| 2 | Robotic rectal cancer surgery:Results from a European multicentre case series of 240 resections and comparative analysis between cases performed with the da Vinci Si and Xi systems显示文摘Introduction:Robotic systems are designed to address the limitations of laparoscopic surgery,leading to a growing interest in robotic rectal surgery.However,certain technical limitations associated with the previous systems(da Vinci S&Si)have arguably slowed down its wholesale adoption.The latest robotic platform,the da Vinci Xi,addresses these limitations.This study aims to examine the short-term surgical outcomes of 240 single-docking fully-robotic rectal cancer resections and compare the outcomes of cases performed with the da Vinci Xi vs Si systems.Materials and methods:All consecutive patients receiving robotic rectal cancer resections from three centres between 2013 and 2018 were identified from prospectively collated databases.The baseline characteristics and short-term surgical outcomes are presented and the da Vinci Xi vs Si system outcomes are analysed.Results:A total of 240 patients were identified(124 Si,116 Xi).Median operation-time and length-of-stay were 260 minutes and 6 days respectively.Conversion and 30-day mortality rates were 0.The da Vinci Si vs Xi system analysis shows that operation-time was lower in the Si group(230 vs 300 min,p=0.000)but length-of-stay,lymph node yield and circumferential resection margin favoured the Xi group(7 vs 5 days,p=0.010;17 vs 21,p=0.000;92.7%vs 99.1%,p=0.020).Conclusion:Single-docking fully-robotic rectal cancer surgery is safe,feasible and can lead to good shortterm outcomes,making it a good alternative to laparoscopic rectal cancer surgery.The new systems technological advances may result in better short-term outcomes but further larger scale observational studies are required if we are to reach such a conclusion. | Sofoklis Panteleimonitis Oliver Pickering Mukhtar Ahmad Mick Harper Tahseen Qureshi Nuno Figueiredo Amjad Parvaiz | 2020 | Laparoscopic, Endoscopic and Robotic Surgery2020，3，1： | 5 |
| 3 | Challenges facing early detection of acute kidney injury in the critically ill显示文摘Recent advances in the detection of acute kidney injury(AKI) afford the possibility of early intervention. Proteomics and genomics have identified many markers of tubular cell injury, some of which are manifest in the urine. One trial has used novel injury biomarkers to recruit patients to an intervention prior to an elevation in plasma creatinine. This trial and other recent studies have shown that the use of biomarkers of injury will depend on the time the patient presents following insult to the kidney, the likely cause of that insult, and the pre-injury renal function of that patient. The definition of AKI is likely to change in the near future to include a measure of injury. | 5 | Composites recycling solutions for the aviation industry显示文摘Ten of thousands of aircraft are expected to retire in the next 20 years.Aircraft manufacturers are gearing up for a new wave of recycling challenges as these aircrafts contain significant higher amount of carbon fibre reinforced polymer composite,which cannot be recycled by the conventional processes designed for metallic alloys.Aircraft manufacturers have been working with the recycling industry to limit unsustainable dismantling that is harmful to the environment and the potential liability risk of re-entry of un-certified salvaged parts back to the aviation market.An organised recycling network and procedures have already been set up for the conventional metallic alloys and will soon be required to include the composite waste.This paper reports the existing aircraft recycling practice and reviews the key recycling technologies for thermoset composites.Energy consumptions of these technologies are sought from the literature and are reported in this paper.Progress in development of reuse options for the recycled fibre is also included with discussion of their advantages and drawbacks.The challenge of working with the fluffy fibre is considered and the benefit of fibre alignment is highlighted for encouraging a widespread use of the fibre. | WONG Kok RUDD Chris PICKERING Steve LIU XiaoLing | 2017 | Science China(Technological Sciences)2017，60，9： | 3 |
