1Risk of second primary malignancies in a population-based study of adult patients with essential thrombocythemia显示文摘AIM:To determine the risk of second primary malignancy(SPM) and survival of patients with essential thrombocythemia(ET).METHODS:We identified all patients with ET diagnosed during 2001 to 2011 from the Surveillance,Epidemiology and End Results(SEER) 18 database.Actuarial and relative survival methods were used to calculate the survival statistics. We utilized the SEER 13 database to calculate SPM.We used multiple primary standardized incidence ratio(SIR) session of the SEER*Stat software(version 8.1.5) to calculate SIR and excess risk of SPM for ET patients.RESULTS:Age standardized five-year cause-specific survival was greater for patients < 50 years vs those ≥ 50 years(99.4% vs 93.5%,P < 0.01).Five-year causespecific survival was lower for men vs women(70.2% vs 79.7%).A total of 201 patients(2.46%) developed SPM at a median age of 75 years.SPMs occurred at an observed/expected(O/E) ratio of 1.26(95%CI:1.09-1.45,P = 0.002) with an absolute excess risk(AER) of 37.44 per 10000 population.A significantly higher risk was noted for leukemia(O/E 3.78; 95%CI:2.20-6.05,P < 0.001; AER 11.28/10000).CONCLUSION:ET patients have an excellent causespecific five-year survival but are at an increased risk of SPM,particularly leukemia, which may contribute to excess deaths.Rajesh Shrestha Smith Giri Ranjan Pathak Vijaya Raj Bhatt 2016World Journal of Clinical Oncology2016,7,4:1

3Clinical approach to diarrheal disorders in allogeneic hematopoietic stem cell transplant recipients显示文摘Diarrhea is a common complication of allogeneic hematopoietic stem cell transplant(HSCT), with an average incidence of approximately 40%-50%. A wide variety of etiologies can contribute to diarrhea in HSCT patients, including medication-induced mucosal inflammation, infections, graft-vs-host disease and cord colitis syndrome in umbilical cord blood transplant. Clinical manifestations can vary from isolated diarrheal episodes, to other organ involvement including pneumonia or myocarditis, and rarely multiorgan failure. The approach for diagnosis of diarrheal disorders in HSCT patients depends on the most likely cause. Given the risk of life-threatening conditions, the development of clinically significant diarrhea requires prompt evaluation, supportive care and specific therapy, as indicated. Serious metabolic and nutritional disturbances can happen in HSCT patients, and may even lead to mortality. In this review, we aim to provide a practical approach to diagnosis and management of diarrhea in the post-transplant period.Shadi Hamdeh Abd Almonem M Abdelrahman Osama Elsallabi Ranjan Pathak Smith Giri Kailash Mosalpuria Vijaya Raj Bhatt 2016World Journal of Hematology2016,5,1:0
4Metamorphic Evolution of the Amphibolites from Bundelkhand Craton, Central India: P-T Constraints and Phase Equilibrium Modelling显示文摘The amphibolites from the Mauranipur and Babina regions are located in the central part of the Bundelkhand Craton(BuC),northern India.During the geodynamic evolution of the BuC,these amphibolites underwent medium-grade metamorphism. This study combines textural observations of amphibolites from two distinct regions(Mauranipur and Babina)with mineral chemistry and phase equilibrium modelling.Observations suggest that the amphibolites of both areas have gone through three stages of metamorphism. The pre-peak stage in the amphibolites from the Mauranipur and Babina regions is marked by the assemblages Ep-Amp-Cpx-Pl-Ilm-Ru-Qz and Ep-Amp-Cpx-Pl-Ab-Ilm-Qz respectively;the peak metamorphic stage is characterized by the mineral assemblages Amp-Cpx-Pl-Ilm-Ru-Qz and Amp-Cpx-Pl-Ilm-Qz-H_(2)O,which is formed during the burial process, and the post-peak stage is represented by the assemblages Amp-Pl-Ilm-Ru-Qz and Amp-Pl-Ilm-Qz-H_(2)O respectively, which is formed by exhumation event.By applying the phase equilibria modelling in the NCFMASHTO system, the P-T conditions estimated from pre-peak,peak to post-peak stages are characterized as 6.7 kbar/510 oC,7.3 kbar/578ºC and>3.0 kbar/>585ºC,respectively,for the Mauranipur amphibolites;and 6.27 kbar/520ºC,5.2 kbar/805ºC and>3.0 kbar/>640ºC respectively for Babina amphibolites. The textural association and P-T conditions of both amphibolites suggest that these rocks were affected by burial metamorphism followed by an exhumation process during subduction tectonism in the BuC.Pratigya Pathak Shyam Bihari Dwivedi Ravi Ranjan Kumar 2022Journal of Environmental & Earth Sciences2022,4,1:0
5Disseminated toxocariasis in an immunocompetent host显示文摘Toxocariasis is a zoonotic infection caused by Toxocara canis,or less commonly,Toxocara cati, which is one of the most common zoonotic infections worldwide. It commonly affects the pediatric and immunocompromised population;however,it has rarely been reported in the immunocompetent adults. Two of the well-recognized syndromes in children are visceral larva migrans and ocular larva migrans.Infection in adults usually ranges from asymptomatic to nonspecific symptoms which makes the diagnosis challenging.A case of 36 year-old male was presented with disseminated toxocariasis with pulmonary and hepatic involvement and striking peripheral eosinophilia.Madan Raj Aryal Paras Karmacharya Amrit Pokharel Smith Giri Ranjan Pathak Richard Alweis 2014Asian Pacific Journal of Tropical Biomedicine2014,4,10:0
