1A rare case of hyaline-type Castleman disease in the liver显示文摘Castleman disease often develops in the neck, mediastinum and pulmonary hilum. Its onset in the peritoneal cavity is very rare. The patient, a woman in her 70s, was referred to our department for a detailed examination of an abdominal mass. On abdominal ultrasonography, computed tomography scan, magnetic resonance imaging and positron emission tomography, a mass approximately 15 mm in diameter was noted in the hepatic S6. We attempted radical treatment and conducted a laparoscope-assisted right lobectomy. 4Effects of an oral iron chelator, deferasirox, on advanced hepatocellular carcinoma显示文摘AIM To evaluate the inhibitory effects of deferasirox(DFX) against hepatocellular carcinoma(HCC) through basic and clinical studies.METHODS In the basic study, the effect of DFX was investigated in three hepatoma cell lines(Hep G2, Hep3 B, and Huh7), as well as in an N-nitrosodiethylamine-induced murine HCC model. In the clinical study, six advanced HCC patients refractory to chemotherapy were enrolled. The initial dose of DFX was 10 mg/kg per day and was increased by 10 mg/kg per day every week, until the maximum dose of 30 mg/kg per day. The duration of a single course of DFX therapy was 28 consecutive days. In the event of dose-limiting toxicity(according to the Common Terminology Criteria for Adverse Events v.4.0), DFX dose was reduced.RESULTS Administration of DFX inhibited the proliferation of hepatoma cell lines and induced the activation of caspase-3 in a dose-dependent manner in vitro. In the murine model, DFX treatment significantly suppressed the development of liver tumors(P < 0.01), and significantly upregulated the mR NA expression levels of hepcidin(P < 0.05), transferrin receptor 1(P < 0.05), and hypoxia inducible factor-1α(P < 0.05) in both tumor and non-tumor tissues, compared with control mice. In the clinical study, anorexia and elevated serum creatinine were observed in four and all six patients, respectively. However, reduction in DFX dose led to decrease in serum creatinine levels in all patients. After the first course of DFX, one patient discontinued the therapy. We assessed the tumor response in the remaining five patients; one patient exhibited stable disease, while four patients exhibited progressive disease. 6Preoperative detection and localization of small bowel hemangioma: Two case reports显示文摘Among the various diagnostic modalities for small bowel hemangioma,video capsule endoscopy(VCE)and double-balloon enteroscopy( BE)can be recommended as part of the work-up in patients with obscure gastrointestinal bleeding(OGIB). 12Calibration of Modified Spalart-Ailmaras Model Parameters for Linear Compressor Cascade Corner Flow显示文摘In this study,Bayesian parameter calibration is applied to Saplart-Allmaras(SA)turbulence model,and the prediction improvement by the calibrated model is demonstrated.The quantity of interest(QOI)is the pitch-wise distribution of Mach number in the comer separation flow region.The 10 model parameters included in the SA model with Rotation-Curvature correction are considered as random variables obeying uniform prior probability distributions.The order of generalized Polynomial Chaos(gPC)used for sensitivity analysis and surrogate model in calibration is incrementally increased during the calibration process.Posterior convergence is obtained at the 3rd order expansion level in this study.At this final level,sensitivity analysis indicates 3 model parameters,cbl,k and cr3 are the most influential random variables,and 3-parameter Bayesian calibration is conducted.The likelihood function in the Bayesian theorem is specified in the form of Gaussian distribution,including experimental uncertainty.The combination of prior and likelihood brings the posterior distribution of model parameters,and Maximum A Posterior(MAP)value is selected as a calibrated parameter set.The flow simulation with calibrated parameters shows a significant increase in the accuracy of the Mach number profile in the comer separation region.The increase in accuracy is attributed to enlarged turbulent viscosity due to the parameter modification of the turbulent viscosity source term.The calibrated parameter is also tested in the off-design flow field,not included in the calibration process.The calibrated CFD again shows improved accuracy for comer separation prediction,and the effectiveness of the parameter set outside of the calibration field is demonstrated.MATSUI Kotaro PEREZ Ethan KELLY TRyan TANI Naoki JEMCOV Aleksandar 2022Journal of Thermal Science2022,31,1:0
13True left-sided gallbladder with variations of bile duct and cholecystic vein显示文摘A left-sided gallbladder without a right-sided round ligament,which is called a true left-sided gallbladder,is extremely rare.A 71-year-old woman was referred to our hospital due to a gallbladder polyp.Computed tomography(CT) revealed not only a gallbladder polyp but also the gallbladder located to the left of the round ligament connected to the left umbilical portion.CT portography revealed that the main portal vein diverged into the right posterior portal vein and the common trunk of the left portal vein and right anterior portal vein.CT cholangiography revealed that the infraportal bile duct of segment 2 joined the common bile duct.Laparoscopic cholecystectomy was performed for a gallbladder polyp,and the intraoperative finding showed that the cholecystic veins joined the round ligament.A true left-sided gallbladder is closely associated with several anomalies; therefore,surgeons encountering a true left-sided gallbladder should be aware of the potential for these anomalies.Hiromichi Ishii Akinori Noguchi Mie Onishi Koji Takao Takahiro Maruyama Hiroaki Taiyoh Yasunobu Araki Takeshi Shimizu Hiroyuki Izumi Naoki Tani Masahide Yamaguchi Tetsuro Yamane 2015World Journal of Gastroenterology2015,21,21:0
