| 1 | Recruitment maneuvers in acute respiratory distress syndrome: The safe way is the best way显示文摘Acute respiratory distress syndrome(ARDS) represents a serious problem in critically ill patients and is associated with in-hospital mortality rates of 33%-52%. Recruitment maneuvers(RMs) are a simple, low-cost, feasible intervention that can be performed at the bedside in patients with ARDS. RMs are characterized by the application of airway pressure to increase transpulmonary pressure transiently. Once non-aerated lung units are reopened, improvements are observed in respiratory system mechanics, alveolar reaeration on computed tomography, and improvements in gas exchange(functional recruitment). However, the reopening process could lead to vascular compression, which can be associated with overinflation, and gas exchange may not improve as expected(anatomical recruitment). The purpose of this review was to discuss the effects of different RM strategies- sustained inflation, intermittent sighs, and stepwise increases of positive end-expiratory pressure(PEEP) and/or airway inspiratory pressure- on the following parameters: hemodynamics, oxygenation, barotrauma episodes, and lung recruitability through physiological variables and imaging techniques. RMs and PEEP titration are interdependent events for the success of ventilatory management. PEEP should be adjusted on the basis of respiratory system mechanics and oxygenation. Recent systematic reviews and meta-analyses suggest that RMs are associated with lower mortality in patients with ARDS. | 2 | Efficacy of neoadjuvant therapy and surgical rescue for locally advanced hepatoblastomas:10 year single-center experience and literature review显示文摘AIM:To report our experience with long-term outcomes after multimodal management therapy.METHODS:An observational retrospective study was performed containing seven patients with hepatoblastoma(Hbl)treated in our institution,a tertiary referral center,from 2003 to 2011.Demographic,preoperative,surgical,and outcome variables were collected.A survival analysis and a review of the current literature related to combination neoadjuvant chemotherapy and surgical resection on Hbl were performed.RESULTS:The median age at surgery was 14.4 mo,with a male to female ratio of 4:3.Pretext staging at diagnosis was as follows:stageⅠ,4 cases;stageⅡ,2 patients;and stageⅢ,1 case.Mean pretreatment tumor volume was 735 cm3.Five out of seven patients received neoadjuvant chemotherapy according to SIOPEL-3 or SIOPEL-6 protocols.Tumor volume and alphafetoprotein levels significantly dropped after neoadjuvant therapy.Surgical procedures performed included hemihepatectomies,segmentectomies and atypical resection.All patients received chemotherapy after surgery.Median postoperative hospital stay was 8 d.All patients were alive and disease-free after a median follow-up period of 23 mo.With regards to the literature review,seventeen articles were found that were related to our search.CONCLUSION:Our series shows how multimodal management of Hbl,exhaustive control and a meticulous surgical approach leads to almost 100%complete resection with optimal postoperative results. | Dolores Ayllon Teran Oscar Gómez Beltran Rubén Ciria Bru Elena Mateos González María José Pea Rosa Antonio Luque Molina Pedro López Cillero Javier Briceo Delgado | 2014 | World Journal of Gastroenterology2014,20,29: | 12 |
| 3 | Intravitreal dexamethasone implants for diabetic macular edema显示文摘AIM: To evaluate the safety and efficacy of a dexamethasone(DEX) intravitreal implant for diabetic macular edema(DME).METHODS: Totally 113 eyes of 84 patients were divided in three subgroups: naive patients(n=11), pseudophakic patients(n=72) and phakic patients(n=30). Inclusive criterion comprised adult diabetic patients with central fovea thickening and impaired visual acuity resulting from DME for whom previous standard treatments showed no improvement in both central macular thickness(CMT) and best corrected visual acuity(BCVA) after at least 3 mo of treatment. Outcome data were obtained from patient visits at baseline and at months 1, 3, 5, 9 and 12 after the first DEX implant injection. At each of these visits, patients underwent measurement of BCVA, a complete eye examination and measurement of CMT and macular volume(MV) carried out with optical coherence tomography(OCT) images. RESULTS: Seventy-three eyes(64.5%) received a single implant, 30(26.5%) received two implants and 10(9%) received three implants. At baseline, average in BCVA, CMT and MV were 43.5±20.8, 462.8±145 and 12.6±2.5 respectively. These values improved significantly at 1 mo(BCVA: 47.2±19.5, CMT: 339.6±120, MV: 11.11±1.4) and 3 mo(BCVA: 53.2±18.1, CMT: 353.8±141, MV: 11.3±1.3)(P≤0.05). At 5 mo(BCVA: 50.9±19.8, CMT: 425±150, MV: 12.27±2.3), 9 mo(BCVA: 48.4±17.6, CMT: 445.5±170, MV: 12.5±2.3) and 12 mo(BCVA: 47.7±18.8, CMT: 413.2±149, MV: 12.03±2.5), improvements in the three parameters were no longer statistically significant and decreased progressively but did not reach baseline values. There were no clinical differences between subgroups. | 4 | Recombinant streptokinase vs phenylephrine-based suppositories in acute hemorrhoids, randomized, controlled trial(THERESA-3)显示文摘AIM: To compare the efficacy and safety of recombinant streptokinase(rSK) and phenylephrine-based suppositories in acute hemorrhoidal disease.METHODS: A multicenter(14 sites), randomized(1:1), open, parallel groups, active controlled trial was done. After inclusion, subjects with acute symptoms of hemorrhoids, who gave their written, informed consent to participate, were centrally randomized to receive, as outpatients, rSK(200000 IU) or 0.25% phenylephrine suppositories, which had different organoleptic characteristics. Treatment was administered by the rectal route, one unit every 6 h during 48 h for rSK, and up to a maximum of 5 d(20 suppositories) for phenylephrine. Evaluations were performed at 3, 5 and 10 d postinclusion. The main end-point was the 5th-day complete clinical response(disappearance of pain and edema, and ≥ 70% reduction of the lesion size). Time to response and need for thrombectomy were secondary efficacy variables. Adverse events were evaluated too. RESULTS: 5thday complete response rates were 83/110(75.5%) and 36/110(32.7%) with rSK and phenylephrine suppositories, respectively. This 42.7% difference(95%CI: 30.5-54.2) was highly significant(P < 0.001). The advantage was detected since the early 3rdday evaluation(37.3% vs 6.4% for the rSK and active control groups, respectively; P < 0.001) and was kept even at the late 10thday assessment(83.6% vs 58.2% for rSK and phenylephrine, respectively; P < 0.001). Time for complete response was significantly shorter(P = 0.031; log-rank test) in the rSK group(median: 4.9 d; 95%CI: 4.8-5.0) with respect to the active control(median: 9.8 d; 95%CI: 9.8-10.0). Thrombectomy was necessary in 1/59 and 8/57 patients with baseline thrombosis in the rSK and phenylephrine groups, respectively(P = 0.016). There were no adverse events attributable to the experimental treatment. | 7 | Recombinant streptokinase vs hydrocortisone suppositories in acute hemorrhoids:A randomized controlled trial显示文摘AIM: To compare the efficacy and safety of recombinant streptokinase(rSK) vs hydrocortisone acetate-based suppositories in acute hemorrhoidal disease.METHODS: A multicenter(11 sites), randomized(1:1:1), open, controlled trial with parallel groups was performed. All participating patients gave their written,informed consent. After inclusion, patients with acute symptoms of hemorrhoids were centrally randomized to receive, as outpatients, by the rectal route, suppositories of rSK 200000 IU of one unit every 8 h(first 3 units)and afterwards every 12 h until 8 administrations were completed(schedule A), one unit every 8 h until 6 units were completed(schedule B), or 25 mg hydrocortisone acetate once every 8 h up to a maximum of 24 administrations. Evaluations were performed at 3, 5,and 10 d post-inclusion. The main end-point was the 5thday response(disappearance of pain and bleeding, and≥ 70% reduction of the lesion size). Time to response and need for thrombectomy were secondary efficacy variables. Adverse events were also evaluated.RESULTS: Groups were homogeneous with regards to demographic and baseline characteristics. Fifth day complete response rates were 156/170(91.8%; 95%CI:87.3-96.2), 155/170(91.2%; 95%CI: 86.6%-95.7%),and 46/170(27.1%; 95%CI: 20.1%-34.0%) with rSK(schedule A and B) and hydrocortisone acetate suppositories, respectively. These 64.6% and 63.9%differences(95%CI: 56.7%-72.2% and 55.7%-72.0%)were highly significant(P < 0.001). This advantage was detected since the early 3rd day evaluation(68.8% and64.1% vs 7.1% for the rSK and active control groups,respectively; P < 0.001) and was maintained even at the late 10 th day assessment(97.1% and 93.5% vs67.1% for rSK and hydrocortisone acetate, respectively;P < 0.001). Time to response was 3 d(95%CI: 2.9-3.1)for both rSK groups and 10 d(95%CI: 9.3-10.7) in the hydrocortisone acetate group. This difference was highly significant(P < 0.001). All subgroup stratified analyses(with or without thrombosis and hemorrhoid classification) showed a statistically significant advantage for the rSK groups. Thrombectomy was necessary in4/251 and 14/133 patients with baseline thrombosis in the rSK and hydrocortisone acetate groups, respectively(P < 0.001). | 8 | Conservative management of perforated duodenal diverticulum: A case report and review of the literature显示文摘Duodenal diverticula are a relatively common condition. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. | 9 | Acute pancreatitis complicated with splenic rupture: A case report显示文摘Atraumatic splenic rupture is an uncommon complication of acute pancreatitis. This report describes the case of a 30-year-old man with acute pancreatitis and splenic vein thrombosis complicated by splenic rupture. The patient was admitted to the emergency department with pain in the upper abdomen that had been present for six hours and was associated with vomiting and sweating. He was diagnosed with acute pancreatitis of alcoholic etiology. Upon computed tomography(CT) of the abdomen, the pancreatitis was scored as Balthazar C grade, and a suspicious area of necrosis affecting 30% of the pancreas with splenic vein thrombosis was revealed. Seventytwo hours after admission, the patient had significant improvement in symptoms. However, he showed clinical worsening on the sixth day of hospitalization, with increasing abdominal distension and reduced hemoglobin levels. A CT angiography showed a large amount of free fluid in the abdominal cavity, along with a large splenic hematoma and contrast extravasation along the spleen artery. The patient subsequently underwent laparotomy, which showed hemoperitoneum due to rupture of the splenic parenchyma. | 10 | Effects of feeding yeast(Saccharomyces cerevisiae), organic selenium and chromium mixed on growth performance and carcass traits of hair lambs显示文摘Yeasts and organic minerals are used in diets to improve health, productive performance and some carcass characteristics of ruminants and non-ruminants. Thirty-two lambs(Pelibuey×Katahdin; BW=(30.55±1.67) kg; n=8) were used in a 56-d feeding experiment to study the effects of different levels of live yeast(Saccharomyces cerevisiae; yeast), selenium(Se) and chromium(Cr) mixed(Se-Cr), and a mixture of yeast-Se-Cr on growth performance and carcass traits. Animals were stratified by body weight(BW) and randomly assigned to one of four treatments: 1) control group(0.0 g kg–1 yeast); 2) yeast(1.50 g kg–1 dry matter intake(DMI) d–1); 3) Se-Cr premix(1.5 mg kg–1 DMI d–1 for each mineral); and 4) yeast-Se-Cr mixture. There were no treatment effects on final BW; whereas lambs fed Se-Cr or yeast-Se-Cr had higher(P<0.05) DMI than animals supplemented with only yeast. Average daily gain(ADG), gain:feed ratio, chop area, dorsal fat and carcass yield were similar(P>0.05) among treatment groups. In conclusion, supplementation with yeast, Se-Cr mixed or yeast-Se-Cr did not improve ADG, final BW, back fat content and carcass yield of growing of Pelibuey×Katahdin lambs. | 12 | Sex-specific differences in white matter microvascular integrity after ischaemic stroke显示文摘background and purpose Functional outcomes after ischaemic stroke are worse in women,despite adjusting for differences in comorbidities and treatment approaches.White matter microvascular integrity represents one risk factor for poor long-term functional outcomes after ischaemic stroke.The aim of the study is to characterise sex-specific differences in microvascular integrity in individuals with acute ischaemic stroke.Methods A retrospective analysis of subjects with acute ischaemic stroke and brain MRI with diffusion-weighted(DWI)and dynamic-susceptibility contrast-enhanced(DSC)perfusion-weighted imaging obtained within 9 hours of last known well was performed.In the hemisphere contralateral to the acute infarct,normal-appearing white matter(NAWM)microvascular integrity was measured using the K_(2) coefficient and apparent diffusion coefficient(ADC)values.Regression analyses for predictors of K_(2) coefficient,DWI volume and good outcome(90-day modified Rankin scale(mRS)score<2)were performed.results 105 men and 79 women met inclusion criteria for analysis.Despite no difference in age,women had increased NAWM K_(2) coefficient(1027.4 vs 692.7×10^(-6)/s;p=0.006).In women,atrial fibrillation(β=583.6;p=0.04)and increasing NAWM ADC(β=4.4;p=0.02)were associated with increased NAWM K_(2) coefficient.In multivariable regression analysis,the K_(2) coefficient was an independent predictor of DWI volume in women(β=0.007;p=0.01)but not men.Conclusions In women with acute ischaemic stroke,increased NAWM K_(2) coefficient is associated with increased infarct volume and chronic white matter structural integrity.Prospective studies investigating sex-specific differences in white matter microvascular integrity are needed. | Mark R Etherton Ona Wu Pedro Cougo Svetlana Lorenzano Hua Li Lisa Cloonan Mark J R J Bouts Arne Lauer Ken Arai Eng H Lo Steve K Feske Karen L Furie Natalia S Rost | 2019 | Stroke & Vascular Neurology2019,4,4: | 2 |
| 13 | Late hepatic artery pseudoaneurysm:A rare complication after resection of hilar cholangiocarcinoma显示文摘We report an unusual pathological entity of a pseudoaneurysm of the right hepatic artery, which developed two years after the resection of a type Ⅱ hilar cholangiocarcinoma and secondary to an excessive skeletonization for regional lymphadenectomy and neoadjuvant external-beam radiotherapy. After a sudden and massive hematemesis, a multidetector computed tomographic angiography (MDCTA) showed a hepatic artery pseudoaneurysm. Angiography with embolization of the pseudoaneurysm was attempted using microcoils with adequate patency of the hepatic artery and the occlusion of the pseudoaneurysm. A new episode of hematemesis 3 wk later revealed a partial revascularization of the pseudoaneurysm. | 15 | 策略将在肝移植以后减少丙肝病毒复发显示文摘 Hepatitis C virus (HCV) is a major health problem that leads to chronic hepatitis, cirrhosis and hepatocellular carcinoma, being the most frequent indication for liver transplantation in several countries. Unfortunately, HCV re-infects the liver graft almost invariably following reperfusion, with an accelerated history of recurrence, leading to 10%-30% of patients progressing to cirrhosis within 5 years of transplantation. In this sense, some groups have even advocated for not retransplanting this patients, as lower patient and graftoutcomes have been reported. However, the management of HCV recurrence is being optimized and several strategies to reduce post-transplant recurrence could improve outcomes, decrease the rate of re-transplantation and optimize the use of available grafts. Three moments may be the focus of potential actions in order to decrease the impact of viral recurrence: the pretransplant moment, the transplant environment and the post-transplant management. In the pre-transplant setting, it is not well established if reducing the pre transplant viral load affects the risk for HCV progression after transplant. Obviously, antiviral treatment can render the patient HCV RNA negative post transplant but the long-term benefit has not yet been fully established to justify the cost and clinical risk. In the transplant moment, factors as donor age, cold ischemia time, graft steatosis and ischemia/reperfusion injury may lead to a higher and more aggressive viral recurrence. After the transplant, discussion about immunosuppression and the moment to start the treatment (prophylactic, pre-emptive or once-confirmed) together with new antiviral drugs are of interest.
