| 1 | Handgrip strength and health outcomes: Umbrella review of systematic reviews with meta-analyses of observational studies显示文摘Purpose:The aim of the present study was to assess both the credibility and strength of evidence arising from systematic reviews with meta-analyses of observational studies on handgrip strength and health outcomes.Methods:An umbrella review of systematic reviews with meta-analyses of observational studies was conducted.We assessed meta-analyses of observational studies based on random-effect summary effect sizes and their p values,95%prediction intervals,heterogeneity,small-study effects,and excess significance.We graded the evidence from convincing(Class I)to weak(Class IV).Results:From 504 articles returned in a search of the literature,8 systematic reviews were included in our review,with a total of 11 outcomes.Overall,nine of the 11 of the outcomes reported nominally significant summary results(p<0.05),with 4 associations surviving the application of the more stringent p value(p<106).No outcome presented convincing evidence.Three associations showed Class II evidence(i.e.,highly suggestive):(1)higher handgrip values at baseline were associated with a minor reduction in mortality risk in the general population(n=34 studies;sample size=1,855,817;relative risk=0.72,95%confidence interval(95%CI):0.670.78),(2)cardiovascular death risk in mixed populations(n=15 studies;relative risk=0.84,95%CI:0.780.91),and(3)incidence of disability(n=7 studies;relative risk=0.76,95%CI:0.660.87).Conclusion:The present results show that handgrip strength is a useful indicator for general health status and specifically for early all-cause and cardiovascular mortality,as well as disability.To further inform intervention strategies,future research is now required to fully understand mechanisms linking handgrip strength scores to these health outcomes. | Pinar Soysal Christopher Hurst Jacopo Demurtas Joseph Firth Reuben Howden Lin Yang Mark A.Tully Ai Koyanagi Petre Cristian Ilie Guillermo F.Lopez-Sanchez Lukas Schwingshackl Nicola Veronese Lee Smith | 2021 | Journal of Sport and Health Science2021,10,3: | 5 |

| 5 | Pediatric interfacility transport effects on mortality and length of stay显示文摘Background We aimed to evaluate the effects of interfacility pediatric critical care transport response time,physician presence during transport,and mode of transport on mortality and length of stay(LOS)among pediatric patients.We hypothesized that a shorter response time and helicopter transports,but not physician presence,are associated with lower mortality and a shorter LOS.Methods Retrospective,single-center,cohort study of 841 patients(<19 years)transported to a quaternary pediatric intensive care unit and cardiovascular intensive care unit between 2014 and 2018 utilizing patient charts and transport records.Multivariate linear and logistic regression analyses adjusted for age,diagnosis,mode of transport,response time,stabilization time,return duration,mortality risk(pediatric index of mortality-2 and pediatric risk of mortality-3),and inotrope,vasopressor,or mechanical ventilation presence on admission.Results Four hundred and twenty-eight(50.9%)patients were transported by helicopter,and 413(49.1%)were transported by ambulance.Physicians accompanied 239(28.4%)transports.The median response time was 2.0(interquartile range 1.4–2.9)hours.Although physician presence increased the median response time by 0.26 hours(P=0.020),neither physician presence nor response time significantly affected mortality,ICU length of stay(ILOS)or hospital length of stay(HLOS).Helicopter transports were not significantly associated with mortality or ILOS,but were associated with a longer HLOS(3.24 days,95%confidence interval 0.59–5.90)than ambulance transports(P=0.017).Conclusions These results suggest response time and physician presence do not significantly affect mortality or LOS.This may reflect the quality of pre-transport care and medical control communication.Helicopter transports were only associated with a longer HLOS.Our analysis provides a framework for examining transport workforce needs and associated costs. | Rod M.Shinozaki Andreas Schwingshackl Neeraj Srivastava Tristan Grogan Robert B.Kelly | 2021 | World Journal of Pediatrics2021,17,4: | 1 |
