| 1 | Aggressive blood pressure treatment of hypertensive intracerebral hemorrhage may lead to global cerebral hypoperfusion:Case report and imaging perspective显示文摘Hypoperfusion injury related to blood pressure decrease in acute hypertensive intracerebral hemorrhage continues to be a controversial topic. Aggressive treatment is provided with the intent to stop the ongoing bleeding. However, there may be additional factors, including autoregulation and increased intracranial pressure, that may limit this approach. We present here a case of acute hypertensive intracerebral hemorrhage, in which aggressive blood pressure management to levels within the normal range led to global cerebral ischemia within multiple border zones. | 2 | Endoscopic management of esophageal varices显示文摘The rupture of gastric varices results in variceal hemorrhage, which is one the most lethal complications of cirrhosis. Endoscopic therapies for varices aim to reduce variceal wall tension by obliteration of the varix. The two principal methods available for esophageal varices are endoscopic sclerotherapy (EST) and band ligation (EBL). The advantages of EST are that it is cheap and easy to use, and the injection catheter fits through the working channel of a diagnostic gastroscope. Endoscopic variceal ligation obliterates varices by causing mechanical strangulation with rubber bands. | 3 | Liver expression of steroid hormones and Apolipoprotein D receptors in hepatocellular carcinoma显示文摘AIM: To evaluate the tissular expression of Androgen (A), Estrogen (E) and Progesterone (Pg) receptors, and Apolipoprotein D (ApoD), in liver tumors from resected hepatocellular carcinoma (HCC) cases in order to assess their possible relationship to prognosis. METHODS: We performed an immunohistochemical study using tissue microarrays (containing more than 260 cancer specimens, from 31 HCC patients and controls) to determine the presence of specif ic antibodies against AR, ER, PgR and ApoD, correlating their findings with several clinico-pathological and biological variables. The staining results were categorized using a semi-quantitive score based on their intensity, and the percentage of immunostained cells was measured. RESULTS: A total of 21 liver tumors (67.7%) were positive for AR; 16 (51.6%) for ER; 26 (83.9%) for PgR and 12 (38.7%) stained for ApoD. We have found a wide variability in the immunostaining score values for each protein, with a median (range) of 11.5 (11.5-229.5) for AR; 11.1 (8.5-65) for ER; 14.2 (4-61) for PgR; and 37.7 (13.8-81.1) for ApoD. A history of heavy ethanol consumption, correlated positively with AR and PgR and negatively with ER status. HCV chronic infection also correlated positively with AR and PgR status. However, the presence of ApoD immunostaining did not correlate with any of these variables. Tumors with a positive immuno-staining for PgR showed a better prognosis. | 9 | 想法行为混淆量表在中国大学生中的修订显示文摘目的:在大学生群体中修订想法行为混淆量表(Thought-Action Fusion Scale,TAFS),并考察其信效度.方法:采用方便取样方法,对样本1的378名大学生施测TAFS、贝克抑郁问卷、状态特质焦虑问卷、Frost完美主义问卷、多元个人传统性量表,以检验量表的效度,并随机选取其中42名学生,在初测4周后进行了重测以检验重测信度;对样本2的273名大学生施测TAFS、白熊压抑量表、帕多瓦量表,以验证强迫观念认知模型的跨文化有效性.结果:对样本1的探索性因素分析获得TAF量表的3个因素(道德、可能性-自我和可能性-他人),3个因素可解释总变异的51%(各条目在所属因子的因素负荷为0.418~0.847);对样本2的验证性因素分析显示,三因素模型拟合良好(χ2/df =2.25,CFI=0.92,TLI=0.90,RMSEA=0.07,SRMR=0.06).TAFS总分的内部一致性系数为0.86,重测信度为0.67;3个因素的内部一致性系数分别为0.84、0.83和 0.86.效标效度分析表明TAFS总分和TAF可能性分量表均与抑郁(r=0.19、0.27)、完美主义(r=0.25、0.27)、迷信信念(r=0.25、0.26)呈正相关,均P<0.05;TAF可能性分量表与特质焦虑呈正相关(r=0.22,P<0.05);TAF道德分量表与以上量表均无相关(P〉0.05).结构方程模型的结果证实了以下模型:想法行为混淆导致思维压抑,而思维压抑导致强迫症状(χ2/df=1.285,CFI=0.988,GFI=0.990,TFL=0.982,RMSEA=0.035).结论:想法行为混淆量表的中文修订版具有良好的信度和效度,可以用来测量对强迫症症状的发展及维持起作用的两种认知偏差. | RODRIGUEZ Marcus A 钱铭怡 高隽 肖征 | 2010 | 中国心理卫生杂志2010，24，11： | 5 |

| 11 | Analysis of the postoperative hemostatic profile of colorectal cancer patients subjected to liver metastasis resection surgery显示文摘BACKGROUND Liver resection surgery has advanced greatly in recent years,and the adoption of fasttrack programs has yielded good results.Combination anesthesia (general anesthesia associated to epidural analgesia) is an anesthetic-analgesic strategy commonly used for the perioperative management of patients undergoing surgery of this kind,though there is controversy regarding the coagulation alterations it may cause and which can favor the development of spinal hematomas.AIM To study the postoperative course of liver resection surgery,an analysis was made of the outcomes of liver resection surgery due to colorectal cancer metastases in our centre in terms of morbiditymortality and hospital stay according to the anesthetic technique used (general vs combination anesthesia).METHODS A prospective study was made of 61 colorectal cancer patients undergoing surgery due to liver metastases under general and combination anesthesia between January 2014 and October 2015.The patient characteristics,intraoperative variables,postoperative complications,evolution of hemostatic parameters,and stay in intensive care and in hospital were analyzed.RESULTS A total of 61 patients were included in two homogeneous groups: general anesthesia (n = 30) and combination anesthesia (general anesthesia associated to epidural analgesia)(n = 31).All patients had normal coagulation values before surgery.The international normalized ratio (INR) in both the general and combination anesthesia groups reached maximum values at 2448 h (mean 1.37 and 1.45 vs 1.39 and 1.41,respectively),followed by a gradual decrease.There was less intraoperative bleeding in the combination anesthesia group (769 mL) than in the general anesthesia group (1200 mL)(P < 0.05).Of the 61 patients,38.8% in the general anesthesia group experienced some respiratory complication vs 6.6% in the combination anesthesia group (P < 0.001).The time to gastrointestinal tolerance was significantly correlated to the type of anesthesia,though not so the stay in critical care or the time to hospital discharge.CONCLUSION Epidural analgesia in liver resection surgery was seen to be safe,with good results in terms of pain control and respiratory complications,and with no associated increase in complications secondary to altered hemostasis. | Guillermo Perez Navarro Ana Maria Pascual Bellosta Sonia María Ortega Lucea Mario Serradilla Martín Jose Manuel Ramirez Rodriguez Javier Martinez Ubieto | 2019 | World Journal of Clinical Cases2019，7，17： | 4 |
