epigenetic targeted therapy.Epigenetic inactivation occurs by hypermethylation of Cp G islands in the promoter regions of tumor suppressor genes.We listed all hyper-and hypomethylation of Cp G islands of several genes in PDAC including its precancerous lesions.Conclusions:The concept of the review would help to understand their biological effects,and to determine whether they may be successfully combined with other epigenetic drugs.However,we need to continue our research to develop more specific DNA-demethylating agents,which are the targets for hypermethylated Cp G methylation sites.Akash Bararia Subhankar Dey Sumit Gulati Supriyo Ghatak Shibajyoti Ghosh Sudeep Banerjee Nilabja Sikdar 2020Hepatobiliary & Pancreatic Diseases International2020,19,3:0
