| 3 | Role of endoscopic ultrasound and cyst fluid tumor markers in diagnosis of pancreatic cystic lesions显示文摘BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant lesions.AIM To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign lesions.METHODS This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different PCLs.RESULTS The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA. Meanwhile, logistic multivariable regression analysis proved that mural nodules, mucin stain, and SPINK1 were independent predictors of malignancy in cystic pancreatic lesions.CONCLUSION EUS examination of cyst morphology with cytopathological analysis and cyst fluid analysis could improve the differentiation between malignant and benign pancreatic cysts. | 6 | 前段光学相干断层扫描在穿透性角膜移植术后移植上皮变化中的应用(英文)显示文摘目的:使用前段光学相干断层扫描(AS-OCT)评估在穿透性角膜移植术(PK)后角膜上皮厚度的变化,并确定上皮厚度在移植排斥反应早期检测中的作用。方法:前瞻性比较观察研究。共纳入20例20眼接受穿透性角膜移植术患者做为研究组,对照组16例16眼。术后2wk、1mo和3mo使用AS-OCT测量角膜厚度。术后3mo,与对照组排斥反应和上皮厚度及分布参数进行比较。结果:术后1mo,研究组的上、下、最大和最小上皮厚度值均显著低于术后2wk(P=0.0004,0.0001,0.0001,0.04),术后3mo与1mo相比无显著差异(P=0.4,0.1,0.8)。与术后2wk相比,1mo时上皮厚度的降低明显高于基质厚度(P=0.04)。角膜上皮厚度变化显示,研究组和对照组的角膜上皮厚度分布模式相似,出现明显的角膜水肿。但是,对照组中央角膜厚度(CCT)明显更高。结论:角膜移植术后移植物上皮重塑的定量评估显示,早期变化有助于角膜移植物厚度的显著改善。角膜上皮厚度和分布模式的变化可作为角膜移植排斥反应的指标。 | Shaimaa A Arfeen Riham SHM Allam Noha M Khalil Ghada A Nassar | 2020 | 国际眼科杂志2020,20,3: | 1 |
| 7 | Type 2 diabetes and thyroid cancer:Synergized risk with rising air pollution显示文摘Diabetes is a complex condition,and the causes are still not fully understood.However,a growing body of evidence suggests that exposure to air pollution could be linked to an increased risk of diabetes.Specifically,exposure to certain pollutants,such as particulate Matter and Ozone,has been associated with higher rates of diabetes.At the same time,air pollution has also been linked to an increased risk of thyroid cancer.While there is less evidence linking air pollution to thyroid cancer than to diabetes,it is clear that air pollution could have severe implications for thyroid health.Air pollution could increase the risk of diabetes and thyroid cancer through several mechanisms.For example,air pollution could increase inflammation in the body,which is linked to an increased risk of diabetes and thyroid cancer.Air pollution could also increase oxidative stress,which is linked to an increased risk of diabetes and thyroid cancer.Additionally,air pollution could increase the risk of diabetes and thyroid cancer by affecting the endocrine system.This review explores the link between diabetes and air pollution on thyroid cancer.We will discuss the evidence for an association between air pollution exposure and diabetes and thyroid cancer,as well as the potential implications of air pollution for thyroid health.Given the connections between diabetes,air pollution,and thyroid cancer,it is essential to take preventive measures to reduce the risk of developing the condition. | Eva M Kruger Shaimaa A Shehata Eman A Toraih Ahmed A Abdelghany Manal S Fawzy | 2023 | World Journal of Diabetes2023,14,7: | 0 |
