1Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved.Shao,P.F. 3Upregulation of vimentin and aberrant expression of E-cadherin /beta-catenin complex in oral squamous cell carcinomas:correlation with the clinicopathological features and patient outcome显示文摘Oral squamous cell carcinoma is a challenging oncology problem.A reliable biomarker for metastasis or high-risk prognosis in oral cancer patients remains undefined.Using quantitative immunohistochemistry,we examined the expression of vimentin,E-cadherin,and beta-catenin in 83 oral squamous cell carcinoma patients,and the relationships between the expression of these markers and specific clinicopathological features were analysed.The high expression of vimentin was observed in 23 of 43(53%) tumours from patients who eventually developed a recurrent tumour and was associated with recurrence and death(P < 0.001 and < 0.001,respectively).The decreased expression of E-cadherin was observed in 36 of 43(84%) tumours from patients who eventually developed a recurrent tumour and was also associated with recurrence and death(P < 0.001 and < 0.001,respectively).Although no correlation between beta-catenin expression in whole-tumour sections and clinicopathological features was observed,decreased beta-catenin expression at the tumour invasive front was closely associated with recurrence and death(P=0.002 and 0.002,respectively).The expression of vimentin and that of E-cadherin were associated with survival and were independent prognostic factors in univariate and multivariate analyses.Our data show that the overexpression of vimentin was closely associated with recurrence and death in oral squamous cell carcinoma patients.The combination of the upregulation of vimentin and aberrant expression of E-cadherin/beta-catenin complexes at the tumour invasive front may provide a useful prognostic marker in oral squamous cell carcinoma.Liu,LK Jiang,XY Zhou,XX Wang,DM Song,XL Jiang,HB Nanjing Med Univ,Coll Stomatol,Dept Oral Pathol,Nanjing 210029,Jiangsu,Peoples R China Nanjing Med Univ,Inst Stomatol,Dept Oral Pathol,Nanjing 210029,Jiangsu,Peoples R China Nanjing Med Univ,Coll Stomatol,Dept Oral & Maxillofacial Surg,Nanjing 210029,Jiangsu,Peoples R China 2010南京医科大学学报(自然科学版)2010,30,5:25

5High expression levels of IKK alpha and IKK beta are necessary for the malignant properties of liver cancer显示文摘IKK-NF-kappa B signaling is regarded as an important factor in hepatocarcinogenesis and a potential target for liver cancer therapy.Therefore,in this study,we analyzed the expression of mRNAs encoding components and targets of NF-kappa B signaling including IKK alpha,IKK beta,RANK,RANKL,OPG,CyclinD3,mammary serine protease inhibitor(Maspin),CyclinD1,c-FLIP,Bcl-x1,Stat3,Cip1 and Cip2 by real-time PCR in 40 patients with liver cancer.After statistical analysis,7 indices including IKK alpha,IKK beta,RANK,Maspin,c-FLIP,Cip2 and cyclinD1 were found to show significant differences between tumor tissue and its corresponding adjacent tissue.When IKK alpha and IKK beta were downregulated in the hepatocellular carcinoma(HCC) cell lines of MHCC-97L and MHCC-97H in vitro,the numbers of BrdU positive cells were decreased in both IKK alpha and IKK beta knockdown cells.Levels of apoptosis were also investigated in IKK alpha and IKK beta knockdown cells.The growth of HCC was inhibited in the subcutaneous implantation model,and lung metastatogenesis was also significantly inhibited in the kidney capsule transplantation model.Downregulation of IKK alpha and IKK beta in HCC cultured in vitro revealed that increased Maspin,OPG and RANKL expression was associated with metastasis of HCC.These findings were associated with downregulation of Bcl-XL and c-FLIP,which may be the reason for increased apoptosis.The therapeutic effect of IKK alpha and IKK beta downregulation depends on extent of NF-kappa B inhibition and the malignant nature of the HCC.We anticipate that IKK-targeted gene therapy can be used in the treatment of HCC,a cancer that is notoriously resistant to radiation and chemotherapy.Jiang,RQ Xia,YX Li,J Deng,L Zhao,L Shi,J Wang,XH Sun,BC Nanjing Med Univ,Liver Transplantat Ctr,Affiliated Hosp 1,Nanjing,Jiangsu Prov,Peoples R China Nanjing Med Univ,Ctr Canc,Nanjing,Jiangsu Prov,Peoples R China 2010南京医科大学学报(自然科学版)2010,30,5:13

7K-ATP channel openers facilitate glutamate uptake by GluTs in rat primary cultured Astrocytes显示文摘Increasing evidence, including from our laboratory, has revealed that opening of ATP sensitive potassium channels(K-ATP channels) plays the neuronal protective roles both in vivo and in vitro. Thus K-ATP channel openers(KCOs) have been proposed as potential neuroprotectants. Our previous studies demonstrated that K-ATP channels could regulate glutamate uptake activity in PC12 cells as well as in synaptosomes of rats. Since glutamate transporters(GluTs) of astrocytes play crucial roles in glutamate uptake and KATP channels are also expressed in astrocytes, the present study showed whether and how KATP channels regulated the function of GluTs in primary cultured astrocytes. The results showed that nonselective KCO pinacidil, selective mitochondrial KCO diazoxide, novel, and blood-brain barrier permeable KCO iptakalim could enhance glutamate uptake, except for the sarcolemmal KCO P1075. Moreover pinacidil, diazoxide, and iptakalim reversed the inhibition of glutamate uptake induced by 1-methyl-4-phenylpyridinium(MPP+). These potentiated effects were completely abolished by mitochondrial K-ATP blocker 5-hydroxydecanoate. Furthermore, either diazoxide or iptakalim could inhibit MPP+-induced elevation of reactive oxygen species (ROS) and phosphorylation of protein kinases C(PKC). These findings are the first to demonstrate that activation of K-ATP channel, especially mitochondrial K-ATP channel, improves the function of GluTs in astrocytes due to reducing ROS production and downregulating PKC phosphorylation. 10Pathological and biochemical alterations of astrocytes in ovariectomized rats injected with D-galactose:A potential contribution to Alzheimer's disease processes显示文摘Astrocytes are implicated in the pathological changes of Alzheimer's disease. Our previous studies have demonstrated that estrogen deprivation and oxidative stress act synergistically to accelerate the progress of Alzheimer's disease. Long-term D-galactose injection combined with ovariectomy may serve as a rodent model for Alzheimer's disease. To address the potential contribution of astroglia to the Alzheimer's disease pathogenesis, we investigated pathological and biochemical alterations of astrocytes under this animal model. Ovadectomized rats injected with D-galactose for 2 weeks showed extensive localization of glial fibrillary acidic protein immunoreactive astrocytes and slightly elevated glutathione levels in the hippocampus without significant impairments in the water maze test and deficits of the cholinergic analyses, compared to the saline-injected rats. Ovariectomized rats injected with D-galactose for 6 weeks, however, exhibited degeneration of astrocytes and decreased glutathione levels in the hippocampus, accompanied with severe dysfunction of behavioral test and deficiency of cholinergic terminals. Electron microscopy further confirmed the pathological changes of astrocytes, especially in the aggregated area of synapse and brain microvessels. Consistent with degeneration of perivascular astrocytic endfeet, analysis of the horseradish peroxidase demonstrated an impairment of the blood-brain barrier permeability. These findings indicate that biochemical and pathological alterations of astrocytes may partially contribute to exacerbating neuronal deficits in the course of Alzheimer's disease. 12The level of oncogene H-Ras correlates with tumorigenicity and malignancy显示文摘Normal bovine adrenocortical cells and some human fibroblasts can be transformed by SV40T and H-Ras in a Ras-dependent manner. We recently reported that high levels of Ras derived from 5' LTR of retrovirrus can induce highly malignant and fast growing tumors, while lower levels of Ras derived from internal ribosome entry site(IRES) promotes slower tumor growth and loss of malignancy. Ras derived from CMV promoters resulted in much lower Ras levels and loss of tumor malignancy and growth. Further studies showed that the tumors formed in the presence of lower levels of Ras and dominant negative P53(P53DD) had fewer apoptotic cells and grew faster than the tumors formed from cells with same level Ras and SV40T. Our studies suggest that low levels of Ras are insufficient to inhibit apoptosis induced by pRb inactivation. 15On the Optimal Design of Cable Driven Parallel Robot with a Prescribed Workspace for Upper Limb Rehabilitation Tasks显示文摘This paper deals with an optimization approach to design a cable driven parallel robot intended for upper limb rehabilitation tasks. The cable driven parallel robots have characteristics that make them best candidate for rehabilitation exercise purposes such as large workspace, re-configurable architecture, portability and cost effectiveness. Here, both the cable tensions that are needed to move a wristband as well as the workspace need to be carefully optimized for fulfilling the prescribed operation tasks. A specific case of study is addressed in this work by referring to LA RM wire driven exercising device (LAWEX), which is applied to upper limbs exercises. To that end, a motion capture system is used to collect quantitative data on the prescribed workspace of a human upper limb. A specific optimization problem is settled up for considering combining two optimization goals, namely, the smallest robot size reaching a prescribed workspace and the minimum cable tension distributions. A sequence of optimization steps is defined using Genetic Algorithms (GAs) applied to LAWEX robot. 16Changes of PTH, 1,25(OH)_2D_3 and Biochemistry in Renal Osteodystrophy.显示文摘The changes of plasma PTH, 1,25(OH)2D3, serum calcium, phosphorus and AKP were observed in 55 uremic patients with renal osteodystrophy. The results showed that: in all types of renal osteodystrophy, plasma PTH were markedly higher than normal (P<0.001), while plasma 1,25(OHH)2D3 and serum calcium were lower than normal (P<0.001). In low bone-turnover uremic osteodystrophy (LTO), plasma phosphorus was markedly lower than that of high bone-turnover uremic osteodystrophy (HTO) and mixe d uremic osteodystrophy (MO) (P<0.05), and plasma 1,25(OH)2D3 was significantly lower than that of HTO (P<0.05). In HTO and MO, serum phosphorus was much higher than mormal (P<0.001). Plasma PTH had a negative linear correlation with plasma 1,25(OH)2D3 (P<0.05), a positive linear correlation with serum AKP (P<0.05), but no linear correlation with the duration of hemodialysis, serum calcium, serum aluminum and bone aluminum content.
