| 1 | Osteopontin: A non-invasive parameter of portal hypertension and prognostic marker of cirrhosis显示文摘AIM: To investigate the relationship between osteopontin plasma concentrations and the severity of portal hypertension and to assess osteopontin prognostic value.METHODS: A cohort of 154 patients with confirmed liver cirrhosis(112 ethylic, 108 men, age 34-72 years)were enrolled in the study. Hepatic venous pressure gradient(HVPG) measurement and laboratory and ultrasound examinations were carried out for all patients. HVPG was measured using a standard catheterization method with the balloon wedge technique. Osteopontin was measured using the enzyme-linked immunosorbent assay(ELISA) method in plasma. Patients were followed up with a specific focus on mortality. The control group consisted of 137 healthy age- and sex- matched individuals.RESULTS: The mean value of HVPG was 16.18 ± 5.6 mm Hg. Compared to controls, the plasma levels of osteopontin in cirrhotic patients were significantly higher(P < 0.001). The plasma levels of osteopontin were positively related to HVPG(P = 0.0022, r = 0.25) and differed among the individual Child-Pugh groups of patients. The cut-off value of 80 ng/m L osteopontin distinguished patients with significant portal hypertension(HVPG above 10 mm Hg) at 75% sensitivity and 63% specificity. The mean follow-up of patients was 3.7 ± 2.6 years. The probability of cumulative survival was 39% for patients with HVPG > 10 mm Hg and 65% for those with HVPG ≤ 10 mm Hg(P = 0.0086, odds ratio(OR), 2.92, 95% confidence interval(CI): 1.09-7.76). Osteopontin showed a similar prognostic value to HVPG. Patients with osteopontin values above 80 ng/m L had significantly lower cumulative survival compared to those with osteopontin ≤ 80 ng/m L(37% vs 56%, P = 0.00035; OR = 2.23, 95%CI: 1.06-4.68).CONCLUSION: Osteopontin is a non-invasive parameter of portal hypertension that distinguishes patients with clinically significant portal hypertension. | 2 | Probiotics in hepatology显示文摘The paper provides a basic review of intestinal micro- flora and its importance in liver diseases. The intestinal microflora has many important functions, above all to maintain the microbial barrier against established as well as potential pathogens. Furthermore, it influences the motility and perfusion of the intestinal wall, stimu- lates the intestinal immune system and therefore also the so-called common mucosal immune system, re- ducing bacterial translocation and producing vitamins. Immune homeostasis at mucosal level results from a controlled response to intestinal luminal antigens. In liver cirrhosis, there are many changes in its function, mostly an increase in bacterial overgrowth and trans- location. In this review, probiotics and their indications in hepatology are generally discussed. According to recent knowledge, these preparations are indicated in clinical practice only for cases of hepatic encephalopa- thy. Probiotics are able to decrease the permeability ofthe intestinal wall, and decrease bacterial translocation and endotoxemia in animal models as well as in clinical studies, which is extremely important in the prevention of complications of liver cirrhosis and infection after liv- er transplantation. Probiotics could limit oxidative and inflammatory liver damage and, in some situations, improve the histological state, and thus non-alcoholic steatohepatitis could be considered as another possible indication. ? 2011 Baishideng. | 4 | Development tendencies of moulding and core sands显示文摘Further development of the technology for making moulding and core sands will be strictly limited by tough requirements due to protection of the natural environment. These tendencies are becoming more and more tense, so that we will reach a point when even processes, that from technological point of view fulfill high requirements of the foundry industry, must be replaced by more ecologically-friendly solutions. Hence, technologies using synthetic resins as binding materials will be limited. This paper presents some predictable development tendencies of moulding and core sands. The increasing role of inorganic substances will be noticed, including silicate binders with significantly improved properties, such as improved knock-out property or higher reclamation strength. Other interesting solutions might also be moulding sands bonded by geo-polymers and phosphate binders or salts and also binders based on degradable biopolymers. Well-ordered Zn O nanorods not only act as a scaffold for growth of MOF/PANI shell but also as Zn source for the formation of MOF. The morphology of ZnO@MOF@PANI composite is greatly influenced by the number of PANI electrodeposition cycles. Their structural and electrochemical properties were characterized with different techniques. The results indicate that the Zn O@MOF@PANI with 13 CV cycles of PANI deposition demonstrates the maximum specific capacitance of 340.7 F g-1 at 1.0 A g-1, good rate capability with84.3% capacitance retention from 1.0 to 10 A g-1 and excellent cycling life of 82.5% capacitance retention after 5000 cycles at high current density of 2.0 A g-1. | 7 | Treatment strategies for colorectal carcinoma with synchronous liver metastases: Which way to go?显示文摘AIM: To offer an up-to-date review of all availabletreatment strategies for patients with synchronous colorectal liver metastases(CLM).METHODS: A comprehensive literature search was performed to identify articles related to the management of patients with synchronous CLM. A search of the electronic databases PubMed, MEDLINE, and Google Scholar was conducted in September 2014.The following search terms were used: synchronous colorectal liver metastases, surgery, stage Ⅳ colorectal cancer, liver-first approach, and up-front hepatectomy.These terms were employed in various combinations to maximize the search. Only articles written in English were included. Particular attention was devoted to studies and review articles that were published within the last six years(2009-2014). Additional searches of the cited references from primary articles were performed to further improve the review. The full texts of all relevant articles were accessed by two independent reviewers.RESULTS: Poor long-term outcomes of patients with synchronous CLM managed by a traditional treatment strategy have led to questions about the timing and sequence of possible therapeutic interventions. Thus,alternative paradigms called reverse strategies have been proposed. Presently, there are four treatment strategies available:(1) primary first approach(or traditional approach) comprises resection of the primary colorectal tumor followed by chemotherapy;subsequent liver resection is performed 3-6 mo after colorectal resection(provided that CLM are still resectable);(2) simultaneous resection of the primary colorectal tumor and CLM during a single operation presents intriguing options for a highly select group of patients, which can be associated with significant postoperative morbidity;(3) liver-first(or chemotherapy-first) approach comprises preoperative chemotherapy(3-6 cycles) followed by liver resection,adjuvant chemotherapy, and resection of the primary colorectal tumor(it is best suited for patients withasymptomatic primary tumors and initially unresectable or marginally resectable CLM); and(4) up-front hepatectomy(or 'true' liver-first approach) includes liver resection followed by adjuvant chemotherapy,colorectal resection, and adjuvant chemotherapy(strategy can be offered to patients with asymptomatic primary tumors and initially resectable CLM).CONCLUSION: None of the aforementioned strategies appears inferior. | 8 | Heavy rockbursts due to longwall mining near protective pillars:A case study显示文摘Rockburst represents a very dangerous phenomenon in deep underground mining in unfavourable conditions such as great depth, high horizontal stress, proximity of important tectonic structures, and unmined pillars. The case study describes a recorded heavy rockburst in the Czech part of the Upper Silesian Coal Basin, which occurred during longwall mining near the protective pillar. The artificial dividing of geological blocks and creation of mining protective pillars(shaft pillars, crosscut pillars etc.) is a dangerous task in light of rockbursts occurring mainly due to overstressing of remaining pillars. A simple model of this situation is presented. Natural and mining conditions are analysed and presented in detail as well as registered seismicity during longwall mining in the area. Recorded rockbursts in the area of interest are described and their causes discussed. Many rockbursts near protective pillars were recorded in this mining region. Methodical instructions for rockburst prevention in proximity of protective pillars as well as for gates driving were devised based on the evaluation of rockburst causes. | 9 | Catheter ablation of atrial fibrillation in elderly population显示文摘Background Although elderly patients have been included in published series of catheter ablation for atrial fibrillation (AF), clinicalbenefit and safety remain still less defined in this population. A retrospective analysis of the results of catheter ablation for AF in a large volumecenter focused on comparison of elderly patients with the rest of the patient cohort was conducted in this study. Methods Consecutive patientswho underwent catheter ablation for AF between January 2001 and December 2016 were analysed. A total population of 3197 patients wasdichotomized by the age of 70 years (394 elderly vs. 2803 younger subjects). Patients were followed in terms of arrhythmia status and sur-vival for a median period of 18 vs. 21 and 35 vs. 57 months, respectively. Results Elderly patients were more frequently females (49% vs.29%, P 〈 0.0001), had a history of hypertension (79% vs. 57%, P 〈 0.0001), diabetes (16% vs. 11%, P 〈 0.01), stroke (9% vs. 6%, P 〈 0.01),coronary/peripheral artery disease (14% vs. 8%, P 〈 0.0001), and CHAzDS2-VASc score (3.1 ± 1.3 vs. 1.5 ± 1.2 s, P 〈 0.0001). Major com-plications were more frequent in elderly (5.3% vs. 3.2%, P = 0.03); however, this difference was driven by vascular complications (3.6% vs.1.9%, P = 0.04). There were comparable rates of cerebrovascular (0.3 vs. 0.3%) or nonvascular complications (1.8 vs. 1.2%). Good arrhyth-mia control was inferior in elderly patients as compared with the rest of the cohort, both without and with antiarrhythmic drugs: 44.2% vs.58.2% (P 〈 0.0001) and 78.2 vs. 83.2% (P 〈 0.01), respectively. Poor arrhythmia control was associated with relative risk of all-cause mor-tality of 2.7 (95% CI: 1.1-6.4) in elderly patients and 1.4 (95% CI: 0.9-2.0) in younger subjects. Conclusions Catheter ablation for AF inelderly patients is safe although somewhat less effective. | 13 | High‑throughput sequencing view on the magnitude of global fungal diversity显示文摘High-throughput DNA sequencing has dramatically transformed several areas of biodiversity research including mycol-ogy.Despite limitations,high-throughput sequencing is nowadays a predominant method to characterize the alpha and beta diversity of fungal communities.Across the papers utilizing high-throughput sequencing approaches to study natural habitats in terrestrial ecosystems worldwide,>200 studies published until 2019 have generated over 250 million sequences of the primary mycological metabarcoding marker,the nuclear ribosomal internal transcribed spacer 2(ITS2).Here we show that at a 97%sequence similarity threshold,the total richness of non-singleton fungal taxa across the studies published so far is 1.08 million,mostly Ascomycota(56.8%of the taxa)and Basidiomycota(36.7%of the taxa).The Chao-1 estimate of the total extant fungal diversity based on this dataset is 6.28 million taxa,representing a conservative estimate of global fungal species richness.Soil and litter represent the habitats with the highest alpha diversity of fungi followed by air,plant shoots,plant roots and deadwood with Chao-1 predictions,for samples containing 5000 sequences,of 1219,569,392,228,215 and 140 molecular species,respectively.Based on the high-throughput sequencing data,the highest proportion of unknown fungal species is associated with samples of lichen and plant tissues.When considering the use of high-throughput sequenc-ing for the estimation of global fungal diversity,the limitations of the method have to be taken into account,some of which are sequencing platform-specific while others are inherent to the metabarcoding approaches of species representation.In this respect,high-throughput sequencing data can complement fungal diversity predictions based on methods of traditional mycology and increase our understanding of fungal biodiversity. | Petr Baldrian TomášVětrovský Clémentine Lepinay Petr Kohout | 2022 | Fungal Diversity2022,,3: | 8 |

| 15 | Comparative analysis of clinicopathological correlations of cyclooxygenase-2 expression in resectable pancreatic cancer显示文摘AIM:To perform a comparative analysis of clinicopathological correlations of cyclooxygenase2 (COX2) expression in pancreatic cancer, examined by monoclonal and polyclonal antibodies.METHODS: The COX2 expression in 85 resection specimens of pancreatic ductal adenocarcinoma was immunohistochemically examined using both monoclonal and polyclonal antibodies. The final immunoscores were obtained by multiplying the percentage of positive cells with the numeric score reflecting the staining intensity.COX2 expression levels were classified into three categories (0, 1+, and 2+) and the clinicopathological correlations were statistically evaluated and analyzed.RESULTS: The positive tumor expression rates of COX2 were 80.5% using monoclonal antibody and 69.4% using polyclonal antibody. In the KaplanMeier analysis, no significant correlations were found between levels of COX2 expression and overall survival (OS), but trends to longer OS were found in COX2 negative cases using monoclonal antibody. Significantly longer disease free survival was revealed in COX2 negative cases using monoclonal antibody (P = 0.019). No correlations between COX2 expression levels and grade (G), tumor (T) status and nodal (N) status were demonstrated. Low histological grade showed a strong association with a longer OS (P < 0.001). Correlation of survival and T status revealed a shorter OS in T3 tumors, but the results reached only marginal statistical significance (P = 0.070). In the multivariate Cox proportional hazards regression model, histological grade, T and N status remained valuable predictors of a worse survival with borderline significance for T [hazards ratio (HR) = 4.18 for G (if G = 3, P < 0.001); HR = 1.64 for T (if T = 3, P = 0.065); HR = 2.53 for N (if N = 1, P = 0.006)]. Higher grade, T or N status was associated with a worse OS. | 18 | Pathological changes in the cellular structures of retina and choroidea in the early stages of alloxan-induced diabetes显示文摘AIM To investigate the temporal sequence of pathological changes in the cellular structures of retina and choroidea in the early stages of diabetes in laboratory animals.METHODS Experimental type 1 diabetes was modeled by three intraperitoneal injections of an alloxan solution into 30 male nonlinear rats at 16 wk of age. The 30 th and 60 th days from the final alloxan injection were chosen as the endpoints. Light and electron microscopy and morphometric and immunohistochemical studies were performed on histological slices of eyeballs from experimental animals.RESULTS Diabetic disturbances progressed to 60 d of the experiment. Thus, in the retina, a partial destruction of photoreceptors accompanied by interstitial edema was observed. The morphometric analysis revealed a reduction in the thickness of the retina. A reduction in the number of blood vessels of the choroid with disturbances of the endothelial cells and the vascular walls and a persistent reduction in the number of melanocytes were observed. | 20 | Influence of chamber misalignment on cased telescoped(CT) ammunition accuracy显示文摘As part of a research program, it was desired to better understand the impact of the rotating chamber alignment with the barrel throat on the precision and accuracy of a novel cased telescoped(CT) ammunition firing rifle. In order to perform the study, a baseline CT ammunition chamber which was concentric with a Mann barrel bore was manufactured. Additionally, six chambers were manufactured with an offset relative to the barrel bore. These chambers were used to simulate a misaligned chamber relative to the bore axis. Precision and accuracy tests were then performed at 200 m in an indoor range under controlled conditions. For this project, 5.56 mm CT ammunition was used. As the chamber axis offset relative to the gun bore was increased, the mean point of impact was displaced away from the target center. The shift in the impact location is explained by the presence of in-bore yaw which results in lateral throw-off and aerodynamic jump components. The linear theory of ballistics is used to establish a relationship between the chamber misalignment and the resulting projectile mean point of impact for a rifle developed to fire CT ammunition.
