3Relationship between PaO_(2)/FiO_(2) and delirium in intensive care:A cross-sectional study显示文摘Background::To investigate the relationship between partial pressure of oxygen(PaO_(2))/fraction of inspired oxygen(FiO_(2))and the probability of delirium in intensive care units(ICUs).Methods::The investigation was a cross-sectional study that involved the collection of data from patients admitted to the Xiang Ya Hospital Cardiothoracic Surgical Care Unit and Comprehensive Intensive Care Unit from 01 September 2016 to 10 December 2016.Delirium was diagnosed using the simplified version of the Chinese Confusion Assessment Method(CAM)for the ICU.Demographic and medical data were obtained within 24 h of each patient admitted in the ICU.The PaO_(2)/FiO_(2) of each patient was recorded 24 h after admission in the ICU.The patients were divided into three groups according to PaO_(2)/FiO_(2) data:normal(PaO_(2)/FiO_(2)≥300 mmHg),slightly low(200≥PaO_(2)/FiO_(2)<300 mmHg),and severely low(PaO_(2)/FiO_(2)<200 mmHg).Baseline characteristics were compared in the three groups.Results of the unadjusted model,minimally adjusted model,and fully adjusted model are presented.Results::A total of 403 participants were included in the study,of which 184(45.7%)developed delirium.Age(P<0.001),Sequential Organ Failure Assessment(SOFA)score(P<0.001),Acute Physiology and Chronic Health Evaluation(APACHE)II score(P<0.001),mechanical ventilation time(P<0.001),history of hypertension(P=0.040),heart disease(P=0.040),sedation(P=0.001),and PaO_(2)/FiO_(2)(P=0.006)were significantly associated with delirium in univariate analysis.Multivariate regression analysis models were used to further analyze the associations between PaO_(2)/FiO_(2) and delirium.In the crude model,for 1 standard deviation(SD)increase in PaO_(2)/FiO_(2),the odds ratio(OR)of delirium was 0.8(95%confidence interval[CI]:0.6-0.9),but there was no significant correlation in the fully adjusted model.There was a non-linear relationship between the PaO_(2)/FiO_(2) and delirium in a generalized additive model.A two-piecewise linear regression model was used to calculate a PaO_(2)/FiO_(2) threshold of 243 mmHg.On the left side of the threshold,the OR was 0.9 and the 95%CI was 0.9-1.0(P=0.013)when PaO_(2)/FiO_(2) increased by 1 SD.Conclusions::PaO_(2)/FiO_(2) was negatively associated with delirium when PaO_(2)/FiO_(2) was below the identified threshold.As a readily available laboratory indicator,PaO_(2)/FiO_(2) has potential value in the clinical evaluation of risk of delirium in ICU patients.Fang Gong Yuhang Ai Lina Zhang Qianyi Peng Quan Zhou Chunmei Gui 2023Journal of Intensive Medicine2023,3,1:0

5Nail pigmentation induced by chemotherapy:an observational study of patients with early-stage breast cancer显示文摘Purpose:Chemotherapy-induced nail pigmentation is a common adverse efect,but prospective studies focussing on its onset,recovery,and severity are few.We aim to evaluate the pattern of chemotherapy-induced nail pigmentation in early-stage breast cancer patients by calculating the comprehensive score based on hyperpigmentation area and color depth of the nail plate.Methods:This prospective,observational study was conducted between February 2019 and December 2019.Early-stage breast cancer patients scheduled to receive anthracyclines combined with cyclophosphamide or taxanecontaining regimens were enrolled.The clinicopathologic characteristics and treatment protocols were collected.The onset,patterns,and duration of nail changes were photographed and recorded regularly.Results:A total of 90 patients were enrolled.The most common nail change was nail pigmentation(n=81,90.0%),followed by onycholysis(n=39,43.3%),Beau’s lines(n=19,21.1%),Mees’lines(n=16,17.8%),Muehrcke’s lines(n=7,7.8%),and hemorrhage(n=1,1.1%).Forty-four(48.9%)patients developed severe nail pigmentation.The median onset time of nail pigmentation was 37 days after the initiation of chemotherapy.At the latest follow-up,55(67.9%)patients achieved remission of melanonychia with the median recovery time of 118 days.The median duration of nail pigmentation was 214 days.Conclusion:Our study revealed the specifc pattern of chemotherapy-induced nail pigmentation,which onsets early and recovers slowly with a high incidence of severe nail pigmentation,in early-stage breast cancer patients.The results provide reference for further intervention studies.Trial Registration:ClinicalTrials.gov Identifer:NCT04215744.Registered 30 December 2019—Retrospectively registered.Kuikui Jiang Simei Shi Qiulian Lin Peng Sun Luan Zhang Zhongyu Yuan Ruoxi Hong Yanxia Shi Xia Liu Jingmin Zhang Jiajia Huang Xiwen Bi Wen Xia Qianyi Lu Qiufan Zheng Shusen Wang Fei Xu 2022Holistic Integrative Oncology2022,1,1:0
6Differences in clinical characteristics between younger and older patients with COVID-19 and their relationship with the length of hospital stay显示文摘Background:The coronavirus disease 2019(COVID-19)pandemic is currently threatening the health of individuals worldwide.We compared the clinical characteristics between younger patients(aged<60 years)and older patients(aged≥60 years)with COVID-19,detected the risk factors associated with a prolonged hospital stay,and examined the treatments commonly used with a particular focus on antiviral therapies.Methods:This retrospective study was conducted at the West Campus,Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology(Wuhan,China).The sample consisted of 123 patients admitted to the hospital between 9th February,2020,and 3rd March,2020.The data related to the demographics,laboratory findings,and treatment were analyzed to identify discrepancies between younger and older patients and those with and without primary diseases.The risk factors that contribute to a prolonged hospital stay were subsequently identified.Results:Patients aged≥60 years required longer hospital stay than younger patients(P=0.001).The percentage of lymphocytes was significantly lower in older patients and those with primary diseases(P=0.016 and P=0.042,respectively).The findings revealed that the risk factors that contributed to the length of hospital stay were age,the number of days of illness before hospitalization,white blood cell(WBC)count and albumin levels at admission,a neutrophil fraction at discharge,and antibiotic treatment.Analysis using a model that consisted of the above five risk factors for predicting prolonged hospital stay(>14 days)yielded an area under the ROC(AuROC)curve of 0.716.Antiviral and antibiotic treatments were administered to 97.6%and 39.0%of patients,respectively.The antiviral drugs most commonly administered were traditional Chinese medicine(83.7%)and arbidol(75.6%).Conclusions:In this study,older patients and those with primary diseases were at a higher risk of worse clinical manifestations.The physicians who treat the patients should pay close attention to the risk factors that contribute to the length of hospital stay,which could be used for predicting prolonged hospital stay.Traditional Chinese medicine and arbidol were the most frequently used antiviral drugs.Nevertheless,the extent to which these medications can effectively treat COVID-19 warrants further investigation.Qianyi Peng Xinhua Ma Zhiyong Liu Chunguang Zhao Lei Zhang Zhaoxin Qian Lina Zhang 2021Journal of Intensive Medicine2021,1,2:0
7Profiling proteomic responses to hexokinase-II depletion in terpene-producing Saccharomyces cerevisiae显示文摘Hexokinase II(Hxk2)is a master protein in glucose-mediated transcriptional repression signaling pathway.De-grading Hxk2 through an auxin-inducible protein degradation previously doubled sesquiterpene(nerolidol)pro-duction at gram-per-liter levels in Saccharomyces cerevisiae.Global transcriptomics/proteomics profiles in Hxk2-deficient background are important to understanding genetic and molecular mechanisms for improved nerolidol production and guiding further strain optimization.Here,proteomic responses to Hxk2 depletion are investi-gated in the yeast strains harboring a GAL promoters-controlled nerolidol synthetic pathway,at the exponential and ethanol growth phases and in GAL80-wildtype and gal80Δbackgrounds.Carbon metabolic pathways and amino acid metabolic pathways show diversified responses to Hxk2 depletion and growth on ethanol,including upregulation of alternative carbon catabolism and respiration as well as downregulation of amino acid synthesis.De-repression of GAL genes may contribute to improved nerolidol production in Hxk2-depleted strains.Seven-teen transcription factors associated with upregulated genes are enriched.Validating Ash1-mediated repression on the RIM4 promoter shows the variation on the regulatory effects of different Ash1-binding sites and the syner-gistic effect of Ash1 and Hxk2-mediated repression.Further validation of individual promoters shows that HXT1 promoter activities are glucose-dependent in hxk2Δbackground,but much weaker than those in HXK2-wildtype background.In summary,inactivating HXK2 may relieve glucose repression on respiration and GAL promoters for improved bioproduction under aerobic conditions in S.cerevisiae.The proteomics profiles provide a better genetics overview for a better metabolic engineering design in Hxk2-deficient backgrounds.Zeyu Lu Qianyi Shen Lian Liu Gert Talbo Robert Speight Matt Trau Geoff Dumsday Christopher B.Howard Claudia E.Vickers Bingyin Peng 2023Engineering Microbiology2023,3,3:0
