| 1 | 在线失眠认知行为治疗干预慢性失眠的开放性随机对照研究显示文摘目的探索在线失眠认知行为治疗(e-aid cognitive behavioral therapy for insomnia,eCBTI)对慢性失眠患者的疗效和治疗依从性。方法本研究是4周的开放性随机对照研究。通过微信平台募集受试者,纳入符合DSM-5中慢性失眠诊断标准的190例患者,通过简单随机方法分为eCBTI组(n=95)和治疗等待组(n=95)。入组后2组均进行失眠严重程度指数(insomnia severity index,ISI)的评估并记录连续1周的睡眠日记,eCBTI组经4周包括5个CBTI核心模块的治疗,治疗等待组不做任何干预,第4周2组再次记录1周的睡眠日记并进行ISI的评估。分别以睡眠效率(sleep efficiency,SE)>85%和ISI<8为失眠痊愈指标进行疗效评估。组间睡眠评估指标的比较采用t检验,痊愈率比较采用χ^2检验。结果eCBTI组55例完成4周治疗,治疗等待组78例完成了2次评估。4周治疗后,eCBTI组与治疗等待组的睡眠效率(11.6%±3.9%与2.9%±1.1%,t=4.63,P=0.036)、睡眠潜伏期(-57.0%±-14.7%与-19.8%±-8.5%,t=13.44,P<0.01)、觉醒次数(-43.4%±-13.0%与-12.2%±-6.1%,t=3.73,P=0.042)、ISI分值(-49.7%±-15.7%与-16.4%±-8.3%,t=11.72,P<0.01)治疗减分率差异均有统计学意义。以睡眠效率>85%为失眠痊愈指标,eCBTI组和治疗等待组痊愈率分为67.3%与26.9%(χ^2=21.36,P<0.01);以ISI<8为失眠痊愈指标,eCBTI组和治疗等待组的痊愈率分为43.6%与6.4%(χ^2=26.22,P<0.01)。结论eCBTI对慢性失眠患者有较好的治疗效果,但治疗依从性欠佳。 | 张斌 张黎黎 陈施雅 罗娴 Dhirendra Paudel 林秋雁 严彩萍 方乐琴 赵昱翰 | 2019 | 中华精神科杂志2019,52,6: | 20 |
| 2 | 喜马拉雅造山带工程效应及中尼铁路工程地质分区显示文摘造山带内动力作用的工程效应是地学与工程领域基础科学研究的前沿问题。结合GIS技术和相关地学理论,对喜马拉雅造山带由构造划分的高喜马拉雅、低喜马拉雅和特提斯喜马拉雅3个地块进行研究,揭示不同地块工程地质特性和灾害效应受内动力制约的普适性规律。提出:在挤压碰撞造山机制作用下快速隆升的高、低喜马拉雅,地震以逆冲型为主,地震强度大、频率高,水平应力相对较大,主应力方向近NE-WS方向,地形向大高差发展、河流下切强烈,山地灾害严重;而属于拆离地系,处于相对沉陷状态的特提斯喜马拉雅,地震以正断型为主,地震活动性相对较弱,水平应力相对较小,主应力方向近E-W方向,地形演化向着减弱地势的趋向发展,雪崩灾害严重;此外,高喜马拉雅特有的海洋性冰川地貌、冰湖和冰川泥石流,可能是控制跨喜马拉雅山铁路线路方案的重要问题。基于上述各地块工程效应存在显著差异的认识,提出以构造划分作为铁路工程地质分区的建议,并以拟建中尼铁路交通廊道为例,绘制了工程地质分区图。研究有助于将造山带理论推进到工程应用层面,为铁路大范围方案比选阶段,广域、高效、低成本地获取信息提供了新途径。 | 黄艺丹 姚令侃 谭礼 郭沉稳 Lalu Prasad Paudel | 2020 | 工程地质学报2020,28,2: | 14 |
| 3 | 缺氧诱导因子1α和血管内皮生长因子在子宫内膜癌中的表达及临床意义显示文摘目的分析缺氧诱导因子1α(HIF-1α)和血管内皮生长因子(VEGF)在子宫内膜癌和癌旁组织中的表达差异,探讨缺氧状态以及这2种蛋白在子宫内膜癌发生、发展中的临床意义。方法选择2011年1月至2012年12月在同济大学附属同济医院接受手术治疗的子宫内膜癌患者128例,采用免疫组织化学法检测术后癌组织和配对癌旁组织中HIF1-α和VEGF蛋白表达,并对患者进行定期随访,分析这2种蛋白表达与患者预后的关系。构建人子宫内膜癌细胞缺氧模型,检测细胞中HIF1-α和VEGF蛋白表达,并观察细胞增殖、侵袭及凋亡情况。结果子宫内膜癌组织中HIF-1α和VEGF蛋白阳性表达率均高于癌旁组织(P均<0.05),HIF-1α蛋白在淋巴结转移阳性、高组织学分级、肿瘤最大径≥4 cm和孕激素受体阳性的患者中阳性率较高(P均<0.05),VEGF蛋白在淋巴结转移阳性、高组织学分级、肌层浸润较深、肿瘤最大径≥4 cm、雌激素受体阳性、孕激素受体阳性和高病理分期的患者中阳性率较高(P均<0.05);HIF-1α阴性患者的5年总生存率高于阳性患者(P<0.05),VEGF阴性患者5年总生存率与阳性患者相比差异无统计学意义(P>0.05)。人子宫内膜癌细胞缺氧模型中,HIF-1α和VEGF蛋白表达均增加(P均<0.05),细胞增殖和侵袭能力均增强(P均<0.05),凋亡减少(P<0.05)。结论 HIF-1α和VEGF蛋白与子宫内膜癌的发展有关,HIF-1α蛋白表达阳性提示预后不良。 | 孙文惠子 Dhruba Paudel 欧阳一芹 宋思蕊 童晓文 李怀芳 王建军 初磊 | 2019 | 第二军医大学学报2019,40,4: | 12 |

| 5 | Antioxidant activity and phenol and flavonoid contents of eight medicinal plants from Western Nepal显示文摘OBJECTIVE: The purpose of this study was to evaluate the antioxidant activity, phenolic and flavonoids contents of selected medicinal plants form the Western region of Nepal.METHODS: The antioxidant activity of selected medicinal plants were determined by using 2, 2-diphenyl-1-picrylhydrazyl radical scavenging activity; total ferric ion reducing ability through spectroscopic analysis. The content of total phenols was determined using Folin-Ciocalteu reagent, whereas Aluminum chloride colorimetric method was used for flavonoid determination.RESULTS: Extracts of Syzygium Operculatus(87%),Astilbe Rivularis(83%) and Mallotus Philippnensis(88%) showed significant free radical scavenging activity with effective concentration(EC50) close to that of ascorbic acid. Syzygium Operculatus(96%),Astilbe Rivularis(97%) and Mallotus Philippnensis(97%) had potent reducing power in concentration dependent fashion. Those plant extract with higher free radical scavenging and ferric reducing effect also showed the greater content of both phenols and flavonoids, suggesting the correlation between polyphenolic content and antioxidant activity. Those plant extracts which showed better antioxidant activity assays, also demonstrated higher total phenol and flavonoid contents. | 6 | 综合医院有自杀倾向的住院患者发生自杀行为的影响因素分析显示文摘目的描述综合医院具有自杀倾向患者的特征,并分析此类患者发生自杀行为的危险因素.方法回顾性连续纳入南方医科大学南方医院2012年10月至2017年10月的住院患者中具有自杀倾向的病例共680例,根据是否发生自杀行为分为自杀倾向组(有自杀倾向无自杀行为,527例)和自杀行为组(有自杀倾向亦有自杀行为,153例).收集他们的性别、年龄、婚姻状况、户籍、入院科室、自杀诱因、自杀方式、精神疾患、躯体疾患及显著的精神症状等资料.采用单因素分析比较两组间的社会人口学和临床特征差异,采用二分类Logistic回归分析研究自杀行为的危险因素.结果自杀倾向组主要为女性[66.6%(351例)]、已婚[77.4%(458例)]、心理科[74.4%(392例)]、抑郁症[66.2%(349例)].自杀行为组中,男性和女性的自杀方式差异有统计学意义(χ^2=12.489,P=0.014),选择跳楼方式的男性较多,药物及割脉方式的女性更多.Logistic回归分析结果表明,入住重症医学科(OR=7.844,95%CI:2.240~27.475,P=0.001)、婚恋受挫(OR=3.646,95%CI:1.217~10.917,P=0.021),肿瘤(OR=4.620,95%CI:1.552~13.755,P=0.006),双相情感障碍(OR=3.734,95%CI:1.157~12.052,P=0.028)是自杀行为的危险因素.结论具有自杀倾向的患者中,入住重症医学科、双相情感障碍、肿瘤、婚恋挫折是发生自杀行为的危险因素,而且两性的自杀方式有所不同,需要临床高度重视,并采取针对性预防措施. | 张菀凌 Paudel Dhirendra 康尹之 杨璐璐 欧浪 邵若洋 张斌 | 2020 | 神经疾病与精神卫生2020,20,1: | 7 |
| 7 | Review of studies on land use and land cover change in Nepal显示文摘Land use and land cover(LULC) in Nepal has undergone constant change over the past few decades due to major changes caused by anthropogenic and natural factors and their impacts on the national and regional environment and climate.This comprehensive review of past and present studies of land use and land cover change(LUCC) in Nepal concentrates on cropland, grassland, forest, snow/glacier cover and urban areas. While most small area studies have gathered data from different sources and research over a short period, across large areas most historical studies have been based on aerial photographs such as the Land Resource Mapping Project in 1986. The recent trend in studies in Nepal is to focus on new concepts and techniques to analyze LULC status on the basis of satellite imagery, with the help of geographic information system and remote sensing tools. Studies based on historical documents, and historical and recent spatial data on LULC, have clearly shown an increase in cropland areas in Nepal,and present results indicating different rates and magnitudes. A decrease in forest and snow/glacier coverage is reported in most studies. Little information is available on grassland and urban areas from past research. The unprecedented rate of urbanization in Nepal has led to significant urban land changes over the past 30 years. | 9 | Effects of Topography and Land use on Woody Plant Species Composition and Beta Diversity in an Arid Trans-Himalayan Landscape, Nepal显示文摘Distribution patterns of plant species are believed to be impacted by small-scale habitat heterogeneity. However, there have been few comparative studies examining how woody vegetation composition and diversity varies with aspects of different orientations in the Trans-Himalayan region at a local scale. Here, we examined the effects of incoming solar radiation on variation in woody species composition and compared the diversity between the northeast- and southwest-facing slopes in a Trans-Himalayan valley of Nepal. We also examined the implicit interactions between slope orientation and land use in determining the compositional variations between the slopes. We selected two pairs of northeast- and southwest-facing slopes where the first pair has a similar land use and differs in exposure only(Pisang site) while the other pair has clear differences in land use in addition to slope exposure(Braka site). In each site, we sampled 72 plots(36 on each slope) in which the presence and absence of woody species, environmental variables, and disturbance were recorded. Correspondence Analysis(CA) results suggested that the woody species composition significantly varied between northeast- and southwest-facing slopes at both sites, and was significantly correlated with measured environmental variables such as radiation index, altitude, and canopy openness. In the Braka site,mean alpha diversity was significantly higher on southwest-facing slopes. In contrast, beta diversity and gamma diversity were greater on northeast-facing slopes at both sites. Our results suggest that topographic variables(e.g., radiation index) affect species composition between the slopes, likely due to their influence on small scale abiotic environmental variables. However, the effects of land use, such as livestock browsing/grazing may interact with the effects of slope exposure, effectively reducing differences in species composition within slopes but enhancing the differences in beta diversity between contrasting slopes in the Braka. We conclude that slope orientation and land use are important factors in structuring the woody species composition and diversity in the arid Trans-Himalayan region. | 10 | 喜马拉雅中部柯西河流域土地覆被现状研究(英文)显示文摘位于喜马拉雅中部的柯西河流域(Koshi River Basin,简称KRB),是恒河支流也是南亚极为重要的跨境流域。流域内海拔落差巨大、生境复杂、生态系统类型完整、土地覆被类型多样且区域差异明显,是全球气候变化的敏感区之一。本研究基于Landsat TM、野外考察及植被图等多源数据,运用3S技术,编制了高精度的柯西河流域土地覆被数据,分析了流域土地覆被现状特征。研究表明:(1)2010年KRB土地覆被从流域源头至下游由雪被和水体(冰川)、裸地、稀疏植被、草地、湿地、灌丛、森林、农田、水体(河流和湖泊)、建设用地等9类组成。其中,以草地、森林、裸地和农田为主,分别占流域面积的25.83%、21.19%、19.31%和15.09%。而对气候变化敏感的冰川面积仅占5.72%。(2)KRB南、北坡土地覆被类型组成与结构迥异。北坡以草地、裸地和冰川分布为主,南坡以森林、农田和裸地为主;草地在北坡的分布面积远高于南坡,二者比例是6.67:1,而森林面积的97.13%分布在南坡,这些森林大多分布在河谷中部和南部平原地区,且与农田交错分布。(3)与环境相适应,流域主要覆被类型的垂直分布也具有明显的地带性特征。土地覆被由低到高,依次为农田、森林、灌丛和农田混合型、草地、稀疏植被、裸地和水体(冰川)的分布。研究结果为土地利用和覆被变化研究、为高山地区尤其是跨境流域的生态系统保护与管理、土地资源利用和可持续发展提供科学依据。 | 吴雪 高俊刚 张镱锂 刘林山 赵志龙 Basanta PAUDEL | 2017 | Journal of Resources and Ecology2017,8,1: | 4 |
| 11 | Is overhydration in peritoneal dialysis patients associated with cardiac mortality that might be reversible?显示文摘AIM To study the relationship between overhydration(OH) in peritoneal dialysis(PD) patients and cardiac mortality.METHODS OH, as measured by body composition monitor(BCM), is associated with increased mortality in dialysis patients. BCM has been used to guide treatment on the assumption that correcting OH will improve cardiac morbidity and mortality although data demonstrating causality that is reversible is limited. We wished to determine if OH in PD patients predicted cardiac mortality, and if there was a correlation between OH and cardiac troponin-T(cT n T) levels. Finally, we wished to determine if improving OH values would lead to a decrement in cT n T. All prevalent PD patients over the study period of 57 mo who had contemporaneous BCM and cT nT measurements were followed irrespective of transplantation or PD technique failure. We also studied a cohort of patients with who had severe OH(> +2L).The Fresenius Body Composition Monitor was used to obtain hydration parameters. cT nT levels were done as part of routine clinical care. Data was analysed using SPSS version 20.0.RESULTS There were 48 deaths in the 336 patients. The patients that died from cardiac or non-cardiac causes were similar with respect to their age, incidence of diabetes mellitus, gender, ethnicity and cause of renal failure. However, the patients with cardiac causes of death had significantly shorter dialysis vintage(10.3 mo vs 37.0 mo, P < 0.0001) and were significantly more overhydrated by BCM measurement(2.95 L vs 1.35 L, P < 0.05). The mean(standard error of the means) hydration status of the 336 patients was +1.15(0.12) L and the median [interquartile range(IQR)] cT nT level was 43.5(20-90) ng/L. The cT nT results were not normally distributed and were therefore transformed logarithmically. There was a statistically significant correlation between Log(cT nT) with the OH value(Spearman r value 0.425, P < 0.0001). We identified a sub-group of patients that were severely overhydrated; median(IQR) hydration at baseline was +2.7(2.3 to 3.7) L. They were followed up for a minimum of 6 mo. Reduction in OH values in these patients over 6 mo correlated with lowering of cT nT levels(Spearman r value 0.29, P < 0.02). CONCLUSION Patients that were overhydrated had higher cT nT, and had deaths that were more likely to be cardiac related. | 13 | 过去百年尼泊尔耕地时空变化(英文)显示文摘In order to advance land use and land cover change(LUCC) research in Nepal, it is essential to reconstruct both the spatiotemporal distribution of agricultural land cover as well as scenarios that can explain these changes at the national and regional levels. Because of rapid population growth, the status of agricultural land in Nepal has changed markedly over the last 100 years. Historical data is used in this study, encompassing soils, populations, climatic variables, and topography. Data were revised to a series of 30 m grid cells utilized for agricultural land suitability and allocation models and were analyzed using a suite of advanced geographical tools. Our reconstructions for the spatiotemporal distribution of agricultural land in Nepal reveal an increasing trend between 1910 and 2010(from 151.2 × 10~2 km^2 to 438.8 × 10~2 km^2). This expanded rate of increase in agricultural land has varied between different eco, physiographic, and altitudinal regions of the country, significantly driven by population changes and policies over the period of this investigation. | 17 | Phytochemical profile and biological activity of Juglans regia显示文摘Juglans regia Linn.(Juglandaceae), popularly known as English or Persian walnut, is a valuable medicinal plant with a potency to cure various diseases in traditional medicine. Since ancient time, different local ethnic groups have used various part of J.regia for a wide array of ailments including helminthiasis, diarrhea, sinusitis, stomach ache, arthritis, asthma, eczema, scrofula, skin disorders, diabetes mellitus, anorexia, thyroid dysfunction, cancer and infectious diseases. Biological activities of J.regia have been reported in several peer review journals and scientific attention is increasing. The present review attempts to provide comprehensive information on plant description, ethnobotanical use, toxicity, phytochemical profile, pharmacology, clinical studies and current research prospective of the J.regia. Currently, there is an immense interest on isolation/identification of active constituents from walnut and screening those active compounds for pharmacological activities. In addition, researchers are performing clinical trials as well as screening various solvent extracts or fractions of J.regia in several animal diseases models to identify promising therapeutic benefits. | 18 | Lung adenocarcinoma with GGO nodules:HRCT radiological and pathological correlation显示文摘Background:Lung cancer is the most common cancer related death in the world for the both male and female as well.Adenocarcinoma is the most common pathological type which is in increasing trend.With recent advancement of screening of lung cancer with HRCT,GGO lesion has been noted frequently.GGO is a nonspecific finding that may be caused by various disorders,including inflammatory diseases,focal fibrosis,atypical adenomatous hyperplasia,bronchoalveolar carcinoma(BAC),and adenocarcinoma.This study intends to analyze the correlation between high resolutions computed tomography(HRCT)findings and the pathological findings of lung adenocarcinoma.Material and methods:Retrospective review of 16 cases of lung adenocarcinoma lesions after surgical resection.Tumors were defined as air containing type based on ratio of maximum dimension of the tumor on mediastinal window to the maximum diameter of the tumor on lung window was≤50%and as solid density if the ratio was>50%.The correlation between CT findings(homogenous/heterogeneous,airbronchogram,pleural tag,speculation,vascular involvement,pleural thickening,margin,shape)and pathological findings were investigated.Results:Of 3 air containing 2 were pre-invasive type and 1 was invasive.Among 13 solid density type all 13 were invasive type.Presence of speculation,heterogeneous appearance was found significantly associated with pathological invasion.Conclusion:Air containing type of small cells lung adenocarcinomas are preinvasive whereas solid densities are invasive.Speculation and heterogeneous are significant factor in invasive adenocarcinoma. | ACHARYA Prashanta PAUDEL Rasmita XU Qiu-zhen | 2015 | 东南大学学报(医学版)2015,34,3: | 2 |
| 19 | 铜绿假单胞菌整合子可变区长度对intⅠ1基因表达的影响显示文摘目的分析铜绿假单胞菌中Ⅰ类整合子可变区的长度对Ⅰ类整合酶基因(intⅠ1)表达水平的影响,以探讨整合酶表达的调控机制。方法将25株临床分离的Ⅰ类整合子阳性铜绿假单胞菌按可变区的长度分为A、B组(A组:1000~2000bp;B组:>2000bp),提取细菌的总RNA并逆转录为cDNA,用实时荧光定量PCR法测定两组细菌intⅠ1mRNA的相对表达量,并比较具有不同长度可变区的细菌中intⅠ1基因的平均表达水平。结果两组细菌中intⅠ1mRNA的相对表达量差异有统计学意义(P<0.05);A组细菌intⅠ1和16SrRNA的Ct值均值及⊿Ct值分别为:17.25、10.34、6.91,B组细菌intⅠ1和16SrRNA的Ct值均值及⊿Ct值分别为:18.91、9.47、9.45,⊿⊿Ct为5.81;A组细菌的intⅠ1mRNA平均表达水平是B组的5.81倍。结论在临床分离的Ⅰ类整合子阳性的铜绿假单胞菌中,短可变区组整合酶表达水平较高,而长可变区组整合酶表达水平较低,提示Ⅰ类整合子中整合酶的表达水平可能受整合子可变区基因盒数目的影响,从而影响整合子继续捕获基因盒的能力。 | 冯银 陈体 Govinda Paudel 袁金玲 黄利华 伍勇 | 2013 | 中华医院感染学杂志2013,23,8: | 2 |
| 20 | 颈深部多间隙脓肿34例临床分析显示文摘目的回顾性分析颈深部多间隙脓肿的临床特征、诊断、治疗和预后。方法总结分析2013年1月~2019年8月我院收治的34例颈深部多间隙脓肿患者的临床资料。结果32例行手术切开引流,2例保守治疗,感染来源以牙源性感染和异物常见,常见致病菌为链球菌,咽痛和颈部肿胀是最常见的临床表现,其中咽旁间隙和颈前间隙最常受累,术后并发症以低蛋白血症和电解质紊乱常见。该组患者33例治愈,1例死于感染性休克,随访1~3个月无复发。结论颈深部多间隙脓肿诊断明确后,及时有效的切开引流和合理抗生素的选择是治疗的关键,重视术后并发症的预防及处理,有利于该疾病的早期治愈。 | 曹武 黄志纯 冯立人 冯旭 孙宝宾 杨鑫 Sharad Paudel | 2020 | 中国耳鼻咽喉头颈外科2020,27,5: | 2 |
