1International validation of the Chinese University Prognostic Index for staging of hepatocellular carcinoma: a joint United Kingdom and Hong Kong study显示文摘The outcome of hepatocellular carcinoma(HCC) patients significantly differs between western and eastern population centers. Our group previously developed and validated the Chinese University Prognostic Index(CUPI) for the prognostication of HCC among the Asian HCC patient population. In the current study, we aimed to validate the CUPI using an international cohort of patients with HCC and to compare the CUPI to two widely used staging systems, the Barcelona Clinic Liver Cancer(BCLC) classification and the Cancer of the Liver Italian Program(CLIP). To accomplish this goal, two cohorts of patients were enrolled in the United Kingdom(UK; n = 567; 2006-2011) and Hong Kong(HK; n = 517; 2007-2012). The baseline clinical data were recorded. The performances of the CUPI, BCLC, and CLIP were compared in terms of a concordance index(C-index) and were evaluated in subgroups of patients according to treatment intent. The results revealed that the median follow-up durations of the UK and HK cohorts were 27.9 and 29.8 months, respectively. The median overall survival of the UK and HK cohorts were 22.9 and 8.6 months, respectively. The CUPI stratified the patients in both cohorts into three risk subgroups corresponding to distinct outcomes. The median overall survival of the CUPI low-, intermediate-, and high-risk subgroups were 3.15, 1.24, and 0.29 years, respectively, in the UK cohort and were 2.07, 0.32, and 0.10 years, respectively, in the HK cohort. For the patients who underwent curative treatment, the prognostic performance did not differ between the three staging systems, and all were suboptimal. For those who underwent palliative treatment, the CUPI displayed the highest C-index, indicating that this staging system was the most informative for both cohorts. In conclusion, the CUPI is applicable to both western and eastern HCC patient populations. The performances of the three staging systems differed according to treatment intent, and the CUPI was demonstrated to be optimal for those undergoing palliative treatment. 2Systemic treatment for inoperable pancreatic adenocarcinoma: review and update显示文摘There have been many clinical trials conducted to evaluate novel systemic regimens for unresectable pancreatic cancer. However, most of the trial results were negative, and gemcitabine monotherapy has remained the standard systemic treatment for years. A number of molecular targeted agents, including those against epidermal growth factor receptor and vascular endothelial growth factor receptors, have also been tested. In recent years, there have been some breakthroughs in the deadlock: three regimens, namely gemcitabine-erlotinib, FOLFIRINOX, and gemcitabine-nab-paclitaxel, have been shown to prolong the overall survival of patients when compared with gemcitabine monotherapy. In addition, emerging data suggested that the membrane protein human equilibrative nucleotide transporter 1 is a potential biomarker with which to predict the efficacy of gemcitabine. 7Time dependent integration of matrix metalloproteinases and their targeted substrates directs axonal sprouting and synaptogenesis following central nervous system injury显示文摘Over the past two decades,many investigators have reported how extracellular matrix molecules act to regulate neuroplasticity.The majority of these studies involve proteins which are targets of matrix metalloproteinases.Importantly,these enzyme/substrate interactions can regulate degenerative and regenerative phases of synaptic plasticity,directing axonal and dendritic reorganization after brain insult.The present review first summarizes literature support for the prominent role of matrix metalloproteinases during neuroregeneration,followed by a discussion of data contrasting adaptive and maladaptive neuroplasticity that reveals time-dependent metalloproteinase/substrate regulation of postinjury synaptic recovery.The potential for these enzymes to serve as therapeutic targets for enhanced neuroplasticity after brain injury is illustrated with experiments demonstrating that metalloproteinase inhibitors can alter adaptive and maladaptive outcome.Finally,the complexity of metalloproteinase role in reactive synaptogenesis is revealed in new studies showing how these enzymes interact with immune molecules to mediate cellular response in the local regenerative environment,and are regulated by novel binding partners in the brain extracellular matrix.Together,these different examples show the complexity with which metalloproteinases are integrated into the process of neuroregeneration,and point to a promising new angle for future studies exploring how to facilitate brain plasticity.Linda L.Phillips Julie L.Chan Adele E.Doperalski Thomas M.Reeves 2014Neural Regeneration Research2014,9,4:1

10On large scale CFD-DEM simulation for gas-liquid-solid,three-phase flows显示文摘Particulate flows in a mixture of gas and liquid,i.e.gas-liquid-solid three-phase flows,are frequently encountered both in nature and industry.In such flows,complex interactions between multiple phases,i.e.particle-particle interactions,fluid-particle interactions and interfacial interactions(such as surface tension and particle wetting),play a crucial role.In literature,simulations of three-phase flows are sometimes performed by incorporating interface capturing methods(e.g.VOF method)into the CFD-DEM coupling model.However,it is practically impossible to perform large(industrial)scale simulation because of the high computational cost.One of the strategies often employed to reduce the computational cost in CFD-DEM is to upscale particle size,which is applied mainly to particle single-phase and fluid-solid two-phase flows.The present work is focused on the scaled-up particle model for gas-liquid-solid three-phase flows.The interaction forces between multiple phases are scaled using the general criteria derived from the continuum assumption of particulate flow.A colour function based interface-capturing method with improved interface smoothness is developed,and the diffusion based coarse graining is employed to ensure sufficient space resolution in CFD even when particle size is increased.It is shown that the model developed is capable of predicting the both particles and fluid behaviour in the original system.Kimiaki Washino Ei L.Chan Tetsushi Kaji Yoshiaki Matsuno Toshitsugu Tanaka 2021Particuology2021,19,6:1

14The impacts of suspended mariculture on coastal zones in China and the scope for Integrated Multi-Trophic Aquaculture显示文摘Introduction:China is responsible for more than 60%of global aquaculture production.As the frontiers of food production have expanded,the cultivation of marine organisms in coastal zones and the open ocean has grown rapidly.The dominant mariculture industry in China is suspended mariculture,which uses net cages,ropes,or other structures suspended in the water column to cultivate aquatic organisms.This systematic,quantitative review provides a clear and comprehensive account of research that has investigated the adverse impacts of suspended mariculture in China and reviews research that has applied Integrated Multi-Trophic Aquaculture(IMTA)systems for mitigating impacts.This work builds on 218 peer reviewed papers that have been published in English-language journals.Outcomes:Eighteen impacts were identified,including chemical,ecological,physical,and socioeconomic impacts.Eighteen measures for improving suspended mariculture were recommended consisting of government department,farm management,and ecological engineering measures.IMTA was the most frequently recommended measure.The capabil-ities of IMTA for bioremediation and increased farm production were the most frequently studied advantages.Seven other benefits have been explored but remain understudied.The current challenges facing the expansion of commercial IMTA include limited use of new technology,limited skills development,decreasing production of low trophic-level species,biogeographic and temporal barriers,and negative system feedbacks.Conclusion:Despite challenges,implementing commercial IMTA is a promising measure for reducing the impacts of sus pended mariculture because it presents a range of secondary benefits that can improve the overall sustainability of aquaculture in the coastal zone.Reece Wartenberg Limin Feng Jia Jun Wu Yim Ling Mak Leo L.Chan Trevor C.Telfer Paul K.S.Lamab.c 2017Ecosystem Health and Sustainability2017,3,6:0

16Middle ear microbiome differences in indigenous Filipinos with chronic otitis media due to a duplication in the A2ML1 gene显示文摘Background:Previously rare A2ML1 variants were identified to confer otitis media susceptibility in an indigenous Filipino community and in otitis-prone US children.The goal of this study is to describe differences in the middle ear microbiome between carriers and non-carriers of an A2ML1 duplication variant that increases risk for chronic otitis media among indigenous Filipinos with poor health care access.Methods:Ear swabs were obtained from 16 indigenous Filipino individuals with chronic otitis media,of whom 11 carry the A2ML1 duplication variant.Ear swabs were submitted for 16S rRNA gene sequencing.Results:Genotype-based differences in microbial richness,structure,and composition were identified,but were not statistically significant.Taxonomic analysis revealed that the relative abundance of the phyla Fusobacteria and Bacteroidetes,and genus Fusobacterium were nominally increased in carriers compared to non-carriers,but were non-significant after correction for multiple testing.We also detected rare bacteria including Oligella that was reported only once in the middle ear.Conclusions:These findings suggest that A2ML1-related otitis media susceptibility may be mediated by changes in the middle ear microbiome.Knowledge of middle ear microbial profiles according to genetic background can be potentially useful for therapeutic and prophylactic interventions for otitis media and can guide public health interventions towards decreasing otitis media prevalence within the indigenous Filipino community.Regie Lyn P.Santos-Cortez Diane S.Hutchinson Nadim J.Ajami Ma.Rina T.Reyes-Quintos Ma.Leah C.Tantoco Patrick John Labra Sheryl Mae Lagrana Melquiadesa Pedro Erasmo Gonzalo d.V.Llanes Teresa Luisa Gloria-Cruz Abner L.Chan Eva Maria Cutiongco-de la Paz John W.Belmont Tasnee Chonmaitree Generoso T.Abes Joseph F.Petrosino Suzanne M.Leal Charlotte M.Chiong 2016Infectious Diseases of Poverty2016,5,1:0
