1Octreotide in Hennekam syndrome-associated intestinal lymphangiectasia显示文摘A number of disorders have been described to cause protein losing enteropathy (PLE) in children. Primary intestinal lymphangiectasia (PIL) is one mechanism leading to PLE. Few syndromes are associated with PIL; Hennekam syndrome (HS) is one of them. The principal treatment for PIL is a high protein, low fat diet with medium chain triglycerides supplementation. Supportive therapy includes albumin infusion. Few publications have supported the use of octreotide to diminish protein loss and minimize hypoalbuminemia seen in PIL. There are no publications on the treatment of PIL with octreotide in patients with HS. We report two children with HS and PLE in which we used octreotide to decrease intestinal protein loss. In one patient, octreotide increased serum albumin to an acceptable level without further need for albumin infusions. The other patient responded more dramatically with near normal serum albumin levels and cessation of albumin infusions. In achieving a good response to octreotide in both patients, we add to the publications supporting the use of octreotide in PIL and suggest that octreotide should be tried in patients with PIL secondary to HS. 2Organosolv Lignin for Non-Isocyanate Based Polyurethanes (NIPU) as Wood Adhesive显示文摘A non-isocyanate-based polyurethane(NIPU)wood adhesive was produced from organosolv lignin,which is a bio-sourced raw material,available in large quantities and produced as a by-product of the paper industry.The formulation of this new lignin-based NIPU adhesive,which is presented,was chemically characterised by Matrix-Assisted Laser Desorption Ionization Time of Flight(MALDI ToF)mass spectrometry and by Fourier Transform Infra-Red(FTIR)spectrometry analyses.The oligomers formed were determined and showed that the three species involved in the NIPU adhesive preparation were formed by the co-reaction of the three reagents used:lignin,dimethyl carbonate,and hexamethylene diamine.Linear and branched structures were both identi-fied.Mechanical properties of the adhesive were determined using the Automated Bonding Evaluation System(ABES)and internal bond(IB)strength test of the laboratory particleboard bonded with it.The adhesive has shown satisfactory mechanical properties after hot pressing at 230℃.Such a temperature is used industrially in the most modern particleboard factories,but since it is hardly feasible for more conventional wood bonding equipment,the reactivity of the NIPU adhesive was successfully increased with the addition of a small percentage of a silane coupling agent.With the addition of the silane,the proposed NIPU adhesive could also be used at a hot-pressing temperature lower than 200℃.Jaša Saražin Antonio Pizzi Siham Amirou Detlef Schmiedl MilanŠernek 2021Journal of Renewable Materials2021,9,5:4
3糖尿病患者视网膜神经节细胞复合体和神经纤维层厚度的变化显示文摘目的观察2型糖尿病(T2DM)患者中视网膜神经纤维层(RNFL)和神经节细胞复合体(GCC)厚度的变化,分析其神经结构与微循环之间的相关关系。方法采用横断面回顾性研究,入选2型糖尿病患者45例71眼、健康受试者36例68眼。采用光学相干断层扫描血管成像技术(OCTA)检测黄斑部的GCC厚度、视盘周围RNFL厚度、视网膜脉络膜毛细血管丛的微血管密度。观察不同分期糖尿病视网膜病变(DR)患眼中RNFL和GCC厚度变化,分析它们与微血管密度的相关关系。结果与对照组相比,NDR组(无DR的糖尿病患者)GCC中局部丢失体积(FLV,P=0.00)、整体丢失体积(GLV,P=0.00)显著升高,可能是视网膜神经结构损伤的早期敏感指标。与对照组相比,轻中度非增殖性糖尿病视网膜病变(NPDR)组的下侧和鼻侧象限RNFL显著变薄(P=0.00),可能是RNFL丢失的敏感区域。重度NPDR组中鼻侧象限RNFL仍然显著变薄,但下侧、颞侧象限RNFL显著增厚(P=0.00)。增殖性糖尿病视网膜病变(PDR)组中RNFL和GCC大部分参数普遍增厚(P=0.00),可能与视网膜水肿导致的视网膜增厚有关。Spearman分析显示GCC、RNFL厚度与视网膜厚度呈正相关。此外,GCC及RNFL厚度与视网膜脉络膜毛细血管包括浅层毛细血管丛(SCP)、深层毛细血管丛(DCP)和脉络膜层毛细血管丛(CCP)的密度呈负相关。结论GCC的FLV和GLV局限性丢失,可能是评价早期视网膜神经结构损伤的敏感指标。随DR的进展,RNFL厚度呈现先变薄后增厚的趋势,还需要扩大样本量进一步研究。董宁 Siham Almansoob 高玲 2020中国动脉硬化杂志2020,28,8:4

5Medical research during the COVID-19 pandemic显示文摘The current pandemic of coronavirus disease 2019(COVID-19)which was first detected in Wuhan,China in December 2019 is caused by the novel coronavirus named severe acute respiratory syndrome coronavirus-2(SARS-CoV-2).The virus has quickly spread to a large number of countries leading to a great number of deaths.Unfortunately,till today there is no specific treatment or vaccination for SARS-CoV-2.Most of the suggested treatment medications are based on in vitro laboratory investigations,experimental animal models,or previous clinical experience in treating similar viruses such as SARS-CoV-1 or other retroviral infections.The running of any clinical trial during a pandemic is affected at multiple levels.Reasons for this include patient hesitancy or inability to continue investigative treatments due to self-isolation/quarantine,or limited access to public places(including hospitals).Additional barriers relate to health care professionals being committed to other critical tasks or quarantining themselves due to contact with COVID-19 positive patients.The best research approaches are those that adapt to such external unplanned obstacles.Ongoing clinical trials before COVID-19 pandemic have the potential for identifying important therapies in the long-term if they can be completed as planned.However,these clinical trials may require modifications due a pandemic such as this one to ensure the rights,safety,and wellbeing of participants as well as medical staff involved in the conduction of clinical trials.Clinical trials initiated during the pandemic must be time-efficient and flexible due to high contagiousness of severe acute respiratory syndrome coronavirus 2,the significant number of reported deaths,and time constraints needed to perform high quality clinical trials,enrolling adequate sample sizes.Collaboration between different countries as well as implementation of innovative clinical trial designs are essential to successfully complete such initiatives during the current pandemic.Studies looking at the long term sequalae of COVID-19 are also of importance as recent publications describe multi-organ involvement.Long term follow-up of COVID-19 survivors is thus also important to identify possible physical and mental health sequellae.Khalid AlNaamani Siham AlSinani Alan N Barkun 2020World Journal of Clinical Cases2020,8,15:2
