| 1 | Hepatitis B virus,HBx mutants and their role in hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC)is one of the leading causes of death induced by cancer in the modern world and majority of the cases are related to chronic hepatitis B virus(HBV)infection.HBV-encoded X protein(HBx)is known to play a pivotal role in the pathogenesis of viral induced HCC.HBx is a multifunctional protein of17 kDa which modulates several cellular processes by direct or indirect interaction with a repertoire of host factors resulting in HCC.HBX might interfere with several cellular processes such as oxidative stress,DNA repair,signal transduction,transcription,protein degradation,cell cycle progression and apoptosis.A number of reports have indicated that HBx is one of the most common viral ORFs that is often integrated into the host genome and its sequence variants play a crucial role in HCC.By mutational or deletion analysis it was shown that carboxy terminal of HBx has a likely role in protein-protein interactions,transcriptional transactivation,DNA repair,cell,signaling and pathogenesis of HCC.The accumulated evidence thus far suggests that it is difficult to understand the mechanistic nature of HBx associated HCC,and HBx mediated transcriptional transactivation and signaling pathways may be a major determinant.This article addresses the role of HBx in the development of HCC with particular emphasis on HBx mutants and their putative targets. | Ashraf Ali Hany Abdel-Hafiz Mohd Suhail Amany Al-Mars Mohammad Khalid Zakaria Kaneez Fatima Sultan Ahmad Esam Azhar Adeel Chaudhary Ishtiaq Qadri | 2014 | World Journal of Gastroenterology2014,20,30: | 29 |
| 2 | Predictors of long term survival after hepatic resection for hilar cholangiocarcinoma:A retrospective study of 5-year survivors显示文摘AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived < 5 years.METHODS:This is a retrospective study of patients with pathologically proven HC who underwent surgical resection at the Gastroenterology Surgical Center,Mansoura University,Egypt between January 2002 and April 2013.All data of the patients were collected from the medical records.patients were divided into two groups according to their survival:patients surviving less than 5 years and those who survived > 5 years.RESULTS:There were 34(14%) long term survivors(5 year survivors) among the 243 patients.Fiveyear survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two groups.The level of CA 19-9 was significantly higher in patients surviving < 5 years(395.71 ± 31.43 vs 254.06 ± 42.19,p = 0.0001).Univariate analysis demonstrated nine variables to be significantly associated with survival > 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival. | Mohamed Abd El Wahab Ayman El Nakeeb Ehab El Hanafy Ahmad M Sultan Ahmed Elghawalby Waleed Askr Mahmoud Ali Mohamed Abd El Gawad Tarek Salah | 2016 | World Journal of Gastrointestinal Surgery2016,8,6: | 10 |
| 3 | Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy显示文摘AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver cirrhosis.METHODS:We studied retrospectively all patients who underwent PD in our centre between January 2002and December 2011.Group A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic livers.The cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were collected.The primary outcome measure was hospital mortality rate.Secondary outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival rate.RESULTS:Only 67/442 patients(15.2%)had cirrhotic livers.Intraoperative blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic patients.Postoperative stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group B.Portal hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with PHT.CONCLUSION:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver cirrhosis.PD is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension. | Ayman El Nakeeb Ahmad M Sultan Tarek Salah Mohamed El Hemaly Emad Hamdy Ali Salem Ahmed Moneer Rami Said Ahmed AbuEleneen Mostafa Abu Zeid Talaat Abdallah Mohamed Abdel Wahab | 2013 | World Journal of Gastroenterology2013,19,41: | 3 |

| 9 | Disaster Collaborative Exercises for Healthcare Teamwork in a Saudi Context显示文摘This study aimed to evaluate the development of healthcare teamwork during and after the collaboration tabletop exercises,through observation and interview methods.Integration and maturity theoretical models were employed to explain the collaborative challenges in teams that may suffer from unequally distributed power,hierarchies,and fragmentation.Using three-level collaboration tabletop exercises and the Command and control,Safety,Communication,Assessment,Treatment,Triage,Transport(CSCATTT)instrument,100 healthcare workers were observed during each step in the implementation of the CSCATTT instrument using two simulated scenarios.The results show a lack of integration and team maturity among participants in the first scenario,leading to the delayed start of the activity,task distribution,and decision making.These shortcomings were improved in the second scenario.In-depth interviews with 20 participants in the second phase of the study revealed improved knowledge and practical skills,self-confidence,and ability in team building within trans-professional groups in the second scenario,which in concordance with the integration theory,was due to the attempts made in the first scenario.Additionally,there was an improvement in the team’s maturity,which in concordance with the maturity theory,was due to the knowledge and practical skills during scenario plays.These results indicate the importance of continuous tabletop training,and the use of CSCATTT as a collaborative instrument,to promote the development of collaboration and to test the concept of preparedness. | Mohammed Ali Salem Sultan Amir Khorram-Manesh Jarle Lφwe Sφrensen Johan Berlin Eric Carlstrōm | 2023 | International Journal of Disaster Risk Science2023,14,2: | 1 |

| 13 | COVID-19 and comorbidities of hepatic diseases in a global perspective显示文摘The worldwide outbreak of coronavirus disease 2019(COVID-19) has challenged the priorities of healthcare system in terms of different clinical management and infection transmission, particularly those related to hepatic-disease comorbidities. Epidemiological data evidenced that COVID-19 patients with altered liver function because of hepatitis infection and cholestasis have an adverse prognosis and experience worse health outcomes. COVID-19-associated liver injury is correlated with various liver diseases following a severe acute respiratory syndrome-coronavirus type 2(SARS-CoV-2) infection that can progress during the treatment of COVID-19 patients with or without pre-existing liver disease. SARS-CoV-2 can induce liver injury in a number of ways including direct cytopathic effect of the virus on cholangiocytes/hepatocytes, immune-mediated damage, hypoxia, and sepsis. Indeed, immediate cytopathogenic effects of SARSCoV-2 via its potential target, the angiotensin-converting enzyme-2 receptor, which is highly expressed in hepatocytes and cholangiocytes, renders the liver as an extra-respiratory organ with increased susceptibility to pathological outcomes. But, underlying COVID-19-linked liver disease pathogenesis with abnormal liver function tests(LFTs) is incompletely understood. Hence, we collated COVID-19-associated liver injuries with increased LFTs at the nexus of pre-existing liver diseases and COVID-19, and defining a plausible pathophysiological triad of COVID-19, hepatocellular damage, and liver disease. | 14 | Opposite Modulatory Effects of Crataegus aronia Aqueous Extract on Platelet Aggregation in Rats显示文摘Objectives:To reveal the mechanisms behind the dual effects of Crataegus aronia(C.aronia)aqueous extract on platelet aggregation by focusing on function,regulation,expression,and signaling of platelets P_(2)Y_(12)receptors.Methods:Adult male Wistar rats(120±10 g)were classified as control received the vehicle,C.aronia(200 mg/kg),and C.aronia(2,000 mg/kg)-treated rats.After treatments for consecutive 7 days,hematological and molecular experiments were conducted to detect alterations in platelet aggregation,thromboxane B2(THXB2)and intracellular reactive oxygen species(ROS)content;protein levels of P_(2)Y_(12),p-Akt,cyclic adenosine monophosphate(c AMP),phosphorylated vasodilator-stimulated-phosphoprotein(p-VASP),nuclear factorκB(NF-κB),P-selectin,etc.in platelets were determined by Western blot;m RNA expressions of P_(2)Y_(12)and some inflammatory markers were determined by real-time polymerase chain reaction.Results:At a concentration of 200 mg/kg,C.aronia inhibited platelet aggregation through multiple interconnected mechanisms including downregulation P_(2)Y_(12)synthesis and expression,stimulating intracellular cAMP levels and protein levels of p-VASP,inhibiting platelets THXB2 release and protein levels of P-selectin.Also,it inhibited platelets level of ROS and of NF-κB,a major signaling pathway that stimulates the expression of P_(2)Y_(12)and THXA2 synthesis.Opposite findings were seen in platelets of rats received C.aronia at a concentration of 2,000 mg/kg.Interestingly,co-administration of N-acetylcysteine prevented all hematological and molecular alterations exerted by the high dose of the extract and inhibited platelet aggregation.Conclusion:Oral administration of C.aronia at low dose inhibits platelet aggregation by reducing THXB2 release,expression of P-selectin and activating c AMP and Akt signaling through two major mechanisms including downregulation of P_(2)Y_(12)and inhibition of ROS-induced activation of NF-κB,an effect that is observed to be in the opposite direction with its high dose. | Abdullah S.Shatoor Ali Shati S.Al Humayed Sultan AL-Qahtani Mahmoud Alkhateeb | 2021 | Chinese Journal of Integrative Medicine2021,27,9: | 1 |
| 15 | Numerical Simulation and Experimental Verification of CMOD in SENT Specimen: Application on FCGR of Welded Tool Steel显示文摘Single-edged notched tension (SENT) specimen is used to study the fatigue crack growth rate (FCGR) behavior of AISI 50100 steel using MTS 810. Calibration tests are run to get plots of crack mouth opening displacement (CMOD) vs. load and CMOD vs. crack length-to-width ratio with the known crack lengths. Numerical simulation is also done to try to establish a relation between crack length and CMOD. FCGR of welded and un-welded specimens are plotted against stress intensity factor range to show the effect of welding on fatigue crack growth rate of AISI 50100 steel. The experimentally obtained CMOD values are compared with values obtained by numerical simulation using ABAQUS/StandardTM software package. | 16 | First report on molecular characterization of Leishmania species from cutaneous leishmaniasis patients in southern Khyber Pakhtunkhwa province of Pakistan显示文摘Objective: To report presence of Leishmania major in Khyber Pakhtunkhwa of Pakistan, where cutaneous leishmaniasis(CL) is endemic and was thought to be caused by Leishmania tropica only. Methods: Biopsy samples from 432 CL suspected patients were collected from 3 southern districts of Khyber Pakhtunkhwa during years 2011–2016. Microscopy on Giemsa stained slides were done followed by amplification of the ribosomal internal transcribed spacer 1 gene. Results: Leishmania amastigotes were detected by microscopy in 308 of 432 samples(71.3%) while 374 out of 432 samples(86.6%) were positive by ribosomal internal transcribed spacer 1 PCR. Subsequent restriction fragment length polymorphism confirmed Leishmania tropica in 351 and Leishmania major in 6 biopsy samples. Conclusions: This study is the first molecular characterization of Leishmania species in southern Khyber Pakhtunkhwa. It confirmed the previous assumptions that anthroponotic CL is the major CL form present in Khyber Pakhtunkhwa province. | 20 | Tailored pancreatic reconstruction after pancreaticoduodenectomy: a single-center experience of 892 cases显示文摘BACKGROUND: Pancreatic reconstruction following pancreaticoduodenectomy(PD) is still debatable even for pancreatic surgeons. Ideally, pancreatic reconstruction after PD should reduce the risk of postoperative pancreatic fistula(POPF) and its severity if developed with preservation of both exocrine and endocrine pancreatic functions. It must be tailored to control the morbidity linked to the type of reconstruction.This study was to show the best type of pancreatic reconstruction according to the characters of pancreatic stump. METHODS: We studied all patients who underwent PD in our center from January 1993 to December 2015. Patients were categorized into three groups depending on the presence of risk factors of postoperative complications: low-risk group(absent risk factor), moderate-risk group(presence of one risk factor) and high-risk group(presence of two or more risk factors). RESULTS: A total of 892 patients underwent PD for resection of periampullary tumor. BMI >25 kg/m^2, cirrhotic liver, soft pancreas, pancreatic duct diameter <3 mm, and pancreatic duct location from posterior edge <3 mm are risk variables for development of postoperative complications. POPF developed in 128(14.3%) patients. Delayed gastric emptying occurred in 164(18.4%) patients, biliary leakage developed in 65(7.3%) and pancreatitis presented in 20(2.2%). POPF in low-, moderate-and high-risk groups were 26(8.3%), 65(15.7%) and 37(22.7%) patients, respectively. Postoperative morbidity and mortality were significantly lower with pancreaticogastrostomy(PG) in high-risk group, while pancreaticojejunostomy(PJ) decreases incidence of postoperative steatorrhea in all groups. CONCLUSIONS: Selection of proper pancreatic reconstruction according to the risk factors of patients may reduce POPF and postoperative complications and mortality.
