| 1 | Metastasized pancreatic carcinoma with neoadjuvant FOLFIRINOX therapy and R0 resection显示文摘Patients with metastasized carcinoma of the pancreas have a very poor prognosis, and long-term survival cannot be expected. This case report describes two patients with an initial diagnosis of metastatic pancreatic cancer, both with hepatic metastases and one with an additional peritoneal carcinomatosis. Initially, both patients were treated intravenously with the FOLFIRINOX chemotherapy regimen, consisting of 5-FU, folinic acid, irinotecan and oxaliplatin. Surprisingly, the FOLFIRINOX treatment resulted in complete resolution of the hepatic metastases in both patients, with no lesions detectable by computed tomography scan. Furthermore, treatment response included decreased diameter of the primary tumor in the tail of the pancreas and disappearance of the additional peritoneal carcinomatosis. Both patients were discussed by our multidisciplinary tumor board, which recommended surgical resections of the carcinoma. The R0 resection of the primary tumor was successful in both cases and, interestingly, the resected tissues showed no evidence of the hepatic metastases intraoperatively. In the first case, the patient received a postoperative 6-mo course of adjuvant chemotherapy with gemcitabine. In the second case, the patient continued to receive the FOLFIRINOX regimen for an additional 6 mo postoperatively. At 12 mo after the operation, a nonresectable retroperitoneal lymph node metastasis was detected in the first patient, whereas the second patient remained in complete remission at the time of this report(5 mo after the adjuvant therapy was discontinued). This case report is the first of its kind to describe two cases of hepatic metastatic pancreatic carcinoma that were resectable following treatment with FOLFIRINOX. | 2 | 应用超硬大磨粒金刚石砂轮实现BK7光学玻璃的超精密磨削显示文摘首先以91μm磨粒杯形铜基金刚石砂轮作为修整器并结合砂轮在线电解修锐技术(ELID,Electrolytic in- process dressing)对151μm磨粒电镀镍基单层金刚石砂轮进行精密高效的修整。在最佳的修整参数下,同时应用测力仪对两个砂轮间磨削力进行监测,并应用共轴光学位移检测系统对砂轮表面状态进行在位监测,151μm砂轮的回转误差被减小至1~2μm范围,同时砂轮上所有金刚石磨粒被修整出平坦表面并拥有恒定的圆周包迹,此时砂轮达到最佳工作状态。然后应用被良好修整的砂轮对光学玻璃BK7进行磨削加工。磨削试验结果和亚表层完整度评价结果表明新开发的大磨粒金刚石砂轮修整技术的可行性,也验证大磨粒金刚石砂轮只要经过精密修整是可以应用于光学玻璃的延展性超精密磨削加工的,并能实现纳米级的表面粗糙度,显示出大磨粒金刚石砂轮在加工难加工材料和硬脆材料中的良好应用前景。 | 赵清亮 于光 EKKARD Brinksmeier OLTMANN Riemer KAI Rickens | 2006 | 机械工程学报2006，42，10： | 9 |
| 3 | Mining seismicity in the Witwatersrand Basin:monitoring,mechanisms and mitigation strategies in perspective显示文摘The Kaapvaal Craton in South Africa hosts one of the largest gold placer deposits in the world. Mining in the Witwatersrand Basin here has been the source of about one third to one half of the gold ever produced in the world. Gold was discovered in the Johannesburg area in 1886 and after 120 years of continuous operation, mining is currently approaching depths of 4 000 m. In spite of the challenges and risks that the industry has had to deal with including rock temperature, ventilation and water, one of the most feared hazards in the basin has been the threat from the ongoing occurrence of seismicity and rockbursts. The problem first manifested itself by way of the occurrence of tremors roughly twenty years after the commencement of mining operations. This paper traces the history of the approach to rockbursts and seismicity during the 120 year history of mining in the basin. It portrays a picture of the mining seismicity in terms of monitoring phases, mechanisms and mitigation strategies. The work of research organizations over the years is highlighted with a brief mention of current regulation strategies on the part of the mining inspectorate. | K. L. Riemer R. J. | 5 | Incidental congenital coronary artery vascular fistulas in adults:Evaluation with adenosine-13N-ammonia PET-CT显示文摘AIM To assess the functionality of congenital coronary artery fistulas(CAFs) using adenosine stress ^(13)N-ammonia positron emission tomography computed tomography(PET-CT).METHODS Congenital CAFs were incidentally detected during coronary angiography(CAG) procedures in 11 adult patients(six males and five females) with a mean age of 64.3 years(range 41-81). Patients were collected from three institutes in the Netherlands. The characteristics of the fistulas(origin, pathway and termination), multiplicity of the origins and pathways of the fistulous vessels were assessed by CAG. Five patients underwent adenosine pharmacologic stress ^(13)N-ammonia PET-CT to assess myocardial perfusion and the functional behavior of the fistula. RESULTS Eleven patients with 12 CAFs, 10 unilateral and one bilateral, originating from the left anterior descending coronary artery(n = 8), right coronary artery(n = 2) and circumflex(n = 2). All fistulas were of the vascular type, terminating into either the pulmonary artery(n = 11) or coronary sinus(n = 1). The CAG delineated the characteristics of the fistula(origin, pathway and termination). Multiplicity of the origins and pathways of the fistulous vessels were common in most fistulas(8/12, 67% and 9/12, 75%, respectively). Multiplicity was common among the different fistula components(23/36, 64%). Adenosine pharmacologic stress ^(13)N-ammonia PET-CT revealed normal myocardial perfusion and ejection fraction in all but one patient, who showed a reduced ejection fraction.CONCLUSION PET-CT may be helpful for assessing the functional status of congenital CAFs in selected patients regarding clinical decision-making.
