1Laparoscopic vs open approach to resection of hepatocellular carcinoma in patients with known cirrhosis:Systematic review and meta-analysis显示文摘AIM: To review the currently available literature comparing laparoscopic to open resection of hepatocellular carcinoma(HCC) in patients with known liver cirrhosis.METHODS: A literature search of MEDLINE, EMBASE, and Cochrane databases was conducted. The search terms used included(laparoscopic OR laparoscopy) AND(hepatic or liver) AND(surgery or resection) AND 'hepatocellular carcinoma' AND(cirrhosis or cirrhotic). Furthermore, to widen the search, we also used the 'related articles' section. Studies reporting a comparison of outcomes and methods of open vs laparoscopic hepatic resection for HCC in patients with liver cirrhosis were included. Meta-analysis of results was performed using a random effects model to compute relative risk(RR) and for dichotomous variablesand standard mean differences(SMD) for continuous variables.RESULTS: A total of 420 patients from 4 cohort studies were included in final analysis. Patients undergoing laparoscopic procedures had statistically less blood loss compared to the open cohort, SMD of-1.01(95%CI:-1.23-0.79), P < 0.001, with a reduced risk of transfusion, RR = 0.19(95%CI: 0.09-0.38), P < 0.001. A wider clearance at tumour resection margins was achieved following a laparoscopic approach, SMD of 0.34(95%CI: 0.08-0.60), P = 0.011. No significant difference was noted between laparoscopic and open resection operative times, SMD of-0.15(95%CI: 0.35-0.05), P = 0.142. The overall RR of suffering from postoperative morbidity is 0.25 in favour of the open surgery cohort(95%CI: 0.17-0.37), P < 0.001. Patients under-going laparoscopic surgery had significantly shorter length of stays in hospital compared to the open cohort, SMD of-0.53(95%CI:-0.73 to-0.32), P < 0.001.CONCLUSION: This review suggests that laparoscopic resection of hepatocellular carcinoma in patients with cirrhosis is safe and may provide improved patient outcomes when compared to the open technique.Ahmed Twaij Philip H Pucher Mikael H Sodergren Tamara Gall Ara Darzi Long R Jiao 2014World Journal of Gastroenterology2014,20,25:16
2Surgical treatment for liver cancer显示文摘Primary liver cancer is amongst the commonest tumors worldwide,particularly in parts of the developing world,and is increasing in incidence. Over the past three decades,surgical hepatic resection has evolved from a high risk,resource intensive procedure with limited application,to a safe and commonly performed operation with a range of indications. This article reviews the approach to surgical resection for malignancies such as hepatocellular cancer,metastatic liver de-posits and neuroendocrine tumors. 3A draft sequence of the rice (Oryza sativa ssp. indica) genome显示文摘The sequence of the rice genome holds fundamental information for its biology, including physiology, genetics, development, and evolution, as well as information on many beneficial phenotypes of economic significance. Using a 'whole genome shotgun' approach, we have pro-duced a draft rice genome sequence of Oryza sativa ssp. in-dica, the major crop rice subspecies in China and many other regions of Asia. The draft genome sequence is constructed from over 4.3 million successful sequencing traces with an accumulative total length of 2214.9 Mb. The initial assembly of the non-redundant sequences reached 409.76 Mb in length, based on 3.30 million successful sequencing traces with a total length of 1797.4 Mb from an indica variant cultivar 93-11, giving an estimated coverage of 95.29% of the rice genome with an average base accuracy of higher than 99%. 4No difference in mortality among ALPPS,two-staged hepatectomy,and portal vein embolization/ligation:A systematic review by updated traditional and network meta-analyses显示文摘Background:There is an ongoing debate on the feasibility,safety,and oncological efficacy of the associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)technique.The aim of this study was to compare ALPPS,two-staged hepatectomy(TSH),and portal vein embolization(PVE)/ligation(PVL)using updated traditional meta-analysis and network meta-analysis(NMA).Data sources:Electronic databases were used in a systematic literature search.Updated traditional metaanalysis and NMA were performed and compared.Mortality and major morbidity were selected as primary outcomes.Results:Nineteen studies including 1200 patients were selected from the pool of 436 studies.Of these patients,315(31%)and 702(69%)underwent ALPPS and portal vein occlusion(PVO),respectively.Ninetyday mortality based on updated traditional meta-analysis,subgroup analysis of the randomized controlled trials(RCTs),and both Bayesian and frequentist NMA did not demonstrate significant differences between the ALPPS cohort and the PVE,PVL,and TSH cohorts.Moreover,analysis of RCTs did not demonstrate significant differences of major morbidity between the ALPPS and PVO cohorts.The ALPPS cohort demonstrated significantly more favorable outcomes in hypertrophy parameters,time to operation,definitive hepatectomy,and R0 margins rates compared with the PVO cohort.In contrast,1-year disease-free survival was significantly higher in the PVO cohort compared to the ALPPS cohort.Conclusions:This study is the first to use updated traditional meta-analysis and both Bayesian and frequentist NMA and demonstrated no significant differences in 90-day mortality between the ALPPS and other hepatic hypertrophy approaches.Furthermore,two high quality RCTs including 147 patients demonstrated no significant differences in major morbidity between the ALPPS and PVO cohorts.Paschalis Gavriilidis Robert P Sutcliffe Keith J Roberts Madhava Pai Duncan Spalding Nagy Habib Long R Jiao Mikael H Sodergren 2020Hepatobiliary & Pancreatic Diseases International2020,19,5:6

6Th1 cytokines promote T-cell binding to antigen-presenting cells via enhanced hyaluronan production and accumulation at the immune synapse显示文摘Hyaluronan(HA)production by dendritic cells(DCs)is known to promote antigen presentation and to augment T-cell activation and proliferation.We hypothesized that pericellular HA can function as intercellular‘glue’directly mediating T cell–DC binding.Using primary human cells,we observed HA-dependent binding between T cells and DCs,which was abrogated upon pre-treatment of the DCs with 4-methylumbelliferone(4-MU),an agent which blocks HA synthesis.Furthermore,T cells regulate HA production by DCs via T cell-derived cytokines in a T helper(Th)subset-specific manner,as demonstrated by the observation that cell-culture supernatants from Th1 but not Th2 clones promote HA production.Similar effects were seen upon the addition of exogenous Th1 cytokines,IL-2,interferon c(IFN-c)and tumor necrosis factor a(TNF-a).The critical factors which determined the extent of DC–T cell binding in this system were the nature of the pre-treatment the DCs received and their capacity to synthesize HA,as T-cell clones which were pre-treated with monensin,added to block cytokine secretion,bound equivalently irrespective of their Th subset.These data support the existence of a feedforward loop wherein T-cell cytokines influence DC production of HA,which in turn affects the extent of DC–T cell binding.We also document the presence of focal deposits of HA at the immune synapse between T-cells and APC and on dendritic processes thought to be important in antigen presentation.These data point to a pivotal role for HA in DC–T cell interactions at the IS.Paul L Bollyky Stephen P Evanko Rebecca P Wu Susan Potter-Perigo S Alice Long Brian Kinsella Helena Reijonen Kelly Guebtner Brandon Teng Christina K Chan Kathy R Braun John A Gebe Gerald T Nepom Thomas N Wight 2010Cellular & Molecular Immunology2010,7,3:3

12“Trends” and variations of global oceanic evaporation data sets from remote sensing显示文摘The variability in global oceanic evaporation data sets was examined for the period 1988-2000.These data sets are satellite estimates based on bulk aerodynamic formulations and include the NASA/Goddard Space Flight Center Satellite-based Surface Turbulent Flux version 2(GSSTF2),the Japanese-ocean flux using remote sensing observations(J-OFURO),and the Hamburg Ocean-Atmosphere Parameters and Fluxes from Satellite version 2(HOAPS2).The National Center for Environmental Prediction(NCEP)reanalysis is also included for comparison.An increase in global average surface latent heat flux(SLHF)can be observed in all the data sets.Empirical mode decomposition(EMD)shows long-term increases that started around 1990 for all remote sensing data sets.The effect of Mt.Pinatubo eruption in 1991 is clearly evident in HOAPS2 but is independent of the long-term increase.Linear regression analyses show increases of 9.4%,13.0%,7.3%,and 3.9% for GSSTF2,J-OFURO,HOAPS2 and NCEP,for the periods of the data sets.Empirical orthogonal function(EOF)analyses show that the pattern of the first EOF of all data sets is consistent with a decadal variation associated with the enhancement of the tropical Hadley circulation,which is supported by other satellite observations.The second EOF of all four data sets is an ENSO mode,and the correlations between their time series and an SOI are 0.74,0.71,0.59,and 0.61 for GSSTF2,J-OFURO,HOAPS2,and NCEP in that order.When the Hadley modes are removed from the remote sensing data,the residue global increases are reduced to 2.2%,7.3%,and <1% for GSSTF2,J-OFURO and HOAPS,respectively.If the ENSO mode is used as a calibration standard for the data sets,the Hadley mode is at least comparable to,if not larger than,the ENSO mode during our study period.CHIU Long S CHOKNGAMWONG R XING Yukun YANG Ruixin SHIE Chung-Lin 2008Acta Oceanologica Sinica2008,27,3:2
13Contribution of genotype and ethnicity to bone mineral density variation in Caucasians and Chinese: a test for five candidate genes for bone mass显示文摘Background Ethnicity is shown to be one of important factors affacting bone mineral density (BMD). The present study was performed to compare the association of six markers for five candidate genes with BMD variation in two populations of different ethnicity, Caucasian and Chinese, and the contribution of genotype and ethnicity to this variation in the populations.Methods The studied restriction fragment length polymorphisms were Bsa H Ⅰ of the calcium-sensing receptor gene, Sac Ⅰ of the α2HS-glycoprotein (AHSG) gene, Pvu Ⅱ and Xba Ⅰ of the oestrogen receptor α gene, Apa Ⅰ of the vitamin D receptor (VDR) gene and BstB Ⅰ of the parathyroid hormone gene. The association of these markers with BMD was analysed by one-way and two-way ANOVA with adjustment for covariates. Results Two polymorphisms, AHSG- Sac Ⅰ and VDR- Apa Ⅰ, showed no association with BMD, while the others were associated with BMD variation at some skeletal sites in either males or females. The polymorphisms indicated clear distinctions between the associations depending on ethnicity, gender and skeletal site. Similar patterns were observed in their contribution to the total population BMD variation. Ethnicity appears to have a larger effect on the total population BMD variation in females than in males. It may account, on the average, for about 2% total population BMD variation at the spine of females and about 1% at the hip of males and females. Conclusion The results of the present study suggest that significant interethnic differentiation at some loci may contribute to the significant interethnic difference in BMD.
