1Mirizzi syndrome type Ⅴa:A rare coexistence of double cholecysto-biliary and cholecysto-enteric fistulae显示文摘Mirizzi syndrome is a rare cause of intermittent obstructive jaundice,where an impacted stone in the cystic duct or Hartmann's pouch mechanically obstructs the common bile duct(CBD) .We report a rare case of double cholecysto-biliary and cholecysto-enteric fistulae,in a 75-year-old female patient,presenting with a right upper quadrant abdominal pain and intermittent obstructive jaundice.Endoscopic retrograde cholangiopancreatography suggested Mirizzi syndrome.Operative findings included erosions of the lateral wall of the CBD and the second portion of the duodenum due to impacted gallstones.The defects were reconstructed primarily and a Kehr tube was inserted.The patient had an uneventful postoperative course and was discharged on the 14th postoperative day.Pavlos Lampropoulos Nikolaos Paschalidis Athanasios Marinis Spiros Rizos 2010World Journal of Radiology2010,2,10:10
2Prevalence of occult hepatitis B virus infection in haemodialysis patients from central Greece显示文摘AIM:To assess the hepatitis B virus(HBV)-DNA and the prevalence of occult HBV infection in end-stage renal failure(ESRF)patients from Central Greece. METHODS:Sera from 366 ESRF patients attending five out of six dialysis units from Central Greece were investigated for HBV-DNA by real-time polymerase chain reaction.Only serum samples with repeatedly detectable HBV-DNA were considered positive.IgG antibodies to hepatitis C virus(anti-HCV)were tested by a third generation enzyme linked immunosorbent assay(ELISA),while IgG antibodies to hepatitis E virus (anti-HEV)were tested by two commercially available ELISAs.RESULTS:HBV-DNA was detected in 15/366 patient (4.1%)and HBsAg in 20/366(5.5%).The prevalenc of occult HBV infection was 0.9%(3/346 HBsAg negative patients).Occult HBV was not associate with a specific marker of HBV infection or anti-HCV o anti-HEV reactivity.There was no significant differenc in HBV-DNA titres,demographic and biochemica features,between patients with occult HBV infectio and those with HBsAg-positive chronic HBV infection. 3Antihypertensive drugs and glucose metabolism显示文摘Hypertension plays a major role in the development and progression of micro-and macrovascular disease.Moreover,increased blood pressure often coexists with additional cardiovascular risk factors such as insulin resistance.As a result the need for a comprehensive management of hypertensive patients is critical.However,the various antihypertensive drug categories have different effects on glucose metabolism.Indeed,angiotensin receptor blockers as well as angiotensin converting enzyme inhibitors have been associated with beneficial effects on glucose homeostasis.Calcium channel blockers(CCBs)have an overall neutral effect on glucose metabolism.However,some members of the CCBs class such as azelnidipine and manidipine have been shown to have advantageous effects on glucose homeostasis.On the other hand,diuretics andβ-blockers have an overall disadvantageous effect on glucose metabolism.Of note,carvedilol as well as nebivolol seem to differentiate themselves from the rest of theβ-blockers class,being more attractive options regarding their effect on glucose homeostasis.The adverse effects of some blood pressure lowering drugs on glucose metabolism may,to an extent,compromise their cardiovascular protective role.As a result the effects on glucose homeostasis of the various blood pressure lowering drugs should be taken into account when selecting an antihypertensive treatment,especially in patients which are at high risk for developing diabetes.Christos V Rizos Moses S Elisaf 2014World Journal of Cardiology2014,6,7:7
4Vulnerabilities and integrity of precise point positioning for intelligent transport systems:overview and analysis显示文摘The implementation of Intelligent Transport System (ITS) technology is expected to significantly improve road safety and traffic efficiency. One of the key components of ITS is precise vehicle positioning. Positioning with decimetre to sub-metre accuracy is a fundamental capability for self-driving, and other automated applications. Global Navigation Satellite System (GNSS) Precise Point Positioning (PPP) is an attractive positioning approach for ITS due to its relatively low-cost and flexibility. However, GNSS PPP is vulnerable to several effects, especially those caused by the challenging urban environments, where the ITS technology is most likely needed. To meet the high integrity requirements of ITS applications, it is necessary to carefully analyse potential faults and failures of PPP and to study relevant integrity monitoring methods. In this paper an overview of vulnerabilities of GNSS PPP is presented to identify the faults that need to be monitored when developing PPP integrity monitoring methods. These vulnerabilities are categorised into different groups according to their impact and error sources to assist integrity fault analysis, which is demonstrated with Failure Modes and Effects Analysis (FMEA) and Fault Tree Analysis (FTA) methods. The main vulnerabilities are discussed in detail, along with their causes, characteristics, impact on users, and related mitigation methods. In addition, research on integrity monitoring methods used for accounting for the threats and faults in PPP for ITS applications is briefly reviewed. Both system-level (network-end) and user-level (user-end) integrity monitoring approaches for PPP are briefly discussed, focusing on their development and the challenges in urban scenarios. 5Development of a quantum-dot-labelled magnetic immunoassay method for circulating colorectal cancer cell detection显示文摘AIM:To detect of colorectal cancer(CRC) circulating tumour cells(CTCs) surface antigens,we present an assay incorporating cadmium selenide quantum dots(QDs) in these paper.METHODS:The principle of the assay is the immunomagnetic separation of CTCs from body fluids in conjunction with QDs,using specific antibody biomarkers:epithelial cell adhesion molecule antibody,and monoclonal cytokeratin 19 antibody.The detection signal was acquired from the fluorescence signal of QDs.For the evaluation of the performance,the method under study was used to isolate the human colon adenocarcinoma cell line(DLD-1) and CTCs from CRC patients' peripheral blood.RESULTS:The minimum detection limit of the assay was defined to 10 DLD-1 CRC cells/mL as fluorescence was measured with a spectrofluorometer.Fluorescenceactivated cell sorting analysis and Real Time RT-PCR,they both have also been used to evaluate the performance of the described method.In conclusion,we developed a simple,sensitive,efficient and of lower cost(than the existing ones) method for the detection of CRC CTCs in human samples.We have accomplished these results by using magnetic bead isolation and subsequent QD fluorescence detection.CONCLUSION:The method described here can be easily adjusted for any other protein target of either the CTC or the host.Maria Gazouli Anna Lyberopoulou Pericles Pericleous Spyros Rizos Gerassimos Aravantinos Nikolaos Nikiteas Nicholas P Anagnou Efstathios P Efstathopoulos 2012World Journal of Gastroenterology2012,18,32:6
6PERFORMANCE ANALYSIS OF INTEGRATED GPS/GLONASS CARRIER PHASE-BASED POSITIONING显示文摘Due to the different signal frequencies for the GLONASS satellites,the commonly-used double-differencing procedure for carrier phase data processing can not be implemented in its straightforward form,as in the case of GPS.In this paper a novel data processing strategy,involving a three-step procedure,for integrated GPS/GLONASS positioning is proposed.The first is pseudo-range-based positioning,that uses double-differenced (DD) GPS pseudo-range and single-differenced (SD) GLONASS pseudo-range measurements to derive the initial position and receiver clock bias.The second is forming DD measurements (expressed in cycles) in order to estimate the ambiguities,by using the receiver clock bias estimated in the above step.The third is to form DD measurements (expressed in metric units) with the unknown SD integer ambiguity for the GLONASS reference satellite as the only parameter (which is constant before a cycle slip occurs for this satellite).A real-time stochastic model estimated by residual series over previous epochs is proposed for integrated GPS/GLONASS carrier phase and pseudo-range data processing.Other associated issues,such as cycle slip detection,validation criteria and adaptive procedure(s) for ambiguity resolution,is also discussed.The performance of this data processing strategy will be demonstrated through case study examples of rapid static positioning and kinematic positioning.From four experiments carried out to date,the results indicate that rapid static positioning requires 1 minute of single frequency GPS/GLONASS data for 100% positioning success rate.The single epoch positioning solution for kinematic positioning can achieve 94.6% success rate over short baselines (<6 km).DAI LiwenHAN ShaoweiChris Rizos 2001Geo-Spatial Information Science2001,4,4:3
7Neutrophil gelatinase-associated lipocalin does not predict acute kidney injury in heart failure显示文摘BACKGROUND Acute cardiorenal syndrome type 1(CRS-1)is defined by a rapid cardiac dysfunction leading to acute kidney injury(AKI).Neutrophil gelatinaseassociated lipocalin(NGAL)is expressed on the surface of human neutrophils and epithelial cells,such as renal tubule cells,and its serum(sNGAL)and urinary have been used to predict AKI in different clinical settings.AIM To characterize CRS-1 in a cohort of patients with acute heart diseases,evaluating the potentiality of sNGAL as an early marker of CRS-1.METHODS We performed a retrospective cohort,multi-centre study.From January 2010 to December 2011,we recruited 202 adult patients admitted to the coronary intensive care unit(CICU)with a diagnosis of acute heart failure or acute coronary syndrome.We monitored the renal function to evaluate CRS-1 development and measured sNGAL levels within 24 h and after 72 h of CICU admission.RESULTS Overall,enrolled patients were hemodynamically stable with a mean arterial pressure of 92(82-107)mmHg,55/202(27.2%)of the patients developed CRS-1,but none of them required dialysis.Neither the NGAL delta value(AUC 0.40,95%CI:0.25-0.55)nor the NGAL peak(AUC 0.45,95%CI:0.36-0.54)or NGAL cutoff(≥140 ng/mL)values were statistically significant between the two groups(CRS-1 vs no-CRS1 patients).The area under the ROC curve for the prediction of CRS-1 was 0.40(95%CI:0.25-0.55)for the delta NGAL value and 0.45(95%CI:0.36-0.54)for the NGAL peak value.Finally,in multivariate analysis,the risk of developing CRS-1 was correlated with age>60 years,urea nitrogen at admission and 24 h-urine output(AUC 0.83,SE=60.5%SP=93%),while sNGAL was not significantly correlated.CONCLUSION In our population,sNGAL does not predict CRS-1,probably as a consequence of the mild renal injury and the low severity of heart disease.So,these data might suggest that patient selection should be taken into account when considering the utility of NGAL measurement as a biomarker of kidney damage.Fiorenza Ferrari Elisa Scalzotto Pasquale Esposito Sara Samoni Flavio Mistrorigo Lilia Maria Rizo Topete Massimo De Cal Grazia Maria Virzì Valentina Corradi Rossella Torregrossa Roberto Valle Stefania Bianzina Nadia Aspromonte Matteo Floris Alessandro Fontanelli Alessandra Brendolan Claudio Ronco 2020World Journal of Clinical Cases2020,8,9:3
8Effects of endoscopic sphincterotomy on biliary epithelium:A case-control study显示文摘AIM: To study the long-term effects of endoscopic sphincterotomy on biliary epithelium. METHODS: This is a prospective case-control study. A total of 25 patients with a median age of 71 years (range 49-89 years) and prior endoscopic sphincterotomy (ES) for benign disease formed the fi rst group. The median time from ES was 42 mo (range 8-144 mo). Another 25 patients with a median age of 76 years (range 44-94 mo) and similar characteristics who underwent current endoscopic retrograde cholangio-pancreatography (ERCP) and ES for benign disease formed the second group (control group). Brush cytology of the biliary tree with p53 immunocytology was performed in all patients of both groups. ERCPs and recruitment were conducted at the Endoscopic Unit of Aretaieion University Hospital and Tzaneio Hospital, Athens, from October 2006 to June 2010. RESULTS: No cases were positive or suspicious for malignancy. Epithelial atypia was higher in the first group (32% vs 8% in the second group, P = 0.034). Acute cholangitis and previous biliary operation rates were also higher in the fi rst group (acute cholangitis, 60% vs 24% in the second group, P = 0.01; previous biliary operation, 76% vs 24% in the second group, P = 0.001). Subgroup analysis showed that previous ES was the main causal factor for atypia, which was not related to the time interval from the ES (P = 0.407). Two patients (8%) with atypia in the fi rst group were p53-positive. CONCLUSION: ES causes biliary epithelial atypia that represents mostly reactive/proliferative rather than premalignant changes.
