| 1 | Comparison of split-dosing vs non-split(morning) dosing regimen for assessment of quality of bowel preparation for colonoscopy显示文摘AIM: To compare(using the Ottawa Bowel Preparation Scale) the efficacy of split-dose vs morning administration of polyethylene glycol solution for colon cleansing in patients undergoing colonoscopy, and to assess the optimal preparation-to-colonoscopy interval.METHODS: Single-centre, prospective, randomized, investigator-blind stud in an academic tertiarycare centre. Two hundred patients requiring elective colonoscopy were assigned to receive one of the two preparation regimens(split vs morning) prior to colonoscopy. Main outcome measurements were bowel preparation quality and patient tolerability.RESULTS: Split-dose regimen resulted in better bowel preparation compared to morning regimen [Ottawascore mean 5.52(SD 1.23) vs 6.02(1.34); P = 0.017]. On subgroup analysis, for afternoon procedures, both the preparations were equally effective(P = 0.756). There was no difference in tolerability and compliance between the two regimens.CONCLUSION: Overall, previous evening- same morning split-dosing regimen results in better bowel cleansing for colonoscopy compared to morning preparation. | 2 | Cholestatic liver diseases:An era of emerging therapies显示文摘Recently the field of cholestasis has expanded enormously reflecting an improved understanding of the molecular mechanisms underlying bile secretion and its perturbation in chronic cholestatic disease. Novel anti-cholestatic therapeutic options have been developed for patients not favorably responding to ursodeoxycholic acid (UDCA), the current standard treatment for cholestatic liver disease. Important novel treatment targets now also include nuclear receptors involved in bile acid (BA) homoeostasis like farnesoid X receptor and G proteincoupled receptors e.g., the G-protein-coupled BA receptor “transmembrane G coupled receptor 5”. Fibroblast growth factor-19 and enterohepatic BA transporters also deserve attention as additional drug targets as does the potential treatment agent norUDCA. | 3 | Assessment of Diversity, Distribution, Conservation Status and Preparation of Management Plan for Medicinal Plants in the Catchment Area of Parbati Hydroelectric Project Stage-Ⅲ in Northwestern Himalaya显示文摘The developmental activities, particularly the construction of hydroelectric projects are causing a great loss of biodiversity in the Indian Himalayan Region. The Himachal Pradesh, a part of IHR is well known for the development of hydroelectric projects. The Parbati H.E. Project is amongst the major projects of the State. The different stages of the project are all causing loss of biodiversity of the area. Stage Ⅲ of the Parbati H.E. Project is a run of the river scheme on the Sainj River downstream of Power House of Parbati H.E. Project Stage Ⅱ. The project shall utilize regulated discharge of Parbati H.E. Project Stage Ⅱ and inflow of River Sainj for power generation, and has been contemplated as a peaking station operating in tandem with Stage Ⅱ. The present study has been undertaken to see the impact of hydroelectric project on the biodiversity, particularly on medicinal plants. A total of 104 species of medicinal plants, belonging to different life forms, i.e., trees (23 spp.), shrubs (22 spp.), herbs (57 spp.) and ferns (2 spp.) were recorded. The species have been analyzed and studied for their distribution, classification, altitudinal zones, part (s) used, indigenous uses, nativity, endemism and rarity.Different parts of these species, such as whole plants, roots (including rhizomes and tubers), leaves, flowers, fruits, seeds, stems, barks, spikes, nuts and insect galls are used by the inhabitants for curing various diseases and ailments. 30 species are native to the Himalayan region, 9 species native to the Himalayan region and adjacent countries also and 65 species are non-natives. 9 species are near endemics. Considering the whole Himalaya as a biogeographic unit (sensu lato), the near endemics are endemic to the Himalaya. Among these species, Zanthoxylum armatum is categorized as Endangered and Valeriana wallichii as Vulnerable. Hedychium spicatum, Rhus javanica, Berberis lycium, Thalictrum foliolossum, Salvia lanata, Rubia cordifolia and Bergenia ligulata may be considered as threatened species due to their over exploitation for trade. 90 species are propagated by seeds, 8 species by seeds and rhizomes/roots/tubers, 4 species by seeds and cuttings, and 2 species by sori. A management plan for the cultivation and conservation of the medicinal plants in the dam submergence area, and the commercially viable medicinal plants with high value in the catchment area is suggested. | S.S. | 4 | Efficacy and safety of standard and anti-reflux self-expanding metal stent: A systematic review and meta-analysis of randomized controlled trials显示文摘BACKGROUND Self-expanding metal stents are the main palliative treatment modality for unresectable esophageal cancer. Gastroesophageal reflux is a common adverse outcome after placement of esophageal stent for cancer involving the gastroesophageal junction and the gastric cardia. Anti-reflux stents with valve have been designed to prevent the acid reflux. The superiority of anti-reflux stent over standard stent in preventing gastroesophageal reflux has not been established well. This study compares the anti-reflux stent and the standard stent in terms of their efficacy to prevent acid reflux.AIM To compare the standard and the anti-reflux stents in terms of their efficacy,safety, and complications.METHODS The meta-analysis included 8 randomized clinical trials(RCTs) to compare pooled outcomes of total 395 patients. Primary outcomes include improvement in reflux symptoms and dysphagia score. Secondary outcomes include complications of stent migration, occlusion, and bleeding.RESULTS A total of eight RCTs were included in the meta-analysis. Compared to the standard stent, the anti-reflux stent showed a trend towards reduction in the dysphagia score without reaching a statistical significance [Standardized mean difference(SMD):-0.33(-0.71, 0.05); P = 0.09, I2: 37%]. There was no statistical difference in the gastrointestinal reflux(GER) scores between the two types of stents [SMD:-0.17(-0.78, 0.45); P = 0.008, I2: 74%]. Therefore, the present attempt has been made to study the diversity, distribution pattern and conservation status of the plant species used fo treating liver diseases/ailments in that region. A tota of 138 species (35 species of trees, 22 shrubs and 8 herbs) belonging to 98 genera in 60 families hav been recorded. Amongst the families, Euphorbiacea (9 species), and altitudinal zone <1,800 m, (i.e., 11 species) are rich in species. Traditionally, variou plant parts, such as roots/rhizomes/tubers (46 species), leaves (31), whole plants (30), barks (15) fruits (13), seeds and unspecified parts (8 each), and inflorescence (1) are used for the treatment of live diseases/ailments. 34 species are native, 3 ar endemic and 15 near endemic. 7 species ar categorized as Critically Endangered (Betula utilis) Endangered (Podophyllum hexandrum, Ephedra gerardiana, and Nardostachys grandiflora) and Vulnerable (Bergenia ligulata, B. stracheyi, and Hedychium spicatum) using new IUCN criteria Available chemical composition of plant parts used fo the treatment of liver diseases/ailments have beengiven.
