| 2 | Helicobacter pylori infection- recent developments in diagnosis显示文摘Considering the recommended indications for Helicobacter pylori(H.pylori)eradication therapy and the broad spectrum of available diagnostic methods,a reliable diagnosis is mandatory both before and after eradication therapy.Only highly accurate tests should be used in clinical practice,and the sensitivity and specificity of an adequate test should exceed 90%.The choice of tests should take into account clinical circumstances,the likelihood ratio of positive and negative tests,the cost-effectiveness of the testing strategy and the availability of the tests.This review concerns some of the most recent developments in diagnostic methods of H.pylori infection,namely the contribution of novel endoscopic evaluation methodologies for the diagnosis of H.pylori infection,such as magnifying endoscopy techniques and chromoendoscopy.In addition,the diagnostic contribution of histology and the urea breath test was explored recently in specific clinical settings and patient groups.Recent studies recommend enhancing the number of biopsy fragments for the rapid urease test.Bacterial culture from the gastric biopsy is the gold standard technique,and is recommended for antibiotic susceptibility test.Serology is used for initial screening and the stool antigen test is particularly used when the urea breath test is not available,while molecular methods have gained attention mostly for detecting antibiotic resistance. | Ana Isabel Lopes Filipa F Vale Mónica Oleastro | 2014 | World Journal of Gastroenterology2014，20，28： | 12 |
| 3 | Autoimmune hepatitis and anti-tumor necrosis factor alpha therapy:A single center report of 8 cases显示文摘This article describes cases of anti-tumor necrosis factor(TNF)-α-induced autoimmune hepatitis and evaluates the outcome of these patients in relation to their immunosuppressive strategy. A retrospective analysis of medical records was performed in our center, in order to detect cases of autoimmune hepatitis(AIH) associated with anti-TNF biologic agents. We describe and analyze eight cases of AIH following anti-TNF therapy, 7 with infliximab and 1 with adalimumab. A distinction should be made between induction of autoimmunity and clinically evident autoimmune disease. Liver biopsy is useful in detecting the role of the TNF-α antagonist in the development of AIH. The lack of relapse after discontinuing immunosuppressive therapy favors, as in this case series, an immune-mediated drug reaction as most patients with AIH have a relapse after treatment is suspended. | 4 | 过瘤胃蛋氨酸、赖氨酸和组氨酸对泌乳奶牛生产性能的影响显示文摘试验旨在研究饲料中单独添加或综合添加过瘤胃(RP)蛋氨酸、赖氨酸和组氨酸及可代谢蛋白(MP)对泌乳后期奶牛生产性能的影响。试验选取72头荷斯坦奶牛,随机分为6组,试验期9周。奶牛随机分配到6个试验组中,2周作为1个变量周期:MPA组,根据美国国家科学研究委员会(2001)标准,在饲料中添加调节MP到243 g·d-1;MPD组,不添加MP,MP含量低于标准量54 g·d-1;MPDM组,在MPD组的基础上加入RP蛋氨酸;MPDL组,在MPD组的基础上加入RP赖氨酸;MPDH组,在MPD组的基础上加入组氨酸;MPDMLH组,在MPD组的基础上加入RP蛋氨酸、RP赖氨酸和RP组氨酸。试验结果表明,与MPA组对比,MPD组的干物质采食量(DMI)、产奶量、乳成分(乳脂肪、乳蛋白和乳糖)及能量校正的牛乳(ECM)、ECM饲料利用效率、牛奶和血浆中的N均降低。在MPD基础上补充RP赖氨酸可使乳蛋白率、血浆中葡萄糖浓度增加,且牛奶中N元素含量有增加的趋势;补充RP组氨酸可使DMI、牛奶浓缩蛋白增加,且乳脂肪、乳蛋白和ECM显著增加;补充3种RP氨基酸时,增加了乳蛋白率,使DMI和乳脂率有增加趋势,也提高了乳脂肪、乳蛋白含量和ECM及ECM饲料利用效率。与MPA相比,MPD组牛奶中N的含量有降低趋势,且血浆中必需氨基酸(除赖氨酸和蛋氨酸)浓度均降低。补充RP赖氨酸、RP蛋氨酸和RP组氨酸提高了血浆中蛋氨酸(除MPDM组)、赖氨酸和组氨酸浓度。饲喂MPD的奶牛血红蛋白浓度降低,血浆生长素含量高于MPA组。MPD组牛奶中总饱和脂肪酸浓度有高于MPA和MPDMLH组的趋势。MPD组的多元不饱和脂肪、牛奶中奇链与支链脂肪酸含量与MPA组相比,有降低趋势。试验结果表明,对于饲料中缺乏MP的奶牛,在饲料中添加组氨酸和添加3种RP氨基酸(赖氨酸、蛋氨酸和组氨酸)均有提高牛奶产量和牛奶组分含量的可能性。 | Giallongo F Harper M T Oh J Lopes J C Lapierre H Patton R A Parys C Shinzato I Hristov A N | 2016 | 饲料博览2016，0，7： | 4 |
| 5 | Adenocarcinoma of the third and fourth portions of the duodenum: The capsule endoscopy value显示文摘Primary adenocarcinoma of the small intestine occurs in over 50% of cases in the duodenum. However,its location in the third and fourth duodenal portions occurs rarely and is a diagnostic challenge. The aim of this work is to report an adenocarcinoma of the third and fourth duodenal portions,emphasizing its diagnostic difficulty and the value of video capsule endoscopy. A man,40 years old,with no medical history,with abdominal discomfort and progressive fatigue,presented four months ago with one episode o f m o d e ra t e m e l e n a. T h e p hy s i c a l e x a m i n a t i o n was normal,except for mucosal pallor. Blood tests were consistent with microcytic,hypochromic iron deficiency anemia with 7.8 g/d L hemoglobin. The upper and lower endoscopy were normal. Additional work-up with video capsule endoscopy showed a polypoid lesion involving the third and fourth portions o f t h e d u o d e n u m. B i o p s y s h o w e d a m o d e ra t e l y differentiated adenocarcinoma. Abdominal computed tomography showed a wall thickening from the third duodenal portion to the proximal jejunum,without distant metastasis. The patient underwent segmental resection(distal duodenum and proximal jejunum) with duodenojejunostomy. The surgical specimen histology confirmed the biopsy diagnosis,with transmural infiltration,without nodal involvement. | 7 | 高羊茅干草部分替代玉米和紫花苜蓿青贮饲料对奶牛消化、泌乳性能的影响显示文摘试验旨在研究高羊茅干草替代玉米或紫花苜蓿青贮对奶牛中性洗涤纤维(NDF)消化率、泌乳性能的影响。选取初产哺乳荷斯坦奶牛(产奶75±35 d)24头,经产荷斯坦奶牛(产奶68±19 d)40头,随机分成4组,共设置32个进料闸门,记录采食量。每组8个进料阀门,以1头牛为1个试验单位,每头牛试验过程中可接触8个进料闸门。试验用高羊茅干草(TF)取代玉米青贮饲料(CS)或紫花苜蓿青贮饲料(AS),试验设计为:33%AS和67%CS(空白组)、60%TF和40%AS(试验1组)、60%TF和40%CS(试验2组)、33%TF和67%CS(试验3组)。试验进行7周,前2周进行预饲。试验结果表明,各组间牛奶产量无显著差异,平均值为40.4 kg·d^(-1)。各组间牛奶中脂肪含量和浓度、蛋白质含量和浓度均无显著差异,平均值为1.58 kg·d^(-1)和3.94%、1.28 kg·d^(-1)和3.15%。与试验1组(22.1 kg·d^(-1))、试验3组(22.7 kg·d^(-1))相比,空白组的干物质摄入量最高,为24.5kg·d^(-1)。各组间总的干物质消化率无显著差异,平均值为66.2%。与试验2组(44.4%)、试验3组(45.3%)相比,空白组的NDF消化率较低,为37.8%,与试验1组(42.4%)相近。各组间奶牛体内NDF消化率(42.3%)与预估的NDF消化率(42.6%)相近。试验结果表明,饲喂高羊茅干草可提高饲料中NDF消化率,部分替代玉米和紫花苜蓿时对产奶量无显著影响,在市场价格较低时,可作为经济实用的奶牛饲料。 | Bender R W Lopes F Cook D E Combs D K | 2016 | 饲料博览2016，0，8： | 2 |
| 8 | Towards curative therapy in gastric cancer:Faraway, so close!显示文摘Although recent diagnostic and therapeutic advances have substantially improved the survival of patients with gastric cancer(GC), the overall prognosis is still poor. Surgery is the only curative treatment and should be performed in experienced centers. Due to high relapse following surgery, complementary and systemic treatment aimed at eradicating micrometastasis should be performed in most cases. Cytotoxic treatments are effective in downstaging locally advanced cancer, but different sensitivities and toxicities probably exist in different GC subtypes. Current treatment protocols are based primarily on clinical data and histological features, but molecular biomarkers that would allow for the prediction of treatment responses are urgently needed.
