| 2 | Ampullary carcinoma:Effect of preoperative biliary drainage on surgical outcome显示文摘AIM: To evaluate the influence of preoperative biliary drainage on morbidity and mortality after surgical resection for ampullary carcinoma. METHODS: We analyzed retrospectively data for 82 patients who underwent potentially curative surgery for ampullary carcinoma between September 1993 and July 2007 at the Singapore General Hospital, a tertiary referral hospital. Diagnosis of ampullary carcinoma was confirmed histologically. Thirty-five patients underwent preoperative biliary drainage (PBD group), and 47 were not drained (non-PBD group). The mode of biliary drainage was endoscopic retrograde cholangiopancreatography (n = 33) or percutaneous biliary drainage (n = 2). The following parameters were analyzed: wound infection, intra-abdominal abscess, intra-abdominal or gastrointestinal bleeding, septicemia, biliary or pancreatic leakage, pancreatitis, gastroparesis, and re-operation rate. Mortality was assessed at 30 d (hospital mortality) and also longterm. The statistical endpoint of this study was patient survival after surgery. RESULTS: The groups were well matched for demographic criteria, clinical presentation and operative characteristics, except for lower hemoglobin in the non-PBD group (10.9 ± 1.6 vs 11.8 ± 1.6 in the PBD group).Of the parameters assessing postoperative morbidity, incidence of wound infection was signif icantly less in the PBD than the non-PBD group [1 (2.9%) vs 12 (25.5%)]. However, the rest of the parameters did not differ signif icantly between the groups, i.e. sepsis [10 (28.6%) vs 14 (29.8%)], intra-abdominal bleeding [1 (2.9%) vs 5 (10.6%)], intra-abdominal abscess [1 (2.9%) vs 8 (17%)], gastrointestinal bleeding [3 (8.6%) vs 5 (10.6%)], pancreatic leakage [2 (5.7%) vs 3 (6.4%)], biliary leakage [2 (5.7%) vs 3 (6.4%)], pancreatitis [2 (5.7%) vs 2 (4.3%)], gastroparesis [6 (17.1%) vs 10 (21.3%)], need for blood transfusion [10 (28.6%) vs 17 (36.2%)] and re-operation rate [1 (2.9%) vs 5 (10.6%)]. There was no early mortality in either group. Median survival was 44 mo (95% CI: 34.2-53.8) in the PBD group and 41 mo (95% CI: 27.7-54.3; P = 0.86) in the non-PBD group. CONCLUSION: Biliary drainage before surgery for ampullary cancer significantly reduced postoperative wound infection. | 3 | High-speed colour-converting photodetector with all-inorganic CsPbBr_(3) perovskite nanocrystals for ultraviolet light communication显示文摘Optical wireless communication(OWC)using the ultra-broad spectrum of the visible-to-ultraviolet(UV)wavelength region remains a vital field of research for mitigating the saturated bandwidth of radio-frequency(RF)communication.However,the lack of an efficient UV photodetection methodology hinders the development of UV-based communication.The key technological impediment is related to the low UV-photon absorption in existing silicon photodetectors,which offer low-cost and mature platforms.To address this technology gap,we report a hybrid Sibased photodetection scheme by incorporating CsPbBr_(3) perovskite nanocrystals(NCs)with a high photoluminescence quantum yield(PLQY)and a fast photoluminescence(PL)decay time as a UV-to-visible colourconverting layer for high-speed solar-blind UV communication.The facile formation of drop-cast CsPbBr_(3) perovskite NCs leads to a high PLQY of up to ~73% and strong absorption in the UV region.With the addition of the NC layer,a nearly threefold improvement in the responsivity and an increase of ~25% in the external quantum efficiency(EQE)of the solar-blind region compared to a commercial silicon-based photodetector were observed.Moreover,timeresolved photoluminescence measurements demonstrated a decay time of 4.5 ns under a 372-nm UV excitation source,thus elucidating the potential of this layer as a fast colour-converting layer.A high data rate of up to 34 Mbps in solar-blind communication was achieved using the hybrid CsPbBr_(3)-silicon photodetection scheme in conjunction with a 278-nm UVC light-emitting diode(LED).These findings demonstrate the feasibility of an integrated high-speed photoreceiver design of a composition-tuneable perovskite-based phosphor and a low-cost silicon-based photodetector for UV communication. | Chun Hong Kang Ibrahim Dursun Guangyu Liu Lutfan Sinatra Xiaobin Sun Meiwei Kong Jun Pan Partha Maity Ee-Ning Ooi Tien Khee Ng Omar F.Mohammed Osman M.Bakr Boon S.Ooi | 2019 | Light(Science & Applications)2019,8,1: | 14 |
| 4 | Preliminary experience with drug-coated balloon angioplasty in primary percutaneous coronary intervention显示文摘We evaluated the clinical feasibility of using drugcoated balloon(DCB) angioplasty in patients undergoingprimary percutaneous coronary intervention(PPCI). Between January 2010 to September 2014,89 STelevation myocardial infarction patients(83% male,mean age 59 ± 14 years) with a total of 89 coronary lesions were treated with DCB during PPCI. Clinical outcomes are reported at 30 d follow-up. Left anterior descending artery was the most common target vessel for PCI(37%). Twenty-eight percent of the patients had underlying diabetes mellitus. Mean left ventricular ejection fraction was 44% ± 11%. DCB-only PCI was the predominant approach(96%) with the remaining 4% of patients receiving bail-out stenting. Thrombolysis in Myocardial Infarction(TIMI) 3 flow was successfully restored in 98% of patients. An average of 1.2 ± 0.5 DCB were used per patient,with mean DCB diameter of 2.6 ± 0.5 mm and average length of 23.2 ± 10.2 mm. At 30-d follow-up,there were 4 deaths(4.5%). No patients experienced abrupt closure of the infarctrelated artery and there was no reported target-lesion failure. | 8 | A study of influence of gravity on bulk behaviour of particulate solid显示文摘This paper examines the influence of gravity on the bulk responses of a granular solid. The loading scenarios in this study include confined compression, rod penetration into a granular medium and discharging through an orifice. Similar loading and flow conditions are likely to be encountered in the stress and deformation regimes that regoliths are subjected to in extraterrestrial exploration activities including in situ resource utilisation processes. Both spherical and non-spherical particles were studied using the discrete element method (DEM). Whilst DEM is increasingly used to model granular solids, careful validations of the simulation outcomes are rather rare. Thus in addition to exploring the effect of gravity, this paper also compares DEM simulations with experiments under terrestrial condition to verify whether DEM can produce satisfactory predictions. The terrestrial experiments were conducted with great care and simulated closely using DEM. The key mechanical and geometrical properties for the particles were measured in laboratory tests for use in the DEM simulations. A series of DEM computations were then performed under reduced gravity to simulate these experiments under extraterrestrial environment. It was found that gravity has no noticeable effect on the force transmission in the confined compression case; the loading gradient in the rod penetration is linearly proportional to the gravity; the mass flow rate in silo discharge is proportional to square root of the gravity and the angle of repose increases with reducing gravity. | 18 | Factors associated with residual gastroesophageal reflux disease symptoms in patients receiving proton pump inhibitor maintenance therapy显示文摘AIM To elucidate the factors associated with residual gastroesophageal reflux disease(GERD) symptoms in patients receiving proton pump inhibitor(PPI) maintenance therapy in clinical practice.METHODS The study included 39 GERD patients receiving maintenance PPI therapy. Residual symptoms were assessed using the Frequency Scale for Symptoms of GERD(FSSG) questionnaire and the Gastrointestinal Symptom Rating Scale(GSRS). The relationships between the FSSG score and patient background factors, including the CYP2C19 genotype, were analyzed.RESULTS The FSSG scores ranged from 1 to 28 points(median score: 7.5 points), and 19 patients(48.7%) had a score of 8 points or more. The patients' GSRS scores were significantly correlated with their FSSG scores(correlation coefficient = 0.47, P < 0.005). In erosive esophagitis patients, the FSSG scores of the CYP2C19 rapid metabolizers(RMs) were significantly higher than the scores of the poor metabolizers and intermediate metabolizers(total scores: 16.7 ± 8.6 vs 7.8 ± 5.4, P < 0.05; acid reflux-related symptom scores: 12 ± 1.9 vs 2.5 ± 0.8, P < 0.005). In contrast, the FSSG scores of the CYP2C19 RMs in the non-erosive reflux disease patients were significantly lower than those of the other patients(total scores: 5.5 ± 1.0 vs 11.8 ± 6.3, P < 0.05; dysmotility symptom-related scores: 1.0 ± 0.4 vs 6.0 ± 0.8, P < 0.01). | 20 | Detecting maize leaf water status by using digital RGB images显示文摘To explore the correlation between crop leaf digital RGB(Red,Green and Blue)image features and the corresponding moisture content of the leaf,a Canon digital camera was used to collect image information from detached leaves of heading-stage maize.A drying method was adopted to measure the moisture content of the leaf samples,and image processing technologies,including gray level co-occurrence matrices and grayscale histograms,was used to extract the maize leaf texture feature parameters and color feature parameters.The correlations of these feature parameters with moisture content were analyzed.It is found that the texture parameters of maize leaf RGB images,including contrast,correlation,entropy and energy,were not significantly correlated with moisture content.Thus,it was difficult to use these features to predict moisture content.Of the six groups of eigenvalues for the leaf color feature parameters,including mean,variance,energy,entropy,kurtosis and skewness,mean and kurtosis were found to be correlated with moisture content.Thus,these features could be used to predict the leaf moisture content.The correlation coefficient(R2)of the mean-moisture content relationship model was 0.7017,and the error of the moisture content prediction was within±2%.The R2 of the kurtosis-moisture content relationship model was 0.7175,and the error of the moisture content prediction was within±1.5%.The study results proved that RGB images of crop leaves could be used to measure moisture content. | Han Wenting Sun Yu Xu Tengfei Chen Xiangwei Su Ki Ooi | 2014 | International Journal of Agricultural and Biological Engineering2014,7,1: | 5 |
