| 6 | Liuwei Dihuang, a traditional Chinese herbal formula, suppresses chronic inflammation and oxidative stress in obese rats显示文摘OBJECTIVE: To investigate the anti-infl ammatory, anti-oxidative stress, and adipokine-ameliorating effects of Liuwei Dihuang(LWDH), a traditional Chinese herbal formula, in obese rats. METHODS: After 2 weeks of acclimation with free access to regular rodent chow and water, obese-prone-caesarean-derived(OP-CD) rats were fed a modified AIN-93 G diet containing 60% energy from fat. Treatment was performed twice daily by gavage feeding with 500, 1 500, or 3 500 mg/kg body weight LWDH suspended in water(n=12 rats per group). Twelve obese-resistant-CD(OR-CD) rats were fed the atherogenic diet and gavaged with water, and served as the normal control. Blood biomarkers of inflammation, oxidative stress and adiponectin were measured post-sacrifi ce and used to determine the treatment effect of LWDH and assess the suitability of OR/OP-CD rats for studying these parameters. RESULTS: After 9 weeks of treatment, LWDH lowered serum C-reactive protein(CRP) and tumour necrosis factor-α(TNF-α) levels. Serum interleukin-6(IL-6) levels showed a tendency towards reduction, but were not signifi cantly different from the OP-CD control. Liver superoxide dismutase(SOD) activity was increased in response to all three doses of LWDH, while the levels of reduced(GSH) and oxidized glutathione(GSSG) and thiobarbituric acid reactive substances(TBARS) were unchanged. Serum adiponectin levels were increased in response to oral administration of LWDH at the dose of either 500 or 1 500 mg/kg body weight. In addition, comparisons between OR-CD and OP-CD rats revealed differential, and for some biomarkers, conflicting characteristics of high-fat diet-fed OP-CD rats in reference to obese human subjects in terms of infl ammatory and oxidative stress biomarkers and circulating adiponectin levels. CONCLUSION: The results show, for the fi rst time, the anti-infl ammatory, anti-oxidative stress and adiponectin-ameliorating effects of LWDH in obese rats. Many studies across decades and all over the world have attempted to illustrate the pathogenesis behind it; however, the pathogenesis of vitiligo remains elusive. This review article, we present the findings behind the most and updated theories behind this psychologically debilitating and disfiguring disease. The discussion begun with the role of genetic predisposition followed by neural theory first proposed in the 1950 s. Wehighlight the autoimmune hypothesis, followed by the reactive oxygen species model, zinc-α2-glycoprotein deficiency hypothesis, viral theory, intrinsic theory and biochemical, molecular and cellular alterations accounting for loss of functioning melanocytes in vitiligo. Many theories were elaborated to clarify vitiligo pathogenesis. It is a multifactorial disease involving the interplay of several factors. | 10 | Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). There are a number of retrospective studies and case reports that have suggested a role for apheresis and insulin infusion in the acute inpatient setting. We report a case of HTGP in a male with hyperlipoproteinemia type Ⅲ who was treated successfully with insulin and apheresis on the initial inpatient presentation followed by bi-monthly outpatient maintenance apheresis sessions for the prevention of recurrent HTGP. We also reviewed the literature for the different inpatient and outpatient management modalities of HTGP. | 15 | Effects of rainfall patterns on runoff and rainfall-induced erosion显示文摘Rainfall-induced erosion involves the detachment of soil particles by raindrop impact and their transport by the combined action of the shallow surface runoff and raindrop impact.Although temporal variation in rainfall intensity(pattern)during natural rainstorms is a common phenomenon,the available information is inadequate to understand its effects on runoff and rainfall-induced erosion processes.To address this issue,four simulated rainfall patterns(constant,increasing,decreasing,and increasing-decreasing)with the same total kinetic energy were designed.Two soil types(sandy and sandy loam)were subjected to simulated rainfall using 15 cm×30 cm long detachment trays under infiltration conditions.For each simulation,runoff and sediment concentration were sampled at regular intervals.No obvious difference was observed in runoff across the two soil types,but there were significant differences in soil losses among the different rainfall patterns and stages.For varying-intensity rainfall patterns,the dominant sediment transport mechanism was not only influenced by raindrop detachment but also was affected by raindrop-induced shallow flow transport.Moreover,the efficiency of equations that predict the interrill erosion rate increased when the integrated raindrop impact and surface runoff rate were applied.Although the processes of interrill erosion are complex,the findings in this study may provide useful insight for developing models that predict the effects of rainfall pattern on runoff and erosion. | Morteza Alavinia Farzin Nasiri Saleh Hossain Asadi | 2019 | International Journal of Sediment Research2019,34,3: | 9 |
| 16 | Total knee arthroplasty for treatment of post-traumatic arthritis: Systematic review显示文摘AIM To review and report functional outcomes, complications,and survivorship associated with total knee arthroplasty(TKA) in the treatment of post-traumatic arthritis(PTA).METHODS We conducted a systematic review according to the PRISMA guidelines. We searched PubMed, Cochrane Library, and SCOPUS in December 2015 for Englishlanguage clinical research studies, both prospective and retrospective, examining the use of TKA for the treatment of PTA. All relevant articles were accessed in full. The manual search included references of retrieved articles.We extracted data on patients' demographics and clinical outcomes, including preoperative diagnosis and pre- and post-operative functional scores. We summarized the data and reported the results in tables and text.RESULTS Sixteen studies, four prospective and ten retrospective,examined patients who underwent TKA for PTA due to fractures of the proximal tibia, patella, and/or distal femur. Eleven studies utilized the Knee Society Scores criteria to assess functional outcomes. All studies utilizing these criteria reported an improvement in functional and knee scores of patients following TKA. Further, studies reported an increased range of motion(ROM) and reduction of pain following surgery. The most commonly reported complications with TKA included infection, stiffness, wound complications, intraoperative rupture of tendons, and osteolysis/polyethylene wear. The overwhelming majority of these complications occurred within the first two years following surgery. Six studies examined the survivorship of TKA with subsequent revision for any reason as an endpoint. | 17 | Mycobacterium avium subspecies paratuberculosis causes Crohn's disease in some inflammatory bowel disease patients显示文摘Crohn’s disease(CD)is a chronic inflammatory condition that plagues millions all over the world.This debilitating bowel disease can start in early childhood and continue into late adulthood.Signs and symptoms are usually many and multiple tests are often required for the diagnosis and confirmation of this disease.However,little is still understood about the cause(s)of CD.As a result,several theories have been proposed over the years.One theory in particular is that Mycobacterium avium subspecies paratuberculosis(MAP)is intimately linked to the etiology of CD.This fastidious bacterium also known to cause Johne’s disease in cattle has infected the intestines of animals for years.It is believed that due to the thick,waxy cell wall of MAP it is able to survive the process of pasteurization as well as chemical processes seen in irrigation purification systems.Subsequently meat,dairy products and water serve as key vehicles in the transmission of MAP infection to humans(from farm to fork)who have a genetic predisposition,thus leading to the development of CD.The challenges faced in culturing this bacterium from CD are many.Examples include its extreme slow growth,lack of cell wall,low abundance,and its mycobactin dependency.In this review article,data from 60 studies showing the detection and isolation of MAP by PCR and culture techniques have been reviewed.Although this review may not be 100%comprehensive of all studies,clearly the majority of the studies overwhelmingly and definitively support the role of MAP in at least30%-50%of CD patients.It is very possible that lack of detection of MAP from some CD patients may be due to the absence of MAP role in these patients.The latter statement is conditional on utilization of methodology appropriate for detection of human MAP strains.Ultimately,stratification of CD and inflammatory bowel disease patients for the presence or absence of MAP is necessary for appropriate and effective treatment which may lead to a cure. | Saleh A Naser Sudesh R Sagramsingh Abed S Naser Saisathya Thanigachalam | 2014 | World Journal of Gastroenterology2014,20,23: | 9 |

| 19 | Development of a new connection for precast concrete walls subjected to cyclic loading显示文摘The Industrialized Building System(IBS) was recently introduced to minimize the time and cost of project construction.Accordingly, ensuring the integration of the connection of precast components in IBS structures is an important factor that ensures stability of buildings subjected to dynamic loads from earthquakes, vehicles, and machineries.However, structural engineers still lack knowledge on the proper connection and detailed joints of IBS structure construction.Therefore, this study proposes a special precast concrete wall-to-wall connection system for dynamic loads that resists multidirectional imposed loads and reduces vibration effects(PI2014701723).This system is designed to connect two adjacent precast wall panels by using two steel U-shaped channels(i.e., male and female joints).During casting, each joint is adapted for incorporation into a respective wall panel after considering the following conditions: one side of the steel channel opens into the thickness face of the panel; a U-shaped rubber is implemented between the two channels to dissipate the vibration effect; and bolts and nuts are used to create an extension between the two U-shaped male and female steel channels.The developed finite element model of the precast wall is subjected to cyclic loads to evaluate the performance of the proposed connection during an imposed dynamic load.Connection performance is then compared with conventional connections based on the energy dissipation, stress, deformation, and concrete damage in the plastic range.The proposed precast connection is capable of exceeding the energy absorption of precast walls subjected to dynamic load, thereby improving its resistance behavior in all principal directions. | Ramin Vaghei Farzad Hejazi Hafez Taheri Mohd Saleh Jaafar Farah Nora Aznieta Abdul Aziz | 2017 | Earthquake Engineering and Engineering Vibration2017,16,1: | 8 |
