1抗氧化剂一、二级预防随机试验中的死亡率系统回顾及汇总分析显示文摘背景:抗氧化剂正用于多种疾病的预防。 目的:评估补充抗氧化剂对一、二级预防随机临床试验参试者死亡率的影响。数据来源及试验选择:检索2005年10月前的电子数据库以及发表的文献目录。所有于成人中进行的、比较单独或联合使用β胡萝卜素、维生素A、维生素C(抗坏血酸)、维生素E及硒剂的安慰剂或空白治疗对照随机试验均纳入分析。使用随机、盲法及随访作为评估入选试验偏倚的指标。采用随机效应分析法分析抗氧化剂对全因死亡率的影响,结果以相对危险度(relative risk,RR)及其95%可信区间(confidence interval,CI)表示。采用汇总回归评估试验协变量的影响。 数据提取:共入选68项随即试验,包括232606例参试者(385篇文章)。 数据整合:将所有低和高偏倚风险抗氧化剂试验汇总后发现,抗氧化剂对死亡率无显著影响(RR,1.02;95%CI,0.98~1.06)。多变量汇总回归分析表明,低偏倚风险试验(RR,1.16;95%CI,1.05~1.29)和硒剂(RR,0.998;95%CI,0.997~0.9995)与死亡率显著相关。在包括180938例参试者在内的47项低偏倚试验中,补充抗氧化剂显著增加死亡率(RR,1.05;95%CI,1.02~1.08)。排除硒剂试验后,在低偏倚风险试验中,β胡萝卜素(RR,1.07;95%CI,1.02~1,11)、维生素A(RR,1.16;95%CI,1.10~1.24)以及维生素E(RR,1.04;95%CI,1.01~1.07)单独或联合使用均显著增加死亡率。维生素C及硒剂对死亡率无显著影响。 结论:β胡萝卜素、维生素A及维生素E治疗可能增加死亡率。维生素C和硒剂对死亡率的影响需要进一步研究。Goran Bjelakovic Dimitrinka Nikolova Lise Lotte Gluud Rose G. 3Diagnostic procedures for submucosal tumors in the gastrointestinal tract显示文摘This review is part one of three, which will present an update on diagnostic procedures for gastrointestinal (GI) submucosal tumors (SMTs). Part two identifies the classification and part three the therapeutic methods regarding GI SMTs. Submucosal tumors are typically asymptomatic and therefore encountered incidentally. Advances in diagnostic tools for gastrointestinal submucosal tumors have emerged over the past decade. The aim of this paper is to provide the readers with guidelines for the use of diagnostic procedures, when a submucosal tumor is suspected. Literature searches were performed to find information on diagnostics for gastrointestinal submucosal tumors. Based on the searches, the optimal diagnostic procedures and specific features of the submucosal tumors could be outlined. Standard endoscppy, capsule endoscopy and push-and-pull enteroscopy (PPE) together with barium contrast X-ray do not alone provide sufficient information, when examining submucosal tumors. 8Development and predictive validity of the cirrhosis-associated ascites symptom scale: A cohort study of 103 patients显示文摘AIM To develop a scale of domains associated with the health-related quality-of-life(HRQOL) in patients with cirrhosis-related ascites.METHODS We initially undertook literature searches and a qualitative study in order to design a cirrhosis-associated ascites symptom(CAS) scale describing symptoms with a potential detrimental impact on health related quality of life(HRQL)(the higher the score, the worse the symptoms). Discriminatory validity was assessed in a validation cohort including cirrhotic patients with(1) tense/severe;(2) moderate/mild; or(3) no ascites(controls). Patients also completed chronic liver disease questionnaire(CLDQ) and the Euro QoL 5-Dimensions 5-Level(EQ-5D-5L) questionnaire evaluating HRQL. The relation between scale scores was analysed using Spearman correlations. RESULTS The final CAS scale included 14 items. The equivalent reliability was high(Chronbach's alpha 0.88). The validation cohort included 103 patients(72% men, mean age 62.4 years). The mean scores for each question in the CAS scale were higher for patients with severe/tense ascites than for mild/moderate ascites and controls. Compared with controls(mean = 9.9 points), the total CAS scale score was higher for severe/tense ascites(mean = 23.8 points) as well as moderate/mild ascites(mean = 18.6 points)(P < 0.001 both groups). We found a strong correlation between the total CAS and CLDQ score(rho = 0.82, P < 0.001) and a moderate correlation between the CAS and the EQ-5D-5L score(0.67, P < 0.001). 9Tumor cells educate mesenchymal stromal cells to release chemoprotective and immunomodulatory factors显示文摘Factors released by surrounding cells such as cancer-associated mesenchymal stromal cells(CA-MSCs)are involved in tumor progression and chemoresistance.In this study,we characterize the mechanisms by which naYve mesenchymal stromal cells(MSCs)can acquire a CA-MSCs phenotype.Ovarian tumor cells trigger the transformation of MSCs to CA-MSCs by expressing pro-tumoral genes implicated in the chemoresistance of cancer cells,resulting in the secretion of high levels of CXC chemokine receptors 1 and 2(CXCR1/2)ligands such as chemokine(C-X-C motif)ligand 1(CXCL1),CXCL2,and interleukin 8(IL-8).CXCR1/2 ligands can also inhibit the immune response against ovarian tumor cells.Indeed,through their released factors,CA-MSCs promote the differentiation of monocytes towards M2 macrophages,which favors tumor progression.When CXCR1/2 receptors are inhibited,these CA-MSC-activated macrophages lose their M2 properties and acquire an anti-tumoral phenotype.Both ex vivo and in vivo,we used a CXCR1/2 inhibitor to sensitize ovarian tumor cells to carboplatin and circumvent the pro-tumoral effects of CA-MSCs.Since high concentrations of CXCR1/2 ligands in patients*blood are associated with chemoresistance,CXCR1/2 inhibition could be a potential therapeutic strategy to revert carboplatin resistance.Augustin Le Naour Melissa Prat Benolt Thibault Renaud Mével Léa Lemaitre Helene Leray Marie-Veronique Joubert Kimberley Coulson Muriel Golzio Lise Lefevre Eliane Mery Alejandra Martinez Gwénael Ferron Jean-Pierre Delord Agnès Coste Bettina Couderc 2020Journal of Molecular Cell Biology2020,12,3:3

15Sap flow for beech(Fagus sylvatica L.)in a natural and a managed forest—effect of spatial heterogeneity显示文摘Aims Beech(Fagus sylvatica L.)is an important species in natural and managed forests in Europe.This drought-sensitive species dominates even-aged stands as well more natural stands composed of a mixture of tree species,age and size classes.This study evaluates the extent that heterogeneity in spacing and tree diameter affect the seasonal availability and use of water.Methods Two stands were evaluated:(i)a heterogeneous forest remnant(NAT)with trees up to;300 years old,a mean top height of 28.4 m and a total of 733 stems ha1with stem diameters averaging 18 cm and(ii)an even-aged 80-year old stand(MAN),with a height of 25 m,and a total of 283 stems ha1 with diameters averaging 38 cm.Stem sap flow,Js(g m2 s1),was continuously measured in 12(MAN)and 13(NAT)trees using 20-mm long heat dissipation sensors.Individual tree measures of sap flow were correlated using non-linear statistical methods with air vapour pressure deficit(D,hPa)and global radiation(Rg,J m2 day1),along with constraints imposed by reductions in soil water content(SWC).SWC was measured as volumetric%using time domain reflectometry.Important Findings The daily integrated Js(Js-sum)for trees growing in the evenly spaced MAN stand and trees in canopy and closed forest positions in NAT stand decreased as the availability of soil moisture was reduced.In the heterogeneous NAT stand,SWC in a recently formed canopy gap remained high throughout the vegetation period.Based on regression models,the predicted relative decrease in Js-sum for dry relative to moist soil water conditions in the closed forest(at mean daily D=10 hPa)was 7–11%for trees near the gap and 39–42%for trees in the closed forest.In MAN,the reduction in Js-sum was 29%in dry relative to moist conditions.Js-sum in the outer 20 mm of the xylem in NAT was lower than that in MAN and the rate of decline in Js with xylem depth was less in NAT than in MAN.In MAN,Js-sum in deep and outer xylem was negatively affected at low soil moisture availability;in NAT,this was the case for only the outer xylem indicating that deep roots could be important in supplying water at times of low soil moisture in the upper soil.Lise Dalsgaard Teis Nørgaard Mikkelsen Annemarie Bastrup-Birk 2011Journal of Plant Ecology2011,4,1:2
