1Literature review on the management of diabetic foot ulcer显示文摘Diabetic foot ulcer(DFU) is the most costly and devastating complication of diabetes mellitus, which affect 15% of diabetic patients during their lifetime.Based on National Institute for Health and Clinical Excellence strategies, early effective management of DFU can reduce the severity of complications such as preventable amputations and possible mortality, and also can improve overall quality of life. The management of DFU should be optimized by using a multidisciplinary team, due to a holistic approach to wound management is required. Based on studies, blood sugar control, wound debridement, advanced dressings and offloading modalities should always be a part of DFU management. Furthermore, surgery to heal chronic ulcer and prevent recurrence should be considered as an essential component of management in some cases. Also, hyperbaric oxygen therapy, electrical stimulation, negative pressure wound therapy, bio-engineered skin and growth factors could be used as adjunct therapies for rapid healing of DFU. 2Effects of rainfall patterns on runoff and rainfall-induced erosion显示文摘Rainfall-induced erosion involves the detachment of soil particles by raindrop impact and their transport by the combined action of the shallow surface runoff and raindrop impact.Although temporal variation in rainfall intensity(pattern)during natural rainstorms is a common phenomenon,the available information is inadequate to understand its effects on runoff and rainfall-induced erosion processes.To address this issue,four simulated rainfall patterns(constant,increasing,decreasing,and increasing-decreasing)with the same total kinetic energy were designed.Two soil types(sandy and sandy loam)were subjected to simulated rainfall using 15 cm×30 cm long detachment trays under infiltration conditions.For each simulation,runoff and sediment concentration were sampled at regular intervals.No obvious difference was observed in runoff across the two soil types,but there were significant differences in soil losses among the different rainfall patterns and stages.For varying-intensity rainfall patterns,the dominant sediment transport mechanism was not only influenced by raindrop detachment but also was affected by raindrop-induced shallow flow transport.Moreover,the efficiency of equations that predict the interrill erosion rate increased when the integrated raindrop impact and surface runoff rate were applied.Although the processes of interrill erosion are complex,the findings in this study may provide useful insight for developing models that predict the effects of rainfall pattern on runoff and erosion.Morteza Alavinia Farzin Nasiri Saleh Hossain Asadi 2019International Journal of Sediment Research2019,34,3:9
3Efficacy and safety of Amla(Phyllanthus emblica L.) in non-erosive reflux disease:a double-blind,randomized,placebo-controlled clinical trial显示文摘Background: Gastroesophageal reflux disease(GERD) is one of the most common gastrointestinal complaints. GERD, caused by the reflux of stomach contents into the esophagus, leads to troublesome symptoms such as heartburn and regurgitation. It is classified into two types: erosive esophagitis,characterized by visible esophageal mucosa erosion in endoscopy, and non-erosive reflux disease(NERD). GERD is a chronic and recurrent disease that impairs the quality of life and imposes socioeconomic and therapeutic burdens to both patients and society.Objective: Due to the failure of the conventional treatments for GERD and to the traditional use of Amla(Phyllanthus emblica L.), in addition to beneficial effects shown in recent studies, we evaluated the safety and efficacy of Amla tablet for improvement of symptoms of patients with NERD.Design, setting, participants and interventions: We designed a double-arm, randomized, double-blind,placebo-controlled clinical trial. Sixty-eight patients who had classic symptoms of GERD(heartburn,regurgitation and epigastralgia) for at least three months before the start of the trial were randomized in two parallel groups. Patients in the Amla group received two 500 mg Amla tablets twice a day, after meals,for 4 weeks. In the control group, patients received placebo tablets similar to the Amla prescription.Main outcome measures: The patients were visited at baseline, and at the end of the 2 nd and 4 th weeks of intervention; their symptoms were measured on a frequency and severity scale for the symptoms of NERD,according to the quality of life in reflux-associated disease questionnaire.Results: Frequencies of heartburn and regurgitation in both groups of the study were significantly reduced after intervention(P < 0.001). 4Corrosion behavior of electron beam processed AZ91 magnesium alloy显示文摘The increasing use of light alloys owing to their high performance makes magnesium alloys very attractive for the use in automotive and biomedical applications.However,it is well known that magnesium and its alloys have poor corrosion resistance in different atmospheric and aqueous environments.As a means of improving corrosion resistance through the microstructure modification,electron beam processing(EBP)was applied on the as-cast AZ91 magnesium alloy.To evaluate the microstructure influence on the corrosion-resistant,the EB processed samples underwent a solution heat treatment and an artificial aging heat treatment.Four different obtained microstructures were investigated by standard microscopy and electrochemical corrosion tests to evaluate the microstructure and its effects on the corrosion resistance of AZ91 alloy.The EBPed specimens show a significant microstructure refinement and homogenous distribution ofβ-phase at the grain boundaries surrounded by supersaturatedα-Mg which acts as a barrier against corrosion.The electrochemical corrosion test of the samples immersed in 3.5 wt%NaCl after 4 weeks indicates that the EBP improves the corrosion resistance of the alloy due to the nobler corrosion potential of supersaturated a-Mg and more stable protective hydroxide films compared to the heat-treated and as-cast conditions.Fatemeh Iranshahi Mohammad Bagher Nasiri Fernando Gustavo Warchomicka Christof Sommitsch 2020Journal of Magnesium and Alloys2020,8,4:7

6Comparison among Ocular Response Analyzer,Corvis ST and Goldmann applanation tonometry in healthy children显示文摘AIM: To explore the relationship between different parameters of Ocular Response Analyzer(ORA) and Corvis ST(CST) in a sample of healthy Iranian school-aged children and the relationship between parameters of these 2 instruments against intraocular pressure(IOP), measured by the Goldmann applanation tonometer(GAT-IOP), age and gender, and find possible correlation between ORA and CST with GAT.METHODS: This cross-sectional study included 90 healthy children. A general interview and complete eye examination were performed. Following successful GATIOP measurement, ORA and CST were conducted. The CST parameters were A 1/2 length(A1 L, A2 L), A 1/2 velocity(A1 V, A2 V), highest concavity deformation amplitude(HCDA), radius of curvature(Ro C), peak distance(PD), central corneal thickness(CCT) and IOP. The ORA parameters were corneal hysteresis(CH), corneal resistance factor(CRF), Goldmann-correlated IOP(IOP-G) and corneal compensated IOP(IOP-CC). Extracted data was analyzed using the Statistical Package for Social Science software.RESULTS: Totally 39 males with age of 9.08±1.60(6-12)y and 51 females with age of 8.96±1.55(6-13)y were included. Many CST parameters were significantly correlated with CH, CRF, IOP-G and IOP-CC. Some CST parameters had a significant correlation with GAT-IOP, including IOP-CST in both eyes and HCDA, A2 L, PD, and RoC in the left eye, but none with age, except A2 L in the right eye. The CRF measurement showed a significant correlation with GAT-IOP in both eyes and CH in the right eye, yet, none with age. Among all CST and ORA parameters, CCT-CST in both eyes and A1 L in right eye had a significant correlation with gender, although this was a negligible negative correlation. Comparison of mean IOP values by different devices showed a significantly highest IOP overestimation by CST and lowest by IOP-CC compared with GAT. Also, IOP-G versus IOP-CST significantly had the lowest IOP overestimation among others. Overall, either low positive correlation or negligible correlation was found between IOP measurements by 3 instruments.CONCLUSION: The study finds the highest IOP overestimation by CST and lowest by IOP-CC compared with GAT. 7Effects of vegetation channel banks and gravel size on flow structure显示文摘Vegetation on river banks and bed roughness are important factors affecting flow structure, sediment transport, erosion and geomorphology in rivers. In this experimental study, the impacts of vegetation on flume walls, grain size of bed gravels and aspect ratio on characteristics of shear stress distribution, Coles' wake parameter, the kinematic energy correction factor (α) and the momentum correction factor (β) have been assessed. Reynolds stress distribution illustrates a three-layer pattern when the aspect ratio is smaller than 2. 11Determination of water balance equation components in irrigated agricultural watersheds using SWAT and MODFLOW models:A case study of Samalqan plain in Iran显示文摘Increasing water demands,especially in arid and semi-arid regions,continuously exacerbate groundwater as the only reliable water resources in these regions.Samalqan watershed,Iran,is a groundwater-based irrigation watershed,so that increased aquifer extraction,has caused serious groundwater depletion.So that the catchment consists of surface water,the management of these resources is essential in order to increase the groundwater recharge.Due to the existence of rivers,the low thickness of the alluvial sediments,groundwater level fluctuations and high uncertainty in the calculation of hydrodynamic coefficients in the watershed,the SWAT and MODFLOW models were used to assess the impact of irrigation return flow on groundwater recharge and the hydrological components of the basin.For this purpose,the irrigation operation tool in the SWAT model was utilized to determine the fixed amounts and time of irrigation for each HRU(Hydrological Response Unit)on the specified day.Since the study area has pressing challenges related to water deficit and sparsely gauged,therefore,this investigation looks actual for regional scale analysis.Model evaluation criteria,RMSE and NRMSE for the simulated groundwater level were 1.8 m and 1.1%respectively.Also,the simulation of surface water flow at the basin outlet,provided satisfactory prediction(R^(2)=0.92,NSE=0.85).Results showed that,the irrigation has affected the surface and groundwater interactions in the watershed,where agriculture heavily depends on irrigation.Annually 11.64 Mm3 water entered to the aquifer by surface recharge(precipitation,irrigation),transmission loss from river and recharge wells 5.8 Mm3 and ground water boundary flow(annually 20.5 Mm^(3)).Water output in the watershed included ground water extraction and groundwater return flow(annually 46.4 Mm^(3))and ground water boundary flow(annually 0.68 Mm^(3)).Overally,the groundwater storage has decreased by 9.14 Mm3 annually in Samalqan aquifer.This method can be applied to simulate the effects of surface water fluxes to groundwater recharge and river-aquifer interaction for areas with stressed aquifers where interaction between surface and groundwater cannot be easily assessed.Shima Nasiri Hossein Ansari Ali Naghi Ziaei 2022Journal of Groundwater Science and Engineering2022,10,1:2
