| 1 | Vasopressors in obstetric anesthesia: A current perspective显示文摘Vasopressors are routinely used to counteract hypotension after neuraxial anesthesia in Obstetrics. The understanding of the mechanism of hypotension and the choice of vasopressor has evolved over the years to a point where phenylephrine has become the preferred vasopressor. Due to the absence of definitive evidence showing absolute clinical benefit of one over the other, especially in emergency and high-risk Cesarean sections, our choice of phenylephrine over the other vasopressors like mephentermine, metaraminol, and ephedrine is guided by indirect evidence on fetalacid-base status. | 2 | Comparative analysis of APACHE-Ⅱ and P-POSSUM scoring systems in predicting postoperative mortality in patients undergoing emergency laparotomy显示文摘BACKGROUND Laparotomy remains one of the commonest emergency surgical procedures.Early prognostic evaluation would aid in selecting the high-risk patients for an aggressive treatment.Awareness about risks could potentially contribute to the quality of perioperative care and optimum utilization of resources.Portsmouth modification of Physiological and operative severity for the enumeration of mortality and morbidity (P-POSSUM) and the acute physiology and chronic health evaluation Ⅱ(APACHE-Ⅱ) have been the most widely used scoring systems for emergency laparotomies.It is always better to have a single scoring system to predict outcomes and audit healthcare organizations.AIM To compare the ability of APACHE-II and P-POSSUM to predict postoperative morbidity and mortality in patients undergoing emergency laparotomy.METHODS All patients undergoing emergency laparotomy at the Tata Main Hospital,Jamshedpur between December 2013 and November 2014 were included in the study.In this observational study,P-POSSUM and APACHE-Ⅱ scoring were done,and the outcome analysis evaluated with mortality being the primary outcome.RESULTS For P-POSSUM,at a cut off value of 63 to predict mortality using receiver operating characteristics curve analysis,the area under the curve was 0.989;and for APACHE-Ⅱ,at the cut off value of 24,the area under the curve was 0.965.CONCLUSION Because the ability of APACHE-Ⅱ to predict mortality was similar to P-POSSUM and APACHE-Ⅱ does not need scoring for intra-operative findings and histopathology reports,APACHE-Ⅱ can be used pre-operatively to assess the risk in patients undergoing emergency laparotomy.However,for audit purposes,either of the two scoring systems can be used. | Deb Sanjay Nag Ankur Dembla Pratap Rudra Mahanty Shashi Kant Abhishek Chatterjee Devi Prasad Samaddar Parul Chugh | 2019 | World Journal of Clinical Cases2019,7,16: | 5 |
| 3 | Glucagon-like peptide-1 receptor agonists favorably address all components of metabolic syndrome显示文摘Cardiovascular death is the leading cause of mortality for patients with type 2 diabetes mellitus. The etiologyof cardiovascular disease in diabetes may be divided into hyperglycemia per se and factors operating through components of metabolic syndrome(Met S). Hyperglycemia causes direct injury to vascular endothelium and possibly on cardiac myocytes. Met S is a cluster of risk factors like obesity, hyperglycemia, hypertension and dyslipidemia. The incidence of this syndrome is rising globally. Glucagon-like peptide-1 receptor agonists(GLP-1RA) are a group of drugs, which address all components of this syndrome favorably. Experimental evidence suggests that they have favorable actions on myocardium as well. Several compounds belonging to GLP-1RA class are in market now and a large number awaiting their entry. Although, originally this class of drugs emerged as a treatment for type 2 diabetes mellitus, more recent data generated revealed beneficial effects on multiple metabolic parameters. We have studied literature published between 2000 and 2016 to look into effects of GLP-1RA on components of Met S. | 4 | Integrating one health in national health policies of developing countries: India’s lost opportunities显示文摘Background:Globally,the threat of infectious diseases,particularly emerging infectious diseases,originating at the human-animal-environment interface,has caught health systems off guard.With forecasts that future pathogen emergence will be centred in hotspots in Asia,Africa,and Latin America,the need to prepare policy frameworks that can combat this threat is urgent.Discussion:Emergence of diseases such as avian influenza and Ebola virus disease,which threatened social disruption,have established the need for intersectoral coordination/collaboration.These events led to the initiation of establishing institutionalised collaborative frameworks in India to adopt a One Health approach to disease prevention and control.However,the gains made in influenza control could not be adapted to other infectious diseases.Intersectoral coordination was briefly carried out,more as a reactive response to threats.The systemic failure to sustain such efforts have therefore,only undermined a coordinated response.The recent draft National Health Policy,2015,has also failed to establish the need for intersectoral coordination in disease control approaches.Neglecting the need to endorse linkages between human health,animal health and husbandry,agriculture,and environmental sectors,has led to duplicative and weak response systems.The absence of health impact assessment with respect to the development agenda in policies,has cast negative effects on the health and wellbeing of man,animal,and the environment.Lack of attention to building core capacity in these critical sectors has further raised challenges in designing and deploying mitigation strategies.With developing countries like India being home to a major portion of the world’s poorest livestock farmers,the absence of a policy discourse that endorses the One Health approach in development and health policies is a major hurdle in eliminating poverty and poverty-related diseases.Conclusions:The adoption of One Health approaches in health and related sectoral policies is a critical policy requirement for India and other developing countries.The goal should be to not just establish preparedness plans,but also to encourage a policy environment where assessment and mitigation of downstream impacts of different agenda are incorporated. | Pranab Chatterjee Manish Kakkar Sanjay Chaturvedi | 2016 | Infectious Diseases of Poverty2016,5,1: | 1 |
| 5 | Sepsis associated delirium mimicking postoperative delirium as the initial presenting symptom of urosepsis in a patient who underwent nephrolithotomy显示文摘We report a case of 70 years old male who underwent percutaneous nephrolithotomy for renal calculi.After an uneventful recovery from anaesthesia,the patient developed delirium which manifested as restlessness,agitation,irritability and combative behavior.All other clinical parameters including arterial blood gas,chest X-ray and core temperature were normal and the patient remained haemodynamically stable.But 45 min later the patient developed florid manifestations of septic shock.He was aggressively managed in a protocolized manner as per the Surviving Sepsis Guidelines in the Critical Care Unit and recovered completely.There are no case reports showing postoperative delirium as the only initial presentation of severe sepsis,with other clinical parameters remaining normal.Both urosepsis and sepsis associated delirium have very high mortality.High index of suspicion and a protocolized approach in the management of sepsis can save lives. | Deb Sanjay Nag Abhishek Chatterjee Devi Prasad Samaddar Harprit Singh | 2016 | World Journal of Clinical Cases2016,4,5: | 1 |
