| 1 | Dose requirement and complications of diluted and undiluted propofol for deep sedation in endoscopic retrograde cholangiopancreatography显示文摘BACKGROUND:In general,the dose requirement and complications of propofol are lower when used in the diluted form than in the undiluted form.The aim of this study was to determine the dose requirement and complications of diluted and undiluted propofol for deep sedation in endoscopic retrograde cholangiopancreatography.METHODS:Eighty-six patients were randomly assigned to either group D (diluted propofol) or U (undiluted propofol).All patients were sedated with 0.02-0.03 mg/kg midazolam (total dose ≤2 mg for age <70 years and 1 mg for age ≥70) and 0.5-1 μg/kg fentanyl (total dose ≤75 μg for age <70 and ≤50 μg for age ≥70).Patients in group U (42) were sedated with standard undiluted propofol (10 mg/mL).Patients in group D (44) were sedated with diluted propofol (5 mg/mL).All patients in both groups were monitored for the depth of sedation using the Narcotrend system.The primary outcome variable was the total dose of propofol used during the procedure.The secondary outcome variables were complications during and immediately after the procedure,and recovery time.RESULTS:All endoscopies were completed successfully.Mean propofol doses per body weight and per body weight per hour in groups D and U were 3.0 mg/kg,6.2 mg/kg per hour and 4.7 mg/kg,8.0 mg/kg per hour,respectively.The mean dose of propofol,expressed as total dose,dose/kg or dose/kg per hour and the recovery time were not significantly different between the two groups.Sedation-related adverse events during and immediately after the procedure were higher in group U (42.9%) than in group D (18.2%) (P=0.013).CONCLUSIONS:Propofol requirement and recovery time in the diluted and undiluted propofol groups were comparable.However,the sedation-related hypotension was significantly lower in the diluted group than the undiluted group. | Somchai Amornyotin Wichit Srikureja Wiyada Chalayonnavin Siriporn Kongphlay | 2011 | Hepatobiliary & Pancreatic Diseases International2011,10,3: | 15 |
| 2 | Characteristics and outcomes of acute upper gastrointestinal bleeding after therapeutic endoscopy in the elderly显示文摘AIM: To characterize the effects of age on clinical presentations and endoscopic diagnoses and to determine outcomes after endoscopic therapy among patients aged ≥ 65 years admitted for acute upper gastrointestinal bleeding (UGIB) compared with those aged < 65 years. METHODS: Medical records and an endoscopy data-base of 526 consecutive patients with overt UGIB admitted during 2007-2009 were reviewed. The initial presentations and clinical course within 30 d after endoscopy were obtained. RESULTS: A total of 235 patients aged ≥ 65 years constituted the elderly population (mean age of 74.2 ± 6.7 years, 63% male). Compared to young patients, the elderly patients were more likely to present with melena (53% vs 30%, respectively; P < 0.001), have comorbidities (69% vs 54%, respectively; P < 0.001), and receive antiplatelet agents (39% vs 10%, respectively; P < 0.001). Interestingly, hemodynamic instability was observed less in this group (49% vs 68%, respectively; P < 0.001). Peptic ulcer was the leading cause of UGIB in the elderly patients, followed by varices and gastropathy. The elderly and young patients had a similar clinical course with regard to the utilization of endoscopic therapy, requirement for transfusion, duration of hospital stay, need for surgery [relative risk (RR), 0.31; 95% confidence interval (CI), 0.03-2.75; P = 0.26], rebleeding (RR, 1.44; 95% CI, 0.92-2.25; P = 0.11), and mortality (RR, 1.10; 95% CI, 0.57-2.11; P = 0.77). In Cox's regression analysis, hemodynamic instability at presentation, background of liver cirrhosis or disseminated malignancy, transfusion requirement, and development of rebleeding were significantly associated with 30-d mortality. To reduce downstream damage and loss of life, it is important to study the process of breaching of the damaged dike. It is not possible to understand the damage process without a clear understanding of the flow patterns encountered. In the present study, data obtained from nine experimental runs are analyzed. The flow patterns and progressive damage of dike overtopping are investigated. Two types of phenomena at the dike surface:erosion; and erosion and sliding, are observed during the overtopping event. Four stages of dike damage can be distinguished. The degradation rate of the dike crest is found to be dependent on the downstream slope of the dike. | 5 | Nitrative and oxidative DNA damage in intrahepatic cholangiocarcinoma patients in relation to tumor invasion显示文摘AIM: Nitrative and oxidative DNA damage such as 8-nitroguanine and 8-oxo-7,8-dihydro-2'-deoxyguanosine(8-oxodG) formation has been implicated in initiation and/or promotion of inflammation-mediated carcinogenesis.The aim of this study is to clarify whether these DNA lesions participate in the progression of intrahepatic cholangiocarcinoma.METHODS: We investigated the relation of the formation of 8-nitroguanine and 8-oxodG and the expression of hypoxia-inducible factor-1α (HIF-1α) with tumor invasion in 37 patients with intra-hepatic cholangiocarcinoma.RESULTS: Immunohistochemical analyses revealed that 8-nitroguanine and 8-oxodG formation occurred to a much greater extent in cancerous tissues than in non-cancerous tissues. HIF-1α could be detected in cancerous tissues in all patients, suggesting low oxygen tension in the tumors.HIF-1α expression was correlated with inducible nitric oxide synthase (iNOS) expression (r= 0.369 and P = 0.025)and 8-oxodG formation (r = 0.398 and P = 0.015).Double immunofluorescence study revealed that iNOS and HIF-1α co-localized in cancerous tissues. Notably, the formation of 8-oxodG was correlated significantly with lymphatic invasion (r = 0.386 and P= 0.018). Moreover, 8-nitroguanine and 8-oxodG in non-cancerous tissues were associated significantly with neural invasion (P = 0.042and P = 0.026, respectively). | 6 | Sedation-related complications in gastrointestinal endoscopy显示文摘Sedation practices for gastrointestinal endoscopic(GIE) procedures vary widely in different countries depending on health system regulations and local circumstances. The goal of procedural sedation is the safe and effective control of pain and anxiety, as well as to provide an appropriate degree of memory loss or decreased awareness. Sedation-related complications in gastrointestinal endoscopy, once occurred, can lead to significant morbidity and occasional mortality in patients. The risk factors of these complications include the type, dose and mode of administration of sedative agents, as well as the patient's age and underlying medical diseases. Complications attributed to moderate and deep sedation levels are more often associated with cardiovascular and respiratory systems. However, sedation-related complications during GIE procedures are commonly transient and of a mild degree. The risk for these complications while providing any level of sedation is greatest when caring for patients already medically compromised. Significant unwanted complications can generally be prevented by careful pre-procedure assessment and preparation, appropriate monitoring and support, as well as post-procedure management. Additionally, physicians must be prepared to manage these complications. | 7 | Anesthetic management for small bowel enteroscopy in a World Gastroenterology Organization Endoscopy Training Center显示文摘AIM:To study the anesthetic management of patients undergoing small bowel enteroscopy in the World Gastroenterology Organization(WGO) Endoscopy Training Center in Thailand.METHODS:Patients who underwent small bowel enteroscopy during the period of March 2005 to March 2011 in Siriraj Gastrointestinal Endoscopy Center were retrospectively analyzed.The patients' characteristics,pre-anesthetic problems,anesthetic techniques,anesthetic agents,anesthetic time,type and route of procedure and anesthesia-related complications were assessed.RESULTS:One hundred and forty-four patients underwent this procedure during the study period.The mean age of the patients was 57.6 ± 17.2 years,andmost were American Society of Anesthesiologists(ASA) class Ⅱ(53.2%).Indications for this procedure were gastrointestinal bleeding(59.7%),chronic diarrhea(14.3%),protein losing enteropathy(2.6%) and others(23.4%).Hematologic disease,hypertension,heart disease and electrolyte imbalance were the most common pre-anesthetic problems.General anesthesia with endotracheal tube was the anesthetic technique mainly employed(50.6%).The main anesthetic agents administered were fentanyl,propofol and midazolam.The mean anesthetic time was 94.0 ± 50.5 min.Single balloon and oral(antegrade) intubation was the most common type and route of enteroscopy.The anesthesia-related complication rate was relatively high.The overall and cardiovascular-related complication rates including hypotension in the older patient group(aged ≥ 60 years old) were significantly higher than those in the younger group.CONCLUSION:During anesthetic management for small bowel enteroscopy,special techniques and drugs are not routinely required.However,for safety reasons anesthetic personnel need to optimize the patient's condition. | Somchai Amornyotin Udom Kachintorn Siriporn Kongphlay | 2012 | World Journal of Gastrointestinal Endoscopy2012,4,5: | 8 |

| 11 | Comparison of early pre-cutting vs standard technique for biliary cannulation in endoscopic retrograde cholangiopancreatography: A personal experience显示文摘AIM: To compare the results and complications of early pre-cutting technique with standard technique. METHODS: From January 2003 to December 2004, a total of 416 consecutive therapeutic biliary ERCP procedures were performed by one endoscopist (T.A.). Data were retrospectively collected according to procedure indication and results. Of these, 293 procedures (70.4%) were done with standard technique (group A) and 123 procedures (29.6%) with early pre-cutting technique in case of difficult cannulation (group B). The results and complications of ERCP were compared. RESULTS: Success rate of first attempt cannulation was 98.0% in group A and 87.8% in group B. The overall incidence of post-ERCP pancreatitis, hemorrhage, perforation and cholangitis was 0%, 0.2%, 0.5% and 0.5%, respectively. Morbidity rate was not significantly different. No procedure-related mortality was occurred. | 12 | Annexin A1: A new immunohistological marker of cholangiocarcinoma显示文摘AIM: To evaluate a new immunohistological marker, annexin A1 (ANXA1), in cholangiocarcinoma (CCA) and hepatocellular carcinoma (HCC). METHODS: Expression of ANXA1 protein was investigated in liver tissues from patients with CCA and HCC by immunohistochemistry. Its expression on differences stages of tumor development was investigated in hamster CCA tissues induced by Opisthorchis viverrini and N -nitrosodimethylamine. Moreover, mRNA expression of ANXA1 was assessed in CCA cell lines by quantitative real-time polymerase chain reaction and silencing of ANXA1 gene expression using small interfering RNA. RESULTS: In human CCA tissue arrays, immunohistochemical analysis revealed that the positive expression of ANXA1 was 94.1% (64/68 cases) consisting of a high expression (66.2%, 45/68 cases) and a low expression (33.8%, 23/68 cases). However, expression of ANXA1 protein was negative in all histologic patterns for HCC (46/46 cases) and healthy individuals (6/6 cases). In hamster with opisthorchiasis-associated CCA, the expression of ANXA1 was observed in the cytoplasm of inflammatory cells, bile duct epithelia and tumor cells. Grading scores of ANXA1 expression were significantly increased with tumor progression. In addition, mRNA expression of ANXA1 significantly increased in all of the various CCA cell lines tested compared to an immortalized human cholangiocyte cell line (MMNK1). Suppressing the ANXA1 gene significantly reduced the matrix metalloproteinase (MMP) 2 and MMP9, and transforming growth factor-β genes, but increased nuclear factor-kB gene expression. CONCLUSION: ANXA1 is highly expressed in CCA, but low in HCC, suggesting it may serve as a new immunohistochemical marker of CCA. The products were characterized by X-ray diffraction(XRD), scanning electron microscopy(SEM),transmission electron microscopy(TEM), Raman spectroscopy, and Fourier transform infrared spectroscopy(FTIR). In this research, XRD patterns of pure ZnO and La-doped ZnO are specified as hexagonal wurtzite ZnO structure with no detection of La2O3 phase. SEM and TEM characterization revealed the flower shape of pure ZnO built-up from petals of hexagonal prisms with hexagonal pyramid tips. Upon doping with La, the flower-shaped ZnO is broken into individual 1D prism-like nanorods. | 15 | Critical Top Tension for Static Equilibrium Configuration of A Steel Catenary Riser显示文摘This paper aims to present the critical top tension for static equilibrium configurations of a steel catenary riser(SCR) by using the finite element method. The critical top tension is the minimum top tension that can maintain the equilibrium of the SCR. If the top tension is smaller than the critical value, the equilibrium of the SCR does not exist. If the top tension is larger than the critical value, there are two possible equilibrium configurations. These two configurations exhibit the nonlinear large displacement. The configuration with the smaller displacement is stable, while the one with larger displacement is unstable. The numerical results show that the increases in the riser's vertical distances, horizontal offsets, riser's weights, internal flow velocities, and current velocities increase the critical top tensions of the SCR. Subsequently, the solutions were processed by 270-W microwave-hydrothermal/solvothermal method. Phase, morphology, vibrational modes and photonic properties were fully characterized by X-ray powder diffraction (XRD), scanning electron microscopy (SEM), transmission electron microscopy (TEM),Fourier transform infrared spectroscopy (FTIR), Raman spectrophotometry, ultraviolet-visible (UV-Vis) absorption and photoluminescence (PL) spectroscopy. Most endoscopic patients require some forms of sedation and/or anesthesia. The goals of this sedation are to guard the patient's safety, minimize physical discomfort, to control behavior and to diminish psychological responses. Generally, moderate sedation for these procedures has been offered by the non-anesthesiologist by using benzodiazepines and/or opioids. Anesthesiologists and non-anesthesiologist personnel will need to work together for these challenges and for safety of the patients. The sedation training courses including clinical skills and knowledge are necessary for the registered nurses to facilitate the patient safety and the successful procedure. | 19 | Effects of Biochars and Other Organic Soil Amendments on Plant Nutrient Availability in an Ustoxic Quartzipsamment显示文摘A sandy soil, Nampong soil(classified as Ustoxic Quartzipsamment), was incubated under controlled condition i) to compare the mineralization of major plant nutrients derived from different types of biochars and other organic soil amendments; ii) to examine their effects on soil properties and plant nutrient availability; and iii) to evaluate the plant nutrient losses in leachate from the rooting zone of soil incorporated with the different amendments. The experiment was arranged in a completely randomized design with 3 replications.Five treatments of soil amendments used were cassava stem base biochar(CSB), rice husk biochar(RHB), chicken manure(CM),compost(CP), and no amendment application(control). The RHB treatment released the highest amounts of mineralized NO-3-N,available P and K(2.30–17.26, 5.50–42.90 and 43.00–187.63 mg kg-1, respectively) while the CM treatment releasing the highest NH+4-N in the range of 1.86–53.67 mg kg-1. The CSB and RHB treatments showed better continuity of mineralization of nutrients than the treatments of CM and CP, particularly in the case of the CSB treatment. In the soil column incubation experiment, the amounts of NH+4-N and NO-3-N in all treatments barely changed on Day 1 to Day 30 of incubation and then the amounts increased markedly on Day 60. On Day 60, the RHB treatment contained a very high amount of NO-3-N(> 250 mg kg-1). This suggests that N would become more available 30 d after the incorporation. The CM treatment gave the highest amounts of organic matter and available P in the ranges of 4.64–8.94 g kg-1and 14.41–36.33 mg kg-1, respectively, during the 60-d column incubation. The CSB treatment tended to have higher available K throughout the measuring period. The NO-3-N was leached from the soil column quite quickly on Day 1 of incubation while the loss of NH+4-N decreased slightly from Day 1 until the end of the measurement.
