202021神经修复学年鉴显示文摘今年,神经修复学领域快速发展,不断获得突破。如2020年神经修复学年鉴中所述,尽管冠状病毒持续大流行造成困难重重,但大多数神经修复学同事都继续在从事他们的研究,并取得令人鼓舞的结果。2021年神经修复学年鉴概述了神经系统疾病发病机制、神经修复机制和临床治疗成果的最新进展。阿尔茨海默病的发病机制和危险因素是最突出热点研究课题之一。β-淀粉样蛋白积累和tau蛋白沉积究竟是神经退行性过程的结果,还是致病原因仍然存在争议。神经发生是一种重要的神经修复机制,但是成人大脑中是否存在神经干细胞?故成人的神经发生可能无法来自内源性神经干细胞。神经修复疗法是2021年的重要研究领域,这些疗法正在改善神经系统疾病患者的生存质量。以细胞治疗为基础的神经疾病和损伤修复方面,学者们做了大量重要探索。但大多数多中心、双盲或观察组盲、安慰剂对照、随机临床试验结果显示,间充质基质细胞或间充质干细胞衍生产品治疗亚急性或恢复期缺血性卒中患者并无有效阳性。与对照组相比,试验组的生存质量的改善没有显著差异。令人兴奋的是,越来越多的临床研究显示脑机接口治疗可使神经功能缺损患者受益。在药物神经修复疗法中,阿杜那单抗(Aduhelm)和甘露寡糖钠分别获得美国食品药品监督管理局和中国国家药品监督管理局批准,用于治疗轻度至中度阿尔茨海默病患者。由于药物使用中相关争议问题,批准这些药物的临床使用在医学和科学界引起了广泛争议。经随机对照试验显示阴性结果或有效证据不足的方法,继续在低水平证据重复研究意义不大。因此,我们强烈建议进行多中心、双盲、安慰剂对照、随机临床试验来寻找更有前景的创新治疗方法,并尽可能促进其临床转化。Hari Shanker Sharma Michael Chopp 陈琳 Anna Sarnowska 薛孟州 敖强 Dario Siniscalco 陈鲁葵 Ziad Hawamdeh 黄红云 陈娣(译) 刘娜(译) 陈琳(审校) 2022神经损伤与功能重建2022,17,12:1
