| 1 | Does Helicobacter pylori infection eradication modify peptic ulcer prevalence? | 2 | Radiomics in colorectal cancer patients显示文摘The main therapeutic options for colorectal cancer are surgical resection and adjuvant chemotherapy in non-metastatic disease.However,the evaluation of the overall adjuvant chemotherapy benefit in patients with a high risk of recurrence is challenging.Radiological images can represent a source of data that can be analyzed by using automated computer-based techniques,working on numerical information coded within Digital Imaging and Communications in Medicine files:This image numerical analysis has been named“radiomics”.Radiomics allows the extraction of quantitative features from radiological images,mainly invisible to the naked eye,that can be further analyzed by artificial intelligence algorithms.Radiomics is expanding in oncology to either understand tumor biology or for the development of imaging biomarkers for diagnosis,staging,and prognosis,prediction of treatment response and diseases monitoring and surveillance.Several efforts have been made to develop radiomics signatures for colorectal cancer patient using computed tomography(CT)images with different aims:The preoperative prediction of lymph node metastasis,detecting BRAF and RAS gene mutations.Moreover,the use of delta-radiomics allows the analysis of variations of the radiomics parameters extracted from CT scans performed at different timepoints.Most published studies concerning radiomics and magnetic resonance imaging(MRI)mainly focused on the response of advanced tumors that under-went neoadjuvant therapy.Nodes status is the main determinant of adjuvant chemotherapy.Therefore,several radiomics model based on MRI,especially on T2-weighted images and ADC maps,for the preoperative prediction of nodes metastasis in rectal cancer has been developed.Current studies mostly focused on the applications of radiomics in positron emission tomogra-phy/CT for the prediction of survival after curative surgical resection and assessment of response following neoadjuvant chemoradiotherapy.Since colorectal liver metastases develop in about 25%of patients with colorectal carcinoma,the main diagnostic tasks of radiomics should be the detection of synchronous and metachronous lesions.Radiomics could be an additional tool in clinical setting,especially in identifying patients with high-risk disease.Nevertheless,radiomics has numerous shortcomings that make daily use extremely difficult.Further studies are needed to assess performance of radiomics in stratifying patients with high-risk disease. | Riccardo Inchingolo Cesare Maino Roberto Cannella Federica Vernuccio Francesco Cortese Michele Dezio Antonio Rosario Pisani Teresa Giandola Marco Gatti Valentina Giannini Davide Ippolito Riccardo Faletti | 2023 | World Journal of Gastroenterology2023,29,19: | 4 |

| 5 | Role of dynamic perfusion magnetic resonance imaging in patients with local advanced rectal cancer显示文摘BACKGROUND The management of rectal cancer patients is mainly based on the use of the magnetic resonance imaging(MRI)technique as a diagnostic tool for both staging and restaging.After treatment,to date,the evaluation of complete response is based on the histopathology assessment by using different tumor regression grade(TRG)features(e.g.,Dworak or Mandard classifications).While from the radiological point of view,the main attention for the prediction of a complete response after chemotherapy treatment focuses on MRI and the potential role of diffusion-weighted images and perfusion imaging represented by dynamiccontrast enhanced MRI.The main aim is to find a reliable tool to predict tumor response in comparison to histopathologic findings.AIM To investigate the value of dynamic contrast-enhanced perfusion-MRI parameters in the evaluation of the healthy rectal wall and tumor response to chemo-radiation therapy in patients with local advanced rectal cancer with histopathologic correlation.METHODS Twenty-eight patients with biopsy-proven rectal adenocarcinoma who underwent a dynamic contrast-enhanced MR study performed on a 1.5 T MRI system(Achieva,Philips),before(MR1)and after chemoradiation therapy(MR2),were enrolled in this study.The protocol included T1 gadolinium enhanced THRIVE sequences acquired on axial planes.A dedicated workstation was used to generate color permeability maps.Region of interest was manually drawn on tumor tissue and normal rectal wall,hence the following parameters were calculated and statistically analyzed:Relative arterial enhancement(RAE),relative venous enhancement(RVE),relative late enhancement(RLE),maximum enhancement(ME),time to peak and area under the curve(AUC).Perfusion parameters were related to pathologic TRG(Mandard’s criteria;TRG1=complete regression,TRG5=no regression).RESULTS Ten tumors(36%)showed complete or subtotal regression(TRG1-2)at histology and classified as responders;18 tumors(64%)were classified as non-responders(TRG3-5).Perfusion MRI parameters were significantly higher in the tumor tissue than in the healthy tissue in MR1(P<0.05).At baseline(MR1),no significant difference in perfusion parameters was found between responders and nonresponders.After chemo-radiation therapy,at MR2,responders showed significantly(P<0.05)lower perfusion values[RAE(%)54±20;RVE(%)73±24;RLE(%):82±29;ME(%):904±429]compared to non-responders[RAE(%):129±45;RVE(%):154±39;RLE(%):164±35;ME(%):1714±427].Moreover,in responders group perfusion values decreased significantly at MR2[RAE(%):54±20;RVE(%):73±24;RLE(%):82±29;ME(%):904±429]compared to the corresponding perfusion values at MR1[RAE(%):115±21;RVE(%):119±21;RLE(%):111±74;ME(%):1060±325];(P<0.05).Concerning the time-intensity curves,the AUC at MR2 showed significant difference(P=0.03)between responders and non-responders[AUC(mm2×10-3)121±50 vs 258±86],with lower AUC values of the tumor tissue in responders compared to nonresponders.In non-responders,there were no significant differences between perfusion values at MR1 and MR2.CONCLUSION Dynamic contrast perfusion-MRI analysis represents a complementary diagnostic tool for identifying vascularity characteristics of tumor tissue in local advanced rectal cancer,useful in the assessment of treatment response. | Davide Ippolito Silvia Girolama Drago Anna Pecorelli Cesare Maino Giulia Querques Ilaria Mariani Cammillo Talei Franzesi Sandro Sironi | 2020 | World Journal of Gastroenterology2020,26,20: | 3 |
| 6 | Cambro-Silurian magmatisms at the northern Gondwana margin(Penninic basement of the Ligurian Alps)显示文摘The Early Paleozoic evolution of the northern margin of Gondwana is characterized by several episodes of bimodal magmatism intruded or outpoured within thick sedimentary basins. These processes are well recorded in the Variscan blocks incorporated in the Ligurian Alps because they experienced low temperature Alpine metamorphism. During the Paleozoic, these blocks, together with the other Alpine basements, were placed between the Corsica-Sardinia and the Bohemian Massif along the northern margin of Gondwana. In this framework, they host several a variegated lithostratigraphy forming two main complexes(Complexs I and II) that can be distinguished by both the protoliths and their crosscutting relationships, which indicate that the acidic and mafic intrusives of Complex II cut an already folded sequence made of sediments, basalts and granitoids of Complex I. Both complexes were involved in the Variscan orogenic phases as highlighted by the pervasive eclogite-amphibolite facies schistosity(foliation II). However, rare relicts of a metamorphic foliation at amphibolite facies conditions(foliation I)is locally preserved only in the rocks of Complex I. It is debatable if this schistosity was produced during the early folding event e occurred between the emplacement of Complex I and II e rather than during an early stage of the Variscan metamorphic cycle.New SHRIMP and LA ICP-MS Ue Pb zircon dating integrated with literature data, provide emplacement ages of the several volcanic or intrusive bodies of both complexes. The igneous activity of Complex I is dated between 507 ± 15 Ma and 494 ± 5 Ma, while Complex II between 467 ± 12 Ma and 445.5 ± 12 Ma.The folding event recorded only by the Complex I should therefore have occurred between 494 ± 5 Ma and 467 ± 12 Ma. The Variscan eclogite-amphibolite facies metamorphism is instead constrained between ~420 Ma and ~300 Ma. These ages and the geochemical signature of these rocks allow constraining the Early Paleozoic tectono-magmatic evolution of the Ligurian blocks, from a middleeupper Cambrian rifting stage, through the formation of an Early Ordovician volcanic arc during the Rheic Ocean subduction, until a Late Ordovician extension related to the arc collapse and subsequent rifting of the PaleoThetys.
