| 1 | Management of acute nonvariceal upper gastrointestinal bleeding:Current policies and future perspectives显示文摘Acute upper gastrointestinal bleeding(UGIB) is a gastroenterological emergency with a mortality of 6%-13%.The vast majority of these bleeds are due to peptic ulcers.Nonsteroidal anti-inflammatory drugs and Helicobacter pylori are the main risk factors for peptic ulcer disease.Endoscopy has become the mainstay for diagnosis and treatment of acute UGIB,and is recommended within 24 h of presentation.Proton pump inhibitor(PPI) administration before endoscopy can downstage the bleeding lesion and reduce the need for endoscopic therapy,but has no effect on rebleeding,mortality and need for surgery.Endoscopic therapy should be undertaken for ulcers with high-risk stigmata,to reduce the risk of rebleeding.This can be done with a variety of modalities.High-dose PPI administration after endoscopy can prevent rebleeding and reduce the need for further intervention and mortality,particularly in patients with high-risk stigmata. | Ingrid Lisanne Holster Ernst Johan Kuipers | 2012 | World Journal of Gastroenterology2012,18,11: | 53 |
| 2 | China’s New Rural Income Support Policy:Impacts on Grain Production and Rural Income Inequality显示文摘This paper analyses the impact of agricultural tax abolition and direct income payments to grain farmers on grain production and rural inequality in China. To separate the impact of the income support measures from recent price trends for grains and inputs, and to account for differences in household responses, we use a village-level general equilibrium model that we calibrate for two villages with different degrees of market access in Jiangxi province. The results show that the income support policy does not reach its goal of promoting grain production. The increased incomes allow farm households to buy more inputs for livestock production and involve other activities that are more profitable than grain farming. Selling of rice outside the villages declines more than rice production, because households in the villages consume more rice when incomes rise. | 4 | Two-fluid modeling of Geldart A particles in gas-fluidized beds显示文摘We have investigated the effect of cohesion and drag models on the bed hydrodynamics of Geldart A particles based on the two-fluid (TF) model. For a high gas velocity U0 = 0.03 m/s, we found a transition from the homogeneous fluidization to bubbling fluidization with an increase of the coefficient C1, which is used to account for the contribution of cohesion to the excess compressibility. Thus cohesion can play a role in the bed expansion of Geldart A particles. Apart from cohesion, we have also investigated the influence of the drag models. When using the Wen and Yu drag correlation with an exponent n = 4.65, we find an under-prediction of the bed expansion at low gas velocities (U0 = 0.009 m/s). When using a larger exponent (n = 9.6), as reported in experimental studies of gas-fluidization, a much better agreement with the experimental bed expansion is obtained. These findings suggest that at low gas velocity, a scale-down of the commonly used drag model is required. On the other hand, a scale-up of the commonly used drag model is necessary at high gas velocity (U0 = 0.2 and 0.06 m/s). We therefore conclude that scaling the drag force represent only an ad hoc way of repairing the deficiencies of the TF model, and that a far more detailed study is required into the origin of the failure of the TF model for simulating fluidized beds of fine powders. | M.A. van der Hoef J.A.M. | 6 | Treatment of acute periprosthetic infections with prosthesis retention: Review of current concepts显示文摘Periprosthetic joint infection(PJI) is a devastating complication after total joint arthroplasty, occurring in approximately 1%-2% of all cases. With growing populations and increasing age, PJI will have a growing effect on health care costs. Many risk factors have been identified that increase the risk of developing PJI, including obesity, immune system deficiencies, malignancy, previous surgery of the same joint and longer operating time. Acute PJI occurs either postoperatively(4 wk to 3 mo after initial arthroplasty, depending on the classification system), or via hematogenous spreading after a period in which the prosthesis had functioned properly. Diagnosis and the choice of treatment are the cornerstones to success. Although different definitions for PJI have been used in the past, most are more or less similar and include the presence of a sinus tract, blood infection values, synovial white blood cell count, signs of infection on histopathological analysis and one ormore positive culture results. Debridement, antibiotics and implant retention(DAIR) is the primary treatment for acute PJI, and should be performed as soon as possible after the development of symptoms. Success rates differ, but most studies report success rates of around 60%-80%. Whether single or multiple debridement procedures are more successful remains unclear. The use of local antibiotics in addition to the administration of systemic antibiotic agents is also subject to debate, and its pro's and con's should be carefully considered. Systemic treatment, based on culture results, is of importance for all PJI treatments. Additionally, rifampin should be given in Staphylococcal PJIs, unless all foreign material is removed. The most important factors contributing to treatment failure are longer duration of symptoms, a longer time after initial arthroplasty, the need for more debridement procedures, the retention of exchangeable components, and PJI caused by Staphylococcus(aureus or coagulase negative). If DAIR treatment is unsuccessful, the following treatment option should be based on the patient health status and his or her expectations. For the best functional outcome, one- or two-stage revision should be performed after DAIR failure. | 12 | Influence of liquid layers on energy absorption during particle impact显示文摘The influence of the thickness of a covering liquid layer and its viscosity as well as the impact velocity on energy loss during the normal impact on a flat steel wall of spherical granules with a liquid layer was studied. Free-fall experiments were performed to obtain the restitution coefficient of elastic–plastic Al2O3 granules by impact on the liquid layer, using aqueous solutions of hydroxypropyl methylcellulose with different concentrations for variation of viscosity (1–300 mPa s). In the presence of a liquid layer, increase of liquid viscosity decreases the restitution coefficient and the minimum thickness of the liquid layer at which the granule sticks to the wall. The measured restitution coefficients were compared with experiments performed without liquid layer. In contrast to the dry restitution coefficient, due to viscous losses at lower impact velocity, higher energy dissipation was obtained. A rational explanation for the effects obtained was given by results of numerically solved force and energy balances for a granule impact on a liquid layer on the wall. The model takes into account forces acting on the granule including viscous, surface tension, capillary, contact, drag, buoyancy and gravitational forces. The dimensions of the bed and other parameters are set to correspond to those of Müller et al. (2008). Various drag models and operational parameters are investigated to find their influence on particle and bubble granular temperatures. Various inlet superficial gas velocities are used in this work to obtain their effect on flow characteristics. It is found that the superficial gas velocity has the most important effect on granular temperatures including bubble granular temperature, particle translational granular temperature and particle rotational granular temperature. The drag force model affects more seriously the large scale variables such as the bubble granular temperature. Restitution coefficient influences all granular temperatures to some degree. Simulation results are compared with experimental results by Müller et al. | 16 | Emerging roles of the intestine in control of cholesterol metabolism显示文摘The liver is considered the major “control center” for maintenance of whole body cholesterol homeostasis. This organ is the main site for de novo cholesterol synthesis, clears cholesterol-containing chylomicron remnants and low density lipoprotein particles from plasma and is the major contributor to high density lipoprotein (HDL; good cholesterol) formation. The liver has a central position in the classical definition of the reverse cholesterol transport pathway by taking up periphery-derived cholesterol from lipoprotein particles followed by conversion into bile acids or its direct secretion into bile for eventual removal via the feces. During the past couple of years, however, an additional important role of the intestine in maintenance of cholesterol homeostasis and regulation of plasma cholesterol levels has become apparent. Firstly, molecular mechanisms of cholesterol absorption have been elucidated and novel pharmacological compounds have been identified that interfere with the process and positively impact plasma cholesterol levels. Secondly, it is now evident that the intestine itself contributes to fecal neutral sterol loss as a cholesterol-secreting organ. Finally, very recent work has unequivocally demonstrated that the intestine contributes significantly to plasma HDL cholesterol levels. METHODS:Microvessel density(MVD) in GEP-NETs was evaluated using endoglin and CD31 immunohistochemistry.In addition,tissue levels of endoglin and VEGF were determined in homogenates by ELISA. RESULTS:Endoglin was highly expressed on tumor endothelial cells.CD31 MVD in GEP-NETs was significantly higher compared to endoglin MVD(P<0.01) .Two-tofour-fold higher tissue levels of endoglin and VEGF were seen in tumors compared to associated normal tissue. This increased endoglin tissue expression in tumors was significantly related to tumor size(P<0.01) ,presence of metastases(P=0.04) ,and a more advanced tumor stage(P=0.02) ,whereas expression of VEGF was not. | 20 | COMPUTATIONAL FLUID DYNAMICS FOR DENSE GAS-SOLID FLUIDIZED BEDS: A MULTI-SCALE MODELING STRATEGY显示文摘Dense gas-particle flows are encountered in a variety of industrially important processes for large scale production of fuels, fertilizers and base chemicals. The scale-up of these processes is often problematic and is related to the intrinsic complexities of these flows which are unfortunately not yet fully understood despite significant efforts made in both academic and industrial research laboratories. In dense gas-particle flows both (effective) fluid-particle and (dissipative) particle-particle interactions need to be accounted for because these phenomena to a large extent govern the prevailing flow phenomena, i.e. the formation and evolution of heterogeneous structures. These structures have significant impact on the quality of the gas-solid contact and as a direct consequence thereof strongly affect the performance of the process. Due to the inherent complexity of dense gas-particles flows, we have adopted a multi-scale modeling approach in which both fluid-particle and particle-particle interactions can be properly accounted for. The idea is essentially that fundamental models, taking into account the relevant details of fluid-particle (lattice Boltzmann model) and particle-particle (discrete particle model) interactions, are used to develop closure laws to feed continuum models which can be used to compute the flow structures on a much larger (industrial) scale. Our multi-scale approach (see Fig. 1 ) involves the lattice Boltzmann model, the discrete particle model, the continuum model based on the kinetic theory of granular flow and the discrete bubble model. In this paper we give an overview of the multi-scale modeling strategy, accompanied by illustrative computational results for bubble formation.
