2Role of innate immunity in the development of hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC)is the most common form of liver cancer worldwide.It is caused by a variety of risk factors,most common ones being infection with hepatitis viruses,alcohol,and obesity.HCC often develops in the background of underlying cirrhosis,and even though a number of interventional treatment methods are currently in use,recurrence is fairly common among patients who have had a resection.Therefore,whole liver transplantation remains the most practical treatment option for HCC.Due to the growing incidence of HCC,intense research efforts are being made to understand cellular and molecular mechanisms of the disease so that novel therapeutic strategies can be developed to combat liver cancer.In recent years,it has become clear that innate immunity plays a critical role in the development of a number of liver diseases,including HCC.In particular,the activation of Toll-like receptor signaling results in the generation of immune responses that often results in the production of proinflammatory cytokines and chemokines,and could cause acute inflammation in the liver.In this review,the current knowledge on the role of innate immune responses in the development and progression of HCC is examined,and emerging therapeutic strategies based on molecular mechanisms of HCC are discussed.Rajagopal N Aravalli 2013World Journal of Gastroenterology2013,19,43:8
3Neuroendocrine carcinoma of the ampulla of Vater:a clinicopathologic evaluation显示文摘BACKGROUND:Only 105 cases of neuroendocrine tumor(NET)of the ampulla of Vater have been described, mostly as single case reports.The incidence of NET is rising.The changes in incidence may result from changes in detection.This study was to determine the relative incidence and clinicopathological characteristics of high- grade neuroendocrine carcinoma(small cell carcinoma and large cell carcinoma)of the ampulla of Vater at a single institution. METHODS:Sections from paraffin blocks of tumors of the ampulla of Vater taken from 45 patients who underwent Whipple's procedure and 6 patients who underwent palliative bypass between September 2003 and January 2007 were subjected to immunohistochemical analysis.The clinical and pathological data from 5 patients diagnosed with NET of the ampulla of Vater were analyzed. RESULTS:The patients were 3 men and 2 women,ranging in age from 39 to 47 years(mean 44 years).Operative procedures included Whipple's procedure in 4 patients and palliative bypass in 1 patient.Histopathological examination revealed large-cell neuroendocrine carcinoma in 2 patients,small cell carcinoma in 2,and carcinoid in 1.Three patients with high-grade neuroendocrine carcinoma who had undergone Whipple's procedure died at postoperatively 7,11,and 13 months.The patient who had undergone palliative triple bypass died 3 months after surgery.CONCLUSIONS:The relative incidence of high-grade neuroendocrine carcinomas of the ampulla of Vater is higher than that generally expected.The tumors behave aggressively and have a dismal prognosis despite aggressive treatment.Eswaran Selvakumar Shanmugasundaram Rajendran Tirupporur Govindaswamy Balachandar Devy Gounder Kannan Satyanesan Jeswanth Palaniappan Ravichandran Rajagopal Surendran 2008Hepatobiliary & Pancreatic Diseases International2008,7,4:7
4Long-term outcome of gastric access loop in hepaticojejunostomy显示文摘BACKGROUND:The incidence of hepaticojejunostomy stricture is 4%-10% in experienced centres. Many access loops have been designed and used to facilitate endoscopic intervention for this complication of hepaticojejunostomy. In this study, we investigated the effectiveness and safety of gastric access loop. METHODS:A retrospective analysis was conducted on 13 patients who had undergone hepaticojejunostomy with gastric access loop between June 1999 and September 2003. Eleven patients were followed up for a mean period of 51 months (range 20-81 months). Two patients were lost to follow up. RESULTS:On follow-up, 8 patients had patent jejuno- gastrostomy (end to side anastomosis between Roux loop of jejunum and stomach) and hepaticojejunostomy. Three patients developed stricture of jejunogastrostomy at 41 months, 63 months and 81 months of follow-up. Among these 3 patients, one also had hepaticojejunostomy stricture. In the patient with hepaticojejunostomy stricture, dilatation of jejunogastrostomy stricture was attempted but failed. None of the patients had any evidence of bile gastritis/cholangitis. There was no procedure related morbidity/mortality. CONCLUSIONS:Further studies involving large numbers of patients are required before wide application of gastric access loop in hepaticojejunostomy though it is a safer option than percutaneous transhepatic manipulations or revision surgery.Eswaran Selvakumar Shanmugasundaram Rajendran Tirupporur G. 6Reinforcement Learning Based Controller Synthesis for Flexible Aircraft Wings显示文摘Aeroelastic study of flight vehicles has been a subject of great interest and research in the last several years. Aileron reversal and flutter related problems are due in part to the elasticity of a typical airplane. Structural dynamics of an aircraft wing due to its aeroelastic nature are characterized by partial differential equations. Controller design for these systems is very complex as compared to lumped parameter systems defined by ordinary differential equations. In this paper, a stabilizing statefeedback controller design approach is presented for the heave dynamics of a wing-fuselage model. In this study, a continuous actuator in the spatial domain is assumed. A control methodology is developed by combining the technique of “proper orthogonal decomposition” and approximate dynamic programming. The proper orthogonal decomposition technique is used to obtain a low-order nonlinear lumped parameter model of the infinite dimensional system. Then a near optimal controller is designed using the single-network-adaptive-critic technique. Furthermore,to add robustness to the nominal single-network-adaptive-critic controller against matched uncertainties, an identifier based adaptive controller is proposed. 8Gene expression profiling of MYC-driven tumor signatures in porcine liver stem cells by transcriptome sequencing显示文摘AIM:To identify the genes induced and regulated by the MYC protein in generating tumors from liver stem cells.METHODS:In this study,we have used an immortal porcine liver stem cell line,PICM-19,to study the role of c-MYC in hepatocarcinogenesis.PICM-19 cells were converted into cancer cells(PICM-19-CSCs)by overexpressing human MYC.To identify MYC-driven differential gene expression,transcriptome sequencing was carried out by RNA sequencing,and genes identified by this method were validated using real-time PCR.In vivo tumorigenicity studies were then conducted by injecting PICM-19-CSCs into the flanks of immunodeficient mice.RESULTS:Our results showed that MYC-overexpressing PICM-19 stem cells formed tumors in immunodeficient mice demonstrating that a single oncogene was sufficient to convert them into cancer cells(PICM-19-CSCs).By using comparative bioinformatics analyses,we have determined that>1000 genes were differentially expressed between PICM-19 and PICM-19-CSCs.Gene ontology analysis further showed that the MYCinduced,altered gene expression was primarily associated with various cellular processes,such as metabolism,cell adhesion,growth and proliferation,cell cycle,inflammation and tumorigenesis.Interestingly,six genes expressed by PICM-19 cells(CDO1,C22orf39,DKK2,ENPEP,GPX6,SRPX2)were completely silenced after MYC-induction in PICM-19-CSCs,suggesting that the absence of these genes may be critical for inducingtumorigenesis.CONCLUSION:MYC-driven genes may serve as promising candidates for the development of hepatocellular carcinoma therapeutics that would not have deleterious effects on other cell types in the liver.Rajagopal N Aravalli Neil C Talbot Clifford J Steer 2015World Journal of Gastroenterology2015,21,7:3
9Outcome of severe acute pancreatitis: Is there a role for conservative management of infected pancreatic necrosis?显示文摘BACKGROUND: Infected pancreatic necrosis is associated with high morbidity and mortality and is mandatory for surgical or radiological intervention. A selected group of patients with CT evidence of infected pancreatic necrosis and a comparatively lower APACHE score may be clinically stable throughout the course of their illness. METHODS: Case records of 52 patients with severe acute pancreatitis admitted from October 2000 to September 2005 were retrospectively analysed to assess the feasibility of conservative management of infected pancreatic necrosis. CT evidence of retroperitoneal air pockets, deteriorated clinical condition, sepsis and positive blood culture were used to diagnose infected pancreatic necrosis. RESULTS: In the 52 male patients reviewed, 24 patients had infected pancreatic necrosis. Eighteen patients who had progressively deteriorated clinical conditions required surgical intervention; five patients of whom (27.8%) died. Six patients with transient end organ dysfunction and stable clinical conditions were treated with prolonged administration of antibiotics and ICU support. All these patients recovered and discharged from the hospital, and no symptoms or readmission happened during follow-up of 6-44 months. CONCLUSIONS: Selected patients with infected pancreatic necrosis who are clinically stable with transient end organ dysfunction can be treated conservatively with a favourable outcome. Necrosectomy associated with high morbidityand mortality in these patients can be avoided. 11Dynamics of a neuron exposed to integer-and fractional-order discontinuous external magnetic flux显示文摘We propose a modified Fitzhugh-Nagumo neuron(MFNN) model. Based on this model, an integerorder MFNN system(case A) and a fractional-order MFNN system(case B) were investigated. In the presence of electromagnetic induction and radiation, memductance and induction can show a variety of distributions. Fractionalorder magnetic flux can then be considered. Indeed, a fractional-order setting can be acceptable for non-uniform diffusion. In the case of an MFNN system with integer-order discontinuous magnetic flux, the system has chaotic and non-chaotic attractors. Dynamical analysis of the system shows the birth and death of period doubling, which is a sign of antimonotonicity. Such a behavior has not been studied previously in the dynamics of neurons. In an MFNN system with fractional-order discontinuous magnetic flux, different attractors such as chaotic and periodic attractors can be observed. 13Synthesis and electrochemical performance of (100-x)Li7P3S11-xLi2OHBr composite solid electrolyte for all-solid-state lithium batteries显示文摘Li7P3S11solid electrolytes with high lithium-ion conductivity are promising candidates for use in all-solidstate lithium batteries.However,this electrolyte’s poor interfacial compatibility with lithium electrodes causes unstable cyclability.In this study,in order to address this problem,(100-x)Li7P3S11-xLi2OHBr(x=0,2,5,10,20,30,40,and 50)electrolytes are prepared by a high energy ball-milling technique and heat-treatment process.The resulting(100-x)Li7P3S11-xLi2OHBr(x=2,5,10,20,30,40,and 50)electrolytes provide improved electrochemical performance with good cycling stability and a wide electrochemical window of up to 10 V(vs.Li/Li+).Moreover,these electrolytes have high ionic conductivity of 10-4–10-5S/cm at room temperature.Particularly,the 90Li7P3S11-10Li2OHBr electrolyte displays the highest conductivity of 4.4×10-4S/cm at room temperature as well as improved cyclability.Moreover,90Li7P3S11-10Li2OHBr shows decreased interfacial resistance between the solid electrolyte and cathode electrode,which was revealed by Electrochemical Impedance Spectroscopy(EIS)analysis.The initial discharge capacity of 90Li7P3S11-10Li2OHBr was found to be 135 m Ah/g when used in a In|solid electrolyte|Li(Ni0.6Co0.2Mn0.2)O2 all-solid-state lithium battery(ASSLB).Thus,we can conclude the addition of Li2OHBr into the Li7P3S11results in enhanced electrochemical properties.Su-Yeon Jung Rajesh Rajagopal Kwang-Sun Ryu 2020Journal of Energy Chemistry2020,29,8:2
14Discrete large eddy simulation显示文摘Despite the intense effort expended towards obtaining a model for describing the turbulent flows of fluid, there is no model at hand that can do an adequate job. This leads us to look for a non-traditional approach to turbulence modeling. In this work we conjoin the notion of large eddy simulation with those of fuzzy sets and neural networks to describe a class of turbulent flows. In previous works we had discussed several issues concerning large eddy simulation such as filtering and averaging. Here, we discuss the use of fuzzy sets to improve the filtering procedure.L.
