1Pancreaticoduodenectomy: a comparison of superior approach with classical Whipple's technique显示文摘BACKGROUND: Pancreaticoduodenectomy (PD) is the standard procedure for resecting tumors arising from the periampullary area and the pancreatic head. Nevertheless this procedure is inherently difficult and associated with high morbidity and mortality. Besides, the technique applied for exposing the portal and superior mesenteric veins is time-consuming, difficult and associated with the risk of major venous injury. Recently we have introduced a modified approach for making this part of the procedure quick, safe and bloodless, which constitutes the subject of this study. METHODS: Patients who underwent pylorus preserving pancreaticoduodenectomy (PPPD) either by superior approach technique (group 1) or by classical Whipple's technique (group 2) were retrospectively identified. Age-sex composition, body mass index (BMI), total operative time, operative blood loss, intraoperative blood transfusion requirement, morbidity, mortality and length of hospital stay were compared between the two groups. RESULTS: Between January 1997 and December 2011, 72 patients underwent PPPD by the superior approach technique (group 1) and 38 underwent PPPD by the classical Whipple's technique (group 2) at our institution. Statistically significant differences were observed in operative time (208.1±46.3 minutes in group 1 vs 322.0±33.8 minutes in group 2), operative blood loss (601.0±250.3 mL in group 1 vs 1371.5±471.8 mL in group 2), and intraoperative blood transfusion requirement [10 (13.9%) patients in group 1 and 24 (63.2%) in group 2]. Among 18 (16.4%) obese patients, significant differences in operativetime, operative blood loss and intraoperative blood transfusion requirement were observed between groups 1 and 2. There was no significant inter-group difference in complication rate among obese patients, but comparing obese patients with normal weight patients revealed higher rates of complications like pancreatic fistula (27.8% vs 6.5%), delayed gastric emptying (16.7% vs 5.4%), and infective complications like wound infection and intra abdominal collection (44.4% vs 5.4%). CONCLUSIONS: On the basis of analytical data, we conclude that the superior approach technique is effective for PD compared with the classical Whipple's technique. It allows fast, safe and virtually bloodless dissection for exposure of the superior mesenteric and portal veins during early steps of PD. 2Impact of reducing and capping agents on carbohydrates for the growth of Ag and Cu nanostructures and their antibacterial activities显示文摘This study presents a viable green synthesis approach to produce Ag and Cu nanoparticles (MNPs) by using carbohydrates such as glucose, fructose, sucrose, and starch as reducing agents and describes theirantimicrobial activities against Escherichia coli DH5a. Optical and diffused light scattering analyses showed the Ag NPs ranged from 20 to 75 nm and the Cu NPs varied from 20 to 160 nm, which supports the differences in their absorption bands (400-434 nm for Ag and 458-641 nm for Cu). The reducing sugars interacted differently with Ag^+ and Cu^2+ based on their size and hydrolysis by NaOH resulting in effective stabilization of Ag^0 and Cu^0 and variation in the bactericidal activities of the MNPs. The antibacterial effects of the MNPs were evaluated by measuring the inhibition zones using E. coli DH5a as a test organism. No growth was observed by restreaking different parts of the clearly inhibited zones into new culture plates indicating the bactericidal efficacy of the Ag and Cu MNPs. The Ag NPs were found to be more effective in terms of the size of their in hibition zones (1.21-1.82 cm) compared with those of the Cu NPs (0.0-1.2 cm). 3New generation Amberlite XAD resin for the removal of metal ions: A review显示文摘The direct determination of toxic metal ions, in environmental samples, is difficult because of the latter's presence in trace concentration in association with complex matrices, thereby leading to insufficient sensitivity and selectivity of the methods used. The simultaneous removal of the matrix and preconcentration of the metal ions, through solid phase extraction,serves as the promising solution. The mechanism involved in solid phase extraction(SPE)depends on the nature of the sorbent and analyte. Thus, SPE is carried out by means of adsorption, ion exchange, chelation, ion pair formation, and so forth. As polymeric supports, the commercially available Amberlite resins have been found very promising for designing chelating matrices due to its good physical and chemical properties such as porosity, high surface area,durability and purity. This review presents an overview of the various works done on the modification of Amberlite XAD resins with the objective of making it an efficient sorbent. The methods of modifications which are generally based on simple impregnation, sorption as chelates and chemical bonding have been discussed. 4Morphometric Analysis of Foramen Magnum in the Determination of Sex Using Computed Tomography显示文摘Background:sex identification by morphological assessment of various bones of the skeleton had been one of the oldest approaches in forensic anthropology and medico-legal cases.Aim and Objective:The aim of this study was to make the morphometric measurements of the foramen magnum in determining the feasibility of sex determination using computed tomography(CT).Materials and Methods:The data for our study was obtained from 100 patients having CT scan of the head and neck region(Siemens Somatom 256 slice CT scanner)with 1mm contiguous axial scans using bone window settings(2000 HU with 400 HU in centre),scan time of one second and exposure parameters 140 kVp and 70mA.The different measurements taken include the antero-posterior(AP)diameter,transverse diameter and area of the foramen magnum.Results:There were 50 males and 50 females with the age range of 18 to 75 years.Males have a higher average value than females in all of the parameters measured on foramen magnum.By using Studenfs T-test,all the measured parameters showed significant difference between the sexes(p<0.01).Conclusion:The expression of sexual dimorphism in the foramen magnum region shows significant difference between the sexes,therefore this area of the skull should be considered a useful area in the identification of sex.Bashir Ahmad Wani Nusrat Nazir Rayees Ahmad Sheikh Altai Hussain Chalkoo Tauseefa Jan 2021Journal of Forensic Science and Medicine2021,7,1:0
