1Do we really need to repair the pronator quadratus after distal radius plating?显示文摘Purpose:Fractures of distal radius are one of the common orthopaedic injuries.Placing the plate on volar surface requires release of underlying pronator quadratus(PQ)muscle.No consensus is present in the literature about the repair or not of the PQ.The purpose of this study was to evaluate the influence of PQ repair on functional outcome and complications.Methods:Retrospectively 83 patients of distal radius fractures managed with volar plating between 2014 and 2016 were evaluated.Demographic data,operative notes and physical therapy records were retrieved.Patients were divided into group A where PQ repair was done and group B where no repair was done.Functional data such as range of motion(ROM),grip strength,visual analogue scale(VAS)score and disabilities of arm,shoulder and hand(DASH)score at 4 weeks,3 months,6 months and finial follow-up were retrieved.Results:Totally 63 patients(n=29 in group A and n=34 in group B)with the mean age of 51.64 years were examined.Patients were followed up for a mean of 35.2 months(range 27.2-47.1 months)in group A and 38.6 months(range 28.6-51.0 months)in group B.Though functional outcome of the affected limb was not significantly different between two groups after 3 months,PQ repair did affect the recovery at an early stage.Repair group had significantly better ROM(p=0.0383)and VAS score at 4 weeks(p=0.017)while grip strength(p=0.014)was significantly better at 3 months.Conclusions:Repair of PQ may provide pain relief and increased ROM in early postoperative period and hence every attempt should be made to achieve the repair.Subodh Pathak Rashid Anjum Rakesh Kumar Gautam Pritam Maheshwari Jatin Aggarwal Aryan Sharma Vineet Pruthi 2019Chinese Journal of Traumatology2019,22,6:3

3Reducing shoulder dislocation without anaesthesia or assistant:Validation of a new reduction manoeuvre显示文摘Purpose:Shoulder dislocation comprises 60%of all major joint dislocations worldwide and a number of reduction techniques are described in the literature w让h varying degrees of success.The description of a large number of techniques speaks for itself that no method is effective all the times and one should be acquainted with more than one technique.An ideal method of reduction should be simple,easily reproducible,relatively painless that can be performed unassisted without sedation or anaesthesia w让h minimal or no further complications.We report our results of using a novel method of anterior shoulder reduction described recently in the literature that claims to fulfil most of the characteristics of an ideal method if not all.Methods:This prospective study was conducted in a tertiary care centre.All the cases of primary anterior shoulder dislocation presenting within three days of injury without any associated fracture or spine trauma with or without greater tuberosity fracture were included.The reduction was done using a novel method by orthopaedic residents in all cases.The need for a second reduction attempt or anaesthesia was considered a treatment failure.Time taken for reduction,pain felt during reduction and complications if any were noted.Results:There were 47(77.04%)males and 14(22.95%)females with a mean age of(37.04±12.63)years.The new technique was effective in locating a shoulder dislocation on the first attempt in 58 of the 61 dislocated shoulders(95.08%).The remaining three shoulders were reduced on second attempt by the same technique.Sedative,pre-medication or anaesthesia was not used in any case.The average time taken for the shoulder reductio n was(130.5±25.8)sec onds and con fide nee interval(95%)124-137 s.Iatrogenic complications were not seen in any of the patients.Conclusion:This relatively painless technique of shoulder reduction is easy to acquire and practice in emergency department.The advantages of this manoeuvre and its associated safety may justly lead surgeons to select it as their primary method for reduction of anterior shoulder dislocations.Rashid Anjum Subodh Pathak Atul Rai Sharma Jatin Aggarwal Aryan Sharma Vineet Pruthi Anil Kumar Chaudhary 2019Chinese Journal of Traumatology2019,22,5:1

6Study of Substrate and Physico-Chemical Base Classification of the Rivers of Nepal显示文摘The rivers in Nepal are classified in terms of geographical regions but a more scientific classification such as on the ba-sis of morphology is clearly lacking. This study was done in 9 rivers namely Jhikhukhola of the Koshi system, Aandhikhola, Arungkhola, East Rapti, Karrakhola, Seti and main channel Narayani of the Gandaki system, and two independent systems within Nepal, Bagmati and Tinau. Among the morphologies, river bed or the substratum was taken as the main variable for the analysis which was categorized into 7 types as rocks, boulders, cobbles, pebbles, gravels, sand and silt. There were 23 sampling sites each with 2 stretches of around 100m in those rivers. The data were taken as a percentage, and to avoid biases it was observed visually by the same person for a complete year in every season. With 23 sites each with 2 stretches and 4 replicates corresponding to 4 seasons, there are altogether 184 observations, each termed as a case, that constitute this work. Canonical Discrimination Analysis (CDA) which is most suitable when the data pool is huge was applied to see if the rivers studied distinguish themselves in terms of its morphology. The result was remarkably successful and was close to the established regional classification of the rivers. 8Contrasting lipid biomarkers in mountain rivers in the Nepal Himalayas: Organic matter characteristics and contribution to the fluvial carbon pool显示文摘The Nepal Himalayas is the source of many glacial and spring-fed river systems crisscrossing the mountainous terrain.There is an increasing recognition of small mountain rivers(SMRs)to have a significant combined export of dissolved and particulate organic carbon to the global carbon flux.We analyzed fluvial sediments from two SMRs and compared the results with two large mountain rivers(LMRs)in Nepal.We investigated the organic matter(OM),its compositional variability,and seasonal export using a suite of lipid biomarkers,namely n-alkanes,n-alkanoic acids,n-alkanols,and sterols.The SMRs indicated a similarity in lipid distribution and were affected by a strong seasonal variability.The LMRs showed a distinct contrast in the distribution of lipids in suspended sediments.Bedload sediments in SMRs were derived from diverse sources with weak terrigenous dominance all-year-round compared to the suspended load.Functional lipids(n-alkanoic acids and n-alkanols)were the major constituents in SMR sediments,indicating better preservation.In contrast,n-alkane concentration dominated over other fractions in suspended sediments retrieved from LMRs.The biomarker trends differentiate SMRs from LMRs with lower transformed/degraded OM in SMRs.A common observation was the strong presence of even carbon compounds in short-chain n-alkanes in SMR bedload sediments and their predominance in suspended sediments in LMRs.Such an unusual trend is attributed to specific biomarker sources from the catchment and ongoing processes in fluvial systems.Topsoil colonized by fungal species under moist acidic conditions and autochthonous bacteria contributes to the organic matter pool in shallow SMRs.In LMRs,the contribution from thermally mature sedimentary hydrocarbons and the diagenetic reduction of nalkanoic acids to n-alkanes are additional contributors to the allochthonous carbon pool.The differences in lipid concentrations,their distribution,seasonality,and the size of rivers suggest differential preservation/degradation of the organic matter pool and their importance in contributing to the carbon budget.Rajendra Bhandari Joyanto Routh Subodh Sharma Rajendra Joshi 2021Geoscience Frontiers2021,12,6:0
9MicroRNA-502-3p regulates GABAergic synapse function in hippocampal neurons显示文摘Gamma-aminobutyric acid(GABA)ergic neurons,the most abundant inhibitory neurons in the human brain,have been found to be reduced in many neurological disorders,including Alzheimer's disease and Alzheimer's disease-related dementia.Our previous study identified the upregulation of microRNA-502-3p(miR-502-3p)and downregulation of GABA type A receptor subunitα-1 in Alzheimer's disease synapses.This study investigated a new molecular relationship between miR-502-3p and GABAergic synapse function.In vitro studies were perfo rmed using the mouse hippocampal neuronal cell line HT22 and miR-502-3p agomiRs and antagomiRs.In silico analysis identified multiple binding sites of miR-502-3p at GABA type A receptor subunitα-1 mRNA.Luciferase assay confirmed that miR-502-3p targets the GABA type A receptor subunitα-1 gene and suppresses the luciferase activity.Furthermore,quantitative reve rse transcription-polymerase chain reaction,miRNA in situ hybridization,immunoblotting,and immunostaining analysis confirmed that overexpression of miR-502-3p reduced the GABA type A receptor subunitα-1 level,while suppression of miR-502-3p increased the level of GABA type A receptor subunitα-1 protein.Notably,as a result of the overexpression of miR-502-3p,cell viability was found to be reduced,and the population of necrotic cells was found to be increased.The whole cell patch-clamp analysis of human-GABA receptor A-α1/β3/γ2L human embryonic kidney(HEK)recombinant cell line also showed that overexpression of miR-502-3p reduced the GABA current and overall GABA function,suggesting a negative correlation between miR-502-3p levels and GABAergic synapse function.Additionally,the levels of proteins associated with Alzheimer s disease were high with miR-502-3p overexpression and reduced with miR-502-3p suppression.The present study provides insight into the molecular mechanism of regulation of GABAergic synapses by miR-502-3p.We propose that micro-RNA,in particular miR-502-3p,could be a potential therapeutic to rget to modulate GABAergic synapse function in neurological disorders,including Alzheimer's disease and Alzheimer's diseaserelated dementia.Bhupender Sharma Melissa MTorres Sheryl Rodriguez Laxman Gangwani Subodh Kumar 2024Neural Regeneration Research2024,19,12:0
10尼泊尔Andhi Khola河研究参照点的识别及确认(英文)显示文摘参照条件是开展河流生态学研究的基础,这些条件不受或很少受到人为干扰。为研究尼泊尔Andhi Khola河上一处河坝的生态影响,采用野外快速生物筛分(RFB)法预先划分出2个研究参照点或最少干扰参照点,于2013年1月和2月采集了生物学样品(大型脊椎动物)和物理-化学样品。对大型无脊椎动物区系使用多栖息地采样(MHS)法。通过一些指标对预选的研究点进行了确认,这些指标是:尼泊尔生物评价法(NEPBIOS)、生物监测评价法(BMWP)、兴都库什喜马拉雅生物评价法(HKHBIOS)、Hilsenhoff法(HILSENHOFF),以及国家卫生基金会水质指标法(NSFWQI)。NEPBIOS、HKHBIOS、HILSENHOFF、RFB以及NSFWQI指标预测2个参照点的河水水质都比较好,达到II级水准。只有BMWP/ASPT水质指标预测2个参照点的河水水质都很好,达到I级水准。如此,预选的这2个水质较好的点(II级)被确认为研究参照点。本研究表明,多度量法适用于即将修建水利工程的河流的监测与评价。Praveen SHARMA Subodh SHARMA Smriti GURUNG 2015Journal of Resources and Ecology2015,6,1:0
