1Pathophysiological 角色和在糖尿病的 nephropathy 的发炎的治疗学的含意显示文摘 Diabetes mellitus and its complications are becoming one of the most important health problems in the world. Diabetic nephropathy is now the main cause of end-stage renal disease. The mechanisms leading tothe development and progression of renal injury are not well known. Therefore, it is very important to f ind new pathogenic pathways to provide opportunities for early diagnosis and targets for novel treatments. At the present time, we know that activation of innate immunity with development of a chronic low grade inflammatory response is a recognized factor in the pathogenesis of diabetic nephropathy. 3Ligation of intersphincteric fistula tract:What is the evidence in a review?显示文摘Broadly,complex fistulas are those that are not low transsphincteric or intersphincteric. The objectives of surgical management are to achieve fistula healing,prevent recurrences and maintain continence. The risk of incontinence associated with treatment ranges from10% to 57%. The objective of this manuscript is to review the current literature to date on the ligation of the intersphincteric fistula tract procedure(LIFT procedure) as a treatment option in these types of fistula.A search was conducted in Medline,PUBMED,EMBASE and ISI Web of Knowledge,and studies published from January 2009 to May 2013 were included. The primary outcomes were fistula healing rates,mean healing time and patient satisfaction with this surgical technique.Eighteen studies were included in this review. The total number of patients included was 592(65% male).The median age reported was 42.8 years. The most common type of fistula included was transsphincteric(73.3% of cases). The mean healing rate reported was74.6%. The risk factors for failure discovered were obesity,smoking,multiple previous surgeries and the length of the fistula tract. The mean healing time was5.5 wk,and the mean follow-up period was 42.3 wk.The patient satisfaction rates ranged from 72% to 100%. No de novo incontinence developed secondary to the LIFT procedure. There is not enough evidence that variants in the surgical technique achieve better outcomes(Bio-LIFT,LIFT-Plug,LIFT-Plus). This review indicates that the LIFT procedure is primarily effective for transsphincteric fistulas with an overall fistula closure of 74.6% and has a low impact on fecal continence. 4Effect of Exogenous Melatonin on Seed Germination and Seedling Growth in Melon(Cucumis melo L.) Under Salt Stress显示文摘An investigation was carried out to evaluate the effect of different application forms and concentrations of melatonin on germination and initial growth of melon plants in several different salinity conditions. We evaluated germination percentage and growth of seedlings and measured total chlorophyll content, relative electrolyte leakage, peroxidase activity, root viability, proline concentration, relative water content,Na^+/K^+ratio in green parts and K^+leakage from roots. Seed germination percentage under salt stress increased with treatments of 10 and50 μmol·L^(-1) melatonin. Seed treatment and plant watering with melatonin at 50 μmol·L^(-1) improved growth parameters and plants showed higher chlorophyll content as well as better biochemical parameters than non-treated plants. 5Increased DNA binding activity of NF-κB,STAT-3,SMAD3 and AP-1 in acutely damaged liver显示文摘AIM: To investigate the role of genes and kinetics of specific transcription factors in liver regeneration, and to analyze the gene expression and the activity of some molecules crucially involved in hepatic regeneration. METHODS: USING gel-shift assay and RT-PCR, transcription factors, such as NF-κB, STAT-3, SMAD3 and AP-1, and gene expression of inducible nitric oxide synthase (iNOS), hepatocyte growth factor (HGF) and c-met were analyzed in an animal model of chemically induced hepatectomy. RESULTS: Gene expression of HGF and its receptor c-met peaked at 3 h and 24 h after acute CCl4 intoxi- cation. iNOS expression was only observed from 6 to 48 h. Transcriptional factor NF-κB had an early activation at 30 min after acute liver damage. STAT-3 peaked 3 h post- intoxication, while AP-1 displayed a peak of activation at 48 h. SMAD3 showed a high activity at all analyzed times. CONCLUSION: TNF-α and IL-6 play a central role in hepatic regeneration. 7Speckle tracking echocardiography to assess regional ventricular function in patients with apical hypertrophic cardiomyopathy显示文摘AIM To explore regional systolic strain of midwall and endocardial segments using speckle tracking echocardiography in patients with apical hypertrophic cardiomyopathy(HCM).METHODS We prospectively assessed 20 patients(mean age 53 ± 16 years,range:18-81 years,10 were male),with apical HCM. We measured global longitudinal peak systolic strain(GLPSS) in the midwall and endocardium of the left ventricle. RESULTS The diastolic thickness of the 4 apical segments was 16.25 ± 2.75 mm. All patients had a normal global systolicfunction with a fractional shortening of 50% ± 8%. In spite of supernormal left ventricular(LV) systolic function,midwall GLPSS was decreased in all patients,more in the apical(-7.3% ±-8.8%) than in basal segments(-15.5% ±-6.93%),while endocardial GLPPS was significantly greater and reached normal values(apical:-22.8% ±-7.8%,basal:-17.9% ±-7.5%). CONCLUSION This study shows that two-dimensional strain was decreased mainly confined to the mesocardium,while endocardium myocardial deformation was preserved in HCM and allowed to identify subclinical LV dysfunction. This transmural heterogeneity in systolic strain had not been previously described in HCM and could be explained by the distribution of myofibrillar disarray in deep myocardial areas. 11Increased incidence of and microbiologic changes in pyogenic liver abscesses in the Mexican population显示文摘BACKGROUND Pyogenic liver abscess(PLA)is a rare disease with an estimated incidence that varies widely across the globe,being as high as 115.4/100000 habitants in Taiwan and as low as 1.1-1.2/100000 habitants in Europe and Canada.Even though there are multiple microorganisms capable of producing an abscess in the liver,including Entamoeba histolytica,fungi,and viruses,most abscesses are derived from bacterial infections.The epidemiology of PLA in Mexico is currently unknown.AIM To describe the clinical,demographic and microbiologic characteristics of PLA in Mexico.METHODS This is a retrospective study carried out in two centers,and included patients seen between 2006 and 2018 with the diagnosis of pyogenic abscess.We collected demographic,clinical,and microbiological information,treatment,complications,and outcomes.A logistic regression analysis was used to determine the association between different variables and mortality rates.RESULTS A total of 345 patients were included in this study.233(67.5%)had confirmed PLA,133(30%)patients had no positive culture and negative serology and 9(2.5%)had mixed abscesses.The mean age was 50 years(ranging from 16-97 years)and 63%were female.65%of the patients had positive cultures for Extended Spectrum Beta-Lactamases(ESBL)-Escherichia coli and Klebsiella pneumoniae.Cefotaxime was administered in 60%of cases.The most common sources of infection were ascending cholangitis and cholecystitis in 34(10%)and 31(9%),respectively.The median length of hospital stay was 14 d.165 patients underwent percutaneous catheter drainage.The inpatient mortality rate was 63%.Immunocompromised state[OR 3.9,95%CI:1.42-10.46],ESBL-Escherichia coli[OR 6.7,95%CI:2.7-16.2]and Klebsiella pneumoniae[OR 4-8,95%CI:1.6-14.4]predicted inpatient mortality by multivariate analysis.CONCLUSION The prevalence of PLA is increasing in Mexico and has a very high mortality rate.ESBL-Escherichia coli and Klebsiella pneumoniae are the most common microorganisms causing PLA and are independent predictors of inpatient mortality.Juanita Pérez-Escobar Wagner Ramirez-Quesada Daniel Alejandro Calle-Rodas Luis Alberto Chi-Cervera Nalu Navarro-Alvarez Jorge Aquino-Matus Juan Pablo Ramírez-Hinojosa Carlos Moctezuma-Velázquez Aldo Torre 2020World Journal of Hepatology2020,12,10:5

14Diabetic gastroenteropathy:An underdiagnosed complication显示文摘This article is an extensive review that provides an update on the pathophysiology,symptoms,diagnosis,and treatment of diabetic gastroenteropathy.There is no reported prevalence,but it has been described that patients with type 1 diabetes have a cumulative incidence at 10 years of 5.2%,and type 2 patients,1%.Also,in the group of type 1 diabetes,it has been observed that women are more likely to present this condition(5.8%vs 3.5%).Many factors are associate with its development(e.g.,hyperglycemia,vagal dysfunction,loss of expression of neural nitric oxide synthase in the myenteric plexus,alterations in the Cajal interstitial cell network,and oxidative stress).Gastrointestinal discomfort could be perceived 70% higher in diabetic patients,describing that 25%of diabetic patients experience gastrointestinal symptoms.Diabetic enteropathy could affect any portion of the gastrointestinal tract,but esophageal alterations were described in more than 60% of diabetic patients,also 60% of them present constipation,and 20%,diarrhea.Gastric emptying scintigraphy is useful to evaluate gastroparesis,therefore,gastric retention of more than 60%at 2 h has a sensitivity of 100% and specificity of 20% for diagnosis;however,other studies such as breath tests,with a sensitivity of 89% and a specificity of 80%,or the endoscopic capsule contribute to the diagnosis.There is no cure;however,management must be multidisciplinary,focused on slowing the progression of diabetic gastroenteropathy,reducing symptoms,and restoring function;that includes nutritional recommendation,maintain glucose levels kept below 180 mg/dL,use of prokinetics,anti-emetics;nowadays,it has been special interest in surgical treatment,such as pyloroplasty,also gastric electrical stimulation appears to be another alternative.Marcio JoséConcepción Zavaleta Jhean Gabriel Gonzáles Yovera Diego Martín Moreno Marreros Luciana del Pilar Rafael Robles Kely Roxana Palomino Taype Karen Nohelly Soto Gálvez Luis Fernando Arriola Torres Julia Cristina Coronado Arroyo Luis Alberto Concepción Urteaga 2021World Journal of Diabetes2021,12,6:4
15A techno-economic and life cycle assessment for the production of green methanol from CO_(2): catalyst and process bottlenecks显示文摘The success of catalytic schemes for the large-scale valorization of CO_(2) does not only depend on the development of active,selective and stable catalytic materials but also on the overall process design.Here we present a multidisciplinary study(from catalyst to plant and techno-economic/lifecycle analysis)for the production of green methanol from renewable H2 and CO_(2).We combine an in-depth kinetic analysis of one of the most promising recently reported methanol-synthesis catalysts(InCo)with a thorough process simulation and techno-economic assessment.We then perform a life cycle assessment of the simulated process to gauge the real environmental impact of green methanol production from CO_(2).Our results indicate that up to 1.75 ton of CO_(2) can be abated per ton of produced methanol only if renewable energy is used to run the process,while the sensitivity analysis suggest that either rock-bottom H2 prices(1.5$kg1)or severe CO_(2) taxation(300$per ton)are needed for a profitable methanol plant.Besides,we herein highlight and analyze some critical bottlenecks of the process.Especial attention has been paid to the contribution of H2 to the overall plant costs,CH4 trace formation,and purity and costs of raw gases.In addition to providing important information for policy makers and industrialists,directions for catalyst(and therefore process)improvements are outlined.Tomas Cordero-Lanzac Adrian Ramirez Alberto Navajas Lieven Gevers Sirio Brunialti Luis MGandía Andrés T.Aguayo S.Mani Sarathy Jorge Gascon 2022Journal of Energy Chemistry2022,31,5:3
16Acute renal injury after partial hepatectomy显示文摘Currently, partial hepatectomy is the treatment of choice for a wide variety of liver and biliary conditions. Among the possible complications of partial hepatectomy, acute kidney injury(AKI) should be considered as an important cause of increased morbidity and postoperative mortality. Difficulties in the data analysis related to postoperative AKI after liver resections are mainly due to the multiplicity of factors to be considered in the surgical patients, moreover, there is no consensus of the exact definition of AKI after liver resection in the literature, which hampers comparison and analysis of the scarce data published on the subject. Despite this multiplicity of risk factors for postoperative AKI after partial hepatectomy, there are main factors that clearly contribute to its occurrence. First factor relates to large blood losses with renal hypoperfusion during the operation, second factor relates to the occurrence of post-hepatectomy liver failure with consequent distributive circulatory changes and hepatorenal syndrome. 17A novel motion tracking system for evaluation of functional rehabilitation of the upper limbs显示文摘Upper limb function impairment is one of the most common sequelae of central nervous system injury, especially in stroke patients and when spinal cord injury produces tetraplegia. Conventional assessment methods cannot provide objective evaluation of patient performance and the effectiveness of therapies. The most common assessment tools are based on rating scales, which are inefficient when measuring small changes and can yield subjective bias. In this study, we designed an inertial sensor-based monitoring system composed of five sensors to measure and analyze the complex movements of the upper limbs, which are common in activities of daily living. We developed a kinematic model with nine degrees of freedom to analyze upper limb and head movements in three dimensions. This system was then validated using a commercial optoelectronic system. 18Risk factors for acute kidney injury after partial hepatectomy显示文摘AIM To identify risk factors for the occurrence of acute kidney injury(AKI) in the postoperative period of partial hepatectomies. METHODS Retrospective analysis of 446 consecutive resections in 405 patients, analyzing clinical characteristics, preoperative laboratory data, intraoperative data, and postoperative laboratory data and clinical evolution. Adopting the International Club of Ascites criteria for the definition of AKI, potential predictors of AKI by logistic regression were identified. RESULTS Of the total 446 partial liver resections, postoperative AKI occurred in 80 cases(17.9%). Identified predictors of AKI were: Non-dialytic chronic kidney injury(CKI), biliary obstruction, the Model for End-Stage Liver Disease(MELD) score, the extent of hepatic resection, the occurrence of intraoperative hemodynamic instability, post-hepatectomy haemorrhage, and postoperative sepsis. 20Characterization of goat whey proteins and their bioactivity and toxicity assay显示文摘Goat whey is an industry-discarded dairy by-product that has interesting nutritional value and a nutrient composition with important functional potential,where proteins have a high biological value and play an important role in the organism’s physiology.Therefore,the aim of this study was to identify,quantify,concentrate and investigate the capacity of goat whey proteins to perform different biological activities and to evaluate their acute toxicity.Concentrated proteins were quantified and identified by Bradford,Kjehdal and SDS-PAGE methods.Antibacterial activity was performed against Listeria monocytogenes,Salmonella spp.,Staphylococcus aureus,Pseudomonas aeruginosa and Escherichia coli microorganisms and showed MIC ranging from 15 to 120μg mL-1,with the highest inhibition for L.monocytogenes growth.Antiproliferative activity was performed against human malignant melanoma cells and their proliferation inhibition was up to 66.8%within 72 h of exposure.The highest antioxidant capacity was obtained at concentration 2 mg that showed 29.69%of inhibition of DPPH radical oxidation.The acute toxicity test was performed using Artemia franciscana larvae in which whey proteins were able to preserve larval survival throughout exposure.Thus,we can conclude that goat’s whey has proteins that can play a positive effect on health,with antimicrobial activity and without toxicity.The present study highlights the great potential of goat’s whey to be developed as a safe biopreservative or as a functional ingredient in the food industry.Maria Isabel Ferreira Campos Paula Perazzo de Souza Barbosa Laura Junqueira Camargo Luciano Da Silva Pinto Bianca Mataribu Catarina Serr˜ao Luis Fernando Marques-Santos Jos´e Hon´orio Lopes Julia Mariano Caju de Oliveira Carlos Alberto de Almeida Gadelha Tatiane Santi-Gadelha 2022Food Bioscience2022,46,2:2
